ABSTRACTPurposePsychological screening in patient with cancer is recommended by clinical guidelines, however most of scales have large number of items, difficulty detection and refer from routine consultations. The specific objective of the study was to develop and validate the ADAF screening for anxiety, depression and coping.Methods/PatientsCross-sectional, multicenter study performed in the medical and radiotherapy oncology services of 5 hospitals in Madrid, coordinated by the Medical Oncology Service of the Hospital Clínico San Carlos (CEIC nº19 / 265-E). To determine the psychometric properties, the ADAF screening questionnaire ADAF was administered, including 5 items (one related to anxiety symptoms, two related to depressive symptoms, one for helplessness coping and one for avoidance coping), and as a gold-standard the HADS and the MiniMAC. Intraclass correlation coefficients and receiver operating characteristic curves were performed. The p value <0.05 was considered significant.ResultsA total of 186 patients completed the evaluation. The correlation coefficients were significant for all dimensions (Anxiety, Depression, Helplessness coping, and Avoidance Coping), with p <0.001. The statistical analysis of ROC curves suggests that the cut-off point for screening is equivalent to a score > 2 points (3 in the case of depression, having two items), with a sensitivity and specificity between 62 and 90%, depending on the item, and an area under the curve above 0.8 for the first 4 items.ConclusionsADAF screening has adequate reliability, good sensitivity and specificity. This instrument is useful and easy to use to identify emotional and coping problems in cancer patients.

PurposeTo evaluate the capability of wide-field spectral-domain optical coherence tomography (SD-OCT) corneal sublayer pachymetry to distinguish between keratoconus and pellucid marginal degeneration (PMD).MethodsThis prospective cross-sectional study included 69 eyes (59 with keratoconus and 10 with PMD) of 69 patients. All patients were examined three subsequent times with the RTVue XR system. Pachymetry maps of the total cornea (CT), the epithelium (ET) and the stroma were generated using the onboard software. For analysis of reliability, the coefficients of variation and intraclass correlation coefficients (ICC) were calculated. Receiver operating characteristic analyses were performed to elaborate the most accurate pachymetric parameters for distinguishing between PMD and keratoconus.ResultsOverall repeatability of (sublayer) pachymetry was comparably good in both keratoconus (ICC ranging between 0.827 and 0.986) and PMD (ICC ranging between 0.753 and 0.998). Measurement reliability in keratoconic eyes was a negative function of Kmax (p<0.05). As compared with keratoconus, PMD exhibited higher CT (526±31 µm vs 503±30 µm; p=0.02) and ET (56±7 µm vs 51±5 µm; p=0.02) in the inferotemporal 2–5 mm sector as well as lower ET in the inferior 7–9 mm sector (52±5 µm vs 57±5 µm; p<0.01). The calculated ratio between CT in the inferotemporal 2–5 mm and in the inferior 7–9 mm sector yielded the highest diagnostic accuracy for distinguishing between PMD and keratoconus with an area under the curve of 0.977 and an optimal cut-off value of 0.90.ConclusionWide-field SD-OCT corneal sublayer pachymetry showed good reliability in PMD and keratoconus and may be useful to differentiate between the two ectatic diseases.

Introduction: One of the popular questionnaires for screening neuropathic pain is the DN4 (Douleur Neuropathique 4) which has been translated into many languages and validated in different populations. So, we wanted to determine the Persian version of this questionnaire’s reliability and validity to differentiate neuropathic pain from other painful non-neuropathic conditions. Methods: This cross-sectional study was conducted on 120 patients who suffered from chronic pain referred to the Physical Medicine and Rehabilitation Department at the three great referral hospitals in Tehran, Iran in 2016. In this study, a modified version of DN4 was used to validate adding a question related to the appearance of pain in an especial joint and adding visual analogue scaling (VAS) score in each item. Results: The modified DN4 questionnaire had high reliability for assessing neuropathic pain with the Cronbach’s alpha coefficient of 0.874. The correlation coefficients obtained by the retest intra-class correlation test ranged from 0.650 to 0.941. Also, the internal consistency of Spearman’s R coefficients ranged from 0.482 to 0.904. Test-retest reliability analysis also showed a higher intraclass correlation (ICC) value in the neuropathic than non-neuropathic pain group (0.85 vs0.73). The modified DN4 questionnaire could effectively discriminate neuropathic pain from non-neuropathic pain with the area under the curve (ROC) of 0.989 (95% CI: 971–1000, P < 0.001). The best cutoff value for the modified DN4 questionnaire to diagnose neuropathic pain was 5, yielding a sensitivity of 98.3% and a specificity of 90.3%. Conclusion: Applying the new modified Persian version of DN4 achieved by adding a VAS score and a new question related to the appearance of pain in an especial joint led to high reliability and validity for this tool to discriminate neuropathic from non-neuropathic pain.

