Trends in Authorship Characteristics in The American Journal of Sports Medicine, 1994 to 2014

Background: Trends in author qualifications, the number of authors per article, and the internationalization of author groups in sports medicine journals have not been widely investigated. Purpose: To examine trends in authorship characteristics in a single prominent sports medicine journal. Study Design: Systematic review. Methods: Articles published in The American Journal of Sports Medicine ( AJSM) in 1994, 2004, and 2014 were reviewed. For each article, the academic degree(s) of the first and last author, the total number of authors, the country of the author group, and academic institution status were recorded. Results: A total of 708 articles met the inclusion criteria: 129 in 1994, 244 in 2004, and 335 in 2014. There were significant differences in the proportion of first authors with an MD degree (80% in 1994, 75% in 2004, 67% in 2014; P = .01), a dual MD/PhD degree (4.7% in 1994, 6.2% in 2004, 9.3% in 2014; P < .001), and a bachelor’s degree (0% in 1994, 0% in 2004, 3.9% in 2014; P < .001). The proportion of last authors with an MD/PhD significantly increased over the 2 decades studied (7% in 1994, 13% in 2004, 17% in 2014; P = .01). The mean number of authors per article also significantly increased (3.8 in 1994, 4.3 in 2004, 5.8 in 2014; P < .0001). The proportion of articles published by an international group and the proportion of articles published by an academic institution increased over the 20-year span as well (both P < .0001). Conclusion: Within the past 2 decades, there has been a significant increase in the average number of authors per article in AJSM, as well as a higher proportion of international groups and academic institutions publishing in the journal. More nonphysicians are publishing in AJSM, with a significantly higher percentage of first authors with a bachelor’s degree as their highest degree. This is likely due to a combination of a general increased interest in research as well as increased competition among medical students. These factors have likely led to larger research groups and thus a significantly higher average number of authors per article.

OBJECTIVE Scientific productivity, as assessed by publication volume, is a common metric by which the academic neurosurgical field assesses its members. The number of authors per peer-reviewed article has been observed to increase over time across a broad range of medical specialties. This study provides an update to this trend in the neurosurgical literature. METHODS All publications from January 1, 1980, to April 30, 2020, were queried from four neurosurgical journals: Neurosurgery, Journal of Neurosurgery (JNS), JNS: Pediatrics, and JNS: Spine. Publication information was acquired from the National Center for Biotechnology Information Entrez database and reconciled with the Scopus database. Publication type was limited to articles and excluded editorials, letters, and reviews. The number of authors and affiliation counts were determined based on structured abstract fields provided in the two databases. RESULTS Between January 1, 1980, and April 30, 2020, the overall increase in author count for the four neurosurgical journals was 0.12 to 0.18 authors per year (p < 0.001). For Neurosurgery, the mean (SD) author count increased from 2.81 (1.4) in 1980–1985 to 7.97 (4.92) in 2016–2020 (p < 0.001). For the JNS, the mean (SD) author count increased from 2.82 (1.04) in 1980–1985 to 7.6 (3.65) in 2016–2020 (p < 0.001). The percentage of articles with more than 10 authors increased from 0.2% to 22.3% in Neurosurgery and from 1.9% to 17.5% in JNS. Only 28% of the author count variation was explained by an increasing number of institutional or departmental affiliations. CONCLUSIONS Author counts for peer-reviewed articles in neurosurgical academic journals have increased significantly during the past 4 decades, with large increases in the numbers of articles with more than 10 authors in the past 5 years. A total of 28% of the variation in this increase can be explained by an increase in multiinstitutional or multidepartmental studies.

Background The incidence of atopic disease tends to increaseover the past few decades and its morbidity interferes with thequality of life and health. Prediction of the disease is importantfor early prevention.Objective To evaluate the relationship between atopicmanifestations, family history (FH) of atopic disease and cordblood IgE (CB-IgE) levels.Methods We conducted an analytic observational study withcohort retrospective design on children with an average age of 3years whose CB-IgE had been measured at delivery inKiaracondong Primary Health Care during October–December2004. Manifestations of atopic disease were recorded using ISAACquestionaire for allergy. Chi-square, Mann-Whitney test, andlogistic regression analysis were used for analysis.Results Cord blood IgE was measured on 124 children after birth.Only 94 children (76%) fulfilled the inclusion criteria. Atopicdisease was found in 17 children (18%), consisting of 8 childrenwith atopic dermatitis, 4 with allergic rhinitis, and 5 suffered fromboth. There were significant differences in the mean value of CB-IgE (Z M-W =4.60; P<0.001) and FH (x 2 =19.059; P<0.001)between atopic and non atopic children. Cut off point of the CB-IgE concentration was 1.4 IU/mL (77.7%). The highest probabilityfor atopic manifestations was found in children who had highCB-IgE and positive FH (P=45%). Relative risk of children withhigh CB-IgE level in positive FH group was 3.636 (95% CI0.943;14.016).Conclusion CB-IgE level and family history of atopic disease arerisk factors for the development of atopic manifestation.

