Abstract P712: Metabolic Syndrome is Associated With Risk of Ischemic Stroke in Patients With Non-Valvular Atrial Fibrillation- A National Estimate

Objective: Primary objective of this study is to find prevalence of metabolic syndrome (MetS) in patients with acute ischemic stroke (AIS) amongst non-valvular atrial fibrillation (NVAF). Secondary aim of this study is to identify the independent components of MetS (obesity, diabetes, hypertension, high TG, low HDL), those predict AIS and compare with CHA 2 DS 2 VASc score (CHF, HTN, age>75, DM, history of stroke/TIA/thromboembolism, history of vascular disease, sex: female) amongst NVAF. Methods: A cross-sectional study was performed to identify cohort of adult hospitalization with secondary diagnosis of NVAF from Nationwide Inpatient Sample of 2008-2017. From patients with NVAF, primary hospitalization for AIS and secondary diagnosis of MetS, its components, and components of CHA 2 DS 2 VASc score were identified using ICD 9 & 10 codes. Weighed analysis using multivariable survey logistic regression was performed to evaluate the predictive potential of MetS and its components as well as to compare them with CHA2DS2VASc for AIS. Results: Amongst 33,462,134 hospitalizations with NVAF, 2.9% were due to AIS. Amongst AIS hospitalizations prevalence of MetS was 0.16% (vs 0.20%) [obesity (8.4 vs 13.7%), DM (31.6 vs 34.1%), HTN (84.3 vs 75.9%), high TG (0.4 vs 0.2%), low HDL (0.02 vs 0.02%)] in comparison to non-AIS. (p<0.001) In regression analysis, patients with MetS [aOR 1.16 (95%CI 1.04-1.30)] and its component DM [1.3 (1.3-1.4)], HTN [2.2 (2.1-2.3)], high TG [1.8 (1.7-2.0)], low HDL [1.02 (0.7-1.4) weak significant] had higher risk of AIS. Amongst CHA 2 DS 2 VASc components besides HTN and DM, history of stroke/TIA, history of vasc. disease, female and age 65-74 and >75 had higher odds of AIS (Table 1) . Conclusion: MetS and its components high TG and low HDL independently predict a high risk of AIS. Future study would evaluate whether adding of high TG and low HDL in CHA 2 DS 2 VASc score and starting anticoagulants amongst patients MetS may mitigate the risk of AIS.

Introduction: The global prevalence of metabolic syndrome (MS) among people living with HIV/AIDS varies from 20% to 33%. Objective: to estimate the prevalence of metabolic syndrome and associated factors in a group of HIV-infected patients on antiretroviral therapy. Methods: This is a cross-sectional study with HIV-infected patients from a reference center in Bahia, Brazil. We evaluated clinical, socio-demographic and anthropometric data. MS was defined according to the guidelines of International Diabetes Federation. Results: We evaluated 152 patients with mean age of 47.3±11.6 years, 59.2% male. The main comorbidities detected were diabetes (3.3%) hypertriglyceridemia (9.3%) and metabolic syndrome (MS,38.2%). Patients with MS were predominantly women (55.2% vs 31.9%; p=0.005), older [52.1 (10.4) vs 44.3 (11.3); p<0.001], and had overweight (74.1% vs 23.4%; p<0.001). After multivariate analysis MS remained associated with age (OR = 1.076; 95% CI: 1.030 – 1.125), female sex (OR = 2.452; 95% CI: 1.114 – 5.374) and family history of hypertension (OR = 3.678; 95% CI: 1.431 – 9.395). Conclusion: Almost half of the HIV-infected patients in Bahia presents with MS which seems to be driven by classical risk factors.