Mild to moderate iodine deficiency is common among women of childbearing age. Data on iodine status in infants are sparse, partly due to the challenges in collecting urine. Urinary iodine concentration (UIC) is considered a good marker for recent dietary iodine intake and status in populations. The aim of this study was to investigate the reliability of iodine concentration measured in two spot-samples from the same day of diaper-retrieved infant urine and in their mothers’ breastmilk. We collected urine and breastmilk from a sample of 27 infants and 25 mothers participating in a cross-sectional study at two public healthcare clinics in Norway. The reliability of iodine concentration was assessed by calculating the intraclass correlation coefficients (ICC) and the coefficient of variation (CV). The ICC for infants’ urine was 0.64 (95% confidence interval (CI) 0.36–0.82), while the ICC for breastmilk was 0.83 (95% CI 0.65–0.92) Similarly, the intraindividual CV for UIC was 0.25 and 0.14 for breastmilk iodine concentration (BIC). Compared to standard methods of collecting urine for measuring iodine concentration, the diaper-pad collection method does not substantially affect the reliability of the measurements.

Abstract Background Canada legalized cannabis use for medical purposes in 1999. Legalization of cannabis for recreational purposes in October 2018 offered the opportunity to assess the impact of recreational legalization on cancer patients’ patterns of use to identify learning points that could be helpful to other countries considering similar legislation.Method Two identical anonymous cross-sectional surveys were administered to cancer patients in British Columbia (2 months before and 3 months following legalization), with the same eligibility criteria. The prevalence of medical cannabis use, the distribution of symptoms leading to use, the most common types of cannabis products and sources, reasons for stopping using cannabis, and barriers to access were assessed.Results The overall response rate was 27%. Both cohorts were similar regarding age (median= 66yrs), gender (53% female), and education (approximately 85% of participants had an education level of high school graduation and higher). Respondents had multiple motives for taking cannabis, including to manage multiple symptoms, to treat cancer, and for recreational reasons. The majority of patients in both surveys did not use the legal medical access system. Comparison of the two cohorts showed that after legalization the prevalence of current cannabis use increased by 26% (23·1% to 29·1%, p-value 0·01), including an increased disclosure of recreational motive for use, from 32% to 40%. However, in the post-legalization cohort more Current Users reported problems getting cannabis (18%) than the pre-legalization cohort (8%), (p-value <0·01). The most common barrier cited was lack of available preferred products, including edibles, as these were only available from illegal dispensaries. Conclusions Results showed that legalization of cannabis for recreational purposes may have an impact on those who use medical cannabis. Impacts include an increase in prevalence of use; problems accessing preferred products legally; higher cost, and difficulties using a legal access system. The desired goal of regulation in reducing harms from use of illegal cannabis products are unlikely to be achieved if the legal process is less attractive to patients than use of illegal sources.

