Testosterone Deficiency, Long-Term Testosterone Therapy, and Inflammation

Objectives: We aimed to evaluate the association of testosterone deficiency with inflammation and how long-term testosterone therapy affects inflammation biomarkers over time. Methods: We conducted a 2-component study. First, we conducted a cross-sectional study using the recently released 2015-2016 National Health and Nutrition Examination Survey (NHANES) data to examine the association between testosterone deficiency and inflammation biomarkers including high sensitivity C-reactive protein (hsCRP), liver enzymes alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in the US general population. Then we conducted a longitudinal study to investigate the longitudinal effect of testosterone therapy on inflammation biomarkers and the risk of cardiovascular events, using data from 776 hypogonadal men based on a registry study in Germany with up to 11 years’ follow-up. Results: The adjusted odds ratios (ORs) describing the associations between testosterone deficiency and hsCRP ≥ 3mg/L, ALT > 40U/L, and AST > 40U/L were 1.81 ( P-value < 0.001), 1.46 ( P-value = 0.009), and 0.99 ( P-value = 0.971), respectively. In the control group, CRP, ALT, and AST levels increased by 0.003 (95%CI: −0.001, 0.007) mg/L, 0.157 U/L (95%CI: 0.145, 0.170), and 0.147 (95%CI: 0.136, 0.159) U/L per month, while in the treatment group, CRP, ALT, and AST levels decreased by 0.05 (95%CI: −0.055, −0.046) mg/L, 0.142 U/L (95%CI: −0.154, −0.130), and 0.148 (95%CI: −0.158, −0.137) U/L per month. Conclusion: Testosterone deficiency was associated with an increased level of inflammation; long-term testosterone therapy alleviated inflammation among hypogonadal men, which may contribute to the reduced cardiovascular risk. Future large trials are warranted to confirm our observational study findings.

Introduction: Purpose of this research is to assess the relationship between long-term pediatric liquid medicines (PLMs) consumption and dental caries in 2–12-years-old chronically ill children. Methods: A cross-sectional study was performed in a public pediatric hospital in India for a period of 6 months. A total of 455 children aged 2–12 years old with various chronic diseases who were receiving PLMs for more than 6 months were selected as the subjects in this study and compared with 531 children of similar age group and diseases who received other forms of medication. Dental caries was measured by DMFT/dmft and data collected were analyzed with SPSS (17th version) using statistical tests such as t-test and one-way ANOVA. Univariable logistic regression was used where the significance was fixed at a a p value of less than 0.001. Results: Children on PLMs had an increased risk of dental caries than those on other forms of medications (OR: 3.142, 95% CI: 2.37–4.15, p < 0.001). The prevalence of dental caries was higher (77.8%) in children consuming PLMs when compared to other forms of medication (52.7%). The mean DMFT and dmft scores were significantly higher in the study group when compared to the control group (p = 0.001). Conclusion: Long-term consumption of pediatric liquid medicines containing sucrose as a risk factor for dental caries among chronically ill children. sugar free options has to be used during prescription to prevent medication-triggered caries.Keywords: Paediatric liquid medicine, medication-triggered caries, sugar-free medication

Introduction: Although the role of inflammation in DVT has been investigated in different studies, there is no definite answer as to whether increased systemic inflammation is the cause or the consequence of DVT. Aim: To follow inflammatory parameters in a cohort of patients with idiopathic DVT. Methods: Out of 49 patients with an acute idiopathic DVT, which were investigated four months after an acute episode (DEVTA 1), 43 patients were included in the follow-up study investigating inflammatory markers and hemostatic markers of endothelial damage five years after an acute DVT (DEVTA 2). A control group consisted of 43 sex and age matched healthy subjects (CONTROLS). Results: The levels of inflammatory markers were significantly higher in DEVTA 2 in comparison to CONTROLS: tumor necrosis factor alpha 2.0 pg/mL (1.1-2.3) vs 1.3 pg/mL (0.8-1.9), p < .001, high sensitivity C-reactive protein 3.2 mg/L (1.5-5.2) vs 1.7 mg/L (0.9-3.0), p = .008, interleukin-6 (IL-6) 2.7 pg/mL (2.0-3.5) vs 2.1 pg/mL (1.5-2.6), p = .025, IL-8 5.0 pg/mL (3.6-7.3) vs 2.4 pg/mL (1.8-2.8), p < .001. IL-10 was significantly decreased (0.9 pg/mL (0.7-1.8) vs 1.8 (1.5-2.2), p < .001. Most of the proinflammatory markers remained elevated in the DEVTA 2 in comparison to DEVTA 1. Markers of endothelial damage were higher in DEVTA 2 in comparison to CONTROLS and higher than in DEVTA 1. Conclusion: Patients with idiopathic DVT have long-term increased inflammatory markers and markers of endothelial damage. These findings favor the hypothesis that inflammation is a cause and not merely a consequence of acute DVT.

