We report the case of heading disorientation following ischemic stroke involving the right posteromedial areas. The patient was administered a new test named the Card Placing Test during which a subject was required to recreate an array of three cards, each of which was randomly placed on eight grids around the subject, before and after the subject’s rotation. Qualitative analysis of his performance after rotation revealed that over half of the errors comprised transposition and rotational offset. His score on the Card Placing Test was compared with those of normal controls(n=11). The results showed that his score on Card Placing Test after rotation was significantly lower than those of controls, whereas there was no significant difference between the case and controls in profile of error types. We infer that the heading disorientation observed in the present case was a result of a derangement of a short-term buffer that integrated information on spatial locations of objects with changes in body directions.

The route efferent vagal fibers travel to reach the left ventricle is not clear and was the subject of this investigation. We measured left ventricular and septal effective refractory period (ERP) changes during vagal stimulation and a constant infusion of norepinephrine, before and after phenol was applied at selected sites of the heart to interrupt efferent vagal fibers that may be traveling in that area. Phenol applied to the atrioventricular (AV) groove between the origin of the right coronary artery anteriorly to the posterior descending branch of the circumflex coronary artery completely eliminated vagal-induced prolongation of ERP in the anterior and posterior left ventricular free wall and reduced, but did not eliminate, ERP prolongation in the septum. A large (3-cm radius) epicardial circle of phenol prevented vagal-induced ERP prolongation within the circle in all dogs, while a small (1-cm radius) epicardial circle of phenol failed to prevent vagal-induced ERP changes within the circle in any dog. An intermediate (2-cm radius) circle eliminated vagal effects on ERP in 13 of 18 dogs. Arcs of phenol, to duplicate the upper portion of the circle, applied sequentially from apex to base eliminated efferent vagal effects only when painted near or at the AV groove. We conclude that the majority of efferent vagal fibers enroute to innervate the anterior and posterior left ventricular epicardium cross the AV groove within 0.25-0.5 mm (depth of phenol destruction) of the epicardial surface.(ABSTRACT TRUNCATED AT 250 WORDS)

The research is led by the problems on student’s skill in writing poetry in eight grade SMPN 3 Cipanas cianjur. Imaginative suggestion method with audio visual media does not only create comfortable atmosphere but also give suggestion that develop students amaginative skill. The research design used in this study is One Group Pre-test Post-test. The design is aimed to find out the result of the subject and to show how well the final results of each subject. Based on the results, it is shown that there is a significant difference of students’ ability of writing poetry before and after using Imaginative Suggestion method With Visual-Audio Media, it is proven by calculation with t-test obtained t arithmetic is 2.68 and t table price t 0,05 ie 2.04. it shows that t count (2.68)> t table (2.04). Then it can be concluded that the results of implementing Imaginative Suggestion method With Visual-Audio Media on Poetry Writing is effective.Keywords: imaginative suggestion method, audio-visual media, poetry writing

Abstract                                                                    This study aims to find out how the integration of information literacy substnce in distance training on biological substance for Madrasah Aliyah (MA) in improving information literacy skills in MA biology teachers. In this study the teacher participated in Distance  Training on MA biological material that was integrated with information literacy substance. The research sample consisted of 26 participants in Distance Training  in  Biology Substance for Biology Teacher Aliyah Madrasah in the Bandung Religious Education Training Center originating from the Ministry of Religion of West Java Province. The research design used was pre-experimental design with one group pretest-posttes design. The design procedure of this study was to take the 1st measurement on the subject (pretest) on the ability of mastery of information literacy, then the subject was treated for a certain period (exposure) through education and training in e-learning based distance training deepening of Biology Substance. The second measurement (posttest) was carried out after the treatment was given, and the prestest measurement results were compared with the results of the posttest measurements, using the t-test statistical test. Based on the t-test it can be concluded that "There is a significant difference in the results of self-assessment of information literacy between before and after doing Distance Learning Training".Abstrak                                                                     Penelitian ini bertujuan untuk mengetahui bagaimana integrasi materi literasi informasi pada kurikulum pendidikan dan pelatihan Diklat jarak jauh (DJJ) pendalaman materi biologi Madrasah Aliyah (MA) dalam meningkatkan keterampilan literasi informasi pada guru biologi MA.  Dalam penelitian ini guru mengikuti Diklat Jarak Jauh  Pendalaman materi biologi MA yang diintegrasikan dengan materi literasi informasi. Sampel penelitian berjumlah 26 orang peserta Diklat Jarak Jauh Teknis Substantif Pendalaman Materi Biologi Bagi Guru Biologi Madrasah Aliyah di Balai Diklat Keagamaan Bandung   yang berasal dari Lingkungan Kementerian Agama Provinsi Jawa Barat.   Desain penelitian yang digunakan adalah pre-eksperimental dengan desain One group pretest-posttes design. Prosedur desain penelitian ini adalah melakukan pengukuran ke-1 pada subyek (pretest) terhadap kemampuan penguasaan literasi informasi,  kemudian subjek diberi perlakuan untuk jangka waktu tertentu (exposure) melalui pendidikan dan pelatihan diklat jarak jauh pendalaman materi biologi MA berbasis e-learning yang terdiri dari tiga kegiatan belajar.  Pengukuran ke-2 (posttest) dilakukan setelah perlakuan diberikan, dan hasil pengukuran prestest dibandingan dengan hasil pengukuran posttest, menggunakan uji statistik Uji-t.  Berdasarkan uji-t dapat disimpulkan bahwa “Terdapat perbedaan yang signifikan hasil penilaian diri penguasaan literasi informasi  antara sebelum dan sesudah melakukan pembelajaran Diklat Jarak Jauh”

