Background:Methotrexate (MTX) represents theanchor drugfor the treatment of rheumatoid arthritis (RA), as well as other rheumatological diseases such as psoriatic arthritis (PsA) and spondyloarthritides (SpA). Despite consolidated clinical efficacy, the use of MTX suffers from relevant limitations. Common issues include subjective intolerance, nausea, malaise, and fatigue, which negatively impact on quality of life and work/social participation.Objectives:In this study, we evaluated the frequency with which patients on MTX therapy opt for self-administration over the weekend or in the evening hours, in order to minimize interference with daily activities, work productivity, and social participation.Methods:A cross-sectional, prospective study was performed in two tertiary referral Rheumatology clinics, which included consecutive patients with RA, PsA, or SpA on MTX therapy. Enrolled patients had not previously received instructions by their healthcare provider as to when during the week or day MTX ought to be administered, and were free to choose or change the weekday and time for self-administration. Data on the route and timing of MTX self-administration was collected using dedicated questionnaires, which included queries on the dose and route of administration, day of the week and time of self-administration, reasons for the patient’s choice, use of folic acid supplementation, and concomitant therapies. Statistical analyses were conducted using a chi-square test; a p-value <0.05 was considered significant.Results:A total of 275 consecutive patients treated with MTX were included, mostly with RA (86%). Patients had an average age of 59.8 years (SD 14.0) and an average disease duration of 134.5 months (SD 127.3). The average MTX dose was 15 mg/wk (SD 4.4); MTX was administered subcutaneously in 68.2% of cases, orally in 17.8%, and intramuscularly in 5%. Regarding the timing of MTX self-administration, 157 patients (57.1%) took MTX in the evening and 119 patients (44.3%) took it during the weekend; even among patients taking MTX in the evening, weekend administration was preferred (93/157, 59.2%, p <0.001). The most frequent reasons leading to MTX self-administration during the evening or weekend were gastrointestinal side effects/nausea (85% of cases), fatigue (63%), interference with work activities (36%) and interference with social activities (29%). Patients who opted for MTX self-administration in the evening or over the weekend were significantly younger (p <0.001), with no significant gender differences or disease duration.Conclusion:The majority of patients with inflammatory arthritis opt for self-administration of MTX in the evening (absolute majority) or during the weekend (relative majority). This choice is dictated by the need to avoid side effects and detrimental repercussions on the individual’s social or working life. Background: Emotional stress is frequently associated with otologic symptoms as tinnitus and dizziness. Stress can contribute to the beginning or worsening of tinnitus. Objective: The objective of the study is to evaluate the presence of stress symptoms in patients with chronic, subjective tinnitus, and correlate its presence to annoyance associated with tinnitus. Methods: This is a cross-sectional study. One hundred and eighty patients with chronic, subjective tinnitus were included. Patients answered the Tinnitus Handicap Inventory (THI) to evaluate the impact of tinnitus in the quality of life and answered the Lipp's inventory symptoms of stress for adults (ISSL). The data obtained was organized using Excel® 2010, mean values, linear regression and p-value were calculated. Results: Of the 180 patients included in the study, 117 (65%) had stress symptoms, 52 of the 117 (44%) were in the resistance phase and 23 of the 117 (20%) in the exhaustion phase, the remaining was in the alert phase. There was a clear progressive increase in stress as THI raised, with more impact of tinnitus in quality of life. Conclusion: The presence of stress symptoms, measured by ISSL was observed in most of our patients with chronic subjective tinnitus, specially in the resistance and exhaustion phases and it is directly associated with tinnitus annoyance.

Diabetes is a common chronic disease that is considered as one of the fastest-growing health problems in the world. Adherence to medications could be an important factor in reducing these complications and improving the quality of life. The purpose of this research was to assess the impact of treatment adherence on health-related quality of life in patients with type 2 diabetes. A multicenter cross-sectional study was carried out among 368 diabetes patients. General Medication Adherence Scale was used to assess the adherence level and EuroQol-5D to assess the quality of life. The results show that 19%, 21%, and 23% of patients had maintained low medication adherence due to patient’s intentional or unintentional behavior due to additional diseases or pills burden and due to financial constraints, respectively. Overall, 43% (n=162) participants had maintained high medication adherence, and 37% (n=138) had maintained low medication adherence to antidiabetic drugs. Nearly one-third (31%) of patients with high overall adherence had perfect health state in comparison with 4% among patients with low adherence. Further, the lower proportion (21%) of patients with high overall adherence had perfect health state in comparison with that among patients with low adherence (34%). In addition to the overall adherence, the association was statistically significant for the domains related to non-adherence due to the patient’s intentional or unintentional behavior (p-value 0.001) and non-adherence due to additional diseases or pills burden (p-value 0.001) after taking into account of socio-demographic and clinical characteristics. In conclusion, the findings suggest that the policymakers should establish an intervention to improve adherence to diabetic treatment, and thus improve the quality of life for the type 2 diabetic patients.

