<b><i>Background:</i></b> Human milk contains a remarkable array of immunological agents that evolved over millions of years to protect the recipient human infant. Furthermore, much of the protection persists long after weaning. However, the scientists who first discovered some components of this immune system have rarely been acknowledged. <b><i>Summary:</i></b> The scientists who made many fundamental immunological discoveries concerning the immune system in human milk include Alfred François Donné, Paul Ehrlich, Lars Å. Hanson, and Jules Bordet. Based upon their discoveries, a wealth of antimicrobial, anti-inflammatory, and immunomodulating agents, and living, activated leukocytes in human milk were later revealed during the last half of the 20th and the first part of the 21st century. Moreover, it was found that human milk enhances the colonization of commensal bacteria that aid to protect the human infant. <b><i>Key Message:</i></b> Their discoveries helped to revitalize breastfeeding in industrialized countries during the past several decades.

Human milk is a very complex fluid with a number of components and multiple functions. New functions are continually being identified. It is clear that human milk can affect the immune system of the breasfed infant. This results both in enhanced vaccine responses and, at times, down-regulation of other immune reactivities, such as transplant rejection and the risk of developing certain immunologic diseases, such as type I diabetes. Breastfeeding presumably gives the infant the possibility for an optimal immune response by providing good nutrition, including a decreased risk of vitamin A deficiency. The control of the intestinal flora and the anti-inflammatory effects of maternal milk also increase the possibilities for an adequate immune response in the infant. Further study is needed of the roles of idiotypic and anti-idiotypic antibodies, growth factors, cytokines, and various anti-inflammatory factors in the maternal milk in the infant's host defence.

Pregnancy is associated with a skewing towards T-helper (Th)2-like cell populations. During the first years of life, the neonatal immune responses towards allergens deviate towards a balanced Th1 and Th2-like immunity. The difference between atopic and nonatopic individuals may be how readily the immune deviation takes place. The high prevalence of allergies in industrialized countries is in contrast with the low prevalence of allergies in Eastern Europe, with a life style similar to that prevailing in Western Europe 40 years ago. The discussion on the impact of environmental changes on the incidence of asthma and other allergies has been limited mostly to the possible effects of a deteriorating air quality, poorly ventilated houses and an increased exposure to certain allergens, notably house dust mites. None of these factors can more than marginally explain the observed regional differences in the prevalence of allergic diseases. The concept of ‘life style’ should therefore be expanded considerably. The mother is a significant ‘environmental factor’ in early infancy. Human milk contains components that enhance the maturation of the immune system of the newborn infant. However, there are considerable individual variations in the composition of human milk. Recent studies indicate an imbalance in the gut flora of allergic, compared with non allergic infants, and in Swedish children compared with Estonian children. As the microbial flora drives the maturation of the immune system, changes in its composition may play a role for the higher prevalence of allergy. The future search for significant environmental factors should be directed towards other areas that have not yet been explored. The intestinal microflora is one of these factors that deserve a closer analysis.

Traditionally, natural medicines have been administered as plant extracts, which are composed of a mixture of molecules. The individual molecular species in this mixture may or may not contribute to the overall medicinal effects and some may even oppose the beneficial activity of others. To better control therapeutic effects, studies that characterized specific molecules and describe their individual activity that have been performed over the past decades. These studies appear to underline that natural products are particularly effective as antioxidants and anti-inflammatory agents. In this systematic review we aimed to identify potent anti-inflammatory natural products and relate their efficacy to their chemical structure and physicochemical properties. To identify these compounds, we performed a comprehensive literature search to find those studies, in which a dose-response description and a positive control reference compound was used to benchmark the observed activity. Of the analyzed papers, 7% of initially selected studies met these requirements and were subjected to further analysis. This analysis revealed that most selected natural products indeed appeared to possess anti-inflammatory activities, in particular anti-oxidative properties. In addition, 14% of the natural products outperformed the remaining natural products in all tested assays and are attractive candidates as new anti-inflammatory agents.

