Children’s Out-of-School Independently Mobile Trips, Active Travel, and Physical Activity: A Cross-Sectional Examination from the Kids in the City Study

Background:The study aim was to determine the association between children’s objectively assessed moderate-to-vigorous physical activity (MVPA) and active trips (AT) and independently mobile trips (IM) during out-of-school hours.Methods:Children aged 9 to 13 years (n = 254) were recruited from 9 schools in Auckland, New Zealand between 2011 and 2012. Children completed travel diaries and wore accelerometers for 7 days. Parents provided demographic information. Geographic information systems-derived distance to school was calculated. Accelerometer data were extracted for out of school hours only. Percentage of time spent in MVPA (%MVPA), AT, and IM were calculated. Generalized estimating equations were used to determine the relationship between daily %MVPA and AT and between daily %MVPA and IM, accounting for age, sex, ethnicity, distance to school, day of the week, and numeric day of data collection.Results:A significant positive relationship was observed between %MVPA and both AT and IM. For every unit increase in the daily percentage of trips made that were AT or IM, we found an average increase of 1.28% (95% CI 0.87%, 1.70%) and 1.15% (95% CI 0.71%, 1.59%) time in MVPA, respectively.Conclusion:Children’s AT and IM are associated with increased MVPA during out-of-school hours.

Purpose: Moderate-to-vigorous physical activity (MVPA) positively influences bone mineral content (BMC) in prepubertal children, but it is unknown whether this relationship is partially mediated by free leptin index. The aim of this study was to examine whether the relationship between MVPA and total body less head (TBLH) BMC is mediated or moderated by free leptin index in prepubertal children. Methods: We performed a cross-sectional analysis on 401 children (194 girls) from baseline examinations of the Physical Activity and Nutrition in Childhood Study. We applied the four-way decomposition mediation analysis method to assess whether free leptin index, measured from fasted blood samples, mediated the relationship between accelerometer-measured MVPA and TBLH BMC measured by dual-energy X-ray absorptiometry. Results: MVPA had a positive controlled direct effect on TBLH BMC in girls and boys (β = 0.010 to 0.011, p < 0.05). There was no mediation or interaction between MVPA, free leptin index and TBLH BMC in girls or boys (β = −0.000 to 0.001, p > 0.05). Conclusion: Our study indicates that MVPA positively influences TBLH BMC through pathways not related to free leptin index in predominantly normal-weight prepubertal children, likely primarily through mechanical loading. The relationships between MVPA, free leptin index and TBLH BMC may be influenced by other factors such as pubertal status and adiposity, so it is unknown whether these observations extend to overweight and obese children at different stages of puberty.

Background: Physical activity is associated with improved quality of life. Patients with an implantable cardioverter defibrillator (ICD) face unique clinical and psychological challenges. Factors such as fear of ICD shock may negatively impact on physical activity, while a sense of protection gained from the ICD may instil confidence to be active. Aim: We aimed to examine the impact of an ICD on physical activity levels and factors associated with amount of activity. Methods: Two cross-sectional studies were conducted. Accelerometer data (seven-day) was collected in March–November 2015 for 63 consecutively recruited hypertrophic cardiomyopathy patients, with or without an ICD, aged ⩾18 years. A survey study was conducted in July–August 2016 of 155 individuals aged ⩾18 years with an inherited heart disease and an ICD in situ. Results: Based on the International Physical Activity Questionnaire, mean leisure time physical activity was 239 ± 300 min/week with 51% meeting physical activity guidelines. Accelerometry showed that mean moderate–vigorous physical activity was the same for patients with and without an ICD (254 ± 139 min/week versus 300 ± 150 min/week, p=0.23). Nearly half of survey participants ( n=73) said their device made them more confident to exercise. Being anxious about ICD shocks was the only factor associated with not meeting physical activity guidelines. Conclusions: Patients with inherited heart disease adjust differently to their ICD device, and for many it has no impact on physical activity. Discussion regarding the appropriate level of physical activity and potential barriers will ensure best possible outcomes in this unique patient group.

