Pulse-Dose Radiofrequency in Athletic Pubalgia: Preliminary Results

Context:Chronic pubalgia affects around 10% of athletes.Objective:To determine the role of pulse-dose radiofrequency (PDRF) in athletes with chronic pubalgia and investigate the causes with imaging.Design:Prospective nonrandomized single-group study.Patients:PDRF was performed on 32 patients with a chronic pain that had been refractory to conservative therapies during the last 3 mo.Intervention:The genital branches of the genitofemoral, ilioinguinal, and iliohypogastric nerves and the obturator nerve were the goals of treatment. A 10-cm, 20-gauge cannula was inserted with a percutaneous access on the upper and lower edges of the iliopubic branch. After the spindle was removed, a radiofrequency needle with a 10-mm “active tip” was inserted. The radiofrequency technique was performed with 1200 pulses at 45 V and 20-ms duration, followed by a 480-ms silent phase.Main Outcome Measures:The follow-up with a clinical examination was performed at 1, 3, 6, and 9 mo after the procedure. During the follow-up visits, the patients were asked to rate their pain on a 0–10 VAS scale.Results:All of the enrolled patients completed the study. Mean VAS score before the treatment was 8.4 ± 0.6. Twenty-four patients had a reduction of pain VAS scores more than 50% during all follow-up visits and started training and physiotherapy in the days after the radiofrequency procedure. Six patients, each treated 2 times, had a reduction more than 50% of VAS scores and could start training and physiotherapy only after the 2nd procedure. One patient had no pain relief with 2 treatments. Pain intensity decreased up to 9 mo in 31 patients (mean VAS scores 3.4 ± 0.5 at 6 mo and 3.8 ± 0.9 at 9 mo). No complications were observed.Conclusions:PDRF is an effective and safe technique in management of chronic pubalgia in athletes.

Background: The skin or the integument is the external organ that protects against mechanical trauma, UV light and infections. The prevalence of skin diseases in India is 10 to 12% of the total population with psoriasis being the major contributor, in India it ranges from 0.44% to 2.8%. In Ayurveda all skin diseases come under the concept of Kushta. Psoriasis has a high resemblance with Ekkushta, which is the type of Kshudrakushta (less severe form of the Kushta). Skin diseases considered as Pitta Doshaja and Rakta Dhatu Pradoshaja Vikara. Acharya Charaka stated that Virechana is the great treatment for Pitta predominance diseases. Aim: To study the role of Virechana Karma in the management of Ekkushta (Psoriasis). Material and Methods: A single group of 5 diagnosed patients of Ekkushta (Psoriasis) were registered at the Kayachikitsa OPD of Pt. Khushilal Sharma Govt. (Auto.) Ayurvede Hospital Bhopal. The patients were treated with Virechana Karma. Duration of study is 21 days and follow up after completion of trial every week for 1 month. Assessment was done on the basis of symptomatic relief after treatment. Observation: The overall effect of the Virechana Karma in all patients suggested that, complete cure was not in any patient. One patient was mild improved, one patient was markedly improved and three patients were moderately improved. None of patients remained unchanged. Conclusion On the basis of this clinical trial it can be concluded that Virechana Karma works effectively in the management of psoriasis.

Background: Bronchial Asthma is a disease of airways that is characterized by hyper-responsiveness of the tracheobronchial tree to a variety of stimuli resulting in widespread spasmodic narrowing of the airway. According to WHO 2016, Asthma affects 235 million people worldwide, out of which 15-20 million people are from India. In India, the prevalence of self-reported asthma is 2% among women aged 15-49 years and 1% among young women aged 15-19 years as well as men aged 15-49 years as per the latest report. Prevalence of asthma is more in urban areas than rural area as due to smoke, pollution and environmental factor. In Ayurveda Bronchial Asthma has a high resemblance with Tamaka Shwasa which comes under Pranavaha Srotas. It is described in Brihatrayee as well as Laghutrayee. Acharya Charak and Sushruta has given detailed description of Tamaka Shwasa. Aim and Objectives: To study the effect of shwasa kuthar rasa in the management of (bronchial asthma) and to find out an effective Ayurvedic medicine for Tamaka Shwasa (Bronchial Asthma). Material and Methods: For this open clinical study, 10 patients of Tamak Shwasa (bronchial asthma) were registered from OPD of Kayachikitsa department and admitted in IPD of Pt. Khushilal Govt (Auto.) Ayurveda Hospital Bhopal. The patient was treated with Shwasa Kuthar Rasa. Duration of study was 30 days and follow up was done after completion of trial every week for 1 month, assessment was done on the basis of symptomatic relief and increase the range of FEV1 and FVC value after treatment. Observation: yielded symptomatic relief after treatment. The overall effect of the treatment in patient suggested that, there is 87.50 % relief in cough (Kasa), 90.01% relief in dyspnoea (Shwasakashta), 93.37% relief in chest tightness (Urashool) and 74.99% relief in wheezing (Ghurghurak) and value of FEV1 and FVC were increased up to 22.29% & 24.43% after treatment. Conclusion: On the basis of result obtained, it can be concluded that Shwasa Kuthar Rasa can be used as an effective medicine in the management of (Bronchial asthma). Objective: To evaluate the results of arthroscopic treatment of various subtalar joint pathologies. Methods: Retrospective study of patients undergoing arthroscopy of the subtalar joint from 2005 to 2013, totaling 10 cases over a mean follow-up of 27.1 months, in which the American Orthopedic Foot and Ankle Society (AOFAS) scale and pain visual analogue scale (VAS) scores before and after surgery were compared. Results: The preoperative AOFAS scores ranged from 35 to 74, with a mean score of 50.1 points, and the postoperative scores ranged from 82 to 100 points, with a mean score of 90.8 points. When comparing the scores, we observed an average gain of 40.1 points. The mean VAS score for the initial pain assessment was 6.5 points, and the mean postoperative score was 1.4 points. Conclusion: Arthroscopic treatment of the reported subtalar pathologies led to encouraging results, with a significant reduction of pain and improvement of functional status. Objective: To assess the long-term outcome of sclerotherapy. Design: Single group study of patients with incompetence of the long saphenous vein. Setting: All treatments were performed in a private office setting. Patients: Sixteen patients with untreated varicosities limited to the greater saphenous vein, which was 10 mm in diameter at the saphenofemoral junction in all cases. Interventions: Angioscopically guided sclerosis of the long saphenous vein. Main outcome measures: Colour-flow ultrasound was used to establish the presence of residual reflux after angioscopic treatment. Results: Total obliteration of saphenofemoral reflux was obtained in 12 veins with angioscopic sclerotherapy. All veins on which follow-up was available (nine veins in seven patients) had undergone substantial recanalization by 12 months after treatment. Conclusion: Angioscopically controlled sclerotherapy effectively obliterates the saphenophemoral junction. Recanalization of the sclerosed segment of vein occurs invariably within 12 months of treatment.

