Attachment Theory and Brief Psychotherapy: Applying Current Research to Clinical Interventions

Recent advances in attachment research provide a framework for defining the content and process of brief psychotherapy with adults. Attachment theory emphasizes a number of issues crucial to therapeutic work. Specifically, attachment relationships are enduring components of a individual's pattern of interpersonal behaviours. Functionally, attachment relationships address security needs. Insecure attachment arises from a representational model based on feared loss of the attachment figure, which predisposes the individual to have little confidence in the attachment figure's availability, responsiveness, and permanence. Behavioural responses to insecure attachment can lead to specific patterns of interpersonal relationships which, in turn, strengthen the representational model. Thus, a relatively stable, self-reinforcing system evolves and results in a consistent inability to experience security within attachment relationships. In this article, the authors describe the current framework for understanding adult attachment relationships and present clinical vignettes illustrating the saliency of attachment theory to common clinical presentations. The goal of the clinical intervention is defined as increasing the “permeability” of the individual's working model of attachment through affective and cognitive re-assessment of attachment experiences and expectations. Finally, the utility of this approach in evaluating the outcome and effectiveness of psychotherapy is highlighted.

The origins of attachment theory and the work of John Bowlby and Mary Ainsworth are described. Four types of child–parent attachment relationships—secure, insecure/anxious, insecure/ambivalent, and insecure/disorganized—are outlined along with the ways each type might manifest itself in the classroom. A longitudinal study, conducted by Alan Sroufe and his colleagues, of the development and effects on learning and interpersonal relationships of different child–parent attachment relationships is described. Teachers too have a history of attachment relationships that can affect how they relate to their students. The chapter describes adult attachment and how one’s attachment history might, positively or negatively, affect one’s ability to build positive, nurturing relationships with students. Specific examples of ways teachers can offset the negative effects of a student’s or their own history of insecure attachment are described.

The present study explores the individual differences in the experience of faith formation using the framework of attachment theory, as it looks at what inspires attachment behaviours toward God. The experience of faith formation is herewith conceptualised in this study as a care-giving experience, watered by reciprocity of proximity with a divine attachment figure. The findings suggest four individual pathways in which the faith of young people was formed. These pathways were seen as remedies for previous insecure attachment experience with unavailable human attachment figures, and as a means of enhancing an already positive attachment with human relational partners. In-depth interviews were conducted with fifteen Christian youths of various racial backgrounds who were active members of different church denominations in Stellenbosch, Western Cape, South Africa. The data collected from the respondents suggests youth faith formation as an attachment phenomenon.

Attachment theory is predicated on the assumption of dyadic relationships between a child and one or a few significant others. Despite its recognition of alloparenting in some cultural environments, current attachment research is heavily biased toward the mother as the major attachment figure in the life of the developing child. This chapter presents evidence that diverse childcare arrangements exist in cultures that differ from Western norms and shows how these are equally normative in their respective cultural contexts. In these settings, alloparenting is neither chaotic nor unstable; it is the norm, not the exception. In all environments, infant care is far more than just an isolated, biopsychological phenomenon: it is an activity deeply imbued with cultural meanings, values, and practices. To account for these multiple levels, the construct of attachment must shift its emphasis away from an individual child toward the network of relationships surrounding a child. Overwhelming evidence on diverse childcare arrangements in non-Western cultures calls the putatively universal model of attachment (derived from the Bowlby-Ainsworth paradigm and still widely applied today) into question. In support of future research, this chapter proposes an inclusive reconceptualization of attachment, informed by research from non-Western cultural settings.

The article considers whether religion in general can be viewed as adaptation and if medieval Catholicism in particular can be seen as a reflection of the human necessity to be emotionally attached to a primary caregiver, especially in the early stages of people’s lives. It observes that in a period of high instability and often regressive child-rearing practices, God and/or a special saint could represent a stable and adequate attachment figure, facilitating relationships which contributed to mental well-being of the devotees. The article also suggests that, due to historic circumstances, the proportion of insecure attachment was probably higher among medieval populace than among modern people, and traces the evidence of such attachment style in The Book of Margery Kempe (late 1430s), arguably the first vernacular biography in English.

In this article, we develop a child-centered network approach to attachment during middle childhood. Following monotropic ideas, current attachment research focuses on parental attachment figures despite the expansion of the children’s social environment during middle childhood, failing to generate a comprehensive and structured overview of all individuals who ensure the children’s feeling of safety. Relying on quantitative methods, these studies are also dominated by an adult perspective, limiting the children’s contributions. While there have been theoretical drafts of attachment networks during childhood, this article constitutes the first practical implementation. Using photo elicitation interviews and participant observations, we developed an innovative assessment strategy that allows children to exhaustively identify and characterize all their attachment figures on sociostructural and functional dimensions, thus positioning the children at the center of their comprehensive attachment networks that collectively contribute to their feeling of security. We combine qualitative and quantitative data to assess the children’s own understanding of their feeling of security and to locate the individual attachment figure on context-specific social dimensions, thus making the research setting, a clan in Cameroon, an inherent part of the methodological development. The data are translated into multidimensional network diagrams to visualize the children’s perception of their attachment environment and the emerging patterns of their selection. We present an exemplary network, supplementing it with observational data to discuss the ecological validity of our approach.