Background: The relationship between moderate to vigorous physical activity (MVPA) and fundamental motor skill (FMS) is inconsistent in early childhood, due to its complex and nonlinear characteristics. This study aimed to analyze the nonlinear relationships between MVPA, FMS, body mass index (BMI), sex, and age in preschoolers. Methods: This cross-sectional study with preschoolers (n = 204; 4.0 [0.8] y old; 99 boys), provided objective physical activity data, FMS assessments, and BMI. The associations between MVPA, FMS, BMI, sex, and age were explored using the network analysis (RStudio and qgraph). Results: Boys were more motor competent than girls in all FMS skills, while girls were more active than boys during the weekend. Older children were less active than their younger peers during these days. MVPA is weak and differently related to each FMS, and the leap skill emerged with the highest betweenness and strength values in the network. Conclusions: For the assessed preschoolers, when considering BMI, age, and sex, the relationships between MVPA and FMS are inconsistent, and leap emerged as the main variable. During early childhood, these variables are connected as part of a complex system in which each skill has a dynamic role within the emerging pattern.

Background:Little is known about the relationship between children’s physical activity (PA) in preschool (in-school) and outside of preschool (out-of-school). This study described this relationship.Methods:Participants were 341 children (4.6 ± 0.3 years) in 16 preschools. Accelerometers measured moderate-to-vigorous physical activity (MVPA) and total physical activity (TPA) in-school and out-of-school. In the full sample, Pearson correlation was used to describe associations between in-school and out-of-school PA. In addition, children were categorized as meeting or not meeting a PA guideline during school. MVPA and TPA were compared between the 2 groups and in-school and out-of-school using 2-way repeated-measures analysis of variance.Results:In the full sample, in-school and out-of-school PA were positively correlated for MVPA (r = .13, P = .02) and TPA (r = .15, P = .01). Children who met the guideline in-school remained comparably active out-of-school. However, those who did not meet the guideline were more active out-of-school than in-school. The groups were active at comparable levels while out-of-school. Identical patterns were seen for MVPA and TPA.Conclusions:Children’s in-school PA was positively associated with out-of-school PA. Children who did not meet the guideline in-school were more active out-of-school than in-school, suggesting preschool and classroom factors may reduce some children’s PA in-school.

AbstractBackgroundObjective methods to measure physical activity (PA) can lead to better cross-cultural comparisons, monitoring temporal PA trends, and measuring the effect of interventions. However, when applying this technology in field-work, the accelerometer data processing is prone to methodological issues. One of the most challenging issues relates to standardizing total wear time to provide reliable data across participants. It is generally accepted that at least 4 complete days of accelerometer wear represent a week for adults. It is not known if this same assumption holds true for pregnant women.AimWe assessed the optimal number of days needed to obtain reliable estimates of overall PA and moderate-to-vigorous physical activity (MVPA) during pregnancy using a raw triaxial wrist-worn accelerometer.MethodsCross-sectional analyses were carried out in the antenatal wave of the 2015 Pelotas (Brazil) Birth Cohort Study. Participants wore the wrist ActiGraph wGT3X-BT accelerometer for seven consecutive days. The daily average acceleration, which indicates overall PA, was measured as milli-g (mg), and time spent in MVPA (minutes/day) was analyzed in 5-minute bouts. ANOVA and Kruskal-Wallis tests were used to compare variability across days of the week. Bland-Altman plots and Spearman-Brown Prophecy Formula were applied to determine the reliability coefficient associated with one to seven days of measurement. Analyses were stratified by sociodemographic factors and nutritional status.ResultsAmong 2,082 pregnant women who wore the accelerometer for seven complete days, overall and MVPA were lower on Sundays compared to other days of the week. Reliability of >=0.80 to evaluate overall PA was reached with at least three monitoring days, whereas six days were needed to estimate reliable measures of MVPA.ConclusionsOur findings indicate that the usual approach obtaining one week of accelerometry in adults is also appropriate for pregnant women, particularly to obtain differences on weekend days and reliably estimate MVPA.

Purpose: Schools provide opportunities for children to accumulate recommended daily moderate to vigorous physical activity (MVPA). This study aimed to determine physical activity (PA) across the school day among elementary school children in Qatar. Methods: This cross-sectional study recruited 181 Grade 1 (5.8 ± 0.4 years) and Grade 4 (9.0 ± 0.5 years) children. Participants wore wrist-based accelerometers for 5 days to assess school-based PA. MVPA during physical education, recess, and class time were compared across sex, grade, and body mass index using general linear models. Results: Only 39.0% of this cohort met the recommended 30 min of MVPA during school hours. Fewer girls (23.2%) than boys (58.1%) achieved the school-based PA guidelines (p < .001). The children engaged in 19.0 ± 12.3%, 19.3 ± 8.6%, and 6.2 ± 3.4% of the designated physical education, recess, and class time in MVPA, respectively. Conclusion: Elementary school children in Qatar engage in inadequate MVPA during the school day, especially girls. Enhancing PA opportunities during physical education and recess is crucial.

