Abstract 20: Kawasaki Disease In France: Incomplete Forms Are Frequent And Associated With A High Frequency Of Cardiac Complications

KD is the main vasculitis affecting children < 5 years and the leading cause of acquired heart disease in children. Its epidemiology is few reported in France. Even if IVIG is still the standard treatment; the management of patients at risk for cardiac complications may change toward reinforced (and new) therapeutic approaches. Kawanet is a clinical and biological data repository aimed to define the epidemiological characteristics of KD in France. Methods: Institutional physicians received information on a national registry for KD. All patients suspected with KD and seen since 2011 were eligible to enter the study. An eCRF was implemented in a web database.The included patients without the AHA international criteria were reviewed by an experts' committee. Results: 468 cases were entered by 84 physicians from 65 centers. The AHA classification gave: 280 complete KD, and 73 incomplete KD. An expert consensus classified 48 other patients as probable leading to 401 patients considered as KD (M229/F72). 67 were excluded (incomplete data or doubful). The median age at diagnosis was 3.1y (2m-14y). Their ethnical backgrounds were: European Caucasian 67%, Eastern Caucasian/North African 15%, afro-Caribbean 13%, Asian 4% and mixed ancestry 1%. The clinical symptoms were (%): conjunctivitis 84, cheilitis 82, diffuse exanthema 74, modification of the extremities 73, oral erythema 66, cervical adenopathy 52, raspberry tongue 49, seat erythema 26, perineal desquamation 18 and BCG erythema 5. The cardiac complications were: coronary dilatation 30%, pericarditis 15%, coronary aneurysm 4%, and myocarditis 3% (1 death). 392/401 (98%) patients received IVIG, 64 (21%) and 5 required 2 and 3 courses. The mean treatment delay was 6 days. The factors associated with the coronary abnormalities were: male gender (p=0.01), young KD onset age (p=0.03), and resistance to IVIG (p=0.03). conclusion: KD diagnosis remains challenging and overdiagnosis represents at least 10% of cases in this registry. Incomplete forms of KD account for 37 % and are associated with coronary dilatation/aneurysm (34%; p<0.01) and a high rate of IVIG resistance. Unlike previous studies, our population is very mixed with 28 % of children from the Middle East and Africa, in whom KD is still few reported.

“Tennis and golfer’s elbow” are common pathologies due to overload of forearm extensors and flexors, and actually occur mostly outside tennis and golf sports. Several differential diagnoses of medial and lateral epicondylitis have to be excluded as there are a number of other conditions with similar clinical symptoms. The high rate of spontaneous recovery has to be considered in treatment. Evidence based conservative treatment comprises excentric physiotherapy, local injections, and physical methods. Surgery is reserved for patients with persistence of symptoms for more than one year despite non-surgical treatment.

Abstract Background: A novel paediatric disease, multi-system inflammatory syndrome in children, has emerged during the 2019 coronavirus disease pandemic. Objectives: To describe the short-term evolution of cardiac complications and associated risk factors in patients with multi-system inflammatory syndrome in children. Methods: Retrospective single-centre study of confirmed multi-system inflammatory syndrome in children treated from 29 March, 2020 to 1 September, 2020. Cardiac complications during the acute phase were defined as decreased systolic function, coronary artery abnormalities, pericardial effusion, or mitral and/or tricuspid valve regurgitation. Patients with or without cardiac complications were compared with chi-square, Fisher’s exact, and Wilcoxon rank sum. Results: Thirty-nine children with median (interquartile range) age 7.8 (3.6–12.7) years were included. Nineteen (49%) patients developed cardiac complications including systolic dysfunction (33%), valvular regurgitation (31%), coronary artery abnormalities (18%), and pericardial effusion (5%). At the time of the most recent follow-up, at a median (interquartile range) of 49 (26–61) days, cardiac complications resolved in 16/19 (84%) patients. Two patients had persistent mild systolic dysfunction and one patient had persistent coronary artery abnormality. Children with cardiac complications were more likely to have higher N-terminal B-type natriuretic peptide (p = 0.01), higher white blood cell count (p = 0.01), higher neutrophil count (p = 0.02), severe lymphopenia (p = 0.05), use of milrinone (p = 0.03), and intensive care requirement (p = 0.04). Conclusion: Patients with multi-system inflammatory syndrome in children had a high rate of cardiac complications in the acute phase, with associated inflammatory markers. Although cardiac complications resolved in 84% of patients, further long-term studies are needed to assess if the cardiac abnormalities (transient or persistent) are associated with major cardiac events.