Our understanding of the tumorigenesis of classical Hodgkin lymphoma (cHL) and the formation of Reed–Sternberg cells (RS-cells) has evolved drastically in the last decades. More recently, a better characterization of the signaling pathways and the cellular interactions at play have paved the way for new targeted therapy in the hopes of improving outcomes. However, important gaps in knowledge remain that may hold the key for significant changes of paradigm in this lymphoma. Here, we discuss the past, present, and future of cHL, and review in detail the more recent discoveries pertaining to genetic instability, anti-apoptotic signaling pathways, the tumoral microenvironment, and host-immune system evasion in cHL.

AbstractThrough considerable effort in research and clinical studies, the immune system has been identified as a participant in the onset and progression of brain injury after ischaemic stroke. Due to the involvement of all types of immune cells, the roles of the immune system in stroke pathology and associated effects are complicated. Past research concentrated on the functions of monocytes and neutrophils in the pathogenesis of ischaemic stroke and tried to demonstrate the mechanisms of tissue injury and protection involving these immune cells. Within the past several years, an increasing number of studies have elucidated the vital functions of T cells in the innate and adaptive immune responses in both the acute and chronic phases of ischaemic stroke. Recently, the phenotypes of T cells with proinflammatory or anti-inflammatory function have been demonstrated in detail. T cells with distinctive phenotypes can also influence cerebral inflammation through various pathways, such as regulating the immune response, interacting with brain-resident immune cells and modulating neurogenesis and angiogenesis during different phases following stroke. In view of the limited treatment options available following stroke other than tissue plasminogen activator therapy, understanding the function of immune responses, especially T cell responses, in the post-stroke recovery period can provide a new therapeutic direction. Here, we discuss the different functions and temporal evolution of T cells with different phenotypes during the acute and chronic phases of ischaemic stroke. We suggest that modulating the balance between the proinflammatory and anti-inflammatory functions of T cells with distinct phenotypes may become a potential therapeutic approach that reduces the mortality and improves the functional outcomes and prognosis of patients suffering from ischaemic stroke.

The 40th Anniversary Edition of Taylor, Walton and Young’s New Criminology, published in 2013, opened with these words: ‘The New Criminology was written at a particular time and place, it was a product of 1968 and its aftermath; a world turned upside down’. We are at a similar moment today. Several developments have been, and are turning, our 21st century world upside down. Among the most profound has been the emergence of a new earth, that the ‘Anthropocene’ references, and ‘cyberspace’, a term first used in the 1960s, which James Lovelock has recently termed a ‘Novacene’, a world that includes both human and artificial intelligences. We live today on an earth that is proving to be very different to the Holocene earth, our home for the past 12,000 years. To appreciate the Novacene one need only think of our ‘smart’ phones. This world constitutes a novel domain of existence that Castells has conceived of as a terrain of ‘material arrangements that allow for simultaneity of social practices without territorial contiguity’ – a world of sprawling material infrastructures, that has enabled a ‘space of flows’, through which massive amounts of information travel. Like the Anthropocene, the Novacene has brought with it novel ‘harmscapes’, for example, attacks on energy systems. In this paper, we consider how criminology has responded to these harmscapes brought on by these new worlds. We identify ‘lines of flight’ that are emerging, as these challenges are being met by criminological thinkers who are developing the conceptual trajectories that are shaping 21st century criminologies.

The development of oncolytic viruses (OVs) has increased significantly in the past 20 years, with many candidates entering clinical trials and three of them receiving approval for some indications. Recently, OVs have also gathered interest as candidates to use in combination with immunotherapies for cancer due to their immunogenic properties, which include immunogenic cell death and the possibility to carry therapeutic transgenes in their genomes. OVs transform non-immunogenic ‘cold’ tumors into inflamed immunogenic ‘hot’ tumors, where immunotherapies show the highest efficacy. However, in monotherapy or in combination with immunotherapy, OVs face numerous challenges that limit their successful application, in particular upon systemic administration, such as liver sequestration, neutralizing interactions in blood, physical barriers to infection, and fast clearance by the immune system. In this regard, the use of mesenchymal stem cells (MSCs) as cells carrier for OV delivery addresses many of these obstacles acting as virus carriers and factories, expressing additional transgenes, and modulating the immune system. Here, I review the current progress of OVs-loaded MSCs in cancer, focusing on their interaction with the immune system, and discuss new strategies to improve their therapeutic efficacy.
