Diagnosis of Ganglions in the Hand and Wrist by Sonography

In the practice of hand surgery, imaging of soft tissue structures can provide useful information for diagnosis and preoperative planning. Sonography with high frequency technique giving high spatial resolution is especially rewarding. In a joint project of the Departments of Radiology and Hand Surgery we evaluated the diagnostic accuracy and clinical value of sonographic assessment of ganglions of the hand and wrist. The material comprises 68 soft tissue lesions clinically suspected to be ganglions. Sonography revealed a ganglion in 53 cases, 34 of which went to surgery. The diagnosis was confirmed in every case. One collapsed ganglion was missed. Nonpalpable ganglions causing clinical symptoms could be demonstrated, and a suspected ganglion could sometimes be ruled out in favor of other diagnoses. Sonography could delineate the entire ganglion and often its connection with the joint space.

Описано применение акустического зондирования для исследования акустических характеристик верхнего слоя моря с использованием широкополосных остронаправленных инвертированных излучателей,устанавливаемых на дно. В основу метода положен принцип регистрации обратного рассеяния и отраженияот поверхности моря акустических импульсов с различной частотой, позволяющий одновременно измерятьрассеяние и поглощение звука и нелинейный акустический параметр морской воды. Многочастотное зондирование позволяет реализовать акустическую спектроскопию пузырьков в приповерхностных слоях моря,проводить оценку газосодержания и получать данные о спектре поверхностного волнения при различных состояниях моря вплоть до штормовых. Применение остронаправленных высокочастотных пучков ультразвукапозволяет разделить информацию о планктоне и пузырьках и определить с высоким пространственным разрешением структуру пузырьковых облаков, образующихся при обрушении ветровых волн, и структуру планктонных сообществ. Участие планктона в волновом движении в толще морской воды позволяет определитьпараметры внутренних волн спектр и распределение по амплитудам в различное время.This paper represents the application of acoustic probingfor the investigation of acoustical properties of the upperlayer of the sea using broadband narrow-beam invertedtransducers that are mounted on the sea bottom. Thismethod is based on the principle of the recording of thebackscattering and reflections of acoustic pulses of differentfrequencies from the sea surface. That simultaneouslyallows measuring scattering and absorption of the soundand non-linear acoustic parameter of seawater. Multifrequencyprobing allows performing acoustic spectroscopy ofbubbles in the near-surface layer of the sea, estimating gascontent, and obtaining data on the spectrum of the surfacewaves in various states of the sea up to a storm. Utilizationof the high-frequency narrow ultrasound beams allows us toseparate the information about plankton and bubbles and todetermine the structure of bubble clouds, created during thebreaking of wind waves, along with the structure of planktoncommunities with high spatial resolution. The participationof plankton in the wave motion in the seawater columnallows determining parameters of internal waves, such asspectrum and distribution of amplitudes at different times.

AbstractAlloplastic facial transplantation has become a new rung on the proverbial reconstructive ladder for severe facial wounds in the past couple of decades. Since the first transfer including bony components in 2006, numerous facial allotransplantations across many countries have been successfully performed, many incorporating multiple bony elements of the face. There are many unique considerations to facial transplantation of bone, however, beyond the considerations of simple soft tissue transfer. Herein, we review the current literature and considerations specific to bony facial transplantation focusing on the pertinent surgical anatomy, preoperative planning needs, intraoperative harvest and inset considerations, and postoperative protocols.

Identification of post-kala-azar dermal leishmaniasis (PKDL) is important due to the long and toxic treatment and the fact that PKDL patients may serve as a reservoir for visceral leishmaniasis (VL). We summarized the published literature about the accuracy of diagnostic tests for PKDL. We searched Medline for eligible studies investigating the diagnostic accuracy of any test for PKDL. Study quality was assessed using QUADAS-2. Data were extracted from 21 articles including 43 separate studies. Twenty-seven studies evaluated serological tests (rK39 dipstick, ELISA, DAT, and leishmanin tests), six studies molecular tests, eight microscopy, and two cultures. Only a few of these studies reported a valid estimate of diagnostic accuracy, as most were case-control designs or used a reference standard with low sensitivity. The included studies were very heterogeneous, for example, due to a large variety of reference standards used. Hence, no summary estimates of sensitivity or specificity could be made. We recommend well-designed diagnostic accuracy trials that evaluate, side-by-side, all currently available diagnostics, including clinical symptoms, serological, antigen, molecular, and parasitological tests and possible use of statistical modelling to evaluate diagnostics when there is no suitable gold standard.

In a prospective investigation the diagnostic accuracy of film-screen and digital radiography in rheumatoid arthritis of hands was compared. Seventy hands of 36 patients with established rheumatoid arthritis were included in the study. Each of 11 joints in every hand was evaluated regarding the following radiologic parameters: soft tissue swelling, joint space narrowing, erosions and periarticular osteopenia. The digital images were obtained with storage phosphor image plates and evaluated in 2 forms; as digital hard-copy on film and on a monitor of an interactive workstation. The digital images had a resolution of either 3.33 or 5.0 lp/mm. ROC curves were constructed and comparing the area under the curves no significant difference was found between the 3 different imaging forms in either resolution group for soft tissue swelling, joint space narrowing and erosions. The film-screen image evaluation of periarticular osteopenia was significantly better than the digital hard-copy one in the 3.33 lp/mm resolution group, but no significant difference was found in the 5.0 lp/mm group. These results support the view that currently available digital systems are capable of adequate diagnostic performance.