Abstract Background In Colombia between 1998-2011 the 23.5% of deaths that were reported were from CardioVascular Disease (CVD), 12.4% were caused by hypertensive disease; well now physical activity has been reported as a protective factor for CVD and Hypertension (HT), those people who perform high levels of physical activity have a relative decrease in developing HT by 19%, however what happens when this physical activity takes place with a duration greater than 4 hours a day, with moderate and high intensities, becoming a sports practice, the same benefits or opposite effects would be had, so the objective of the study was to determine the relationship between aerobic capacity and blood pressure in soccer players in the city of Bogotá. Methods Quantitative cross-sectional research; blood pressure of 64 players and the aerobic capacity was assessed using the Legger test (VO2 Max). Results It was evidenced that 4.6% of the players have hypotension, 50.8% normotension, 30.8% prehypertension and 12.3% stage I hypertension; As regards for VO2 max, 7.7% had an excellent capacity, 73.8% good and 12.3% favorable, there was no relationship between the variables. Conclusions Soccer players, although they have good levels of VO2 max, they do not present a decrease in blood pressure data, being mostly within a normal range. Key messages Although physical activity is a protective factor for HT. Soccer training does not show a decrease in blood pressure, possibly could be a risk factor for HT and CVD.

Metabolically unhealthy obesity (MUO) was associated with increased cardiometabolic risk. However, it is not fully understood which lifestyle factors are associated with MUO in youth. We aimed to investigate the relationship between MUO and lifestyle patterns in youth. 7-17 years old youth with overweight (N=48; 60.4% girls) and obesity (N=71; 56.3% girls) were recruited in the Pediatric Clinic, Luxembourg (cross-sectional study). Eating and sedentary habits, moderate to vigorous physical activity (MVPA) and vigorous physical activity (VPA) were assessed. 72.3% of the participants had MUO. Multivariable logistic regressions showed that MUO is most likely to be associated with moderate to low weekly levels of MVPA [OR: 2.09 (95% CI: 1.07 &ndash; 4.09; p = 0.030)] and moderate to high weekly soda and lemonade drinks consumption [OR: 2.92 (95% CI: 1.32 &ndash; 6.48; p = 0.008)]. After adjustment for age, gender and Tanner stages, MUO was most likely to be associated with moderate to high soda and lemonade drinks consumption [OR: 2.72 (95% CI: 1.21&ndash; 6.12; p = 0.016)] and with Tanner stages [OR: 2.06 (95% CI: 1.08&ndash; 3.94; p = 0.029)]. We support the promotion of MVPA and the moderation in the sugar-sweetened beverages consumption to manage cardiometabolic health in youth with obesity.

Background: School environment provides several intervention opportunities for physical activity. The aim of this study was to examine the relationship between objectively assessed moderate to vigorous physical activity (MVPA) and academic achievement in adolescents. Methods: Cross-sectional data were collected from 301 students aged between 13 and 15 years (46.51% boys), enrolled at 7 secondary schools in the city of Huesca (Spain). Participants wore accelerometers during a 7-day period, and their academic achievement was calculated from the average marks of all subjects. Structural equation modeling and quadratic regression analysis were performed to test both linear and nonlinear explanatory models. One-way analysis of variance was also performed to explore the effect of gender and the percentage of compliance with MVPA recommendations. Results: MVPA on weekdays and higher levels of body mass index were negatively associated with academic achievement. According to the curvilinear relationship, those students whose MVPA levels were closer to the daily recommendation were more likely to obtain higher academic achievement. However, a significant association was only shown in the case of boys. Conclusions: Adolescents who satisfy the international recommendations tend to obtain better academic achievement. Therefore, MVPA for adolescents should be prescribed within some beneficial time margins (50–70 min/d).

The aim of this study was to explore the relationship between insulin resistance (IR) and objectively measured habitual moderate-to-vigorous physical activity (MVPA) in First Nations youth. A cross-sectional study was conducted in 2 rural villages in northern British Columbia, Canada. Thirty-nine healthy youth (16 males and 23 females; age = 11.8yrs ± 2.2; range = 8.8–18.5yrs) participated. PA was measured with ActiGraph GT1M accelerometers. The homeostasis model assessment estimate of IR (HOMA-IR) was used to define IR. Duration of MVPA was inversely related to HOMA-IR (r=−.44, p < .01). From the regression model, 30 min of habitual MVPA corresponded to HOMA-IR levels that were 15% lower. In conclusion, these findings suggest that active First Nations youth have lower HOMA-IR values.