Background: The outbreak of COVID-19 in China in late 2019 was an unprecedented catastrophe that also involved many other countries, including Iran. Concerning the danger of disease contagion, it is necessary to detect asymptomatic or mild cases, especially in hospital staff who are highly exposed to the disease. Objectives: In this serosurvey study, we aimed to estimate IgG seroprevalence among hospital staff in two public hospitals to determine local transmission and infection risk factors, as well as protective immunity among high-risk populations. Methods: Screening was offered to the hospital staff of two public hospitals in Shiraz, Iran. Screening involved the measurement of IgG antibodies against SARS-CoV-2. Besides, a checklist that consisted of questions about sociodemographic, occupational, and epidemiological characteristics was completed by the participants. Results: Among 494 participants in this study, 29 (5.8%) had anti-SARS-CoV-2 IgG in their blood. Besides, 320 (64.8%) had at least one of the clinical symptoms within six months before this survey. Among participants with positive PCR, nine (21.4%) had anti-SARS-CoV-2 IgG, while this figure was seven (33.3%) for individuals with positive CT scans. Non-proper disposal of used protective equipment or infectious wastes (OR = 26.5), rotational daily work shifts (OR = 7.5), being anxious about getting COVID-19 (OR = 3.8), and age (OR = 1.06) were the significant determinants of having anti-SARS-CoV-2 IgG in the hospital staff. Conclusions: It is essential to continue training and giving technical consultations about COVID-19, especially the proper disposal of used protective equipment or infectious wastes in rotational daily shift workers.

The purpose of the study the improvement of early diagnostics of Lyme disease on the based of clinical and epidemiological analysis of the main clinical forms of the acute course of the disease in new natural foci in the Krasnodar Krai. Patients and methods. There was analyzed the clinical course of disease and epidemiological data for 207 patients in the early period of acute course of Lyme disease within the period from 2004 to 2013. Results. In the territory of the Krasnodar Krai and the Republic of Adygea there has formed a set of foci of tick-borne borreliosis (TB), i.e. Lyme disease (LD). The infection of cases is observed not only in the natural foci of disease, but also within the city limits. The disease is registered mainly in the erythematous form (74% of patients), non-erythematous form accounts for 26%. The average age of patients was 41,1±1,83, years, males 38%, females 62%. In the non-erythematous form initial clinical symptoms are recorded in average in 11,6 ± 2,20 days after tick suction; in the erythematous form in 6,4 ± 0,70 days ( p

AbstractBackgroundCreating a computational infrastructure to analyze the wealth of information contained in data repositories that scales well is difficult due to significant barriers in organizing, extracting and analyzing relevant data. Shared Data Science Infrastructures like Boa can be used to more efficiently process and parse data contained in large data repositories. The main features of Boa are inspired from existing languages for data intensive computing and can easily integrate data from biological data repositories.ResultsHere, we present an implementation of Boa for Genomic research (BoaG) on a relatively small data repository: RefSeq’s 97,716 annotation (GFF) and assembly (FASTA) files and metadata. We used BoaG to query the entire RefSeq dataset and gain insight into the RefSeq genome assemblies and gene model annotations and show that assembly quality using the same assembler varies depending on species.ConclusionsIn order to keep pace with our ability to produce biological data, innovative methods are required. The Shared Data Science Infrastructure, BoaG, can provide greater access to researchers to efficiently explore data in ways previously not possible for anyone but the most well funded research groups. We demonstrate the efficiency of BoaG to explore the RefSeq database of genome assemblies and annotations to identify interesting features of gene annotation as a proof of concept for much larger datasets.