Abstract The body temperature of 10 clinically healthy green iguanas (Iguana iguana) was measured using a thermographic camera (FLIR E6, Flir Systems Sweden) before and after the food was offered. For each animal there were performed a total of 6 measurements (3 before feeding and 3 after the food was offered). The purpose of this experiment was to observe the thermographic pattern of the body before and after the feeding, since herbivore reptiles tend to bask after the feeding to increase the body temperature that will help them afterwards digest the food. The animals were housed in individual vivariums with every animal having a basking spot available. The pictures were taken outside the vivarium in an adjacent room. The animals were handled with gloves and transported in a cardboard box in order to avoid heat transfer between the handler and the iguana that would have produced thermal artefacts. Each individual was placed on a table on a styrofoam slate, again, to avoid the heat transfer between the table and the animal`s body. For each animal a total of 4 pictures were taken (up, front, left and right). The pictures were analysed with the FLIR Tools program that is provided by the manufacturer and 3 temperatures were taken into consideration (the head temperature, body temperature on the right side and body temperature on the left side). The temperatures were compared between them and with the temperature of the vivariums that consisted of the average between the temperature in 3 different spots (basking spot, the feeding bowl site and the coldest spot) measured with an infrared thermometer GM300 (Benetech, China). The temperature of the body was dependent on the vivarium temperature and it was a significant temperature difference between the measurements before the feeding and after the feeding. Also we discovered a significant difference between the head temperature and the body temperature on the left side before the feeding that disappeared after the animals ate. There was also a significant difference between the temperature on right side and on left side of the animals both before and after the feeding. No significant temperature difference was observed between the head and the right side of the body neither before nor after the feeding.

ObjectivesTo determine the short-term impact of a soft opt-out organ donation system on consent rates and donor numbers.DesignBefore and after observational study using bespoke routinely collected data.SettingNational Health Service Blood and Transplant.Participants205 potential organ donor cases in Wales.InterventionsThe Act and implementation strategy.Primary and secondary outcomesConsent rates at 18 months post implementation compared with 3 previous years, and organ donor numbers 21 months before and after implementation. Changes in organ donor register activity post implementation for 18 months.ResultsThe consent rate for all modes of consent was 61.0% (125/205), showing a recovery from the dip to 45.8% in 2014/2015. 22.4% (46/205) were deemed consented donors: consent rate 60.8% (28/46). Compared with the 3 years before the switch there was a significant difference in Welsh consent rates (χ2 p value=0.009). Over the same time period, rest of the UK consent rates also significantly increased from 58.6% (5256/8969) to 63.1% (2913/4614) (χ2 p value<0.0001), therefore the Wales increase cannot be attributed to the Welsh legislation change. Deceased donors did not increase: 101 compared with 104. Organ donation registration increased from 34% to 38% with 6% registering to opt-out.ConclusionThis is the first rigorous initial evaluation with bespoke data collected on all cases. The longer-term impact on consent rates and donor numbers is unclear. Concerns about a potential backlash and mass opting out were not realised. The move to a soft opt-out system has not resulted in a step change in organ donation behaviour, but can be seen as the first step of a longer journey. Policymakers should not assume that soft opt-out systems by themselves simply need more time to have a meaningful effect. Ongoing interventions to further enhance implementation and the public’s understanding of organ donation are needed to reach the 2020 target of 80% consent rates. Further longitudinal monitoring is required.

