Abstract Introduction Impacts of age and gender on sleep have been reported in normal population, but rarely in chronic insomnia disorder (CID). This study aimed to investigate difference in sleep characteristics of CID according to gender and age. Methods The participants with drug-naïve CID and aged between 40 and 79 years were recruited. We compared subjective and objective sleep parameters between the middle-aged (40-64 years, N=86) and the elderly (65-79 years, N=50), and between men (N=45) and women (N=91). The subjective sleep quality and habitual sleep time were measured by Pittsburgh Sleep Quality Index (PSQI). The participants were asked to wear an actigraph for 4 days to obtain objective sleep parameters. Results In the PSQI, the elderly reported earlier bedtime and wake-up time (p=0.018; p=0.026), reduced total sleep time (TST) and sleep efficiency (p=0.003; p=0.011), and low sleep quality (p=0.034) compared to the middle-aged. However, according to the actigraphy, differences were observed only in the bedtime (p=0.016) and the wake-up time (p=0.002) between the two age groups. Between genders, the actigraphy showed that the male patients woke up earlier than the female group (p=0.015); except for this finding, there was no significant gender effect. Meanwhile, regarding gender and age interactions, the elderly women with CID showed longer time in bed (TIB) with increase in both TST and wake after sleep onset (WASO) compared to the middle-aged women. The elderly men showed decreased TIB and TST, and slightly decreased WASO than the middle-aged men. Conclusion The elderly with CID show more subjective sleep complaints than the middle-aged CID despite little difference in objective sleep characteristics, which suggests that the elderly CID may seek medical help more than the middle aged. As women with CID get older, they increase time spent in bed to maintain sleep time, but with resultant increase in wake. Support None

Background Yoga nidra is practised by sages for sleep. The practice is simple to use and has been clearly laid out, but its role in the treatment of chronic insomnia has not been well studied. Methods In this randomized parallel-design study conducted during 2012–16, we enrolled 41 patients with chronic insomnia to receive conventional intervention of cognitive behavioural therapy for insomnia (n=20) or yoga nidra (n=21). Outcome measures were both subjective using a sleep diary and objective using polysomnography (PSG). Salivary cortisol levels were also measured. PSG was done before the intervention in all patients and repeated only in those who volunteered for the same. Results Both interventions showed an improvement in subjective total sleep time (TST), sleep efficiency, wake after sleep onset, reduction in total wake duration and enhancement in subjective sleep quality. Objectively, both the interventions improved TST and total wake duration and increased N1% of TST. Yoga nidra showed marked improvement in N2% and N3% in TST. Salivary cortisol reduced statistically significantly after yoga nidra (p=0.041). Conclusion Improvement of N3 sleep, total wake duration and subjective sleep quality occurred following yoga nidra practice. Yoga nidra practice can be used for treatment of chronic insomnia after supervised practice sessions.

Purpose: To explore the effects of travel related to international rugby sevens competition on sleep patterns. Methods: A total of 17 international male rugby sevens players participated in this study. Actigraphic and subjective sleep assessments were performed daily during 2 separate Sevens World Series competition legs (Oceania and America). The duration of each competition leg was subdivided into key periods (pretour, precompetition, tournament 1, relocation, tournament 2, and posttour) lasting 2 to 7 nights. Linear mixed models in combination with magnitude-based decisions were used to assess (1) the difference between preseason and key periods and (2) the effect of travel direction (eastward or westward). Results: Shorter total sleep time (hours:minutes) was observed during tournament 2 (mean [SD], 06:16 [01:08]), relocation (06:09 [01:09]), and the pretour week (06:34 [01:24]) compared with the preseason (06:52 [01:00]). Worse sleep quality (arbitrary units) was observed during tournament 1 (6.1 [2.0]) and 2 (5.7 [1.2]), as well as during the relocation week (6.3 [1.5]) than during the preseason (6.5 [1.8]). When traveling eastward compared with westward, earlier fall-asleep time was observed during tournament 1 (ES − 0.57; 90% CI, −1.12 to −0.01), the relocation week (−0.70 [−1.11 to −0.28]), and the posttour (−0.57 [−0.95 to −0.18]). However, possibly trivial and unclear differences were observed during the precompetition week (0.15 [−0.15 to 0.45]) and tournament 2 (0.81 [−0.29 to 1.91]). Conclusion: The sleep patterns of elite rugby sevens players are robust to the effects of long-haul travel and jet lag. However, the staff should consider promoting sleep during the tournament and relocation week.

