Objectives. This study investigates the effect of Qingshen Granules (QSG) on chronic renal failure patients and the HIF-1α/Wnt/β-catenin signaling pathway. Methods. Subjects were randomly divided into treatment and control groups, with 42 patients in each group. Participants in the treatment group received 10 g oral doses of QSG 3 times a day, for 12 weeks, whereas subjects in the control group were given a placebo. The effective rates of traditional Chinese medicine (TCM) symptom, serum creatinine (Scr), and estimate glomerular filtration rate (eGFR) as well as the serum levels of HIF-1α, Wnt1, β-catenin, α-SMA, and E-cadherin were evaluated. Results. Eighty patients completed the treatment program and two dropped out. After 12 weeks, the effective rates of TCM symptom and eGFR were found to be higher in the treatment group than in the control group, with statistically significant differences (P = 0.024 and 0.019, respectively). Meanwhile, lower levels of HIF-1α, Wnt1, β-catenin, α-SMA, and E-cadherin were detected in the treatment group, and the differences were statistically significant (P ≤ 0.001, P = 0.001, P ≤ 0.001, P ≤ 0.001, and P = 0.039). No adverse events occurred during the study. Conclusions. QSG can alleviate the clinical symptoms of chronic renal failure (CRF) and protect renal function in patients by influencing the HIF-1α/Wnt/β-catenin signaling pathway. The treatment exhibits no adverse effects and is thus safe to be used by humans.

Background: Amplification of inflammation in acute exacerbation of chronic obstructive pulmonary disease (COPD) increases inflammatory mediators and oxidative stress in the airways, pulmonary and systemic circulation that are characterized by increased plasma level of IL-6 and MDA, resulting in worsening of clinical symptoms. Xanthones in mangosteen pericarp have anti-inflammatory and antioxidant effects, potentially as an adjuntive therapy in acute exacerbations of COPD. Methods: The aim of this study was to determine the effect of mangosteen pericarp extract to clinical improvements, plasma level of IL-6 and MDA of acute exacerbation COPD patients. A clinical trial of experimental with pretest and posttest was conducted on 34 acute exacerbation of COPD patients in Dr. Moewardi Hospital Surakarta and Dr. Ario Wirawan Lung Hospital Salatiga from April until May 2016. The sample was taken by consecutive sampling. Subjects were divided by randomized double blind technique into the treatment group (n=17) received mangosteen pericarp extract 2x1100mg/day and control group (n = 17) received placebo. Clinical improvements were measured in CAT score and length of stay. CAT score, plasma level of IL-6 and MDA were measured on admission and at discharge. Length of stay based on the number of days of care in hospitals. Results: There was significant difference (p=0,011) towards decreased of IL-6 plasma level between treatment group (-2,17 ± 3,46 pg/ mL) and control group (+1,67 ± 6,81 pg/mL). There were no significant difference towards decreased of length of stay (p=0,34) between treatment group (4,12 ± 1,54 days) and control group (5,24 ± 2,49 days), towards decreased of CAT score (p=0,252) between treatment group (-19,18 ± 3,96) and control group (-18,24 ± 2,75), and towards decreased of MDA plasma level (p=0,986) between treatment group (+0,03 ± 0,36μmol/L) and control group (+0,35 ± 1,58). Conclusions: The addition of mangosteen pericarp extract 2x1100mg/day during hospitalization was significantly lowered plasma levels of IL-6, but were not significant in lowering the CAT score, shortening the length of stay, and reducing the increase in plasma level of MDA.

Objective. To ascertain the efficacy and safety of Ganji Formulation (GF) for patients with Hepatocellular carcinoma (HCC) who had undergone surgery. Materials and Methods. A total of 262 HCC patients who had undergone liver resection, local ablation, or transcatheter arterial chemoembolization (TACE) were divided randomly into the treatment group and control group. The former was treated with GF and the later with placebo, both for 6 months. The primary endpoint was overall survival (OS). Second endpoints were disease-free survival (DFS) or time to disease progression (TTP). Results. OS of the treatment group was significantly longer than that of the control group (P < 0.05). Subgroup analysis showed that, for patients who received TACE, the TTP was significantly longer in the treatment group than in the control group (P < 0.05). However, for patients who underwent liver resection or local ablation, there was no significant difference in DFS between the two groups (P > 0.05). Conclusion. GF could improve postoperative cumulative survival and prolong the TTP. This clinical trial number is registered with ChiCTR-IOR-15007349.

Introduction: Chronic renal failure is clinical syndrome caused by declining renal function, lasting progressively and irreversible. Physical change due to organ function decline will affect client psychologicaly and socially. These changes cause self esteem disorder where client tend to use maladaptive coping with manifestation of frequent irritating, angry, anxiety, and depression. Therapeutic communication between nurse and client that lead to client healing process which planned in conscious, purposive manner and this activity is centered on client. Method: This research used quasi experimental. Total sample in this research was 12 clients with chronic renal failure that meet inclusion criteria grouped in two groups: treatment group and control group. Independent variable was therapeutic communication while dependent variable was self esteem. In this research data analysis used was Wilcoxon Sign Rank Test with significance α ≤ 0,05 and Mann Whitney Test with significance α ≤0,05. Result: Result obtained after therapeutic communication intervention provided was p = 0,002 where there is significant difference on client self esteem before and after therapeutic communication intervention given. Result showed that there is significant difference between treatment group and comparative group with p = 0.026. Discussion: It is concluded that therapeutic communication has effect on improving self esteem in client with chronic renal failure. Therapeutic communication helps client to express his/her feeling and problems clients self esteem were improved.