Traditional taxonomic publications have served as a biological data repository accumulating vast amounts of data on species diversity, geographical and temporal distributions, ecological interactions, taxonomic relations, among many other types of information. However, the fragmented nature of taxonomic literature has made this data difficult to access and use to its full potential. Current anthropogenic impact on biodiversity demands faster knowledge generation, but also making better use of what we already have. This could help us make better-informed decisions about conservation and resources management. In past years, several efforts have been made to make taxonomic literature more mobilized and accessible. These include online publications, open access journals, the digitization of old paper literature and improved availability through online specialized repositories such as the Biodiversity Heritage Library (BHL) and the World Spider Catalog (WSC), among others. Although easy to share, PDF publications still have most of their biodiversity data embedded in strings of text making them less dynamic and more difficult or impossible to read and analyze without a human interpreter. Recently developed tools as GoldenGATE-Imagine (GGI) allow transforming PDFs in XML files that extract and categorize taxonomically relevant data. These data can then be aggregated in databases such as Plazi TreatmentBank, where it can be re-explored, queried and analyzed. Here we combined several of these cybertaxonomic tools to test the data extraction process for one potential application: the design and planning of an expedition to collect fresh material in the field. We targeted the ground spider Teutamus politus and other related species from the Teutamus group (TG) (Araneae; Liocranidae). These spiders are known from South East Asia and have been cataloged in the family Liocranidae; however, their relations, biology and evolution are still poorly understood. We marked-up 56 publications that contained taxonomic treatments with specimen records for the Liocranidae. Of these publications, 20 contained information on members of the TG. Geographical distributions and occurrences of 90 TG species were analyzed based on 1,309 specimen records. These data were used to design our field collection in a way that allowed us to optimize the collection of adult specimens of our target taxa. The TG genera were most common in Indonesia, Thailand and Malaysia. From these, Thailand was the second richest but had the most records of T. politus. Seasonal distribution of TG specimens in Thailand suggested June and July as the best time for collecting adults. Based on these analyses, we decided to sample from mid-July to mid-August 2018 in the three Thai provinces that combined most records of TG species and T. politus. Relying on the results of our literature analyses and using standard collection methods for ground spiders, we captured at least one specimen of every TG genus reported for Thailand. Our one-month expedition captured 231 TG spiders; from these, T. politus was the most abundant species with 188 specimens (95 adults). By comparison, a total of 196 specimens of the TG and 66 of T. politus had been reported for the same provinces in the last 40 years. Our sampling greatly increased the number of available specimens, especially for the genera Teutamus and Oedignatha. Also, we extended the known distribution of Oedignatha and Sesieutes within Thailand. These results illustrate the relevance of making biodiversity data contained within taxonomic treatments accessible and reusable. It also exemplifies one potential use of taxonomic legacy data: to more efficiently use existing biodiversity data to fill knowledge gaps. A similar approach can be used to study neglected or interesting taxa and geographic areas, generating a better biodiversity documentation that could aid in decision making, management and conservation.

Abstract OBJECTIVE To evaluate findings for patients with carotid cavernous fistulae or dural arteriovenous fistulae (AVFs) who underwent transvenous embolization via different transvenous approaches. METHODS Retrospective analysis of data for 31 patients (age range, 17–81 yr; mean age, 59.3 yr) with carotid cavernous fistulae (n = 6) or dural AVFs (cavernous sinus [CS], n = 11; transverse/sigmoid sinus, n = 14) was performed. The AVFs were treated with coils via different transvenous approaches, in 56 procedures. Doppler ultrasonography and time-resolved, two-dimensional, magnetic resonance projection angiography were performed to confirm the treatment. The mean clinical follow-up period was 32.5 months. RESULTS A total of 34 transvenous procedures were performed for 17 AVFs of the CS. Eleven patients with AVFs of the CS (63%) were cured with respect to clinical symptoms, and six patients experienced improvement (37%). The approach via the internal jugular vein and inferior petrosal sinus (n = 15) was possible in 60% of cases, with complete occlusion of the fistula in 78% of cases. With the approach via the facial vein (n = 8), there was a 50% success rate. The superior ophthalmic vein approach (n = 5) was associated with a high rate of technical success (100%), with a rate of complete fistula occlusion of 80%. We encountered complications, with transient morbidity, in four cases (23.5%). For 14 dural AVFs of the transverse/sigmoid sinus, 22 transvenous procedures were performed; 12 patients were cured (85.7%) and 2 experienced improvement (14.3%). The technical success rate was 86%, with complete occlusion in 42% of cases. Minor complications occurred in six cases (42.9%) but did not lead to permanent morbidity. CONCLUSION Transvenous treatment of CS and transverse/sigmoid sinus AVFs can be effective if all transvenous approaches, including combined surgical/endovascular approaches, are considered.

Background The overall severity of cardiac disease secondary to acute SARS-CoV-2 (severe acute respiratory syndrome coronavirus-2) infection in children appears to be much lower when compared with adults. However, the newly described multisystem inflammatory syndrome in children (MIS-C) associated with coronavirus disease 2019 (COVID-19) has been associated with cardiac complications. Methods We reviewed the clinical course and cardiac testing results in pediatric patients hospitalized with MIS-C at 2 large hospital systems in the New York City metropolitan area over a 3-month period. Results Of the 33 patients (median age 2.8 years) in the study cohort, 24 (73%) had at least one abnormality in cardiac testing: abnormal electrocardiogram (48%), elevated brain natriuretic peptide (43%), abnormal echocardiogram (30%), and/or elevated troponin (21%). Electrocardiogram and echocardiogram abnormalities all resolved by the 2-week outpatient follow-up cardiology visit. Conclusion While 73% of pediatric patients with MIS-C had evidence of abnormal cardiac testing on hospital admission in our study, all cardiac testing was normal by outpatient hospital discharge follow-up. Cardiac screening tests should be performed in all patients diagnosed with MIS-C given the high rate of abnormal cardiac findings in our study cohort.

