Absorption of Penicillin V from Mixture Formulations. Comparison of Potassium and Benzathine Salts

The absorption rate and serum level curves of two commercial phenoxymethylpenicillin mixture preparations were compared in adult volunteers. Both the potassium salt (Primcilliri®) and the benzathine salt (V-Pen ped forte®) of V-penicillin were rapidly absorbed and the mean peak serum levels were obtained with both preparations within the first hour after single dose administration. The mean peak level obtained with the potassium salt was several-fold higher (p < 0.001) and was also reached within a shorter time than that of the benzathine preparation. These results confirm the earlier evidence, obtained with other formulae, for the superior bioavailability of the potassium salt of V-penicillin as compared to most other derivatives used in V-penicillin mixtures.

The bioavailability of the calcium and potassium salts of Phenoxymethylpenicillin (dose 38,000 I.U./kg) was investigated in eight healthy adult volunteers. Administration of the calcium salt as an aqueous oral mixture resulted in a mean peak plasma concentration of 8·52 mg/l (SD 1·96) and that of the potassium salt mixture in a concentration of 8·40 mg/ml (SD 2·61), p > 0·1. The median time-to-peak levels were 0·75 h and 1·0 h, respectively (p > 0·1). The mean AUC for the calcium salt mixture was 16·94 mg·h/l (SD 3·31) and for the potassium salt 15·84 mg·h/l (SD 4·76), p > 0·09. These findings confirm that an aqueous mixture of calcium phenoxymethylpenicillin is equivalent to a mixture of potassium Phenoxymethylpenicillin.

Ten cases of pneumococcal pneumonia were treated with cephazolin 500 mg. q8h for at least 5 days. In every case therapy was accompanied by clinical improvement and eradication of the organism. Ten patients with E. coli bacteriuria (5 symptomatic) were treated with cephazolin 500 mg. q12h for 7 to 10 days. In every case the pathogen was eliminated during therapy but in one case bacteriologic relapse occurred following cessation of therapy. In 10 cases of bacteriuria caused by P. mirabilis, Klebsiella sp, Enterobacter, and Enterococcus, the urine became sterile during treatment, but relapse was common. Initial and final creatinine clearances obtained in 29 patients who received an average of 12.9 g. of cephazolin showed no tendency toward loss of renal function. Serial serum levels of cephazolin were determined following the first 500 mg. dose in 18 patients. The peak serum level occurred at one hour with a serum half life of approximately 2.2 hours. For 13 of these 18 patients serial serum levels were also obtained following the last dose of cephazolin. At this time the mean peak level occurred at 2 hours but again the serum half life was approximately 1.9 hours.

Abstract. In 21 hyperprolactinaemic patients without other signs of pituitary dysfunction the mean basal serum level of TSH was 4.4 ± 0.47 μU/ml that was significantly (P < 0.001) higher than controls (2.5 ± 0.16 μU/ml and oestrogen treated individuals (2.4 ± 0.29 μU/ml). The TSH increase was more pronounced (P < 0.05) in hyperprolactinaemic patients without sellar enlargement and with moderately elevated plasma prolactin levels (155 ± 42 μg/ml) than in patients with sellar enlargement and higher plasma prolactin levels (857 ± 306 μg/ml). The serum levels of thyroxine and triiodothyronine in the hyperprolactinaemic patients did not differ significantly from controls. Patients with thyroid antibodies were excluded. The increased basal serum level of TSH in hyperprolactinaemia is compatible with the concept of a reduced dopaminergic tonus as the mechanism for both changes. In patients with advanced hyperprolactinaemia and sellar enlargement the high prolactin level may induce some inhibition of TSH release and explain their lower basal serum level of TSH that was probably not due to pituitary compression as they responded normally to TRH. The TSH response to TRH was significantly (P < 0.05) correlated to the basal serum TSH in all groups. The regression lines were very similar for hyperprolactinaemic patients and controls suggesting that in hyperprolactinaemia the thyrotroph has not changed its mode of response to TRH. In contrast, oestrogen treated subjects in addition to dependence on basal serum TSH levels showed a genuinely augmented response to TRH (164.6 ± 20.3%, P < 0.01) compared to controls.