In religious learning, students are expected to be able to understand the daily prayers taught in class. To achieve these goals, the right method is needed so students can follow these prayers. One way that can be used to prepare daily prayers is the sima'i method. This type of research is classroom action research. The results in this study are students can memorize correctly and both on the accuracy of speech, fluency, and attitude. This study concludes that the sima'i method has a positive influence on increasing the ability to memorize daily prayers on the subject. The results are taken based on the comparison of data between before and after being given treatment in the form of applying the sima'i method to the topic. Dalam pembelajaran agama, siswa diharapkan mampu mamahami doa-doa harian yang diajarkan di kelas. Untuk mencapai tujuan tersebut, diperlukan metode yang tepat agar siswa dapat memahami doa-doa tersebut. Salah satu metode yang dapat digunakan untuk mengajarkan doa-doa harian adalah metode sima’i. Jenis penelitian ini adalah penelitian tindakan kelas. Hasil dalam penelitian ini adalah siswa-siswa dapat menghafal dengan benar dan baik pada ketepatan ucapan, kelancaran dan sikap. Simpulan penelitian ini adalah metode sima’i memberikan pengaruh positif terhadap peningkatan kemampuan menghafal doa-doa harian pada subjek. Hasil tersebut diambil berdasarkan perbandingan data antara sebelum dan sesudah diberikan perlakuan berupa penerapan metode sima’i pada subjek.

Introduction: The use of ketamine and tramadol as postoperative analgesics for pediatric are still relatively rare, especially orally administrated. As an analgesic, ketamine blocks the NMDA receptor, the main excitatory transmitter in CNS; whereas tramadol blocks serotonin and norepinephrine uptake, thus preventing pain transmission on the spinal cord. Objective: The aim of this study is to compare the effectiveness of oral ketamine and oral tramadol as analgesics for postoperative acute pain in children. Method: A double-blind randomized clinical trial was conducted at Dr. Soetomo Hospital. The hospital ethical committee had approved this study. The subject includes thirty children aged 5-10 years old who fulfilled the inclusion criteria. They were divided into either ketamine groups or the tramadol group, in which each group consisting of fifteen patients. The regimen dosage that been given was 2mg/kg tramadol and ketamine as postoperative oral analgesics in the form of simple syrup. The FLACC table was used to evaluate pain score before and after administration of drugs (30-minutes, 1-hour, 2-hours, 3-hours, 4-hours, and at discharge from the recovery room). Result and Discussion: Based on the quantitative parameter of the FLACC (scale 0-10), there was a significant difference (p<0.05) between the first-hour postoperative administration and patient discharge from the recovery room. The patient of ketamine group had far lower FLACC value compared to the tramadol group. Rescue analgesics in the form of intravenous fentanyl were given to one patient (6.7%) in the ketamine group and  four patients (26.7%) in the tramadol group. Conclusion: Ketamine proved to be a better and more effective postoperative oral analgesic compared to tramadol in this study.

The Aim of study was analysis of the effect of nicergoline on the cerebral, cardiac, systemic hemodynamics of patients after ischemic stroke, taking into account the hemispheric localization of the ischemic focus. Materials and methods. A comprehensive clinical and neurological examination was carried out in 38 elderly patients (mean age 64.3 ± 2.5 years) who had undergone atherothrombotic ischemic stroke in the carotid system (recovery period). There were 20 patients with left hemispheric stroke and 18 with right hemispheric stroke. All patients had arterial hypertension (AH). These were ACE inhibitors (enalapril 10-20 mg 1 tablet 2 times a day) and hydrochlorothiazide at a dose of 12.5 mg. Patients received nicergoline at a dose of 4 mg IV drip for 10 days. A comprehensive examination was carried out before and after treatment with nicergoline. The Results of the study allowed us to conclude that the course of nicergoline intake in patients after ischemic atherothrombotic stroke improves the subjective state and reduces the severity of neurological disorders, cerebral hemodynamics, increases linear systolic blood flow velocities (LBFVsys) and reduces peripheral resistance in individual extra- and intracranial vessels of the carotid and vertebro-basilar basins. In patients with right and left hemispheric localization of stroke under the influence of nicergoline, LBFVsys increases in the right and left ICA, PCA, and VA. In addition, LBFVsys increases in patients with right hemispheric stroke in the right MBA; in patients with left hemisphere - in two MBA and BA. In ischemic stroke patients, nicergoline affects systemic and cardiac hemodynamics: it reduces the systemic and cardiac hemodynamics, decreases the systemic vascular resistance and increases the ejection fraction, changes the structure of the relationship between systemic and cerebral hemodynamics, forming a positive relationship between blood pressure and hemodynamics in the vessels of the vertebro-basilar basin. Thus, the complex positive effect of nicergoline on various levels of cerebral, systemic and cardiac hemodynamics in ischemic stroke patients gives grounds to recommend the inclusion of this drug in the rehabilitation system of this category of patients.

