Bouts of Vigorous Physical Activity and Bone Strength Accrual During Adolescence

Purpose:We examined the influence of vigorous physical activity (VPA) bout frequency on bone strength accrual across adolescence, independent of total volume of VPA.Methods:We measured VPA (6 metabolic equivalents; total volume and bout frequency <5 min in duration) annually using waist-worn accelerometers (ActiGraph GT1M) in 309 adolescents (9–20 y at baseline: 99, <13 y; 126, 13–18 y; 84, >18 y) over a maximum of 4 years. We applied finite element analysis to high-resolution peripheral quantitative computed tomography scans of the distal tibia (8% site) to estimate bone strength (failure load; F.Load, Newtons). We fit a mixed effects model with maturity offset (years from age at peak height velocity) as a random effect and sex, ethnicity, tibia length, lean body mass, and VPA (volume and bout frequency) as fixed effects.Results:VPA volume and bout frequency were positively associated with F.Load across adolescence; however, VPA volume did not predict F.Load once VPA bout frequency was included in the model. Participants in the upper quartile of VPA bout frequency (∼33 bouts per day) had 10% (500 N) greater F.Load across adolescence compared with participants in the lowest quartile (∼9 bouts per day;P = .012). Each additional daily bout of VPA was associated with 21 N greater F.Load, independent of total volume of VPA.Conclusion:Frequent VPA should be promoted for optimal bone strength accrual.

The objectives of this study were (i) to assess whether daily minutes of moderate to vigorous physical activity (MVPA) or vigorous physical activity (VPA) and impact counts (acceleration peaks ≥3.9g) independently predict variance in bone strength in children and youth and (ii) to estimate bone strength gain associated with increases in daily MVPA, VPA, or impact counts. We recorded 7-day activity of 49 participants (mean age 11.0 years, SD 1.7) using accelerometers and estimated radius and tibia bone strength using peripheral quantitative computed tomography. We used linear regression models adjusted for sex, body mass, and muscle area to address our objectives. Daily MVPA (mean 50 min, SD 23) and VPA (mean 17 min, SD 11) or impacts (mean 71 counts, SD 59) did not predict variance in radius strength. Daily VPA (β = 0.24) predicted variance in tibia strength at the distal and shaft sites, and shaft strength was also predicted by MVPA (β = 0.20) and impact counts (β = 0.21). Our models estimated a 3%–6%, 4%, or 4%–11% gain in tibia strength after increasing daily MVPA by 10–20 min, VPA by 5 min, or impacts by 30–100 counts, respectively. In conclusion, daily minutes of MVPA or VPA and impact counts are independent predictors of tibia but not radius strength. Objective recording of activities associated with forearm bone strength and trials testing the efficacy of increasing daily MVPA, VPA, and related impacts on bone strength development in children and youth are warranted.

Abstract Background Physical activity, a high-frequency health behavior, varies by where children live, learn, and play. Children accumulate physical activity in adult-led in-school and out-of-school settings. Youth sport is a potential setting for physical activity, but there are differences in youth sport participation based on age, sex, and socioeconomic status. There is a gap in understanding demographic influences on youth sport participation and how these factors interact to influence physical activity. This study examines influences of grade, sex, and family income on youth sport participation and these factors and youth sport participation on moderate-to-vigorous physical activity of children in rural communities. Methods Children (n = 418 3rd–6th graders) living in two rural communities completed the online Youth Activity Profile as part of Wellscapes, a type 3 hybrid implementation-effectiveness community randomized trial. Mixed models with community as a random effect examined main effects and interactions of grade, sex, and family income on youth sport participation and these factors and youth sport participation on moderate-to-vigorous physical activity. Results About 80% of children engaged in youth sport, and full-pay lunch students were almost four times more likely to have youth sport participation than students with free/reduced lunch (OR = 3.91, 95% CI = 1.95, 7.8). Females and 6th graders (p < 0.05) had lower physical activity than comparison groups. Males with higher family income had greater physical activity; females with higher family income had less physical activity. For 6th graders, high family income had less effect on physical activity than similar 3rd-5th graders (p < 0.01). Conclusions While a fairly high percentage of children participate in youth sports, there are disparities in rural communities on youth sport participation and physical activity outcomes based on age, sex, and family income.