IL-5 is an important role cytokine on the RA. IL-5 has an important role on eosinophils. ARIA-WHO made classification of RA based on how long the clinical symptoms and the impact on quality of life. The aim of this study was to know the classification of RA with IL-5 on RA study. This study used a cross-sectional method with 39 samples. The examination of IL-5 used ELISA. The highest classification of RA was medium-severe persistent of 43.58% with the mean value IL-5 was 56.25 pg/ml. Based on the test of Kruskal Wallis, it was obtained p-value = 0.664. Conclusion: There was no significant relation between classification RA and IL-5.

Abstract The aim of this study was to evaluate the impact of malocclusion severity on the oral health-related quality of life (OHRQoL) of non-white adolescents. A cross-sectional study was conducted with 585 non-white Brazilian adolescents (12-15 years). The Dental Aesthetic Index (DAI) was used for the clinical assessment of malocclusion and Oral Health Impact Profile (OHIP-14) on OHRQoL. Deep bite and transverse occlusal relationships were assessed in association with the DAI. The adolescents with DAI 3 and 4 were distributed into 4 groups: G1 - individuals without transverse occlusal relationships or deep bite; G2 - individuals with only posterior crossbite; G3 - individuals with only deep bite; and G4 - individuals with Brodie bite. The backward stepwise procedure was used to select variables on each level, eliminating variables with a p≤0.20. From the logistic regression analyses, the adjusted odds ratios were estimated with the respective 95% confidence intervals. The adolescents with severe malocclusion divided into the G2, G3 and G4 showed p-value of 0.0501, 0.1475, and 0.5407, respectively, but did not remain in the final model. Malocclusion severity had no impact on the OHRQoL of non-white adolescents.

Background: A heart attack is one of the most common cardiovascular diseases, affecting different dimensions of the patients’ quality of life due to the disease’s disabling and limiting nature. Therefore, this study aimed to examine the impact of myocardial infarction on patients’ quality of life referring to the Shahid Madani Medical & Training Hospital, in East Azerbaijan Province in 2017, in Iran. Methods: This was a cross-sectional study. A total of 220 patients with a heart attack were selected in this study. Sampling was conducted using the systematic random method. Data were collected using the Minnesota Living with Heart Failure Questionnaire (MLHFQ). Descriptive statistics (frequency, mean) and statistical analyses (Mann-Whitney test, Spearman correlation, and Generalized Linear Models Regression) were performed at a significance level of P-value < 0.05 using SPSS22. Results: In this study, the mean score of the total effect of a heart attack on the patients’ quality of life was 30.09 ± 17.73. The mean score of the effect of a heart attack on the patient’s quality of life in the physical, emotional, and general subscales was 34.85 ± 24.24, 26.63 ± 27.73, and 27.51 ± 17.83, respectively. There was a statistically significant association between education status (P-value = 0.006), income status (P- value = 0.000), and the mean score of the effect of a heart attack on the patient’s quality of life; also a positive and significant correlation was found between age (CC = 0.135, P-value = 0.046), the number of hospitalization (CC = 0.187, P-value = 0.006) and the mean score of the effect of a heart attack on the patient’s quality of life. Conclusion: A heart attack has a considerable impact on the patients’ quality of life, particularly on the physical dimension. Hence, health professionals should pay more attention to these vulnerable groups and offer rehabilitation services suitable to these groups to reduce the impact of the disease.

Rheumatoid arthritis is a autoimmune that causes a long period of inflammation in the joints. This might cause a deprcrease in the quality of life of a person resulting in limited activity and depression. The purpose of this study was to determine the relationship between self management and the quality of life of patients with rheumatoid arthritis . the research method uses quantitative research by using descriptive correlation research methods. Then this research uses a Cross sectional approach design research method and correlation design with a sampling total sampling, and the number of sample in this study was 25 respondents. The measuring instrument used in this research in a questionnaire. Data analysis was carried out using the chi-square tets, the result of this study showed that 18 people (51%) had a good self management and 17 people  (49%) had a good quality of life. This show that there is a relationship between self managemnet and quality of life of rheumatoid arthritis patient at Grandmed Lubuk Pakam hospital in 2021 with the result of the chi-square test (p Value 0,001). Therefore, it is hoped that patients will be able to further improve self management so that the quality of life is getting better.

