Clinical relevance of the relationship between Trabecular Bone Score and metabolic syndrome

The Trabecular Bone Score (TBS) is an indirect measurement of bone quality, and studies have shown that TBS is an independent predictor of fracture risk. This cross-sectional investigation aimed to explore the relationship between metabolic syndrome (MetS) and TBS using data from the 2005–2006 US National Health and Nutrition Examination Survey. The association between individual MetS components and TBS was examined. There was a significant linear decrease in TBS with an increase in the number of MetS components. The β coefficients of TBS among participants with 3 and ≥4 MetS components were −0.015 and −0.041 (p=0.006 and p<0.001, respectively). Among participants with MetS, high systolic blood pressure, abdominal obesity, and high serum levels of triglycerides and glucose were significantly associated with lower TBS in fully adjusted models (p<0.05). Furthermore, there was a significant linear decrease in TBS with an increase in the number of MetS components in both sexes. TBS significantly decreased with an increasing number of MetS components in a US population. The components of MetS, including systolic blood pressure, waist circumference, and serum levels of triglyceride and glucose, exhibited a negative association with TBS.

Prevalence of hypertension in adult males was highest in PuskesmasBasuki Rahmat (16.2%) as many as 648 cases of hypertension patients which hasincreased compared to 2012 by 12% in cases of hypertension totaled 482 patients((Dinkes Kota, 2013). This study aims to determine the relationship of body massindex (BMI) and waist circumference (WC) with systolic blood pressure (BP) in adultmales in Puskesmas Basuki Rahmat Bengkulu City in 2015. The study design wasdescriptive analytic with cross sectional approach. Study location was in PuskesmasBasuki Rahmat City of Bengkulu. Samples were selected using purposive samplingtotalling 68 people. Criterias sample were being adult males who occupied inPuskesmas Basuki Rahmat City of Bengkulu, aged ≥ 18 years, agreed to beinterviewed,and able to communicate actively. Data were obtained include BMI, WCand systolic BP adult males and processed using computer software with pearsoncorrelation analysis.The results showed that there was relationship between BMI andsystolic BP (p = 0.0005; r = 0.395); between WC and systolic BP (p = 0.004 and r =0.347) in adult males. This study concluded that there was relationship between BMIand WC with systolic BP in adult males in Puskesmas Basuki Rahmat Bengkulu Cityin 2015.

Background In view of the increasing prevalence of obesity in children, it is necessary to investigate the relative performance of different indicators used for its assessment and health consequences. Objectives To examine concordance between various indicators used for assessing obesity among adolescents and to examine their ability to predict risk of high systolic blood pressure. Design Cross-sectional study, from two schools catering to affluent class. Subjects Children in age 9–16 yr (n = 1146 boys and 1036 girls). Measurements Body weight, height, skinfold thickness at triceps (TSFT) and body fat percent by trained investigators and blood pressure measurement by a pediatrician using sphygmomanometer. Results Prevalence of overweight was lowest with criterion of TSFT (11.7% in boys; 7.6% in girls) and was highest using criterion of body fat percent (53.7% in boys and 28.4% in girls). Body mass index (BMI) had high significant correlation with each of the indicator and with systolic blood pressure (SBP) as well, in both sexes. All the indicators with conventional cut offs showed poor sensitivity for predicting high SBP. However, receiver operating characteristics (ROC) cut-offs improved sensitivity considerably, but the values were much lower compared to conventional cut-offs. Conclusions There is considerable disparity in the estimates of overweight children obtained by different indicators. Lower values of ROC cut-offs highlights the need for population specific customized classification systems for assessing obesity in view of the probable population differences in relative risks of non-communicable adult diseases.