Background: The role of short-lasting (<30 sec) runs of atrial fibrillation (AF) in ischemic stroke pathophysiology is currently unknown. Although these non-sustained attacks are considered as a risk factor for future development of longer lasting, classical AF episodes, prior research has highlighted that associated clinical stroke features are not entirely similar between these two types of arrhythmias. In this study we determined the prevalence of short-lasting AF in stroke-free controls and compared it to a consecutive series of ischemic stroke patients. Methods: A total 235 controls, without any prior history stroke or AF, were evaluated with ECG and 24-hour Holter monitoring for the presence of <30-sec or ≥30-sec lasting AF episodes. The results were compared to a consecutive series of ischemic stroke patients without prior history of AF (n=456). Univariate and multivariate analyses were performed to determine demographic and cardiovascular factors related to <30-sec lasting AF and its association with ischemic stroke. Results: Expectedly, the frequency of newly diagnosed ≥30-sec lasting AF, detected either on ECG or Holter monitoring, was significantly higher in patients with ischemic stroke (18% vs. 2%; p<0.01). Non-sustained AF was positively related to old age (p<0.01), female gender (p=0.01) and hypertension (p<0.01) in univariate analyses. In multivariate analyses, after adjustment for demographic and cardiovascular risk factors, presence of non-sustained AF was significantly higher among both cryptogenic (OR 1.78; 95% CI 1.02-3.10) and non-cryptogenic (OR 1.84; 95% CI 1.15-2.94) stroke patients with respect to controls. Conclusion: Our study shows a higher prevalence of non-sustained AF episodes in ischemic stroke patients in comparison to controls. Whether this cross-sectional association translates into causality in terms of stroke pathophysiology will be the subject of future studies.

The aim of this study was to estimate the prevalence of metabolic syndrome (MS) and its components in obese Mexican adolescents and to compare the clinical, anthropometric, and biochemical characteristics between patients with and without MS by sex. We conducted a cross-sectional study with a sample of 110 obese adolescents (boys and girls) from 8 to 16 years old (BMI ≥95th percentile), who were recruited in the pediatric obesity clinic of a third-level care hospital. A frequency analysis was used to estimate the prevalence of MS and its components, and the assessments were compared between the sexes and between the groups with and without MS using the Kruskal–Wallis test. The prevalence of MS was 62%. In order of prevalence, the following components of MS were observed in the sample: abdominal obesity (88%), high triglycerides (TG) (85%), low HDL-C (60%), hypertension (35%), and hyperglycemia (5%). In the groups with MS, hypertension (P<0.001), waist circumference (P=0.003), and TG (P=0.012) were significantly higher, and HDL-C (P<0.001) was significantly lower. In conclusion the prevalence of MS and its components is high among obese Mexican-Hispanic children. These findings show the importance of preventing and treating obesity in the early stages of life in order to decrease the incidence rates of cardiovascular disease and type 2 diabetes mellitus.

Background. Metabolic Syndrome (MetS) is a major public health concern. Objective. The aim of this study was to estimate the frequency of MetS, its components, and factors associated with MetS amongst apparently healthy individuals in Pakistan. Methods. A retrospective cross-sectional study was conducted at the executive Clinics of Aga Khan Hospital, Pakistan. Medical records of patients aged ≥18 years visiting the clinics from July 2011 to December 2011 were consecutively reviewed. Records in which either MetS components data or 10% of overall data was missing were excluded. A total of 1329 participants’ records was included in final analysis. Data was analyzed using SPSS version 19 and multivariable logistic regression was used to identify the factors associated with MetS. Results. A total of 847 (63.7%) participants had MetS; mean age of the participants were 47.6 ± 11.6 years. About 70.4% were males and 29.6% were females. Approximately 70% of participants had BMI ≥25 kg/m2. MetS was associated with male gender (AOR = 2.1; 95% C.I: 1.6–3.2) and history of diabetes among parents (AOR = 3.0; 95% C.I: 1.6–6.0). Conclusion. This study shows that a large proportion of population has MetS and is overweight or obese. This requires urgent interventions on part of health care providers’ especially family physicians. Educating masses about life style factors can make a difference. Further researches on this issue are warranted.