  Objective:  To determine if Ehretia microphylla (Tsaang Gubat) decoction tea and placebo can improve the symptoms of mild intermittent allergic rhinitis in comparison to loratadine and control tea.   Methods: Study Design:            Double blind, Randomized Controlled Trial Setting:                       Tertiary Government Training Hospital Subjects:                    Twenty-four patients diagnosed with mild intermittent allergic rhinitis from October 2015 to July 2016 were randomly divided into a treatment group given Ehretia microphylla (Tsaang Gubat) decoction tea and placebo, and a control group given control tea and loratadine, both taken for 7 days. Patients underwent pre–  and post–intervention evaluation by anterior rhinoscopy, Sino-nasal Outcome Test 22 (SNOT 22) Questionnaire and 10-point Visual Analog Scale (VAS). Data were encoded and subjected to statistical analysis using Mann Whitney U test, and Wilcoxon Signed Rank test.   Results: Age and gender of the treatment and control group participants were comparable. Prior to intervention, no differences in symptoms were noted between both groups on SNOT 22 and VAS scores. After intervention, no differences in symptoms were noted between the 2 groups on SNOT 22 and VAS scores either. Comparison of pre- (30.4 ± 17.3) and post- (7.2 ± 6.5) intervention mean SNOT 22 scores of the loratadine control group with pre- (32.5 ± 23.7) and post- (7.8 ± 10.4) intervention mean SNOT 22 scores of the Ehretia Microphylla treatment group showed significant improvement of symptoms in both groups. Likewise, comparison of pre- and post-intervention mean VAS scores of the loratadine control group and pre- and post-intervention mean VAS scores of the Ehretia Microphylla treatment group based on symptoms of sneezing, rhinorrhea, nasal congestion and pruritus showed significant improvement of symptoms in both groups (p-values of < .001).                         Conclusion: Ehretia microphylla (Tsaang Gubat) decoction tea may improve symptoms of allergic rhinitis (sneezing, rhinorrhea, pruritus, and nasal congestion) and be taken as an alternative to loratadine in patients with mild intermittent allergic rhinitis. Further clinical trials with more participants may provide stronger evidence for this conclusion.   Keywords: Allergic rhinitis, tsaang gubat, ehretia microphylla, loratadine

Objective. Painful diabetic neuropathy (PDN) is a prevalent debilitating consequence of diabetes mellitus with lack of satisfactory therapeutic options. Methylcobalamin (MeCbl) is one of vitamin B12 analogs with known neurotrophic effects. We aimed to determine if MeCbl can relieve PDN. Materials and methods. This was a randomized (1:1) double-blind placebo-controlled trial involving PDN patients. Treatment and control group received daily 12.5 mg oral amitriptyline bid with either 500 µg of intravenous MeCbl or saline injection given on alternating days, respectively, for a 9-consecutive day period. PDN was assessed with douleur neuropathique 4 (DN4) questionnaire. Numeric pain rating scale (NPRS) was used to monitor pain intensity and treatment response. All investigators and patients were kept blinded throughout the study period. Outcomes. 42 patients, 21 on each arm had completed the study. The NPRS reduction can already be observed as early as day 2 post-intervention. Both the treatment and control group demonstrated sustained reduction of NPRS by almost one point per each time point of evaluation in the first three days (p<0.001). NPRS reduction remained until the end of the study period. The treatment group had a significantly lower NPRS score by 1.29 than that of the control group during the entire study period (95% CI -1.84 – -0.75; p < 0.001). Treatment group experienced significantly higher NPRS reduction when compared with control (4.19±1.54 vs. 2.1± 0.83; 95% CI 1.32-2.87; p < 0.001), i.e. 62.6% from baseline. Conclusions. MeCbl significantly and safely relieved PDN in a relatively rapid onset.