Background: Previous studies have examined the most cited articles in orthopaedic sports medicine research and the journals in which they were published. Purpose: To analyze the manuscript submission patterns of authors who published manuscripts in various orthopaedic sports medicine journals. Study Design: Cross-sectional study. Methods: All articles published in the March 2017 and April 2017 issues of Arthroscopy, the American Journal of Sports Medicine ( AJSM), Knee Surgery, Sports Traumatology, Arthroscopy ( KSSTA), and the Orthopaedic Journal of Sports Medicine ( OJSM) were searched. In addition, the past 50 sports medicine–related articles published in the Journal of Bone and Joint Surgery ( JBJS) dating back from April 2017 to May 2015 were searched. The corresponding author of each article was asked whether the publishing journal was the first journal of submission. If the article was previously submitted elsewhere, authors were asked which other journals, the dates of submission, and the order of submission. The proportion of articles that were initially submitted to each journal, the mean number of submissions prior to publication, and the mean duration from initial submission to date of publication were calculated for each journal. Results: A total of 298 articles were included in this study, and 221 (74%) corresponding authors responded to the survey. The mean number of submissions before acceptance was 1.05 for AJSM, 1.18 for JBJS, 1.20 for KSSTA, 1.38 for Arthroscopy, and 2.19 for OJSM. The percentage of articles that were submitted to their accepting journal first (ie, not previously submitted elsewhere) was 95% for AJSM, 85% for JBJS, 82% for KSSTA, 68% for Arthroscopy, and 13% for OJSM. Conclusion: Among the orthopaedic sports medicine journals included in this study, articles published in AJSM have the highest percentage of first submissions.

Abstract Background Authorship trends in medical journals were studied in many disciplines, mostly in western countries' journals. We aimed at studying the authorship trends in the Egyptian Orthopaedic Journal as an example of a specialized Egyptian medical journal. Results A total of 397 articles were eligible for analysis. The mean number of authors per article was 2 ± 1 (range from 1 to 6), 161 (40.6%) articles were single authorship. The degree of the first author was reported in 305 (76.6%), the first author carried an M.D. degree in 302 (99%) articles, and in three (1%), the first author carried a master’s degree. No authors with a bachelor's degree were reported. Forty-two institutions contributed to the publications, 14 (33.3%) international and 28 (66.7%) Egyptian national institutions. In 368 (92.7%) articles, all the authors were from the same institution, and 29 (7.3%) articles were published as a cooperation between different institutions with a mean 1.1 ± 0.3 institution per article. International contribution to the journal was found in 21 (5.3%) articles. The orthopedic department from Cairo university was the most contributing department to the journal publications. Conclusions The old trend of single authorship prevails in the journal publications with a notable deficiency in young researchers’ contribution to the journal and low incidence of international contribution and poor national institution cooperation.

Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.

Background: Even though the decompression of the cellulitis phase of Ludwig’s angina (LA) by surgical or pharmacological approach is well documented, it is unclear which approach is more effective. Objective: We aim to compare the outcome of treatment between surgical versus pharmacological decompression in patients with LA. Subjects and Methods: A retrospective cohort study was designed. Data were collected from the case notes of patients that met the inclusion criteria from 2004 to 2018 at the University of Benin Teaching Hospital, Nigeria.The data were age, gender, type of decompression approach, length of hospital stay (LOS) and airway compromise. Result: A total of 62 patients comprising 37(59.7%) surgical decompression group and 25(40.3%) pharmacological decompression group were studied. Thirty-six (58.1%) males and 26 (41.9%) females were studied. Their mean age and standard deviation were 40.6 years and 11.9 years respectively. The mean length of hospital stays between the pharmacological and surgical decompression groups were 8.05 days and 13.8 days respectively. The incidence of airway compromise in the surgical decompression group was 19.9% lower than that of the pharmacological decompression group (P=0.47), which was not significant. The type of decompression approach also failed to influence the incidence of airway compromise (P = 0.41). Conclusion: The use of surgical versus pharmacological decompression does not significantly alter the incidence of airway compromise in the management of LA. The Patients that had surgical decompression had a shorter stay in the hospital as compared to those who had pharmacological decompression. This was not statistically significant.

Scrub typhus (Orientia tsutsugamushi), is a strict intracellular bacterium which is reported to be a recent threat to parts of southern India. There is re-emergence of scrub typhus during the past few years in Chennai. Scrub typhus is an acute febrile illness which generally causes non-specific symptoms and signs. The clinical manifestations of this disease range from sub-clinical disease to organ failure to fatal disease. This study documents our laboratory experience in diagnosis of scrub typhus in patients with fever and suspected clinical symptoms of scrub typhus infection for a period of two years from April 2014 to April 2016 using immunochromatography and IgM ELISA methods. The study was conducted on 648 patients out of whom 188 patients were found to be positive for scrub typhus. Results also showed that pediatric (0 -12 years) and young adults (20 – 39 years) were more exposed to scrub typhus infection and female patients were more infected compared to male. The study also showed that the rate of infection was higher between September to February which also suggested that the infection rate is proportional to the climatic condition. Statistical analysis showed that the mean age of the patients in this study was 37.6, standard deviation was 18.97, CV % was 50.45.