Purpose: Purpose of this study is to evaluate the measuring consistency of central refraction between multispectral refraction topography (MRT) and autorefractometry.Methods: This was a descriptive cross-sectional study including subjects in Sun Yat-sen Memorial Hospital from September 1, 2020, to December 31, 2020, ages 20 to 35 years with a best corrected visual acuity of 20/20 or better. All patients underwent cycloplegia, and the refractive status was estimated with autorefractometer, experienced optometrist and MRT. We analyzed the central refraction of the autorefractometer and MRT. The repeatability and reproducibility of values measured using both devices were evaluated using intraclass correlation coefficients (ICCs).Results: A total of 145 subjects ages 20 to 35 (290 eyes) were enrolled. The mean central refraction of the autorefractometer was −4.69 ± 2.64 diopters (D) (range −9.50 to +4.75 D), while the mean central refraction of MRT was −4.49 ± 2.61 diopters (D) (range −8.79 to +5.02 D). Pearson correlation analysis revealed a high correlation between the two devices. The intraclass correlation coefficient (ICC) also showed high agreement. The intrarater and interrater ICC values of central refraction were more than 0.90 in both devices and conditions. At the same time, the mean central refraction of experienced optometrist was −4.74 ± 2.66 diopters (D) (range −9.50 to +4.75D). The intra-class correlation coefficient of central refraction measured by MRT and subjective refraction was 0.939.Conclusions: Results revealed that autorefractometry, experienced optometrist and MRT show high agreement in measuring central refraction. MRT could provide a potential objective method to assess peripheral refraction.

Background Early detection and intervention for neurodevelopmental disorders are effective. Several types of paper questionnaires have been developed to assess these conditions in early childhood; however, the psychometric equivalence between the web-based and the paper versions of these questionnaires is unknown. Objective This study examined the interformat reliability of the web-based parent-rated version of the Autism Spectrum Screening Questionnaire (ASSQ), Attention-Deficit/Hyperactivity Disorder Rating Scale (ADHD-RS), Developmental Coordination Disorder Questionnaire 2007 (DCDQ), and Strengths and Difficulties Questionnaire (SDQ) among Japanese preschoolers in a community developmental health check-up setting. Methods A set of paper-based questionnaires were distributed for voluntary completion to parents of children aged 5 years. The package of the paper format questionnaires included the ASSQ, ADHD-RS, DCDQ, parent-reported SDQ (P-SDQ), and several additional demographic questions. Responses were received from 508 parents of children who agreed to participate in the study. After 3 months, 300 parents, who were among the initial responders, were randomly selected and asked to complete the web-based versions of these questionnaires. A total of 140 parents replied to the web-based format and were included as a final sample in this study. Results We obtained the McDonald ω coefficients for both the web-based and paper formats of the ASSQ (web-based: ω=.90; paper: ω=.86), ADHD-RS total and subscales (web-based: ω=.88-.94; paper: ω=.87-.93), DCDQ total and subscales (web-based: ω=.82-.94; paper: ω=.74-.92), and P-SDQ total and subscales (web-based: ω=.55-.81; paper: ω=.52-.80). The intraclass correlation coefficients between the web-based and paper formats were all significant at the 99.9% confidence level: ASSQ (r=0.66, P<.001); ADHD-RS total and subscales (r=0.66-0.74, P<.001); DCDQ total and subscales (r=0.66-0.71, P<.001); P-SDQ Total Difficulties and subscales (r=0.55-0.73, P<.001). There were no significant differences between the web-based and paper formats for total mean score of the ASSQ (P=.76), total (P=.12) and subscale (P=.11-.47) mean scores of DCDQ, and the P-SDQ Total Difficulties mean score (P=.20) and mean subscale scores (P=.28-.79). Although significant differences were found between the web-based and paper formats for mean ADHD-RS scores (total: t132=2.83, P=.005; Inattention subscale: t133=2.15, P=.03; Hyperactivity/Impulsivity subscale: t133=3.21, P=.002), the effect sizes were small (Cohen d=0.18-0.22). Conclusions These results suggest that the web-based versions of the ASSQ, ADHD-RS, DCDQ, and P-SDQ were equivalent, with the same level of internal consistency and intrarater reliability as the paper versions, indicating the applicability of the web-based versions of these questionnaires for assessing neurodevelopmental disorders.