Background Light physical activity is known to reduce atrial fibrillation risk, whereas moderate to vigorous physical activity may result in an increased risk. However, the question of what volume of physical activity can be considered beneficial remains poorly understood. The scope of the present work was to examine the relation between physical activity volume and atrial fibrillation risk. Design A comprehensive systematic review was performed following the PRISMA guidelines. Methods A non-linear meta-regression considering the amount of energy spent in physical activity was carried out. The first derivative of the non-linear relation between physical activity and atrial fibrillation risk was evaluated to determine the volume of physical activity that carried the minimum atrial fibrillation risk. Results The dose–response analysis of the relation between physical activity and atrial fibrillation risk showed that physical activity at volumes of 5–20 metabolic equivalents per week (MET-h/week) was associated with significant reduction in atrial fibrillation risk (relative risk for 19 MET-h/week = 0.92 (0.87, 0.98). By comparison, physical activity volumes exceeding 20 MET-h/week were unrelated to atrial fibrillation risk (relative risk for 21 MET-h/week = 0.95 (0.88, 1.02). Conclusion These data show a J-shaped relation between physical activity volume and atrial fibrillation risk. Physical activity at volumes of up to 20 MET-h/week is associated with reduced atrial fibrillation risk, whereas volumes exceeding 20 MET-h/week show no relation with risk.

AbstractObjectiveTo evaluate habitual levels of physical activity in a nationally representative sample of adults in Ireland.DesignCross-sectional survey using a self-administered questionnaire. Usual levels of work, recreational and household activities were evaluated in relation to anthropometric, demographic and socio-economic characteristics. The amount and intensity of all activities were quantified by assigning metabolic equivalents (METS) to each activity.SettingRepublic of Ireland and Northern Ireland, 1997–1999.SubjectsRandom sample of 1379 adults aged 18–64 years.ResultsMen were approximately twice as active in work and recreational activity (139.7 ± 83.9 METS) as women (68.5 ± 49.8 METS; P < 0.001), but women were three times more active in household tasks (65.9 ± 58.7 METS vs. 22.6 ± 24.6 METS; P < 0.001). Overall levels of physical activity declined with increasing age, particularly leisure activity in men. In women the decline in work activity was offset by spending more time in household pursuits. Twenty-five per cent of the subjects were extremely overweight (body mass index (BMI) > 28kg m−2) or obese (BMI > 30kg m−2). Fewer obese subjects reported higher levels of work and leisure activities. However, a higher percentage of obese women reported participation in the higher levels of household activities. Participation rates in recreational activities were low. Walking was the most important leisure activity of both men (41%) and women (60%). In terms of hours per week spent in vigorous physical activity, men were more active than women, professional and skilled non-manual women were more active than women in other social classes, and younger subjects (aged 18–35 years) were more active than older subjects.ConclusionsThe holistic approach used in the assessment of physical activity in this study has revealed important and subtle differences in the activity patterns of men and women. Failure to fully characterise the respective activity patterns of men and women could lead to ill-informed public health policy aimed at promoting and sustaining lifetime habits of physical activity. The results suggest that simple population-focused programmes to promote physical activity are unlikely to offer the same chance of long-term success as more sensitive and individualised strategies.

Background:This study compared physical activity levels among early, average, and late maturing boys and girls.Methods:Physical activity was assessed with an Actigraph accelerometer in 161 (76 boys, 85 girls) 9 to 14 year olds over 7 consecutive days. Anthropometric variables were measured and the maturity offset (ie, years from peak height velocity) was predicted. Biological maturity groups (early, average, and late) were created based on the mean estimated age at peak height velocity for boys and girls separately.Results:Levels of moderate-to-vigorous physical activity (MVPA) were similar between early, average, and late maturing boys and girls after adjusting for differences in chronological age. Levels of MVPA progressively declined across chronological age in boys and girls (P < .001) and gender differences existed at 10-, 12-, and 13-years, with boys having higher levels than girls (P < .05). When aligned according to biological age, gender-related differences in MVPA did not exist.Conclusions:Within this sample of 9 to 14 year old boys and girls, there were no significant differences in MVPA among early, average, and late maturing individuals.

