Abstract Background Cannabis use is growing domestically due to recent legalization in many jurisdictions. There are two main species of cannabis, Cannabis sativa and Cannabis indica, and thousands of different commercially available cannabis strains. Although there are multiple reports of cannabis allergy in the literature, to our knowledge, there is no prior published report of selective cannabis strain allergy. Case presentation A 31-year-old male was referred for allergy assessment due to several episodes of localized pruritus and erythema after direct contact with various strains of cannabis. He had noted that the severity of his reaction appeared to be strain dependent. He developed a severe local reaction involving bilateral periorbital edema shortly after coming into direct contact with one particular strain of cannabis. He denied any adverse symptoms after inhalation of cannabis. Fresh skin prick testing was performed to various strains of cannabis and had positive testing to the three of the five tested strains. Conclusions We believe this is the first reported case of selective cannabis strain allergy based on patient history and skin prick testing. This case report outlines the variability in different strains of cannabis and stresses the importance of further research into cannabis allergen identification. Multiple cannabis allergens should be included and incorporated into commercial extracts when they become routinely available.

Abstract The strain-dependent vertical and horizontal wicking of as-prepared cotton/lycra elastic woven fabric was systematically studied. The experimental results revealed that the fabric exhibited a strain strengthening effect. A higher tensile strain results in a higher equilibrium wicking height, and vice versa. Moreover, the results indicated that the proposed Laughlin–Davies model is capable of tracking well the experimental data and replicating the wicking characteristics of fabric under different levels of stretch. In addition, the wetting time and wicking area of fabric under different strains and height regimes were examined during horizontal wicking. It was found that the wetting time decreased with an increase of strain and/or water drop height. The strain-enhanced and height-weakened effects of wicking area were revealed. The spreading mechanism of water drop in elastic fabric was also proposed. Such fundamental work provides a basic support for the in-depth investigation of wicking behavior of complex stretchable textile structures.

Abstract Background Anti-streptavidin antibodies are causal determinants of analytical interference during Thyroid function tests, and numerous reports have detailed such interference, with anti-streptavidin antibodies attracting attention. Case presentation We conducted a straightforward investigation of interference due to anti-streptavidin antibodies, with a case of a 60-year-old Japanese man who consulted our department for inconsistencies between his clinical course and Thyroid function tests. Experiments were conducted using Cobas8000 e602, which employs assay procedures with pre-wash to evaluate FT4 and FT3 levels. Conclusions To our knowledge, this is the first published report to clearly investigate such interferences using a combination of polyethylene glycol precipitation, heterophilic blocking tube precipitation, streptavidin-coated magnetic particle precipitation, and different instruments with or without pre-wash. Clinicians should consider that interferences caused by anti-streptavidin antibodies could lead to a misdiagnosis of thyrotoxicosis. Moreover, discussions between laboratory specialists, clinicians, and manufacturers are required to identify interferences and avoid unnecessary examinations and inappropriate treatment.

There has been an increasing rate of cannabis consumption globally, especially among the youths. This study therefore evaluated the neurological behaviours and some brain marker hormones and enzymes of cannabis administered rats. Twenty six albino rats were divided into four groups based on oral cannabis administration (control, Cannabis sativa, Cannabis indica and the combination of the two). At the end of seven days, open field test was conducted on the rats. Also, brain neuro-chemicals, activities of antioxidant enzymes and lipid peroxidation were evaluated using spectrophotometry. The results of the Open-Field Test showed an appreciable increase in the level of ambulation (line crossing), grooming, urination and stretched attend posture in the rats administered with Cannabis indica, Cannabis sativa and the combination when compared with the control. Norepinephrine was significantly lower (p < 0.05) in the rat groups administered with the combination of Cannabis indica and Cannabis sativa. The control group however had the lowest dopamine level. Superoxide dismutase (SOD) was significantly lower (p < 0.05) in the rats administered the combination of both Cannabis indica and Cannabis sativa. The brain level of reduced glutathione (GSH) was significantly higher in the rats administered with Cannabis indica. Malondialdehyde (MDA) was significantly higher in the rats administered with Cannabis sativa than the other rat groups. Histopathological evaluation of the brain also revealed various damages in the brain cells of rats administered with cannabis compared to the normal brain structure of the control rats. It is thus said that consumption of C. sativa or C. indica alone produced mild effect on the brain cells and physiology in rats. However, combination of C. sativa and C. indica produced a severe synergistic effect on the neurological function of the exposed rat.