Background: The use of HMB supplement is prevalence among athletes to reduce proteolysis and increase muscle mass. Therefore, the purpose of this study was to investigate the effect of short-term beta-hydroxy-beta-methylbutyrate (HMB) supplementation on serum cortisol and testosterone levels in wrestling men following an exhaustive exercise. Methods: Sixteen male wrestlers were randomly divided into two groups including: HMB supplementation (40 mg/kg body weight; n=8) and placebo (n=8). Both groups, after two weeks of supplementation, performed an exhaustive exercise. Blood was obtained before supplementation (Base), before exhaustive exercise, immediately after exhaustive exercise, one hour and 24 hours’ after exhaustive exercise. Circulating concentrations of cortisol and testosterone were assayed. Results: The results of this study indicated that there was no significant difference between the two groups at serum cortisol levels of resting state (before exhaustive exercise) and after exhaustive exercise (immediately, one hour and 24 hours later) (p<0.05). However, there was significant difference between the HMB and placebo groups at serum testosterone levels and testosterone/cortisol of resting state (before exhaustive exercise) and after exhaustive exercise (p<0.05). Conclusion: In general, short-term beta-hydroxy beta-methyl butyrate (HMB) supplementation does not affect the serum cortisol in male wrestlers before and after an exhausting exercise. However, HMB supplementation can lead to a significant increase in serum testosterone and T/C before and after an exhausting exercise.

Abstract Objectives Dietary advanced glycation end products (AGEs) are a group of compounds formed by the Maillard reaction. Carboxymethyl-lysine (CML), a marker of AGEs in foods, may contribute to cardiometabolic diseases. However, CML health effects and its absorption and excretion in healthy individuals remained controversial. For this reason, the aim of this study was to evaluate serum and urinary excretion CML levels in healthy adults after a short-term diet high in CML. Methods A randomized clinical trial was conducted in 30 healthy adults (18–35 years old). After signing the informed consent, subjects were randomly assigned to a low or a high CML diet. All participant followed a diet with low CML content for seven days (washing period) before the 5-days intervention. During the 5 days participants received all the foods according to their group. Blood and 24-hour urine samples were collected before and after the intervention to evaluate serum and urinary CML levels. CML measurement in foods, serum, and urine was carried out by HPLC-ESI-ITMS/MS. Comparison between groups was carried out by a t-test. The research was approved by the Institutional Review Board and the clinical trial registration number NCT03208946. Results Biochemical and anthropometric variables were similar between groups (P > 0.05). The average consumption of CML was 1.93 ± 1.47 mg/day for the low CML diet and 5.03 ± 2.22 mg/day for the high CML diet, whereas caloric, protein and fat intake were similar. Serum levels decreased significantly in both groups (low diet: basal 488.1 ± 135.4 μg/L, final 383.2 ± 78.6 μg/L (P = 0.007)) (high diet: basal 463.5 ± 90.3 μg/L, final 390.3 ± 99.1 μg/L (P = 0.02)). No significant difference was found when comparing the final CML levels between groups (P = 0.47). The excretion of CML in urine was adjusted by creatinine (mg) and the high CML group had a greater excretion (4.5 ± 8.0 ug CML/mg) when compared to the low CML group (−1.5 ± 7.2 ug CML/mg) (P = 0.04). Conclusions A high CML diet increases urinary CML excretion levels in healthy adults when compared to a low CML diet. There were no significant differences in serum CML levels between both groups. However, a greater decrease was observed in the subjects who followed a diet with a low content of AGEs. Funding Sources University of Guanajuato. CONACYT.