In this commentary I argue that the European Union has been functioning as an insecure object of collective attachment for large parts of the European population for many years. According to attachment theory, in relationships of asymmetrical power insecure attachment is formed as the narrative constructed by the most powerful party overwrites the authentic experience of the weakest, generating conflicted representation of self and the attachment object. That attachment object may be interpersonal or collective. The EU narrative on how it safeguards democracy and citizen well-being contradicts the true experience of many Europeans who struggle to make ends meet in neoliberal Europe. On this basis, an insecure collective bond with the EU is established, as the latter fails to recognize and address the needs of many of its citizens.

Stress-related illness presents an ever-increasing burden to society, and thus has become the target of numerous complementary and integrative medicine interventions. One such clinical intervention, mindfulness meditation, has gained eminence for its demonstrated efficacy in reducing stress and improving health outcomes. Despite its prominence, little is known about the mechanics through which it exerts its treatment effects. This article details the therapeutic mechanisms of mindfulness with a novel causal model of stress, metacognition, and coping. Mindfulness is hypothesized to bolster coping processes by augmenting positive reappraisal, mitigating catastrophizing, and engendering self-transcendence. Reviews of stress and mindfulness are then framed by the perspective of second-order cybernetics, a transdisciplinary conceptual framework which builds on extant theory by highlighting the recursion between the individual and their environment.

Background.Attachment theory proposes that psychological functioning and affect regulations are influenced by the attachment we form with others. Early relationships with parents or caregivers lay the foundations for attachment styles. These styles are proposed to influence how we relate to others during our life can be modified by the relationships and events we experience in our lifespan. A secure attachment style is associated with a capacity to manage distress, comfort with autonomy and the ability to form relationships with others, whereas insecure attachment can lead to dysfunctional relationships, emotional and behaviour avoidance. Attachment theory provides a useful framework to inform our understanding of relationship difficulties in people with psychosis. This paper aims to complement recent systematic reviews by providing an overview of attachment theory, its application to psychosis, including an understanding of measurement issues and the clinical implications offered.Method.A narrative review was completed of the measures of attachment and parental bonding in psychosis. Its clinical implications are also discussed. The paper also explores the link between insecure attachment styles and illness course, social functioning and symptomatology. The following questions are addressed: What are the key attachment measures that have been used within the attachment and psychosis literature? What are the results of studies that have measured attachment or parental bonding in psychosis and what clinical implications can we derive from it? What are some of the key questions for future research from these findings in relation to the onset of psychosis research field?Results.The most commonly used measures of attachment in psychosis research are reviewed. Self-report questionnaires and semi-structured interviews have mainly been used to examine attachment styles in adult samples and in recent years comprise a measure specifically developed for a psychosis group. The review suggests that insecure attachment styles are common in psychosis samples. Key relationships were observed between insecure, avoidant and anxious attachment styles and psychosis development, expression and long-term outcome.Conclusions.Attachment theory can provide a useful framework to facilitate our understanding of interpersonal difficulties in psychosis that may predate its onset and impact on observed variability in outcomes, including treatment engagement. Greater attention should be given to the assessment of attachment needs and to the development of interventions that seek to compensate for these difficulties. However, further investigations are required on specifying the exact mechanisms by which specific attachment styles impact on the development of psychosis and its course.

Purpose – The purpose of this paper is to describe some of the basic features of attachment theory, and explore how they relate to the development of the “social mind” and the work of therapeutic communities (TC). Design/methodology/approach – The author describes the essentials of attachment theory in humans; and the development of both secure and insecure states of mind. The author will set out how insecure attachment systems are associated with deficits in mentalising processes which are fundamental to the activity of the social mind. Findings – The author suggests how attachment to a TC can promote mentalising processes. The author draws on the work of other speakers in the conclusions about how to “grow” secure minds and societies. Research limitations/implications – This paper is a brief over view only and does not address attachment process to TC in any depth. Practical implications – Attachment theory could help both service users and therapists who work in TCs understand some of the difficulties people have in engaging at the start. Attachment theory also gives a guide to what a “good enough” experience in a TC might look like. Originality/value – There is little existing discussion of the application of attachment theory to TCs.

All treatments for adult attachment insecurity include in some form a set of principles and methods that can be termed therapist-as-good-attachment-figure. This relational context is widely and appropriately accepted as a foundation for any attachment-focused therapy. After highlighting some of the principles of this approach, this article describes a therapeutic model that includes using patient-and-therapist co-created imagery of positive attachment experience. This imagery method is intrapersonal, in that it focuses on the patient's inner experience of mental representations of attachment relationships; it is interpersonal, in that the process calls upon the therapist to be highly attuned and responsive — as a good attachment figure — to the patient experiencing the imagery; and it is metainterpersonal, in that the patient experiences the imagined interaction with the positive attachment relationships in the context of the therapist supporting and participating in the process. The use of imagery in this way can be a valuable contribution towards relational healing and adult earned secure attachment.