Objectives: This study aimed to explore the association between bedtime at night and systolic blood pressure (SBP) in adults.Methods: We conducted a cross-sectional study composed of 7,642 individuals from the National Health and Nutrition Examination Survey (NHANES). Bedtime was defined as the response to the question: “What time do you usually fall asleep on weekdays or workdays?” SBP was taken using the average of all measured values. Multiple linear regression analyses were done to explore the relationship between bedtime and SBP.Results: The bedtime was changed from categorical variable to continuous variable for data analysis, and a significantly negative association was identified between bedtime and SBP (β, −0.23 [95% CI, −0.43, −0.02]). With the delay of bedtime, the SBP showed a gradual decrease trend, and it was dropped to the lowest at 0:00. After 0:00, the SBP was gradually increased with the delay of sleep time. The stratified analyses showed that in the female group, with the delay of bedtime, the range of SBP was decreased more obviously at 0:00. In the 18–45 year group, bedtime had little effect on SBP. Among ≥45 years old group, this trend was still the same. In the black group, an obvious downward trend was found at 22:00.Conclusion: With the delay of bedtime, the SBP had shown a gradual decrease trend, and it was dropped to the lowest at 0:00. After 0:00, the SBP was gradually increased with the delay of sleep time. Bedtime and SBP showed a U-shaped relationship.

Background: Hypertension is a highly prevalent health problem which incidence is greatest among the elderly. Hypertension may increase creatinine level and leads to other health problems like diabetes mellitus, kidney damage, and cardiovascular disease. Wrist circumference is a simple anthropometric measurement that can be used to identify hypertension and increasing level of serum creatinine.Objectives: To analyze the correlation of wrist circumference with blood pressure and creatinine level among the elderly.Materials and Methods: This was a cross-sectional study with a purposive sampling method. Subjects of this study were 84 women aged 60 years old or above at Unit Rehabilitasi Sosial Pucang Gading Semarang. The independent variable of this study was wrist circumference, and the dependent variables were systolic blood pressure, diastolic blood pressure, and creatinine level. The result was analyzed using the Spearman-rho test.Results: The participants of this research were 49% women aged 60-65 years old, with an average age was 65.5 years old. The prevalence of hypertension was 61.9%. Most hypertension incidence in this research was caused by high systolic blood pressure (50%), and the rest was caused by high diastolic blood pressure (3.9%) and both (46.1%). The level of creatinine was normal with an average level was 0.75 mg/dL. There was no correlation of wrist circumference with systolic blood pressure systolic (r=0.15; p=0.19), diastolic blood pressure (r=0.1; p=0.38), and creatinine serum (r=0.18; p=0.09) among elderly.Conclusions: There was no correlation of wrist circumference with blood pressure and creatinine level among the elderly.

BackgroundMetabolic syndrome (MetS) has a potential connection with thyroid disease, but its relationship with thyroid nodules (TNs) is still controversial. This study aims to clarify the relationship between MetS and TNs, and this relationship in the subgroup of gender.MethodsThe recent nationwide cross-sectional study called Thyroid Disorders, Iodine Status, and Diabetes Epidemiological survey provided the newest data on the relationship between MetS and TNs from China and included 56,729 subjects. We also researched related literature in PubMed, EMBASE, Cochrane Library, and MEDLINE until Oct 30, 2020, in order to perform a meta-analysis. The relevant articles were examined, and the eligible studies were included to assess the association between MetS and TNs.ResultsThe meta-analysis included 15 studies (involving 468,845 subjects). Of these, 14 studies were from the databases, and one study was this cross-sectional data. The meta-analysis showed that TNs were associated with a higher prevalence of MetS (OR=1.87, 95% CI: 1.44–2.45) and the components of MetS, including central obesity (OR=1.41, 95% CI: 1.15–1.72), hypertriglyceridemia (OR=1.13, 95% CI: 1.10–1.15), low high-density lipoprotein cholesterolemia (OR=1.11, 95% CI: 1.02–1.20), abnormal blood pressure (OR=1.68, 95% CI: 1.62–1.75), and hyperglycemia (OR=1.59, 95% CI: 1.46–1.74). Central obesity displayed gender differences, being a risk factor in males (OR=1.38, 95% CI: 1.02–1.86) but not in females (OR=1.47, 95% CI: 0.97–2.23).ConclusionTNs were indeed associated with a higher prevalence of MetS. In addition, its component diseases, such as central obesity, hypertriglyceridemia, abnormal blood pressure, and hyperglycemia, were also associated with TNs. Females with MetS or its components had a higher risk of suffering from TNs than males.