Metabolic syndrome (MetS) can worsen cerebral arterial atherosclerosis stenosis in patients with stroke; however, its effect on patients without stroke remains ambiguous. This study explored the association of MetS and its individual components with asymptomatic intracranial arterial stenosis (aICAS) and asymptomatic extracranial arterial stenosis (aECAS) among older Chinese adults. A total of 1988 participants from the Kongcun Town study aged ≥40 years and without a history of stroke were enrolled. The baseline data were obtained via face-to-face interviews. MetS was defined according to International Diabetes Federation criteria. Detection of aICAS was conducted using transcranial Doppler ultrasound, followed by diagnosis via magnetic resonance angiography. The evaluation of aECAS was performed using bilateral carotid ultrasonography. The aICAS and aECAS groups were 1:1 matched separately to the non-stenosis group by age and sex. The association between MetS and aICAS or aECAS was analyzed using multivariate logistic regression. Among the 1988 participants, 909 were diagnosed with MetS. The prevalence of MetS was higher in the aICAS group than in the non-stenosis group (P &lt;0.001), but did not differ significantly between the aECAS and non-stenosis groups. The prevalence of aICAS increased with the number of MetS components from 3.4% in the ≤ 1 component group to 12.7% in the ≥4 components group (P for trend &lt;0.001). After adjusting for confounding factors, MetS components associated with aICAS included central obesity, elevated triglyceride levels, and elevated blood pressure. None of the MetS components was associated with aECAS. MetS was positively associated with aICAS, but not with aECAS. Further, different components play different roles in the pathological process leading to aICAS.

Objective: The aim of the present study was to investigate the epidemiological pattern of factors associated with ischemic stroke among patients under 50 years old. Methods: The present cross-sectional study was performed on 197 Stroke patients. Individuals with confirmed ischemic stroke based on of CT scan were included in the study. Demographic information included age, sex, history of smoking, place of residence, season of disease incidence, history of contraceptive use in women, history of hypertension, having high blood pressure at the onset of diabetes, hyperlipidemia, heart disease and patient weight. Finally, the collected data was analyzed using SPSS ver. 22. Results: The mean age of participants was 40.18 years. The mean weight, height, body mass index (BMI) were 79.83, 168.63, and 28.12, respectively. A total of 60.4% of the participants were male and 39.6% were female. Moreover, 66.9% were urban residents, 38.6% had complications in winter, 67% were smokers, and 50% used contraceptives. The history of hypertension, diabetes, hyperlipidemia, and heart disease was seen in 69%, 49.7, 66%, and 73.6% of cases, respectively. Also, 53.8% of patients had high blood pressure upon admission. Conclusion: The present study showed that a history of heart disease and a history of hypertension in the past and nicotine use are very common in people with stroke. Considering the above mentioned epidemiological factors, it is recommended to prioritize the mentioned factors when identifying new cases

Abstract Background The prevalence of metabolic syndrome (MetS) of comprehensive population are always lower than mono- population. However, studies on MetS and its components of it and its subgroups (different ages, genders, races, regions) based on big data and using a same diagnosis criterion are rare. Method: A total of 9,745,640 Chinese adults aged ≥ 18 years in Xinjiang,the largest autonomous region of Multi-ethnic, were enrolled in the study. MetS was defined by modified Adults Treatment Panel (ATP III) criteria.Prevalence of metabolic syndrome and its components were calculated. To further explore the correlates of MetS and its components logistic regression were used. Results The overall prevalence of MetS was 20.85% [Sex: 20.06% in female, 21.56% in man; Age: 39.22% in 60 year older followed by 40–59 year 26.32% and 18–39 years 9.56%; Ethnicity: 28.19% in Hui followed by Han (26.39%), Uyghur(18.56%), Other(18.61%), Kazak(17.98%), Mongolian (17.87%), Kyrgyz(14.44%)], living in city and town (23.03%) or north(24.78%) had higher prevalence of MetS than living in rural (19.94%) and South (17.66%). Although the prevalence of hypertension, smoking, drinking and lack of physical activity in Kyrgyz, Mongolian and Kazakh were higher than those of other ethnic groups, the risk of MetS was lower than that of other. The results also showed that although their risk of central obesity and hypertension was higher than that of Han, Hui, Uygur and other, the risk of high-TG, low HDL-C and Elevated-FPG was significantly lower than that of other ethnic groups. Among them, the Mongolian had the lowest risk of high-TG and low-HDL-C, while the Kazakh had the lowest risk of elevated-FPG. In addition, except that the risk of central obesity in males was lower than that in females, the other components of males were higher than those in females, but the risk of MetS was lower than that in females. The risk of MetS and its components in the elderly was significantly higher than that in the young, and physical activity did not show reduce the risk of MetS in the general population. Conclusion The reason why the prevalence of MetS in the comprehensive population is lower than that in most mono-population may be that some subgroups of the population have the ability to resist risk factors in terms of genetic structure such as Kyrgyz, Mongolian, Kazak, and the influence in this aspect may exceed that of lifestyle. Within each subgroup, the prevalence of MetS is still increasing. Central obesity is an important risk factor for women with MetS. Elevated-FPG and high-TG are the most important risk factors of MetS in Han, Hui. Climate and lifestyle are the main causes of MetS in northern Xinjiang and city and town residents.