To explore the clinical value of octreotide therapy and nursing intervention in patients with acute pancreatitis. 120 patients with acute pancreatitis admitted to the Department of Digestive Medicine from June 2018 to August 2019 were selected. Two groups of patients were treated with octreotide. They were randomly divided into observation group (intensive nursing intervention) and control group (routine nursing) with 60 patients in each group. Compared with the control group (81.67%), the total effective rate (93.33%) was higher in the observation group (P<0.05%); After treatment, C reactive protein, lactate dehydrogenase, WBC and hemodiastase of patients in the observation group were better than those in the control group (P<0.05); compared with the control group (18.33%), the occurrence rate of hypoglycemia in the observation group (6.67%) was lower (P<0.05%); At the same time, the Nursing Satisfaction Scale of the observation group (95.00%) was higher than that of the control group (78.33%), (P<0.05%). Patients with acute pancreatitis treated with octreotide and strengthened nursing intervention can obtain more obvious therapeutic effect, so that the clinical symptoms and related laboratory indicators of patients have been significantly improved. At the same time, the clinical value was significant with a lower occurrence rate of hypoglycemia and higher nursing satisfaction scale.

Abstract Background and Objective: Detection of radiolucent soft-tissue foreign bodies is a challenging problem, which is especially further complicated when retained foreign body is highly suggested by clinicians but radiography is negative. So, blind exploration is sometimes hazardous for patients. The purpose of this study was to determine the accuracy of ultrasonography (USG) in detecting radiolucent soft-tissue foreign bodies in the extremities. Materials and Methods: From November 2011 to January 2012, patients with clinically suspected radiolucent soft-tissue foreign body and negative radiography were evaluated by USG with a 12-MHz linear array transducer. The patients with positive clinical and USG examination were included in our study and underwent exploration or USG removal. Results: Fifty-one patients underwent foreign body removal under ultrasonography-guided or surgical exploration and 47 patients had foreign body (31, 12, 3, and 1 case had thorn, wood, glass, and plastic, respectively). Ultrasound was positive in 50 patients. USG falsely predicted the presence of foreign body in four cases and was falsely negative in one of the cases. Accuracy, sensitivity, and positive predictive value were determined as 90.2%, 97.9%, and 92%, respectively. Conclusions: The real-time high-frequency USG is a highly sensitive and accurate tool for detecting and removing radiolucent foreign bodies which are difficult to be visualized by routine radiography.

With an ever-growing aging population, the prevalence of normal pressure hydrocephalus (NPH) is increasing. Clinical symptoms of NPH include cognitive impairment, gait disturbance, and urinary incontinence. Surgery can improve symptoms, which leads to the disease's alternative name: treatable dementia. The Evans index (EI), defined as the ratio of the maximal width of the frontal horns to the maximum inner skull diameter, is the most commonly used index to indirectly assess the condition of the ventricles in NPH patients. EI measurement is simple, fast, and does not require any special software; in clinical practice, an EI &gt;0.3 is the criterion for ventricular enlargement. However, EI's measurement methods, threshold setting, correlation with ventricle volume, and even its clinical value has been questioned. Based on the EI, the z-EI and anteroposterior diameter of the lateral ventricle index were derived and are discussed in this review.

Background: Timely diagnosis and intervention of acute appendicitis reduces morbidity and mortality associated with the disease condition. The study aimed to evaluate the etiology of acute appendicitis, to analyze the sensitivity of modified Alvarado scoring system and radiology in the diagnosis of acute appendicitis and to correlate the observations of laboratory tests, operative findings with the histopathological report of specimen of appendix.Methods: This was a prospective study done on 100 patients with clinical symptoms of acute right lower abdominal pain suggestive of appendicular origin during the period from February 2015 to January 2016 in the department of surgery thorough clinical assessment, laboratory investigations, ultrasound findings as were done for all patients. After confirming the diagnosis of AA the patients had operative intervention and specimens were sent for histopathological study.Results: Male preponderance was seen in the study. Majority of them belongs to 21 to 30 years age group (50%). Faecolith was the most common etiological factor observed (58%). Abdominal pain (100%) was the most common clinical symptom. Alvarado score had sensitivity of 95.74% and specificity of 66.67% in diagnosing AA. In correlation to histopathological findings, ultrasonography findings showed 100% positive visualization rate in all 71 cases. Elevated ESR (94%) had high diagnostic accuracy as confirmed by HPE finding (96.81%) which is statistically significant (p<0.000).Conclusions: Alvarado scoring system, elevated ESR levels and USG findings of the appendix can be considered as adjuncts to clinically diagnose the AA, to improve the diagnostic accuracy thereby consequently the rate of negative appendicectomy can be reduced and thus decreases the complication rates.