Background: C-reactive protein test is one of clinical assessments to minimize risks of infection in infants. However, its procedure may cause pain. Pain in the infant may result in negative metabolic behavior, physiology and metabolic response. Objective: This study was to describe the infant's pain response by administering a combination of breastfeeding and an effleurage massage on the blood sampling procedure of C-reactive protein examination.         Methods: This was a descriptive observational cross-sectional study. There were 30 infants selected using consecutive sampling technique, which 15 samples assigned in an intervention group (combination of breastfeeding and effleurage massage) and a control group. Premature Infant Pain Profile (PIPP) instrument was used to measure pain. Univariate analysis was performed with the aim to describe data in mean and median.Results: The average of pain response at 1-minute observation in the combination of breastfeeding and effleurage massage group was 7.47 ± 1.356, and the average of pain response in the control group was 10.80 ± 1.897. The average pain at 5-minutes in the intervention group was 3.53 ± 1.922 and control group was 6.00 ± 1.852. Conclusions: Pain responses in the combination of breastfeeding and effleurage massage group were lower than the pain response in the control group.

Objective: to determine the prevalence of explained and unexplained recurrent miscarriages (RM) and to find out if there is a significant relationship between recurrent miscarriages and consanguinity. Methods: A cross sectional in which the cases group included all women with RM attending the outpatient clinic at Sultan Qaboos University Hospital from July 2006 to April 2012 and the controls group included women with no history of RM after matching them with cases for age (case to control ratio was 1:1). The main outcome measures were the prevalence of consanguinity in women with or without recurrent miscarriages. Results: During study period a total of 290 women with RM were seen. Of which, 150 (51.7%) women had unexplained RM. Control group with no history of RM were 300 women. Consanguinity rate among cases (49.5%) %) was less than the controls (52.7 %%). Both first cousin and second cousin marriages were more common in the controls than the cases and it was not statistically significant (p value 0.476, chi squared test). Conclusion: In this study we found that more than half of RM cases were unexplained and there was no significant association between RM and consanguinity.

INTRODUCTION:Cerebrovascular accident (commonly called stroke) is dened as an abrupt onset of a neurologic decit that is attributable to a focal vascular cause. Thus, the diagnosis of stroke is clinical and laboratory studies including brain imaging are used to support the diagnosis AIMS AND OBJECTIVES: To evaluate the serum levels of high sensitivity C-reactive protein (hsCRP) in different types of cerebrovascular accidents on admission. MATERIALS AND METHODS: This study was an institution based cross-sectional study designed to investigate the association of hs-CRP levels with stroke and its types in Indian patients. The study was done in the Department of Medicine, Darbhanga Medical College and Hospital, Laheriasarai, Bihar from march 2019 to March 2020. 50 patients of either sex above 12 years of age admitted in Medicine Department of DMCH, Laheriasarai, Bihar with clinically or radiologically diagnosed stroke. RESULT: The mean level of hsCRP (mean ± s.d.) of the cases in our study was 7.65±7.01 with range 1.4 – 26 and the median was 3.7 and 58% of the cases were having level of hsCRP≥3 (p=0.023).Mean level of hsCRPof cases was signicantly higher than that of control (t98=7.25;p=0.001). CONCLUSION:We concluded that hs-CRPlevel is increased in cases of cerebrovascular accident- ischemic as well as haemorrhagic, suggesting an inammatory response in acute cerebrovascular accident. hs-CRP level is increased in patients with ischemic cerebrovascular accident dramatically but not in haemorrhagic cerebrovascular accident which might be considered as useful adjunct method for determining type of stroke in patients with cerebrovascular problems.

Abstract Objectives The aim of the present study is to compare cardiometabolic biomarker levels (MARKERS) from a cohort using multiple (2+) dietary supplements (MDS) manufactured by Shaklee Corporation for 3–5 yr (SHAKLEE) to those from the age-matched MDS users from NHANES 2007–10 (NHANES). Methods Each subject from SHAKLEE [n = 58; mean age: 48 yr (22–79 yr)], using MDS manufactured by Shaklee Corp for 3–5 yr, voluntarily signed the IRB approved informed consent form before the study participation. Body mass index (BMI), systolic and diastolic blood pressures (SBP and DSP) were measured, and approximately 30 mL of 12-h fasting blood sample was collected. Pregnant women and people with a history of cancer were excluded. MARKERS from SHAKLEE were compared to those from NHANES [n = 1952; mean age: 51 yr (22–79 yr)]. MARKERS included total cholesterol (TC), HDL-c, TC/HDL-c ratio, LDL-c, triglycerides (TG), high sensitivity C-Reactive Protein (CRP), glucose, hemoglobin A1c (HbA1c), and Insulin levels in the blood as well as BMI, SBP, and DBP. Statistical analyses were performed using independent samples t-tests, and P < 0.05 was considered significantly different between groups. Results SHAKLEE had significantly lower TC (189 vs. 201 mg/dL), TC/HDL-c ratio (3.1 vs. 4.0), LDL-c (103 vs. 118 mg/dL), TG (81 vs. 131 mg/dL), glucose (93 vs. 107 mg/dL), HbA1c (5.1 vs. 5.7%), insulin (8.3 vs. 13.4 mIU/L), BMI (26.7 vs. 29.0), and SBP (110 vs. 122 mmHg), and higher HDL-c (69 vs. 55 mg/dL) but had higher DBP (76 vs. 71 mmHg) than NHANES. There was no significant difference in CRP although CRP was slightly lower in SHAKLEE than NHANES (2.9 vs. 3.8 mg/L). Conclusions The present study showed that 3–5 yr MDS users from the Shaklee cohort had healthier pattern in cardiometabolic biomarkers than the age-matched MDS users from NHANES 2007–2010.