Abstract Introduction Poor sleep quality is a health condition that impacts the quality of life of the older population. In the literature, there are questions about which objective sleep parameters should be considered to describe precisely the definition of sleep quality. There is ongoing debate with this term usually being used in relation to subjective sleep perception. This study aimed to investigate which objective and subjective sleep parameters contribute to a measurement of sleep quality in older adults. Methods A cross-sectional study using a representative sample of adults from the city of São Paulo, Brazil was performed. We used a dataset from the 2015 Epidemiological Study of Sleep from the City of São Paulo (EPISONO), including only individuals aged 60 years or more. We used exploratory factor analysis and structural equation modelling to identify relevant variables to a descriptive model of sleep quality. Results A total of 152 older adults were included. The final model consists of two factors, objective sleep quality which comprises sleep efficiency, total sleep time and sleep latency, and poor sleep perception, constituted by scores in the Pittsburgh Sleep Quality Index and Insomnia Severity Index. Conclusion The results suggested that sleep quality had both an objective (sleep efficiency, total sleep time, latency of sleep onset) and subjective dimensions (subjective questionnaires). These results may be useful in the clinical scenario, serving as leads for a better understanding of the sleep quality in aging patients. Future studies may also benefit from this descriptive model to further researches other associations, such as sleep and pain in this population. Emerging evidence suggests that adequate intake of omega-3 polyunsaturated fatty acids (n-3 PUFAs), which include docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), might be associated with better sleep quality. N-3 PUFAs, which must be acquired from dietary sources, are typically consumed at suboptimal levels in Western diets. Therefore, the current placebo-controlled, double-blind, randomized trial, investigated the effects of an oil rich in either DHA or EPA on sleep quality in healthy adults who habitually consumed low amounts of oily fish. Eighty-four participants aged 25–49 years completed the 26-week intervention trial. Compared to placebo, improvements in actigraphy sleep efficiency (p = 0.030) and latency (p = 0.026) were observed following the DHA-rich oil. However, these participants also reported feeling less energetic compared to the placebo (p = 0.041), and less rested (p = 0.017), and there was a trend towards feeling less ready to perform (p = 0.075) than those given EPA-rich oil. A trend towards improved sleep efficiency was identified in the EPA-rich group compared to placebo (p = 0.087), along with a significant decrease in both total time in bed (p = 0.032) and total sleep time (p = 0.019) compared to the DHA-rich oil. No significant effects of either treatment were identified for urinary excretion of the major melatonin metabolite 6-sulfatoxymelatonin. This study was the first to demonstrate some positive effects of dietary supplementation with n-3 PUFAs in healthy adult normal sleepers, and provides novel evidence showing the differential effects of n-3 PUFA supplements rich in either DHA or EPA. Further investigation into the mechanisms underpinning these observations including the effects of n-3 PUFAs on sleep architecture are required.

Objectives: the current study aimed to investigate the relationship between physical activity (PA) level and inhibitory control performance and then to determine whether this association was mediated by multiple sleep parameters (i.e., subjective sleep quality, sleep duration, sleep efficiency, and sleep disturbance). Methods: 180 healthy university students (age: 20.15 ± 1.92 years) from the East China Normal University were recruited for the present study. PA level, sleep parameters, and inhibitory control performance were assessed using the International Physical Activity Questionnaire (IPAQ), the Pittsburgh Sleep Quality Index Scale (PSQI), and a Stroop test, respectively. The data were analyzed using structural equation modeling. Results: A higher level of PA was linked to better cognitive performance. Furthermore, higher subjective sleep quality and sleep efficiency were associated with better inhibitory control performance. The mediation analysis revealed that subjective sleep quality and sleep efficiency mediated the relationship between PA level and inhibitory control performance. Conclusion: our results are in accordance with the literature and buttress the idea that a healthy lifestyle that involves a relatively high level of regular PA and adequate sleep patterns is beneficial for cognition (e.g., inhibitory control performance). Furthermore, our study adds to the literature that sleep quality and sleep efficiency mediates the relationship between PA and inhibitory control performance, expanding our knowledge in the field of exercise cognition.

Abstract Introduction Several studies have shown the negative impact of COVID-19-related confinement measures (spring 2020) on sleep features and quality. Here we aim to follow-up on these data by assessing self-reported sleep characteristics during the second wave of the pandemic (autumn 2020) in Italy, where the government responded with a second, partial, lockdown. Methods Using a longitudinal approach, 214 participants (Mage=36.78±14.2y; 159F) who had participated in a previous survey (April 2020 – Total Lockdown, TL) completed the same online survey from November 10th to December 1st (Partial Lockdown, PL). In each survey, participants had to respond to a set of questionnaires, including the Pittsburgh Sleep Quality Index (PSQI), referring to their current situation and, retrospectively, to their situation before the lockdown, resulting in four time-points (pre-TL, TL, pre-PL, PL). Results Linear mixed-model analysis showed that bedtime was delayed from pre-TL (23:46) to TL (24:42) and then linearly advanced in pre-PL (24:02) and PL (23:56). The same pattern emerged for rise time (pre-TL: 07:48, TL: 9:05, pre-PL: 8:28, PL: 7:58) and time in bed, which increased from pre-TL (8h4min) to TL (8h24min) and then linearly decreased (pre-PL: 8h15m, PL: 8h2m). Subjective sleep quality decreased in the two lockdowns compared to the period with no restrictions. The proportion of poor sleepers (PSQI>5) increased from 39.7% (pre-TL) to 48.6% in TL and again from 36.9% (pre-PL) to 47.7% in PL. Conclusion Sleep habits and quality showed different profiles across four time-points of the COVID-19 pandemic and related restrictive measures. Sleep timing alterations appeared during the first lockdown, recovered after the confinement period, and almost returned to baseline during the second lockdown (likely due to a normalization of working schedules). Instead, subjective sleep quality markedly worsened during both lockdowns relative to the preceding respective months. These data suggest that subjective sleep quality is particularly sensitive to changes in life habits and psychological factors, independently of sleep habits. Considering that the pandemic situation may continue for several months, there is a need for interventions targeting sleep quality. Support (if any) N/A

