Impact of the Physical Environment of Residential Health, Care, and Support Facilities (RHCSF) on Staff and Residents

The physical environment of residential health, care, and support facilities (RHCSF) is a critical component in providing supportive and resident-centered care for the elderly. Although past reviews of the literature have focused on specific settings for the elderly or specific residential populations of the elderly, a systematic literature review that covers the range of settings and populations has been lacking. This article provides an overview and synthesis of the most recent empirical evidence addressing the impact of the physical environment on residents and staff of RHCSFs. The review also identifies gaps where future research is needed. This review found 66 studies examining the relationship between the built environment and outcomes in three broad domains of resident quality of life, resident safety, and staff and organizational outcomes. The studies address a range of topics including the impact on elderly residents of the facility scale and size, outdoor environments, and environmental quality.

 Resumo: Com o aumento da população idosa, e por ser uma fase da vida frágil e conforme os aparecimentos de doenças, com o encerramento trajetória profissional, perda familiar, mudança de hábitos, tudo isso vem o sentimento de frustações, é comum que nessa fase os idosos fiquem deprimidos. Tornando assim bastante importante o acompanhamento qualificado e humanizado por parte dos profissionais de saúde. Por tanto cabe a estes profissionais proceder em buas de uma assistência adequada. Objetivo desse estudo é analisar e descrever a importância da assistência em saúde ao paciente com depressão na terceira idade. Metodologia: trata-se de uma revisão integrativa da literatura, que foi realizada através da pesquisa bibliográfica com os seguintes descritores: Terceira idade, depressão, assistência da enfermagem e saúde do idoso.com os estudos dos artigos selecionados os resultados foram distribuídos em três categorias temáticas: Fatores contribuintes para depressão na terceira idade; Ações da enfermagem para prevenir e amenizar a depressão na terceira idade; A importância da assistência qualificada do profissional da enfermagem ao idoso com depressão. Notou-se o quanto é importante uma assistência adequada e humanizada da enfermagem ao idoso com depressão, elaborando assim ações que vá ajudar o idoso na prevenção à amenização dos sintomas da depressão. Espero que esse estudo ajude com futuras pesquisa relacionado a esse tema, fortalecendo o interesse dos enfermeiros a saúde do idoso.   Palavras-chaves: Depressão. Terceira idade. Assistência. Saúde do idoso.Abstract: With the increase of the elderly population, and because it is a fragile stage of life and according to the onset of diseases, with the closure of the professional trajectory, family loss, change of habits, all this comes with the feeling of frustration, it is common for the elderly at this stage get depressed. Thus, qualified and humanized monitoring by health professionals is very important. Therefore, it is up to these professionals to provide adequate assistance. The aim of this study is to analyze and describe the importance of health care for patients with depression in the elderly. Methodology: this is an integrative literature review, which was carried out through bibliographical research with the following descriptors: Third age, depression, nursing care and elderly health. With the studies of the selected articles, the results were divided into three categories thematic: Contributing factors for depression in old age; Nursing actions to prevent and alleviate depression in old age; The importance of qualified nursing professional assistance for the elderly with depression. It was noted how important an adequate and humanized nursing care for the elderly with depression is, thus developing actions that will help the elderly in preventing the alleviation of symptoms of depression. I hope this study will help with future research related to this topic, strengthening nurses' interest in the health of the elderly. Keywords: Depression. Third Age. Assistance. Elderly health.

For some people, dementia is considered as a disease that is common in elderly, regardless the impact of dementia. Taking care for the elderly with dementia brings stress for the family. It can cause and increase the family burden. Brain vitalization gymnastics is one of the methods to improve memory. The study aims to investigate the effect of brain vitalization activity on dementia incidence in elderly at Budi Luhur Nursing Home of Yogyakarta. The study used Quasi Experimental with Pretest-Posttest control group and randomized sampling system. The samples were taken randomly as many as 26 respondents and were divided into two groups namely 13 respondents of experimental group and 13 respondents of control group. The statistical test used Wilcoxon Match Pairs Test. The result showed that Wilcoxon Match pairs test obtained p value 0,003, which is smaller than 0,005. There is an effect of brain vitalization activity on dementia incidence in elderly at Budi Luhur Nursing Home of Yogyakarta

Health care systems for elderly people should aim to delay the onset of illness, reducing the final period of infirmity and illness to the shortest possible time. The most effective way to achieve this is by health education and preventative medicine to maintain mobility and function. Changes in life style even in late life may result in improved health, effectively decreasing the incidence of chronic diseases associated with advancing age. This paper presents the problems experienced by elderly persons with chronic diseases and disabilities with indications for meaningful therapeutic interventions.

Profit margins at most hospitals across the country declined or stagnated last year, reflecting growing pressure on them to reduce costs. And health care executives said many hospitals would be under even greater pressure in 1995 if Congress enacted proposals that would slash spending for medical care for the elderly and the poor. At investor-owned hospitals, the outlook is brighter, because many of them have moved aggressively to merge and cut costs. Profit at these hospitals has risen in the 1990s.

Purpose The purpose of this paper is to explore the impact of self-care on the prevalence of loneliness among elderly people living in retirement homes and older people living in their homes/communities. Design/methodology/approach The research was conducted through standardized SELSA-L assessment loneliness questionnaires and the Self Care Assessment Worksheet for self-care assessment. The results were processed using the Kruskal–Wallis test. Findings The obtained results have shown that impaired self-care ability affects the prevalence of loneliness among the elderly almost the same in both groups of participants. However, regarding the relationship between the state of health and self-care, only a statistically significant difference in the prevalence of loneliness is found in the case of the participants living in their homes, with the worst health condition affecting the poorer psychological care. Research limitations/implications It is necessary to point out the limitations of the research, primarily sample limitations and the selected design of the study. The sample consisted of two different and relatively small groups of participants which could adversely affect the representativeness of the sample and reduce the possibility of generalising the results. The next limiting factor is the age distribution the authors used in the research, where the age of the participants as a very important variable was collected by age range and not precisely which consequently resulted in inequality in subgroup sizes. Thus, the middle age (75–85) covers up to ten years, which is a huge range at an older age and can mean major differences in functional ability, and can impact the self-care assessment. Practical implications Nurses are indispensable in care for the elderly and they need to promote and encourage self-care of the elderly through health care. Elderly people living in retirement homes should be allowed to participate equally in health care in order to preserve their own autonomy and dignity. However, to benefit those who live in their homes, nurses should be connected to the local community and thus stimulate various forms of preventative (testing blood sugar levels, blood pressure and educating on the importance of preventive examinations) or recreational activities in the environment of elderly people with the goal of preserving their functional abilities. Originality/value The impact of self-care on loneliness was not sufficiently researched, and this paper contributed to understanding the complexity of loneliness phenomena among the elderly with the aim of developing a model of prevention.

Clinician teamwork is effective at improving many health care outcomes, and the physical environment is an important part of a system that facilitates teamwork. This review critically evaluates and synthesizes the empirical evidence on the impacts of the physical environment and surrounding ecosystem on clinician teamwork in health care facilities using a systems-based lens. The systematic search yielded 2,323 titles and abstracts between 2007 and 2017, which were reviewed and resulted in 34 articles examining physical environment’s impact on clinician teamwork. The Systems Engineering for Patient Safety (SEIPS) framework is utilized to thematically structure findings to examine the interdependent nature of built and nonbuilt factors that have a documented impact on clinician teamwork. This study found consistent evidence of the interdependence of these systems factors in facilitating or hampering teamwork, as defined by Salas and colleagues’ “Big Five.” The review identifies current gaps in the literature and suggests where future research is needed.