ABSTRACT : Background : Hemodialysis is a process in which blood is separated from harmful substances or poisons and excess fluid is then discharged through a dialysis device. Quality of life is a quality that is felt in everyday life one of which is the physical dimension, namely the impact of anemia experienced by patients. Purpose : to identify the relationship between anemia and quality of life in patients with chronic kidney failure who underwent hemodialysis for less than 6 months. Method: quantitative research design with analytic survey through a cross sectional approach. The population in this study were all patients with chronic kidney failure who did hemodialysis at the Rasyida Kidney Hospital in Medan. “The sample in this study amounted to 40 respondents with purposive sampling. Researchers requested laboratory results for the patient's hemoglobin level in the patient's medical record, and then the patient filled out a WHOQOL quality of life questionnaire”. Results: Patients with chronic kidney failure who underwent hemodialysis <6 months, the majority were male 26 people (65.0%), aged> 50 years (50.0%), married 38 people (95.0%), high school education was 17 people (42.5%), and had 14 self-employed jobs (35.0%). The majority experienced not anemia as many as 16 people (40,0%) and the majority experienced poor quality of life as many as 22 people (55,0%). Kualitas hidup merupakan kualitas “yang dirasakan dalam kehidupan sehari-hari salah satunya adalah dimensi fisik, yaitu dampak anemia yang dialami pasien”.Tujuan : Penelitian untuk mengidentifikasi hubungan antara anemia dan kualitas hidup pada pasien yang gagal ginjal kronik yang menjalani hemodialisa kurang dari 6 bulan.Metode : Penelitian ini menggunakan kuantitatif dengan survey analitik yang melalui pendekatan cross sectional. Populasi dalam penelitian ini yakni seluruh pasien gagal ginjal kronik yang melakukan hemodialisis di Rumah Sakit Khusus Ginjal Rasyida Medan “Sampel dalam penelitian ini berjumlah 40 responden  dengan purposive sampling”. Peneliti meminta hasil laboratorium kadar hemoglobin pasien pada rekam medis pasien, dan kemudian pasien mengisi kuisioner kualitas hidup WHOQOL. Hasil : Pasien gagal ginjal kronik yang menjalani hemodialisa < 6 bulan mayoritas berjenis kelamin laki-laki 26 orang (65,0%), berusia >50 Tahun (50,0%), berstatus sudah menikah 38 orang (95,0%), pendidikan terakhir SMA 17 orang (42,5 %), dan memiliki pekerjaan wiraswasta 14 orang (35,0%). Mayoritas tidak mengalami anemia 16 orang (40,0%), anemia ringan 13 orang (32,5%), anemia sedang 11 orang (27,5%) dan mayoritas mengalami kualitas hidup buruk sebanyak 22 orang (55,0%). Hasil uji bivariate Chi Square terdapat “hubungan antara anemia dan kualitas hidup pada pasien gagal ginjal kronik yang menjalani hemodialisa kurang dari 6 bulan dengan nilai p-value 0,002”. Kata Kunci : Anemia, Kualitas Hidup, Gagal Ginjal Kronik, Hemodialisa

Type 1 diabetes is a disease that can’t be cured but the quality of life of the patients can be maintained as much as possible. This study aimed to analyze the quality of life of adolescents with type 1 diabetes in Malang.  This study used cross sectional design. The sampling technique used total sampling by taking all adolescents (10-19 years) with type 1 diabetes who are still active in IKADAR Malang which were 24 adolescents. The quality of life of adolescents with type 1 diabetes was measured by the quality of life for youth questionnaire.  The result showed that the total score of the quality of life of adolescents with type 1 diabetes was 74.4±11.4 with the highest score was the impact on activities (92.3±12.4) and the lowest score was the parent issues (57.3±29.2). Higher score quality of life was in boys (75.5±12.9), age 10-14 years (75.3±11.7), disease duration  1-5 years (83.0±3.5) and last HbA1c was <7.5% (83.3±4.2). Healthcare providers especially nurse are expected to keep monitoring and improving the quality of life of adolescents with type 1 diabetes.

Introduction: Strabismus is known to affect health related quality of life in adults. This study was conducted to compare the quality of life among strabismic and non strabismic visually normal adults.Methods: A prospective cross sectional study was conducted at Nepal Eye Hospital for two years. Fifty three adults with strabismus and fifty three adults without strabismus and normal vision were enrolled. Twenty open ended adult strabismus questionnaire (AS-20) developed by Hatt et al were used to assess the impact of strabismus on patient’s health related quality of life. All the participants reviewed AS-20 instructions for patients prior to completing the questionnaire. The overall AS-20 score was calculated as a mean of all 20 item scores. The psychosocial subscale was calculated as a mean of items 1-10 (inclusive). Function subscale was calculated as a mean of items 11-20 (inclusive).The mean overall and subscale scores were compared between strabismic patients and non-strabismic visually normal adults using Mann Whitney U test.Results: Mean overall scores decreased for patients with strabismus as compared to non strabismic visually normal adults. Mean psychosocial scale among strabismic patients and non strabismic patients was 47.53 ± 24.840 and 95.42 ± 9.631 respectively and mean function subscale among the two groups were 63.57±22.652 and 83.91±13.592 respectively. There was statistical difference in the overall score, psychosocial as well as function subscale between the strabismic and non strabismic visually normal adults (p value 0.00).Conclusion: The overall quality of life appears to be adversely affected in people with strabismus in comparison to the ones without strabismus. Strabismus, being a treatable condition, should not be ignored and corrective measures should be undertaken.