The discrepancy between natural sleep-wake rhythm and actual sleep times in shift workers can cause sleep loss and negative daytime consequences. Irregular shift schedules do not follow a fixed structure and change frequently, which makes them particularly harmful and makes affected individuals more susceptible to insomnia. The present study compares insomnia symptoms of non-shift workers, regular shift workers, and irregular shift workers and takes into account the moderating role of the Big Five personality traits and levels of perfectionism. Employees of an Austrian railway company completed an online survey assessing shift schedules, sleep quality and duration, daytime sleepiness, and personality traits. A total of 305 participants, of whom 111 were non-shift workers, 60 regular shift workers, and 134 irregular shift workers, made up the final sample. Irregular shift workers achieved significantly worse scores than one or both of the other groups in time in bed, total sleep time, sleep efficiency, sleep duration, sleep quality, sleep latency, and the number of awakenings. However, the values of the irregular shifts workers are still in the average range and do not indicate clinical insomnia. Participants working regular shifts reported the best sleep quality and longest sleep duration and showed the least nocturnal awakenings, possibly due to higher conscientiousness- and lower neuroticism scores in this group. Agreeableness increased the effect of work schedule on total sleep time while decreasing its effect on the amount of sleep medication taken. Perfectionism increased the effect of work schedule on time in bed and total sleep time. Generalization of results is limited due to the high percentage of males in the sample and using self-report measures only.

Poor sleep quality during pregnancy is associated with both antepartum and postpartum depression and adverse birth outcomes. This study evaluated both objective and subjective sleep quality and the effects on the subsequent course of antepartum depressive symptoms in psychiatric patients. This observational explorative study was embedded in an ongoing study focusing on pregnant women with a mental disorder and was performed in 18 patients (24–29 weeks pregnant). Depressive symptoms were assessed throughout pregnancy using the Edinburgh Postnatal Depression Scale (EPDS) with 5-week intervals. Sleep was assessed with actigraphy, the Pittsburgh Sleep Quality Index (PSQI) and sleep diaries at the start of the study. We studied correlations between sleep parameters and EPDS scores cross-sectionally using Spearman correlation. Next, we studied the course of antepartum EPDS scores over time per sleep parameter using generalized linear mixed modelling analysis. Objectively measured fragmentation index, total PSQI score and 4 PSQI subscales (sleep quality, sleep duration, sleep disturbances and daytime dysfunctions) were significantly correlated with EPDS scores when measured cross-sectionally at the start. Six objectively and subjectively measured sleep parameters had moderate to large effects on the course of depressive symptoms through the third trimester, but these effects were not statistically significant. More research is necessary to explore the causality of the direction between sleep problems and antepartum depressive symptoms we found in psychiatric patients.

Abstract Background and objective The extent to which adult sleep varies depending on the day of the week has not yet been systematically investigated with electroencephalography (EEG) data. Whether such effects exist and whether they are related to age, gender, and employment status was retrospectively analyzed based on data from an experimental double-blind cross-over study in which effects of electromagnetic fields of a cell phone base station on the sleep of a general rural population had been examined. Methods The sleep of 397 adults (age 45.0 ± 14.2 years, range 18–81 years; 50.9% women) from ten different rural German villages was recorded for 12 nights with ambulatory devices. Self-reported sleep quality was recorded in morning and evening protocols. Friedman tests were used for statistical analysis of the comparison between the days, and the Kruskal–Wallis and Mann–Whitney U tests were used for pairwise comparisons of independent parameters between groups. Results For the present analysis, data from 335 participants were considered. Overall, the differences between nights were small and the quality of sleep was good. Three of the five objective and all six self-rated sleep parameters differed significantly between the days of the week. While the objective and the self-estimated total sleep time were longest on Sunday nights, the qualitatively poorest values occurred on Monday nights. People who worked fulltime had the longest sleep latencies on Sunday nights. Friday nights were rated the best. Conclusion The objective and self-rated sleep quality varied relatively little in a rural adult population over the course of the week, being worst on Monday nights and best on Friday nights.