Hope is a crucial issue in patients with cancer. Hope can be increased by providing social support. Social support can be obtained from the nurses caring behavior. This research aimed to analyze the relationship between nurses caring behavior and recovery hope level of cancer patients undergoing chemotherapy program at Baladhika Husada Hospital in Jember. A cross-sectional study was conducted to 112 respondents using the Caring Behavior Inventory (CBI-24) questionnaire and Scale of Hope. The data were analyzed applying the Kendall Tau B (τ) correlation. The results showed that there was a relationship between nurses caring behavior with the level of recovery expectancy of cancer patients undergoing chemotherapy programs at Baladhika Husada Hospital in Jember (p-value <0.001; τ = 0.375). Nurse caring behavior increases patient comfort and makes patients more enthusiastic about undergoing therapy. Patients feel valued and get more information from nurses. Therefore, the patient's recovery hopes can increase. This study concludes that nurses caring behavior can increase the expectation of recovery of cancer patients. Abstrak Harapan adalah hal krusial pada pasien kanker. Harapan dapat ditingkatkan dengan pemberian dukungan sosial yang bisa didapat dari perilaku caring perawat. Tujuan dari penelitian ini adalah untuk menganalisis hubungan perilaku caring perawat dengan tingkat harapan sembuh pasien kanker yang menjalani program kemoterapi di Rumah Sakit Baladhika Husada Jember. Penelitian ini menggunakan metode cross sectional dan 112 responden didapatkan dengan teknik sampel purposive samping. Data didapatkan melalui kuesioner CBI-24 dan kuesioner skala harapan. Analisis pada penelitian ini menggunakan uji korelasi Kendall Tau B (τ). Hasil menunjukkan terdapat hubungan antara perilaku caring perawat dengan tingkat harapan sembuh pasien kanker yang menjalani program kemoterapi di Rumah Sakit Baladhika Husada Jember (p value < 0,001; τ = 0,375). Perilaku caring perawat dapat meningkatkan kenyamanan pasien dan membuat pasien lebih semangat menjalankan terapi yang dijalani. Pasien merasa dinilai dan mendapatkan informasi lebih dari perawat. Maka dari itu, harapan sembuh pasien dapat meningkat. Kesimpulan dari penelitian ini adalah perilaku caring perawat dapat meningkatkan harapan sembuh pasien kanker.

Abstract Background Malnutrition is common in cancer patients. The NUTRISCORE is a newly developed cancer-specific nutritional screening tool and was validated by comparison with the Patient-Generated Subjective Global Assessment (PG-SGA) and Malnutrition Screening Tool (MST) in Spain. We aimed to evaluate the performance of the NUTRISCORE, MST, and PG-SGA in estimating the risk of malnutrition in Chinese cancer patients. Methods Data from an open parallel and multicenter cross-sectional study in 29 clinical teaching hospitals in 14 Chinese cities were used. Cancer patients were assessed for malnutrition using the PG-SGA, NUTRISCORE, and MST. The sensitivity, specificity, and areas under the receiver operating characteristic curve were estimated for the NUTRISCORE and MST using the PG-SGA as a reference. Results A total of 1000 cancer patients were included. The mean age was 55.9 (19 to 92 years), and 47.5% were male. Of these patients, 450 (45.0%) had PG-SGA B and C, 29 (2.9%) had a NUTRISCORE ≥5, and 367 (36.7%) had an MST ≥ 2. Using the PG-SGA as a reference, the sensitivity, specificity, and area under the curve values of the NUTRISCORE were found to be 6.2, 99.8%, and 0.53, respectively. The sensitivity, specificity, and area under the curve values of the MST were 50.9, 74.9%, and 0.63, respectively. The kappa index between the NUTRISCORE and PG-SGA was 0.066, and that between the MST and PG-SGA was 0.262 (P < 0.05). Conclusions The NUTRISCORE had an extremely low sensitivity in cancer patients in China compared with the MST when the PG-SGA was used as a reference.

