Social Capital and Self-Perceived Health in Lone Mothers: A Multilevel Cross-Sectional Study across Europe

This study aimed to explore the distribution of social capital and its relation to self-perceived health in lone mothers across Europe. Data were drawn from the European Social Survey Round 5. The sample was restricted to women (15–64 years), not cohabiting with a partner, and with children (≤ 18 years) living in the household. Social capital was measured using variables, representing both structural (political engagement, social support, and social activity) and cognitive (generalized trust, institutionalized trust, reciprocity, and a feeling of safety) components. Individual-level measurements: age, educational attainment, employment status, income level, and household economy. Country-level measurements: family policy model and collective social capital. A multilevel analysis was conducted. The results revealed cross-country variance in the level of lone mothers’ social capital. After adjustment for individual-level and country-level measurements, only reciprocity and a feeling of safety were related to good self-perceived health among lone mothers in Europe.

IntroductionExposure to secondhand aerosol from e-cigarette (SHA) may pose harmful effects to bystanders. This study aims to investigate the prevalence, duration and determinants of SHA exposure in various indoor settings in 12 European countries.MethodsIn 2017–2018, we conducted a cross-sectional study, the TackSHS survey, on a representative sample of the population aged ≥15 years in 12 European countries (Bulgaria, England, France, Germany, Greece, Ireland, Italy, Latvia, Poland, Portugal, Romania and Spain). We described the prevalence and duration of exposure to SHA in several indoor settings among 11 604 e-cigarette non-users. Individual-level and country-level characteristics associated with SHA exposure were also explored using multilevel logistic regression analyses.ResultsOverall, 16.0% of e-cigarette non-users were exposed to SHA in any indoor setting at least weekly, ranging from 4.3% in Spain to 29.6% in England. The median duration of SHA exposure among those who were exposed was 43 min/day. ‘Other indoor settings’ (eg, bar and restaurant) was reported as the place where most of e-cigarette non-users were exposed (8.3%), followed by workplace/educational venues (6.4%), home (5.8%), public transportation (3.5%) and private transportation (2.7%). SHA exposure was more likely to occur in certain groups of non-users: men, younger age groups, those with higher level of education, e-cigarette past users, current smokers, those perceiving SHA harmless and living in countries with a higher e-cigarette use prevalence.ConclusionsWe found inequalities of SHA exposure across and within European countries. Governments should consider extending their tobacco smoke-free legislation to e-cigarettes to protect bystanders, particularly vulnerable populations such as young people.Trial registration numberNCT02928536.

ObjectiveTo explore the independent contribution of individual-level and country level socioeconomic status (SES) determinants to disease activity and physical function in patients with spondyloarthritis (SpA).MethodsData from the cross-sectional, multinational (n=22 countries worldwide) COMOSPA (COMOrbidities in SpA) study were used. Contribution of individual SES factors (gender, education) and country of residence to Ankylosing Spondylitis Disease Activity Score (ASDAS) and Bath Ankylosing Spondylitis Functional Index (BASFI) was explored in multilevel regression models, adjusting for clinical and demographic confounders. Next, the additional effects of national macroeconomic indicators (gross domestic product [GDP], Human Development Index, healthcare expenditure and Gini index) were explored. The mediating role of uptake of biologic disease-modifying antirheumatic drugs between education or GDP and ASDAS was explored by testing indirect effects.ResultsIn total, 3370 patients with SpA were included: 65% were male, with a mean age of 43 (SD 14), ASDAS of 2.0 (SD 1.1) and BASFI score of 3.1 (SD 2.7). In adjusted models, patients with low education and female patients had an OR of 1.7 (95% CI 1.3 to 2.2) and an OR of 1.7 (95% CI 1.4 to 2.0), respectively, of having ASDAS ≥2.1. They also reported slightly worse function. Large country differences were observed independent of individual SES and clinical confounders. Patients from less SES developed countries have worse ASDAS, while patterns for BASFI were insignificant. Uptake of biologicals did not mediate the relationship between individual-level or country-level SES and disease activity.ConclusionsIndividual-level and country-level health inequalities exist also among patients with SpA. Women and lower educated persons had worse disease activity and somewhat worse physical function. While patients in less socioeconomically developed countries had higher disease activity, they reported similar physical function.

ObjectiveInequalities exist between the 23-valent pneumococcal polysaccharide vaccination (PPSV23) rate in each municipality among Japanese older adults. Exploring individual-level and community-level intervenable factors is necessary to improve the vaccination rates. We examined the associations between community-level and individual-level social capital and the PPSV23 vaccination among older Japanese adults using multilevel Poisson regression analyses.DesignCross-sectional study.SettingWe used data from the Japan Gerontological Evaluation Study, conducted between 3 October 2016 and 10 January 2017 in 631 districts, 39 municipalities and 18 prefectures.ParticipantsThe target population comprised persons aged 65 years or older who are physically and cognitively independent (that is, not certified as needing long-term care). Further, 180 021 older adults from 39 Japanese municipalities were enrolled.Primary outcome measureThe primary outcome was the PPSV23 vaccination among the Japanese older adults aged 65 years or older who did not have physical or cognitive disabilities.ResultsAfter adjusting for municipality-, community-, individual-levels effects with multiple imputation, 137 075 individuals who participated in one/more of the civic participation (participation of social groups), social cohesion (social tie), or reciprocity (mutual exchange of social support) were significantly associated with more vaccinations than those without the three social capitals among the 137 075 older adults (13.0% (95% CI 11.0% to 14.9%), 5.0% (95% CI 2.4% to 7.6%) or 33.9% (95% CI23.6% to 44.2%) increase, respectively, p>0.001 for all). The rich (≥+1 SD) community-level civic participation was significantly associated with 3.4% increase [95% CI 0.02% to 6.78%, p<0.05] of the PPSV23 vaccination among the older adults compared to those with the poor or standard one.ConclusionsOlder adults with one/more of the three social capitals at the both levels received more PPSV23 vaccinations than those without those social capitals. Therefore, fostering of those social capitals may improve the inequality of the PPSV23 vaccination rate among older adults in each municipality.