Hyperuricemia has been described as associated with the risk of development metabolic syndrome; however the relationship between the uric acid level and particular parameters of metabolic syndrome remained unclear. We performed a cross-sectional study on a cohort of 833 dyslipidemic patients and correlated their levels of uric acid with parameters of insulin resistance, lipid metabolism, C-reactive protein, anthropometric parameters. We also defined patients with hypertriglyceridemic waist phenotype and compered their uric acid levels with those without this phenotype. We found that levels of uric acid are associated with parameters of metabolic syndrome. Specifically, dyslipidemia characteristic for metabolic syndrome (low HDL cholesterol and high triglycerides) correlates better with uric acid levels than parameters of insulin resistance. Also waist circumference correlates better with uric acid levels than body mass index. Patients with hypertriglyceridemic waist phenotype had higher levels of uric acid when compared with patients without this phenotype. Serum uric acid levels are even in low levels linearly correlated with parameters of metabolic syndrome (better with typical lipid characteristics than with parameters of insulin resistance) and could be associated with higher cardiovascular risk.

Aims: The aim of this study was to investigate associations between psychosocial work exposure and the presence of biological and imaging biomarkers of cardiovascular disease. Methods: This cross-sectional study was conducted in a sub-cohort of the Swedish CArdioPulmonary bioImage Study (SCAPIS). Psychosocial exposure was evaluated with the job demand–control model, and analysed according to the standard categorization: high strain, active, passive and low strain (reference). Biomarkers (blood pressure, high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterol, coronary artery calcification (CAC) and metabolic syndrome) were measured, or derived through measurements, from clinical examinations. Gender-specific prevalence ratios (PRs) and 95% confidence intervals (CIs) were calculated with regression models and adjusted for age, education, smoking, physical activity, general life stress and body mass index (BMI). Results: The analyses included 3882 participants (52.5% women). High strain (high demands–low control) was linked to increased PR for low HDL cholesterol in women, adjusted for all covariates (PR 1.76; 95% CI 1.25–2.48). High strain was also related to moderately increased PR for metabolic syndrome in men, after adjustments for all covariates except BMI (PR 1.25; 95% CI 1.02–1.52). In addition, passive work (low demands–low control) was associated with diastolic hypertension in women (fully adjusted: PR 1.29; 95% CI 1.05–1.59). All relationships between psychosocial factors and LDL cholesterol or CAC (both genders), or hypertension (men), were non-significant. Conclusions: Poor psychosocial job conditions was associated with the presence of low HDL cholesterol and diastolic hypertension in women, and metabolic syndrome in men. These findings contribute to the knowledge of potential pathways between stressful work and coronary heart disease.

Metabolic syndrome (MetS) is a cluster of cardiometabolic risk factors. Many people may be unaware of their risk for MetS. A cross-sectional, descriptive study was conducted among hospitalized patients with at least one cardiometabolic risk factor in Mainland China. This study assessed the MetS knowledgelevel(through MetS Knowledge Scale, MSKS) and examined the potential predictors by regression analysis. A total of 204 patients aged 58.5 ± 10.1 years (55% males) participated in this study. The majority of participants had no history of hypertension (54%), dyslipidemia (79%), or diabetes (85%). However, 56% of these participants had at least three cardiometabolic risk factors, indicating the presence of MetS. The average MSKS was very low (mean = 36.7±18.8, possible range = 0–100), indicating the urgent needs of MetS education in current practice. Predictors of better MetS knowledge included higher educational level, history of dyslipidemia, and normal high-density lipoprotein cholesterol (F (8, 195) = 9.39, adjusted R2 = 0.192, p< 0.001). In conclusion, adults with cardiometabolic risk factors are at risk of developing MetS, but with a low level of knowledge. Specific health education on MetS should be provided, particularly for those with limited formal education or inadequate lipid management.