Objective We evaluated the measurement properties for item and domain scores of the Infant with Clefts Observation Outcomes Instrument (iCOO). Design Cross-sectional (before lip surgery) and longitudinal study (preoperative baseline and 2 days and 2 months after lip surgery). Setting Three academic craniofacial centers and national online advertisements. Participants Primary caregivers with an infant with cleft lip with or without cleft palate (CL  ±  P) scheduled to undergo primary lip repair. There were 133 primary caregivers at baseline, 115 at 2 days postsurgery, and 112 at 2 months postsurgery. Main Outcome Measure(s) Caregiver observation items ( n = 61) and global impression of health and function items ( n = 8) across eight health domains. Results Mean age at surgery was 6.0 months (range 2.7-11.8 months). Five of eight iCOO domains have scale scores, with Cronbach’s alphas ranging from 0.67 to 0.87. Except for the Facial Skin and Mouth domain, iCOO scales had acceptable intraclass correlation coefficients (ICCs) ranging from 0.76 to 0.84. The internal consistency of the Global Impression items across all domains was 0.90 and had acceptable ICCs (range 0.76-0.91). Sixteen out of 20 (nonscale) items had acceptable ICCs (range 0.66-0.96). As anticipated, iCOO scores 2 days postoperatively were generally lower than baseline and scores 2 months postsurgery were consistent with baseline or higher. The iCOO took approximately 10 min to complete. Conclusions The iCOO meets measurement standards and may be used for assessing the impact of cleft-related treatments in clinical research and care. More research is needed on its use in various treatment contexts.

Background: The American Orthopaedic Foot & Ankle Society (AOFAS) score is one of the most common and adapted outcome scales in hallux valgus surgery. However, AOFAS is predominantly physician based and not patient based. Although it may be straightforward to derive statistical significance, it may not equate to the true subjective benefit of the patient’s experience. There is a paucity of literature defining MCID for AOFAS in hallux valgus surgery although it could have a great impact on the accuracy of analyzing surgical outcomes. Hence, the primary aim of this study was to define the Minimal Clinically Important Difference (MCID) for the AOFAS score in these patients, and the secondary aim was to correlate patients’ demographics to the MCID. Methods: We conducted a retrospective cross-sectional study. A total of 446 patients were reviewed preoperatively and followed up for 2 years. An anchor question was asked 2 years postoperation: “How would you rate the overall results of your treatment for your foot and ankle condition?” (excellent, very good, good, fair, poor, terrible). The MCID was derived using 4 methods, 3 from an anchor-based approach and 1 from a distribution-based approach. Anchor-based approaches were (1) mean difference in 2-year AOFAS scores of patients who answered “good” versus “fair” based on the anchor question; (2) mean change of AOFAS score preoperatively and at 2-year follow-up in patients who answered good; (3) receiver operating characteristic (ROC) curves method, where the area under the curve (AUC) represented the likelihood that the scoring system would accurately discriminate these 2 groups of patients. The distribution-based approach used to calculate MCID was the effect size method. There were 405 (90.8%) females and 41 (9.2%) males. Mean age was 51.2 (standard deviation [SD] = 13) years, mean preoperative BMI was 24.2 (SD = 4.1). Results: Mean preoperative AOFAS score was 55.6 (SD = 16.8), with significant improvement to 85.7 (SD = 14.4) in 2 years ( P value < .001). There were no statistical differences between demographics or preoperative AOFAS scores of patients with good versus fair satisfaction levels. At 2 years, patients who had good satisfaction had higher AOFAS scores than fair satisfaction (83.9 vs 78.1, P < .001) and higher mean change (30.2 vs 22.3, P = .015). Mean change in AOFAS score in patients with good satisfaction was 30.2 (SD = 19.8). Mean difference in good versus fair satisfaction was 7.9. Using ROC analysis, the cut-off point is 29.0, with an area under the curve (AUC) of 0.62. Effect size method derived an MCID of 8.4 with a moderate effect size of 0.5. Multiple linear regression demonstrated increasing age (β = −0.129, CI = −0.245, –0.013, P = .030) and higher preoperative AOFAS score (β = −0.874, CI = −0.644, –0.081, P < .001) to significantly decrease the amount of change in the AOFAS score. Conclusion: The MCID of AOFAS score in hallux valgus surgery was 7.9 to 30.2. The MCID can ensure clinical improvement from a patient’s perspective and also aid in interpreting results from clinical trials and other studies. Level of Evidence: Level III, retrospective comparative series.