BackgroundMalaria remains a public health issue particularly in sub-Saharan Africa with special features of seriousness in young children and pregnant women. Older children and adults reported to have acquired a semi-immune status and therefore show low parasitaemia and less pronounced symptoms if any. We aim to explore parasitaemia and symptoms in adolescents and adults with malaria compared to younger children in the Lambaréné area.MethodsWe conducted a cross-sectional study in CERMEL for a month in 2018. Adults and children were screened for malaria at health facilities in Lambaréné and Fougamou and their respective surrounding villages in the central region of Gabon. Malaria infection was determined either by rapid diagnostic test (RDT) or by thick blood smear (TBS). Body temperature, history of fever, age, sex, haemoglobin level, and location were collected.ResultsFrom 09 January to 09 February 2018, a total of 596 patients were screened and 280 (47%) belonged to the adult population. Plasmodium falciparum infection in adults was diagnosed based on the detection of P. falciparum in thick blood films. Prevalence among the adults was 56% (n=141); 34% of adult patients had a parasite count between 1015–116,079 pf/µl. Fever was found in 31% of adults and several symptoms were reported by the same patient. Vomiting, loss of appetite, and fatigue were significantly more present in the adult population than in children.ConclusionThis study shows a high rate of Plasmodium falciparum infection in adults associated to clinical symptoms and high level of parasitaemia. Adults seem to become an at-risk population, suggesting that malaria programmes should consider including adults in the implementation of a malaria prevention programme.

Huntington’s disease is a rare neurodegenerative disease caused by a cytosine–adenine–guanine (CAG) trinucleotide expansion in the Huntingtin (HTT) gene. Although Huntington’s disease (HD) is well studied, the pathophysiological mechanisms, genes and metabolites involved in HD remain poorly understood. Systems bioinformatics can reveal synergistic relationships among different omics levels and enables the integration of biological data. It allows for the overall understanding of biological mechanisms, pathways, genes and metabolites involved in HD. The purpose of this study was to identify the differentially expressed genes (DEGs), pathways and metabolites as well as observe how these biological terms differ between the pre-symptomatic and symptomatic HD stages. A publicly available dataset from the Gene Expression Omnibus (GEO) was analyzed to obtain the DEGs for each HD stage, and gene co-expression networks were obtained for each HD stage. Network rewiring, highlights the nodes that change most their connectivity with their neighbors and infers their possible implication in the transition between different states. The CACNA1I gene was the mostly highly rewired node among pre-symptomatic and symptomatic HD network. Furthermore, we identified AF198444 to be common between the rewired genes and DEGs of symptomatic HD. CNTN6, DEK, LTN1, MST4, ZFYVE16, CEP135, DCAKD, MAP4K3, NUPL1 and RBM15 between the DEGs of pre-symptomatic and DEGs of symptomatic HD and CACNA1I, DNAJB14, EPS8L3, HSDL2, SNRPD3, SOX12, ACLY, ATF2, BAG5, ERBB4, FOCAD, GRAMD1C, LIN7C, MIR22, MTHFR, NABP1, NRG2, OTC, PRAMEF12, SLC30A10, STAG2 and Y16709 between the rewired genes and DEGs of pre-symptomatic HD. The proteins encoded by these genes are involved in various biological pathways such as phosphatidylinositol-4,5-bisphosphate 3-kinase activity, cAMP response element-binding protein binding, protein tyrosine kinase activity, voltage-gated calcium channel activity, ubiquitin protein ligase activity, adenosine triphosphate (ATP) binding, and protein serine/threonine kinase. Additionally, prominent molecular pathways for each HD stage were then obtained, and metabolites related to each pathway for both disease stages were identified. The transforming growth factor beta (TGF-β) signaling (pre-symptomatic and symptomatic stages of the disease), calcium (Ca2+) signaling (pre-symptomatic), dopaminergic synapse pathway (symptomatic HD patients) and Hippo signaling (pre-symptomatic) pathways were identified. The in silico metabolites we identified include Ca2+, inositol 1,4,5-trisphosphate, sphingosine 1-phosphate, dopamine, homovanillate and L-tyrosine. The genes, pathways and metabolites identified for each HD stage can provide a better understanding of the mechanisms that become altered in each disease stage. Our results can guide the development of therapies that may target the altered genes and metabolites of the perturbed pathways, leading to an improvement in clinical symptoms and hopefully a delay in the age of onset.