<p class="18">Abstract: Objective: To observe the clinical effect of “Chaihu Shugan Powder Zuojin Pill” in the treatment of non-erosive gastroesophageal reflux disease. Methods: Sixty patients with non-erosive gastroesophageal reflux disease were randomly divided into treatment group and control group (30 cases in each group). The patients were treated with “Zaohu Shugan Powder” and “Zangjin Pills” and omeprazole respectively. For 8 weeks, the clinical efficacy and changes in clinical symptoms were observed. Results: The total effective rate was 93.3% in the treatment group and 80.0% in the control group. There was significant difference between the two groups before and after treatment (P &lt;0.05). Conclusion: “Chaihu Shugan Powder Zuojin Pill” can effectively improve the clinical symptoms of non-erosive gastroesophageal reflux disease.</p>

Smog warning systems are components of adaptation strategies that are adopted by governments around the world to protect their citizens from extreme episodes of air pollution. As part of a growing research stream on the effectiveness of these systems, this article presents the results of a study on the impacts of an automated phone warning and advising system for individuals vulnerable to air pollution. A sample of 1328 individuals were recruited and randomly assigned to treatment and control groups. The treatment group received smog warning while the control group did not. Data were collected via three phone surveys, two before and one after issuing the smog warning. The comparison between treatment and control groups indicates that exposure to a smog warning improved information on the occurrence of smog episodes (n = 484, OR = 5.58, p = 0.00), and knowledge on protective behaviors. Furthermore, members of treatment group were more likely to avoid exposure to smog episodes by spending more time inside with the windows closed than usual (n = 474, OR = 2.03, p = 0.00). Members of treatment group who take medication in the form of aerosol pumps also kept these devices on themselves more frequently than those of control group (n= 109, OR = 2.15, p = 0.03). The system however had no discernible effects on the awareness of air pollution risks, reduction of health symptoms related to smog and the use of health system services. The absence of health benefits could be related to the lower actual exposure to air pollution of such vulnerable groups during winter.

Background: Older people is vulnerable to COVID-19 relates to presence of co-morbidities diseases and low immunity.Undernutrition condition making them prone to anemia (low hemoglobin/Hb) and low lymphocyte level will brings a higher risk for COVID-19 infection. Objective: To assess the effect of potato almond orange cookies on weight, Hb, and lymphocyte levels of undernourished older people amidst the COVID-19 pandemic. Methods: This 3-week randomized controlled trial involved 48 subjects > 60 years, divided randomly into treatment group (potato almond orange cookies) and control group (potato almond cookies). Lymphocyte and Hb collected before and after the study. Both groups received nutrition education. Analysis was performed using independent t-test and paired t-test. Results: Weight gained in the treatment group (0.7 kg) was significantly different from the control group (p=0.002), with 0.3 point change observed in the BMI of the treatment group. Changes were also seen in vitamin C intake in treatment group (+10.3 mg, p = 0.03) and vitamin E intake in both groups (+4.6 mcg, p = 0.001 and +2.9 mcg, p = 0.001, respectively), but not for inter-group. Weight, Hb and lymphocyte level increased in both groups, with the highest weight increased in the treatment group while the highest Hb increased in the control group. The lymphocyte increased 1 point in both groups. Weight, Hb and lymphocyte level differences were insignificant between the two groups. Conclusion: Potato almond orange consumption tend to increase weight, Hb and lymphocyte level in undernourished older people during COVID-19 pandemic.

Objective: To observe the clinical effect of "heterozygous treatment" intervening the damp-heat constitution. Method: 106 cases with damp-heat constitution were randomly divided into the observation group and control group, 53 cases for each group. Lianpu drink was given to the two groups, and the observation group was treated with scraping, acupuncture, cupping, constitution care and popularization of constitution science for "heterozygous treatment" based on the control group. 70 days later, "constitution classification and determination table of traditional Chinese medicine" was used to determine, and statistics was applied to analyze the change of the symptoms of the two groups before and after the intervention. Results: in the observation group, compared to before the intervention, symptoms like dirty and oily complexion, yellow greasy tongue, bitter taste, dullness and scanty dark urine were significantly improved (P < 0.05), and improvement of the above symptoms was greater than the control group (P < 0.05) .Conclusion: There is no significant difference between the two groups (P > 0.05). Conclusion: the "heterozygous treatment" method can significantly improve the clinical symptoms of people with damp-heat constitution, with a better role in regulating.

Backgrounds: Asthma is a chronic airway inflammation which is a manifestation of complex interactions between cells and moleculer mediators. The aims asthma management is to reach asthma in controlled state. Providing an additional therapy in these circumstances is necessary to control asthma. Quercetin as an adjunctive therapy in asthma therapy may improve clinical symptoms and lung function. Methods: Experimental clinical trials of pretest of and postest design were conducted on 34 patient’s asthma at Dr. Moewardi Hospital Surakarta in 6 October–10 December 2018. Subject of treatment group (n= 17) was given quercetin 500 mg per day within 28 days, the control group (n=17) received only standar therapy asthma. Decreased airway inflammation was assessed based on the percentage of blood eosinophil and IL-5. Clinical improvement was assessed by ACT score while lung function used FEV1. Results: Quercetin decreased the inflammatory airways in patients with asthma evidenced by significant decrease in plasma IL-5 of treatment groups but no significant differences between treatment and control group, the mean decrease of blood eosinophil in the treatment group was significant. Querectin improved lung function with decrease FEV1 of treatment groups but no significant differences between treatment and control group and there was a clinical improvement with significant ACT score enhancement in the treatment group. Conclusion: The administration of quercetin significantly reduced inflammation based on decreased levels of eosinophils. There are improvement of lung function and clinical symptoms after quercetin.