Objective: To study the factors that influence the use of opioids in the management of chronic noncancer pain (CNCP) by primary care providers (PCPs) for patients returning from a pain specialist.Design: A survey of PCPs.Setting: Two physician groups in the Minneapolis-St. Paul metropolitan area.Participants: Two seventy-six PCPs surveyed and 80 surveys returned. Main outcome measures: Participants rated the importance of specific concerns regarding the role of pain specialists and the use of opioids in the management of CNCP. Past experience with pain specialists, comfort using opioids, and opinions regarding a trilateral opioid agreement were also examined.Results: The top concerns for PCPs were as follows: the use of opioids in patients with chemical dependency or psychological issues, the escalation of opioid dosing, and the use of opioids in pain states without objective findings. They also ranked highly the importance of coordinating the return of patients from a pain specialist with explicit opioid instructions and the availability of consultation by phone or a timely follow-up visit. PCPs were supportive of the concept of a trilateral opioid agreement.Conclusions: PCPs have significant concerns regarding the prescribing of opioids in CNCP. They desire closer collaboration with pain specialists, including more explicit plans of care when patients are transferred back to them. The trilateral agreement may provide one framework for better collaboration.

Abstract. Background: Little is known about the role of resilience in the likelihood of suicidal ideation (SI) over time. Aims: We examined the association between resilience and SI in a young-adult cohort over 4 years. Our objectives were to determine whether resilience was associated with SI at follow-up or, conversely, whether SI was associated with lowered resilience at follow-up. Method: Participants were selected from the Personality and Total Health (PATH) Through Life Project from Canberra and Queanbeyan, Australia, aged 28–32 years at the first time point and 32–36 at the second. Multinomial, linear, and binary regression analyses explored the association between resilience and SI over two time points. Models were adjusted for suicidality risk factors. Results: While unadjusted analyses identified associations between resilience and SI, these effects were fully explained by the inclusion of other suicidality risk factors. Conclusion: Despite strong cross-sectional associations, resilience and SI appear to be unrelated in a longitudinal context, once risk/resilience factors are controlled for. As independent indicators of psychological well-being, suicidality and resilience are essential if current status is to be captured. However, the addition of other factors (e.g., support, mastery) makes this association tenuous. Consequently, resilience per se may not be protective of SI.

The study dealt with tax evasion in the medical profession, where the problem was the existence of many cases of tax evasion, especially tax evasion in the income tax of medical professions. The aim of the study is to try to shed light on the phenomenon of tax evasion and the role of the tax authority in the development of controls and means that reduce the phenomenon of tax evasion. The most important results of the low level of tax awareness and lack of knowledge of the tax law and the unwillingness to read it and the sense of taxpayers unfairness of the tax all lead to an increase in cases of tax evasion and in suggested tightening control and follow-up on the offices of auditors, through the investigation and auditing The reports of certified accountants and the use of computers for this purpose in order to raise the degree of confidence in these reports and bring them closer to the required truth and coordination and cooperation with the Union of Accountants and Auditors and inform them about each case of violations of the auditors and accountants N because of its great influence in the rejection of the organization of the accounts and not to ratify fake accounts lead to show taxpayers accounts on a non-truth in order to tax evasion.

This study aims to determine the role of the Ombudsman of the Republic of Indonesia in North Sumatra Representative in the Supervision of Population and Civil Registry Service in Medan City. This study uses a qualitative approach with descriptive methods describing information about the data obtained from the field in the form of written and oral data from the parties studied. Data is collected based on interviews and documentation. The results of this study indicate that the role of the Ombudsman of the Republic of Indonesia in North Sumatra was carried out in the form of external supervision. External supervision is supervision carried out by the Ombudsman of the Republic of Indonesia Representative of North Sumatra to the Medan Population and Civil Registry Service. Actions taken in the supervision process are incoming reports, follow-up of the first report and follow-up of the report. Based on the research that has been carried out, it has been concluded that the role of the Ombudsman of the Republic of Indonesia in the North Sumatra Representative in supervising the service provider of the Population and Civil Registry services is carried out in the form of external supervision. In supervising the handling of public reports of alleged poor service in the area of population administration, it has been effective, with several efforts to handle reports such as: (1) Clarification; (2) Investigation; (3) Recommendations; (4) Monitoring.