This study evaluated the effects of exergaming on college students’ energy expenditure (EE), moderate-to-vigorous physical activity (MVPA), light physical activity (LPA), rating of perceived exertion (RPE), and enjoyment compared to traditional treadmill exercise, and sex differences. Sixty college students (30 female; X ¯ age = 23.6 ± 4.1 years) completed three 20-min exercise sessions on Xbox 360 Kinect Just Dance (Microsoft, Redmond, WA, USA), Xbox 360 Kinect Reflex Ridge (Microsoft, Redmond, WA, USA), and treadmill walking. Their EE and PA were assessed by ActiGraph accelerometers (ActiGraph Co.; Pensacola, FL, USA); RPE every four min; enjoyment via an established scale. Significant exercise-type by sex interaction effects were observed for RPE (p < 0.01): females reported significantly lower RPE during exergaming sessions but significantly higher RPE during treadmill walking. Results revealed significant main effects for all outcomes between exercise sessions (all p < 0.01): treadmill walking resulted in significantly higher metabolic equivalents (METs), MVPA, and EE (p < 0 .01), yet lower LPA (p < 0.01), compared to the two exergaming sessions. Participants’ RPE was significantly higher during treadmill walking than during exergaming sessions, with exergaming eliciting significantly higher enjoyment (all p < 0.01). College students find exergaming more enjoyable and report lower RPE compared to traditional treadmill exercise, though not yet matching the moderate physiological intensity level.

ObjectivesWe compared the ability of physical activity and sitting time questionnaires (PAQ) for ranking individuals versus continuous volume calculations (physical activity level (PAL), metabolic equivalents of task (MET), sitting hours) against accelerometry measured physical activity as our criterion.MethodsParticipants in a cohort from the Tromsø Study completed three questionnaires; (1) The Saltin-Grimby Physical Activity Level Scale (SGPALS) (n=4040); (2) The Physical Activity Frequency, Intensity and Duration (PAFID) questionnaire (n=5902)) calculated as MET-hours·week-1 and (3) The International Physical Activity questionnaire (IPAQ) short-form sitting question (n=4896). We validated the questionnaires against the following accelerometry (Actigraph wGT3X-BT) estimates: vector magnitude counts per minute, steps∙day-1, time (minutes·day-1) in sedentary behaviour, light physical activity, moderate and vigorous physical activity (MVPA) non-bouted and ≥10 min bouted MVPA.ResultsRanking of physical activity according to the SGPALS and quartiles (Q) of MET-hours∙week-1 from the PAFID were both positively associated with accelerometry estimates of physical activity (p<0.001) but correlations with accelerometry estimates were weak (SGPALS (PAL): r=0.11 to 0.26, p<0.001) and weak-to-moderate (PAFID: r=0.39 to 0.44, p<0.01). There was 1 hour of accelerometry measured sedentary time from Q1 to Q4 in the IPAQ sitting question (p<0.001) and also weak correlations (r=0.22, p<0.01).ConclusionRanking of physical activity levels measured with PAQs appears to have higher validity than energy expenditure calculations. Self-reported sedentary time poorly reflects accelerometry measured sedentary time. These two PAQs can be used for ranking individuals into different physical activity categories supporting previous studies using these instruments when assessing associations with health outcomes.

This study investigated the ability of published cut points for the RT3 accelerometer to differentiate between levels of physical activity intensity in children with cerebral palsy (CP). Oxygen consumption (metabolic equivalents; METs) and RT3 data (counts/min) were measured during rest and 5 walking trials. METs and corresponding counts/min were classified as sedentary, light physical activity (LPA), and moderate to vigorous physical activity (MVPA) according to MET thresholds. Counts were also classified according to published cut points. A published cut point exhibited an excellent ability to classify sedentary activity (sensitivity = 89.5%, specificity = 100.0%). Classification accuracy decreased when published cut points were used to classify LPA (sensitivity = 88.9%, specificity = 79.6%) and MVPA (sensitivity = 70%, specificity = 95–97%). Derivation of a new cut point improved classification of both LPA and MVPA. Applying published cut points to RT3 accelerometer data collected in children with CP may result in misclassification of LPA and MVPA.