Background. Infective endocarditis is associated with considerable morbidity and mortality. Guidelines addressing prophylaxis and management of infective endocarditis do not extensively deal with concomitant pregnancy, and case reports on infective endocarditis are scarce. This is the first published report of infective endocarditis byStaphylococcus lugdunensisin a pregnant woman.Case Presentation. We report a single case of a 35-year-old woman in her 24th week of pregnancy who was admitted to our intensive care unit with fever and suspected infectious endocarditis. Blood culture detectedStaphylococcus lugdunensis. A vegetation and severe mitral regurgitation due to complete destruction of the valve confirmed the diagnosis. An interdisciplinary panel of cardiologists, maternal-fetal medicine specialists, cardiac and plastic surgeons, infectiologists, anesthesiologists, and neonatologists was formed to determine the best therapeutic strategy.Conclusions. Timing and indications for surgical intervention to prevent embolic complications in infective endocarditis remain controversial. This original case report illustrates how managing infective endocarditis byStaphylococcus lugdunensisparticularly in the 24th week of pregnancy can represent a therapeutic challenge to a broad section of specialties across medicine. Critical cases like this require a thorough weighing of risks and benefits followed by swift action to protect the mother and her unborn child.

Differences in LPS responsiveness influence the outcome of patients with sepsis. The intensity of the response is highly variable in patients and strain dependent in rodents. However, the role of the liver for initiating the LPS response remains ill defined. We hypothesize that hepatic LPS uptake is a key event for initiating the LPS response. In the present study, the severity of the LPS-induced inflammatory response and the hepatic LPS uptake was compared in two rat strains (Lewis (LEW) rats and Brown Norway (BN) rats). Using a transplantation model, we demonstrated the decisive role of the liver. The expression of hepatic TNF-α, IL-6, and IL-1β mRNA levels in BN rats was significantly lower than that in LEW rats. LEW rats were sensitized to LPS via G-CSF pretreatment. Sensitization caused by G-CSF pretreatment induced severe liver injury and mortality in LEW rats, but not in BN rats (survival rate: 0% (LEW) versus 100% (BN), p<0.01). LEW rats presented with higher liver enzymes, more alterations in histology, and higher expression of caspase 3 and higher cytokines levels. One of the reasons could be the increased hepatic LPS uptake, which was only observed in LEW but not in BN livers. Using the transplantation model revealed the decisive role of the LPS responsiveness of the liver. Injection of LPS to the high-responding LEW recipient before transplantation of a low-responder BN liver resulted in a 50% survival rate. In contrast, injecting the same dose of LPS into the high-responding LEW recipient after transplanting the low-responding BN liver resulted in a 100% survival rate. The severity of inflammatory response in different strains might be related to the differences in hepatic LPS uptake. This observation suggests that the liver plays a genetically defined decisive role in modulating the inflammatory severity.

Di antara pengguna ganja, beragam efek yang dihasilkan, terutama euphoria (rasa gembira) yang berlebihan, serta hilangnya konsentrasi untuk berpikir di antara para pengguna tertentu.Efek negatif secara umum adalah bila sudah menghisap maka pengguna akan menjadi malas dan otak akan lamban dalam berpikir. Namun, hal ini masih menjadi kontroversi, karena tidak sepenuhnya disepakati oleh beberapa kelompok tertentu yang mendukung medical marijuana dan marijuana pada umumnya. Selain diklaim sebagai pereda rasa sakit, dan pengobatan untuk penyakit tertentu (termasuk kanker), banyak juga pihak yang menyatakan adanya lonjakan kreatifitas dalam berfikir serta dalam berkarya (terutama pada para seniman dan musisi.Berdasarkan penelitian terakhir, hal ini (lonjakan kreatifitas), juga di pengaruhi oleh jenis ganja yang digunakan. Salah satu jenis ganja yang dianggap membantu kreatifitas adalah hasil silangan modern “Cannabis indica” yang berasal dari India dengan “Cannabis sativa” dari Barat, dimana jenis Marijuana silangan inilah yang merupakan tipe yang tumbuh di Indonesia.Efek yang dihasilkan juga beragam terhadap setiap individu, dimana dalam golongan tertentu ada yang merasakan efek yang membuat mereka menjadi malas, sementara ada kelompok yang menjadi aktif, terutama dalam berfikir kreatif (bukan aktif secara fisik seperti efek yang dihasilkan Methamphetamin). Marijuana, hingga detik ini, tidak pernah terbukti sebagai penyebab kematian maupun kecanduan. Bahkan, di masa lalu dianggap sebagai tanaman luar biasa, dimana hampir semua unsur yang ada padanya dapat dimanfaatkan untuk berbagai keperluan. Hal ini sangat bertolak belakang dan berbeda dengan efek yang dihasilkan oleh obat-obatan terlarang dan alkohol, yang menyebabkan penggunanya menjadi kecanduan hingga tersiksa secara fisik, dan bahkan berbuat kekerasan maupun penipuan (aksi kriminal) untuk mendapatkan obat-obatan kimia buatan manusia itu