Background: Systolic blood pressure during exercise is the blood pressure as measured by aneroid sphygmomanometer at the peak exercise (treadmill), namely when the subject has experienced fatigue or heart rate has reached > 85% of Maximum Heart Rate. Purpose:This research purpose is to determine the relationship between LDL level and systolic blood pressure at the exercise peak of  police personnel in Southeast Sulawesi. Methods: This research used analytical-observasional study with cross sectional aproach. The independent variables used in this research are LDL level while the dependent variables was systolic blood pressure during the peak exercise. Research location is in Maxima Laboratory Kendari.The subjects of this study were 95 people based on the inclusion and exclusion criteria that selected by Purposive Sampling method. Data collection was gained by taking the secondary data and was processed by using statistical test data of Independent T-test with a significance p-value <0.05. Result: The result of bivariate analysis showed that there was a relationship between LDL level and systolic blood pressure at the exercise peak on the police personnel in Southeast Sulawesi with a p-value = 0,000 (<0.05). Conclusion: This study can be conlcuded that there was a relationship between LDL level and systolic blood pressure at the peak exercise of police personnel in Southeast Sulawesi.Keywords: low density lipoprotein, systolic blood pressure, peak exercise. ABSTRAKLatar belakang : Puncak tekanan darah sistolik pada saat exercise adalah tekanan darah sistolik yang diukur dengan menggunakan sfignomanometer air raksa pada saat puncak exercise, dimana subyek sudah mengalami kelelahan atau denyut jantung telah mencapai 85% dari denyut jantung maximal. Tujuan: Tujuan penelitian ini adalah untuk mengetahui hubungan antara kadar LDL plasma dengan tekanan darah sistolik pada puncak exercise pada personil Kepolisian Daerah Sulawesi Tenggara. Metode: penelitian ini merupakan penelitian analitik-observasional dengan metode cross sectional. variabel independen adalah kadar LDL plasma dan dependen variabel adalah tekanan darah sistol pada puncak exercise. penelitian dilakukan di Laboratorium Klinik Maxima Kendari. sebanyak 95 sampel penelitian yang memenuhi kriteria inklusi dan eksklusi diambil dengan metode purposive sampling. Data tekanan darah saat puncak exercise dan kadar LDL plasma diambil dari data sekunder rekam medik pasien. Data kemudian dianalisa dengan uji statistik Independent T-test dan bermakna bila nilai p <0,05. Hasil: Analisa statistik menunjukkan adanya hubungan antara kadar LDL plasma dengan tekanan darah sistolik pada puncak exercise pada personil POLDA Sulawesi Tenggara, dengan nilai p=0,000. Simpulan : Pada penelitian ini dapat disimpulkan bahwa terdapat hubungan antara kadar LDL plasma dengan tekanan darah sistolik pada puncak exercise pada personil POLDA Sulawesi Tenggara.Kata Kunci: low density lipoprotein, tekanan darah sistolik, puncak exercise

A review of pathological mechanisms that can explain the relationship between periodontitis and cardiovascular disease (CVD) is necessary to improve the management of both conditions. Metabolic syndrome is a combination of obesity, hypertension, impaired glucose tolerance or diabetes, hyperinsulinemia, and dyslipidemia. All these have been examined in recent years in terms of their relationship to periodontitis. Reviewed data indicate an association between some of them (body mass index, high-density lipoprotein-cholesterol [HDL-C], triglycerides, high blood pressure, among others) and periodontitis. Oxidative stress may act as a potential common link to explain relationships between each component of metabolic syndrome and periodontitis. Both conditions show increased serum levels of products derived from oxidative damage, with a pro-inflammatory state likely influencing each other bidirectionally. Adipocytokines might modulate the oxidant/anti-oxidant balance in this relationship.
