Haemophagocytic lymphohistiocytosis (HLH) is an immune dysregulation disorder with variable presentations and non-specific features making it extremely difficult to diagnose early in the clinical course. Here, we are presenting a case of a young man who presented in cardiogenic shock with findings of anterolateral wall ischaemia on ECG. Echocardiography findings were consistent with takotsubo cardiomyopathy (TCM). Cardiac catheterisation showed clean coronary arteries and pulmonary artery pressure measurements showed high output cardiac failure. After extensive workup, the patient was diagnosed with HLH. In spite of aggressive supportive and definitive therapy, he eventually died due to a complicated clinical course. We did a comprehensive literature review and found that this is the first reported case of HLH presenting as TCM as the initial clinical manifestation.

ABSTRACT Introduction: Immune-mediated peripheral neuropathy is the term applied to a spectrum of peripheral nerve disorders where immune dysregulation plays a role. Therefore, they are treatable. We analyzed the cases seen in the past 3 years by us and evaluated the clinical, laboratory, and outcome parameters in these patients. Patients and Methods: Consecutive patients seen by the authors and diagnosed as immune-mediated neuropathy were analyzed for etiology, pathology, and outcome assessed. Results: A total of sixty patients, 31 acute and 29 chronic neuropathies, were identified. Their subtypes treatment and outcome assessed. Males were significantly more in both acute and chronic cases. Miller Fisher 4, AMAN 1, paraplegic type 1, motor dominant type 19, Sensory-motor 1, MADSAM 3, Bifacial 2. Nonsystemic vasculitis was seen in 16 out of 29 chronic neuropathy and HIV, POEMS, and diabetes mellitus one each. Discussion: There is a spectrum of immune-mediated neuropathy which varies in clinical course, response to treatment, etc., Small percentage of uncommon cases are seen. In this group, mortality was nil and morbidity was minimal. Conclusion: Immune-mediated neuropathies are treatable and hence should be diagnosed early for good quality outcome.

Bronchogenic cysts are foregut-derived developmental anomalies most commonly encountered in the mediastinum and rarely in the abdomen or retroperitoneally. A comprehensive review of the English-language literature of subdiaphragmatic bronchogenic cysts (sBCs) revealed only 48 reported cases of sBC. Although most cases are incidentally discovered, preoperative differential diagnosis often includes tumors with malignant potential and necessitates surgical resection to obtain a definitive diagnosis. Herein, we describe a case of a 46-year-old female presenting with intermittent left flank pain, upon which computed tomography demonstrated a retroperitoneal mass. Upon resection, histopathology revealed the mass to be a thin-walled cystic mass lined by ciliated columnar cells and cartilage, consistent with a subdiaphragmatic bronchogenic cyst. A comprehensive literature review of sBC was also performed.

AbstractSince Takotsubo cardiomyopathy was first described by Sato in 1990, multiple cases have been reported, but only few in children, among whom this type of cardiomyopathy is to some extent underappreciated. A series of children with this syndrome were therefore reviewed, drawing comparison with cases reported by others. The review addresses the current challenges in diagnosis, presentation, triggers, clinical course, management, and possible pathogenic mechanisms of the entity.

Today a significant dilemma of research involves the sparse number of rigorous, empirical research studies to guide development of best practices of writing. The authors describe the purpose of their book as “a tool and guide for master’s-level students, doctoral-level students, and new and experienced researchers” to approach writing a CLR in a through and investigative way. The thrust and challenge is for researchers to concentrate on improving their methods and practices when reviewing topics with strong potential for shaping long-term educational policies.

The paper is briefly outlining the expected changes in the economic and financial pre-conditions for Slovak farming during the period after the country’s accession to the EU. The starting point of authors reasoning is the current piece of knowledge, saying that the economic performance of individual farms is widely differentiated and the ability of individual farms to meet the future challenges and to use the opportunities is very unevenly distributed among them. Also the competitiveness if a particular agricultural commodity varies. Despite of expectations concerning the price development after accession, not all commodities will be competitive on the single market. The economic situation of farms will considerably benefit from direct payments. Nevertheless, the fact that the Slovak government opted for the Single Area payment Scheme will have some adverse effects on possible gains from payments in productive regions and in farms with high output performance. In order to mitigate these redistribution effects the author is proposing to implement a different scheme of allocation of national top-up is to direct payments. In the final part of his paper the author gives a comprehensive review on payments aimed to promote rural development and payments from the Guidance section of the EAGGF targeted at enhancement of farm re-structuring.