AbstrakAda anyak penelitian yang membuktikan transfer kolesterol dari ibu ke janin melalui lapisan trofoblas yang membawa partikel LDL (Low Density Lipoprotein) dan HDL (High Density Lipoprotein). Pengambilan dan pemanfaatan LDL oleh plasenta merupakan mekanisme alternatif oleh janin untuk memperoleh asam lemak dan asam amino esensial. Tujuan penelitian ini untuk mengetahui hubungan kadar LDL dan HDL serum ibu hamil aterm dengan berat lahir bayi. Penelitian ini merupakan studi observasional dengan rancangan cross sectional. Dilakukan pemeriksaan kadar LDL dan HDL serum terhadap 31 sampel ibu hamil aterm yang dipilih secara consecutive sampling, kemudiaan saat bayi dari sampel lahir dilakukan penimbangan berat lahir bayi dalam 1 jam setelah lahir dengan keadaan tanpa pakaian. Data dianalisis menggunakan uji korelasi Pearson dilanjutkan dengan uji regresi linier sederhana, nilai p<0.05 dianggap bermakna secara statistik. Rerata kadar LDL serum ibu hamil aterm 138,52±37,86 mg/dl dengan 7 sampel (22,60%) kadar LDL <101 mg/dl. Rerata kadar HDL serum ibu hamil aterm 53,32±17,39 mg/dl dengan 13 sampel (41,90%) kadar HDL <48 mg/dl. Rerata berat lahir bayi 3150,00±489,89 gram dengan 2 sampel (6,50%) memiliki bayi dengan berat<2500 gram. Terdapat hubungan positif antara kadar LDL serum ibu hamil aterm dengan berat lahir bayi, kekuatan hubungan lemah (r=0,258), secara statistik tidak bermakna (p=0,161). Terdapat hubungan positif antara kadar HDL serum ibu hamil aterm, kekuatan hubungan sangat lemah (r=0,035), secara statistik tidak bermakna (p=0,850). Kesimpulan penelitian tidak terdapat hubungan kadar LDL dan HDL serum ibu hamil dengan berat lahir bayi.Kata kunci: kadar LDL serum, kadar HDL serum, ibu hamil aterm, berat lahir bayiAbstractMany studies proved that the transferring of cholesterol from mother to fetus through the trophoblastic layer carried LDL (Low Density Lipoprotein) and HDL (High Density Lipoprotein) particles. Uptake and usage of LDL by placenta to the fetus is an alternative mechanism to obtain fatty acids and essential amino acids. The objective of this study was to determine whether there is a relationship between LDL and HDL serum level of pregnant women at term with infant birth weight. This study was an observational study with cross sectional design. Examination of LDL and HDL serum level to 31 term pregnancy sample choose by consecutive sampling, and then infant’s birth weight was counted within 1 hour after birth without clothes. The data analyzed with Pearson correlation statistical test followed by simple linier regression statistical test. The mean of LDL serum level term pregnancy was 138,52±37,86mg/dlwith7 samples(22.60%) in LDL levels<101 mg/dl. The mean of HDL serum level at term pregnancy was 53,32±17,39 mg/dlwith 13 samples (41,90%) in HDL levels<48 mg/dl. The mean of infant birth weight was 3150,00±489,89 grams with 2 samples (6,50%) had infants weighing < 2500 grams. There is a positive relationship between LDL serum levels term pregnancy with birth weight infants, the strength of the relationship is weak (r =0,258), were not significant statistically (p=0,161). There is a positive relationship between HDL serum levels at term pregnancy with birth weight infants, the strength ofthe relationshipis veryweak(r =0,035), were not significant statistically (p=0,850). In conclusion there was no correlation of serum levels of LDL and HDL at term pregnant with birth weight.

Objective: Low dose mifepristone (RU486) is highly effective in emergency post-coital contraception (EC), although the mechanism(s) of action remains unclear. We studied the endocrine actions of 10 mg mifepristone administered orally as a single dose to eight healthy volunteers (aged 20–45 years) during the late follicular phase. Methods: Serum levels of LH, FSH, oestradiol, progesterone, leptin, mifepristone, cortisol, and gluco-corticoid bioactivity (GBA) were measured before and 1, 2, 4 and 8 h after ingestion of mifepristone on cycle day 10 or 11 (study day 1), and follow-up was continued for 10 days. Ovarian ultrasonography was performed on study days 1 and 7. Similar measurements were carried out during a control cycle. Results: Mifepristone postponed ovulation, as evidenced by a 3.4±1.1 day (means±s.d.) delay (P < 0.005) in the LH surge and 3.6±4.0 day prolongation of the treatment cycle (P = 0.08). During the mifepristone cycle, an LH surge was displayed by five subjects when serum mifepristone levels had declined to 9.5±7.1 nmol/l. During the day of mifepristone administration, circulating GBA (P < 0.001) and leptin (P < 0.001) levels declined. On the day after mifepristone administration, mean serum FSH and leptin levels were lower than pretreatment values (3.8±1.8 IU/l vs 5.2±1.1 IU/l, n = 7, P < 0.05; 28.9±6.7 μg/l vs 33.2±9.0 μg/l, n = 7, P < 0.05 respectively), and the corresponding difference in the mean serum oestradiol concentration was borderline (452±252 pmol/l vs 647±406 pmol/l, n = 7, P = 0.056). In contrast to the control cycle, individual leptin levels declined during the follow-up after ingestion of mifepristone (n = 8, P < 0.01). Conclusions: These data showed that the commonly employed dose of mifepristone for EC delays ovulation and prolongs the menstrual cycle, when given during the late follicular phase. The mechanism of action of mifepristone may include a reduction of FSH secretion via a decrease in circulating leptin.