The use of dietary supplements is high among athletes and non-athletes alike, as well as able-bodied individuals and those with impairments. However, evidence is lacking in the use of dietary supplements for sport performance in a para-athlete population (e.g., those training for the Paralympics or similar competition). Our objective was to examine the literature regarding evidence for various sport supplements in a para-athlete population. A comprehensive literature search was conducted using PubMed, SPORTDiscus, MedLine, and Rehabilitation and Sports Medicine Source. Fifteen studies met our inclusion criteria and were included in our review. Seven varieties of supplements were investigated in the studies reviewed, including caffeine, creatine, buffering agents, fish oil, leucine, and vitamin D. The evidence for each of these supplements remains inconclusive, with varying results between studies. Limitations of research in this area include the heterogeneity of the subjects within the population regarding functionality and impairment. Very few studies included individuals with impairments other than spinal cord injury. Overall, more research is needed to strengthen the evidence for or against supplement use in para-athletes. Future research is also recommended on performance in para-athlete populations with classifiable impairments other than spinal cord injuries.

Abstract Background The Open Payments Program, as designated by the Physician Payments Sunshine Act is the single largest repository of industry payments made to licensed physicians within the United States. Though sizeable in its dataset, the database and user interface are limited in their ability to permit expansive data interpretation and summarization. Objectives We sought to comprehensively compare industry payments made to plastic surgeons with payments made to all surgeons and all physicians to elucidate industry relationships since implementation. Methods The Open Payments Database was queried between 2014 and 2019, and inclusion criteria were applied. These data were evaluated in aggregate and for yearly totals, payment type, and geographic distribution. Results 61,000,728 unique payments totaling $11,815,248,549 were identified over the six-year study period. 9,089 plastic surgeons, 121,151 surgeons, and 796,260 total physicians received these payments. Plastic surgeons annually received significantly less payment than all surgeons (p=0.0005). However, plastic surgeons did not receive significantly more payment than all physicians (p = 0.0840). Cash and cash equivalents proved to be the most common form of payment; Stock and stock options were least commonly transferred. Plastic surgeons in Tennessee received the most in payments between 2014-2019 (mean $ 76,420.75). California had the greatest number of plastic surgeons to receive payments (1,452 surgeons). Conclusions Plastic surgeons received more in industry payments than the average of all physicians but received less than all surgeons. The most common payment was cash transactions. Over the past six years, geographic trends in industry payments have remained stable.

Abstract Background Hepatocellular carcinoma (HCC) is one of the leading causes of cancer-related deaths in Africa. In Africa, the major causes of HCC include chronic infection with hepatitis B virus (HBV) and/or hepatitis C virus (HCV). Knowledge of the changes in the incidence of viral hepatitis-associated HCC over time and the factors responsible for such changes is key in informing policies for the prevention of viral hepatitis-associated HCC in Africa. Aim The study aimed to systematically summarize the changes in the prevalence of viral hepatitis among HCC patients and the overall effect of the prevalence of viral hepatitis on the incidence of HCC over the past four decades in Africa (1980–2019). Methods A literature search was conducted in MEDLINE (PubMed), Google Scholar, Science Direct, Scopus, Web of Science, and African wide web for articles published on viral hepatitis-associated HCC in Africa from 1980 to 2019. The abstracts of the articles were screened for eligibility and those meeting the inclusion criteria were retrieved and reviewed. Results A total of 272 studies were included in the analysis. Viral hepatitis-related HCC incidence changed by 1.17% (95% confidence interval (CI): 0.63–1.71, p < 0.001), 0.82% (95% CI: 0.45–1.18, p < 0.001), and 3.34% (95% CI: 2.44–4.25, p < 0.001) for every 1% change in the prevalence of HBV, HCV, and hepatitis D virus (HDV) respectively, per decade. The incidence of HBV-related HCC decreased by − 0.50% (95% CI: − 0.74 – − 0.25, p < 0.001) over the last 40 years, while HCV-related HCC increased. Conclusion Overall, the incidence of viral hepatitis-associated HCC has not declined, mainly due to no decline in the prevalence of HCV, HDV, and the high number of chronic hepatitis B carriers on the African continent. There is an urgent need for the allocation of resources for the implementation of treatment and preventive programs for HBV, HCV, HDV, and HCC in Africa. This systematic review is registered with PROSPERO®, number CRD42020169723.