Abstract Background Life satisfaction is an important component in designing strategies to improve health outcomes in different groups of society. This study aimed to investigate the effect of subjective socioeconomic status (SSS), social capital (SC), self-rated health (SRH), and physical activity (PA) on life satisfaction (LS) in Iran. Methods This cross-sectional study was conducted on 1187 people (643 men and 544 women) lived in five western cities in Iran. The sampling method was multistage clustering. Data collection tool was a five part questionnaire including demographic characteristics, socioeconomic status ladder, social capital scale, a question to measure physical activity, and the life satisfaction scale. Data were analyzed using independent t-test, one way ANOVA, and Ordinal Logistic Regression. Result Life satisfaction was higher in married men and women compared to single and widows (p < 0.05). Among the variables included in the main model, the significant predictors were college education (− 0.500), marriage (coefficient = 0.422), age 25–34 years (coefficient = − 0.384), SRH (coefficient = 0.477), male sex (coefficient = 0.425), SSS (coefficient = 0.373), trust (coefficient = 0.115), and belonging and empathy (coefficient = 0.064). Conclusion SRH and SSS were significant predictors of life satisfaction in west Iranian society. Being married was associated with higher LS, but college education affects LS adversely.

ABSTRACT Background The gold standard to assess salt intake is 24-h urine collections. Use of a urine spot sample can be a simpler alternative, especially when the goal is to assess sodium intake at the population level. Several equations to estimate 24-h urinary sodium excretion from urine spot samples have been tested in adults, but not in children. Objective The objective of this study was to assess the ability of several equations and urine spot samples to estimate 24-h urinary sodium excretion in children. Methods A cross-sectional study of children between 6 and 16 y of age was conducted. Each child collected one 24-h urine sample and 3 timed urine spot samples, i.e., evening (last void before going to bed), overnight (first void in the morning), and morning (second void in the morning). Eight equations (i.e., Kawasaki, Tanaka, Remer, Mage, Brown with and without potassium, Toft, and Meng) were used to estimate 24-h urinary sodium excretion. The estimates from the different spot samples and equations were compared with the measured excretion through the use of several statistics. Results Among the 101 children recruited, 86 had a complete 24-h urine collection and were included in the analysis (mean age: 10.5 y). The mean measured 24-h urinary sodium excretion was 2.5 g (range: 0.8–6.4 g). The different spot samples and equations provided highly heterogeneous estimates of the 24-h urinary sodium excretion. The overnight spot samples with the Tanaka and Brown equations provided the most accurate estimates (mean bias: −0.20 to −0.12 g; correlation: 0.48–0.53; precision: 69.7–76.5%; sensitivity: 76.9–81.6%; specificity: 66.7%; and misclassification: 23.0–27.7%). The other equations, irrespective of the timing of the spot, provided less accurate estimates. Conclusions Urine spot samples, with selected equations, might provide accurate estimates of the 24-h sodium excretion in children at a population level. At an individual level, they could be used to identify children with high sodium excretion. This study was registered at clinicaltrials.gov as NCT02900261.

BACKGROUND The majority of patients use the internet to search for health information. While there is a vast repository of searchable information online, much of the content is unregulated and therefore potentially incorrect, conflicting, or confusing. Abortion information online is particularly prone to being inaccurate as anti-choice websites publish purposefully misleading information in formats that appear as neutral resources. To understand how anti-choice websites appear neutral, we need to understand the specific website features of anti-choice features that impart trust to viewers. OBJECTIVE We sought to identify characteristics of false or misleading abortion websites that make these websites appear trustworthy to the public. METHODS We conducted a cross-sectional study using Amazon.com Inc’s Mechanical Turk platform. We used validated questionnaires to ask participants to rate eleven anti-choice sites and one neutral site identified by experts, focusing on site content, creators, and design. We collected socio-demographic data and participant views on abortion. We used a composite measure of “mean overall trust” as our primary outcome. Using correlation matrices, we determined which website characteristics were most associated with overall trust. Finally, we used linear regression to identify participant characteristics associated with overall trust. RESULTS Our analytic sample included 498 participants ranging from ages 22 to 70, 50.1% of whom identified as female. Across eleven anti-choice sites, creator trust (“I believe that the creators of this site are honest and trustworthy”) had the highest correlation coefficient with overall trust (0.70). Professional appearance (0.59), look and feel (0.59), perception that the information is created by experts (0.59), association with a trustworthy organization (0.58), valued features and functionalities (0.54), and interactive capabilities (0.52) all demonstrated strong relationships with overall trust. At the individual level, pro-choice leaning was associated with higher overall trust of the neutral site (B: -0.43, CI: -0.87, 0.01) and lower overall trust of the anti-choice sites (B: 0.52, CI: 0.05, .99). CONCLUSIONS The overall trustworthiness of anti-choice websites is most associated with design characteristics and perceived trustworthiness of the site creator. Those who believe that access to abortion should be limited are more likely to trust anti-choice websites.