The underlying basis of the antibiotic treatment of sinusitis is still unclear. Critical to the use of antibiotics in patients with sinusitis is the tissue penetration of the curative agent. However, there is scant information available regarding maxillary mucosal tissue levels in patients with this disease. We present the results of plasma and sinus tissue concentrations of loracarbef in six patients and one control. Levels were taken at 2 and 3 hours after the oral administration of the medication. The mean sinus tissue level was 2.5 mcg/gm and 2.7 mcg/gm at 2 and 3 hours after administration respectively. Although therapeutic efficacy cannot be predicted by the concentrations attained in a specific body fluid or tissue, we believe that these levels are excellent based on mean inhibitory concentrations. Additional studies are underway to document the penetration of this and comparable other drugs in multiple sinuses.

Background: Growth differentiation factor-15 (GDF-15), a member of transforming growth factors, is a stress-responsive marker whose levels may significantly increase in response to pathological stresses associated with inflammatory tissue injuries such as unstable angina pectoris (USAP). This study evaluated the diagnostic value of GDF-15 in patients with USAP. Methods: The present cross-sectional study recruited 39 patients with USAP criteria and 30 patients with stable angina pectoris (SAP), referred to Shahid Beheshti Hospital, Kashan, Iran. All the patients with USAP had at least 1 coronary artery stenosis (>50%) in angiography. The control group comprised 42 healthy individuals. The serum levels of GDF-15 were measured in all the participants by ELISA. Also analyzed were the relationship between GDF-15 levels and thrombolysis in myocardial infarction (TIMI) and the Global Registry of Acute Coronary Events (GRACE) risk scores in the patients with USAP to determine the severity of the disease. Result: The study population consisted of 111 subjects, 62 women and 49 men, divided into 3 groups of USAP (n=39, mean age=60.07±14.10 y), SAP (n=30, mean age=67.56±9.88 y), and control (n=42, mean age=61.21±7.76 y). The mean serum level of GDF-15 in the USAP group was significantly different from the other 2 groups (P<0.001), while no significant difference was observed in this regard between the SAP and control groups (P=0.797). No correlation was found between the mean GDF-15 serum level and the GRACE (P=0.816) and TIMI (P=0.359) risk scores in the USAP group. Conclusion:  The mean serum level of GDF-15 exhibited a rise in our patients with USAP. GDF-15 may be a diagnostic biomarker of USAP and its severity.

Abstract Backgrounds: Multiple Sclerosis (MS) is a chronic inflammatory and autoimmune disease linked to several inflammatory and dietary parameters. This study was carried out to determine the relationship between serum leptin, orexin-A, and TGF-β levels with BMI in MS patients.Methods and results: In this cross-sectional study, 25 relapsing-remitting multiple sclerosis (RRMS) patients and 40 healthy controls were enrolled. The serum level of Leptin, Orexin-A, and TGF- were measured by the Enzyme-linked immunosorbent assay (ELISA). The data was analyzed using descriptive statistics, t-test, Chi-square test, and Linear regression test. A total of 65 volunteers, including 25 MS patients and 40 healthy, were enrolled in the study. The mean age of individuals in the case and control groups was 38.04 ± 7.53 and 40.23 ± 5.88. There were no statistically significant differences between the case and control groups regarding gender, age, alcohol, and cigarette use (P>0.05). The mean serum levels of Orexin-A and TGF-ß were lower among multiple sclerosis patients than in healthy controls, but leptin was higher (42.8 vs. 18.9 ng/ml, P<0.001). The relationship between BMI and serum levels of Orexin-A, TGF-ß, and Leptin among Multiple Sclerosis patients was not statistically significant (P > 0.05).Conclusion: Our results showed that the serum levels of Orexin-A and TGF-β were significantly lower. The serum level of leptin was higher among multiple sclerosis patients than among healthy controls. Also, there was no statistically significant relationship between BMI and serum levels of Orexin-A, TGF-ß, and Leptin among multiple sclerosis patients.

Introduction: Vitamin D deficiency is a major public health problem. Low vitamin D levels associated with adverse health consequences such as musculoskeletal health, cognitive decline and progression of cancer and death. The lack of vitamin D associated with major risk factors for cardiovascular disease (CVD) includes hypertension is considered. The ability to assess the general health, disease and treatment outcomes through saliva as a non-invasive, inexpensive and simple method of interest is located. The aim of this study was a comparative study of salivary and serum levels of vitamin D3 in patients with a history of developing high blood pressure and a healthy person.Methods: This study was a case - control survey, in which 40 patients with high blood pressure were examined. The control group including 40 healthy subjects. Both groups were matched in terms of age and gender. After collecting samples of serum and saliva, the amount of vitamin D level samples were measured using ELISA method by electrochemiluminescence (ELC), and then analyzed the results using software SPSS 16 and statistical test including Chi Square Test, Independent-Samples, linear regression model, the Mann-Whitney Test and Spearman correlation coefficient.Results: There was no significant difference in the mean serum levels of vitamin D among patients and healthy subjects (p= 0.588). In addition, there was no significant difference in the mean salivary levels of vitamin D between patients and healthy subjects (p= 0.833). There was no significant relationship between salivary and serum level of vitamin D in healthy individuals (p= 0.095). As well as there was no significant correlation between salivary and serum level of vitamin D in patients (p= 0.5).Conclusions: This study showed that vitamin D is a measurable marker in saliva, but its analysis in saliva, may not be a reliable tool for determining the vitamin D levels.