Background. Plantar cutaneous conditions are the most common causes of foot discomfort limiting ambulation. Identification of indwelling foreign bodies is greatly facilitated by dermoscopy, allowing early detection of the character of the underlying pathological etiology. Furthermore, polarized dermoscopy provides ideal conditions of illumination and three- dimensional visualization of the involved site, without direct contact with the cutaneous surface. This unprecedented report describes its implementation for a solitary hair implanted into plantar skin.  Case Presentation. A patient presented with localized plantar discomfort diagnosed as a soft tissue reaction to an embedded single human hair, folded and rooted inside the epidermis. Under magnified vision the phenomenon was characterized, the hair was extracted, the source was manipulated with a hypodermic needle and the lesion was removed with ensuing complete resolution. Conclusions. Previous reports of hair slivers embedded inside the skin were attributed to barbers or animal grooming and the treatment was dictated by the severity of the complications. In this relatively mild case the intervention was greatly facilitated by therapeutic dermoscopy. Therapeutic dermoscopy is advocated for use to evaluate and treat many diverse body surfaces, especially the plantar skin as it raises the likelihood of successful treatment of embedded foreign bodies.  Precisely how a single hair may become lodged into normal plantar skin remains to be explained

Abstract Background: COVID-19 has been recently declared by WHO a global health pandemic. Theoretically, it might affect all age groups but it is not known if vertical transmission during pregnancy occurs. We hereby report a case about the first Jordanian newborn delivered to COVID-19 infected mother. Case presentation: A late preterm female was delivered by Cesarean section to COVID-19 mother who was diagnosed after presenting with a dry cough, nasal congestion, headache, and sore throat in the context of direct contact with other confirmed patients. The infant’s clinical examination was reassuring throughout the hospital stay. COVID-19 was not detected by RT-PCR tests performed on the amniotic fluid and on two samples of the newborn’s nasopharyngeal swabs indicating no vertical transmission of the virus. After 10 days of hospital stay and following two negative consecutive RT-PCR assays on the mother’s nasopharyngeal swabs, both the mother and the infant were discharged home in stable clinical conditions. Conclusion: Vertical transmission is not likely among the routes of COVID-19 transmission. However, data about more number of deliveries to COVID-19 infected mothers is needed to support this conclusion.

Abstract Cannabis cultivated as the main raw material for textiles is another than the one used for its psychotropic effects. From a botanical point of view, however, plants are virtually imperceptible. What is changing is the concentration level of the active principle, depending on the type of cannabis. Cannabis Indica has a higher amount of resin and a higher cannabidiol percentage than the Sativa variety. The hemp cultivated for fibers and especially Cannabis sativa L, which includes the types grown in Romania and generally in Europe, have a low content of narcotic and hallucinogenic substances (0.2-0.3%). This hemp is specially formulated and used for the production of fibrer.The hempcultivated for drug is not grown in. Romania. Because there are no processing plants factories, all the production in Romania is sold for export. Subsidies are given for the cultivation of these varietes. The benefits are great because textiles obtained from hemp are superior to those from countries like China and Germany. The hempcultivated for drug is not grown in Romania.

“But nobody ever died from an overdose of marijuana” has become one of the rallying cries of those who support legalization of marijuana for the treatment of chronic pain and a plethora of other disorders. There is little doubt that the Cannabis Sativa and Cannabis Indica plants produce substances that may well have medicinal value. However, much is yet to be learned. The endocannabinoid system, like the plants themselves, is very complex. Hemp oil and cannabidiol are flooding the market. Unregulated by any federal agencies, the claims and labeling are often unsupported and misleading, if not false. The issue of medical marijuana is here to stay. The clinician treating patients with chronic pain needs to become familiar with the nuances of these products and recognize that they are not without side effects and drug-drug interactions.
