An Epidemiological Study of Dementia in a Rural Population of Elderly Women

A study of 365 women aged 70–79 in a rural community was carried out using the Cambridge Examination of Mental Disorders in the Elderly (CAMDEX). Prevalence rates of dementia are reported by severity for the 70–74 and 75–79 age groups. Differential diagnosis was made according to CAMDEX guidelines. Senile dementia of Alzheimer type accounted for half the dementia cases. The prevalence rates overall did not differ from those reported in other recent studies, but the rates for levels greater than mild/moderate were lower, despite the inclusion of subjects in institutions.

AbstractThe MentDis_ICF65+ Project is an epidemiological study of mental disorders in people 65 to 85 years old in several European cities, including Madrid. Its aim is to determine the lifetime, 12-month, and 1-month prevalence of the main mental disorders in the elderly. The relationship of age and sex with each mental disorder was examined. The sample was collected through random sampling of people over 65 in Madrid, and consisted of 555 persons between 65 and 85 years old. The CIDI65+ was administered. Estimates of prevalence and odds ratios (OR) were made using sample frequencies and according to sex and age. Excluding nicotine dependence, 40.12% of the sample was found to have suffered a mental disorder at some time in their lives, 29.89% in the past year, and 17.70% were currently suffering from a mental disorder. The disorders with the highest prevalence rates were anxiety disorders, alcohol-related disorders, and mood disorders. Elderly women had a higher risk of suffering an anxiety disorder (OR men/women 0.42; CI 0.25–0.68) with a significance level of p < .001, while elderly men were more affected by any substance-related disorder (OR men/women 3.96; CI 1.62–11.07) with a significance level of p < .001. Each disorder’s prevalence decreased with age (OR 65–74/75–85, 1.85; CI 1.25–2.75) with a significance level of p < .01. Results show higher prevalence rates than previous studies reported. The main implications of this study, and the need to adapt mental health services for people over 65, are highlighted.

The age-specific prevalence of dementia, its sex difference, and the relative prevalence of important types of dementia were studied in the elderly people in a Korean rural community. A two-stage approach was employed, involving screening and clinical assessment. The prevalence among individuals aged 65 and over was found to be 10.8%, with rates of 7.2% in men and 14.5% in women. The dementia was of the Alzheimer type in 60.0% of cases, multi-infarct dementia in 12.0%, mixed dementia of Alzheimer type and multi-infarct in 10.7%, alcoholic dementia in 8.0%, and others and unclassifiable in 9.3%. The prevalence of dementia of the Alzheimer type was significantly higher in women and rapidly increased with age in both sexes. The prevalence of multi-infarct dementia was not related to sex or age. Alcoholic dementia was identified only in men. These findings indicate that the prevalence of dementia in rural Korea is similar to that reported in Western countries and that the prevalence of dementia of the Alzheimer type in rural Korea is greater than that of multi-infarct dementia.

SummaryIn vivo NMR 31p spectroscopy is a non invasive, non ionizing method of exploration of energy and phospholipid metabolism in the brain. This study consisted of comparing 31p spectra in five patients with Senile Dementia of Alzheimer Type (SDAT) with those of four controls of similar ages. Abnormal phosphonionocsters (PME) concentrations, either high or low, were found in the patients, but statistical analysis did not elicit any significant difference relative to controls.

Objective: To investigate the perception and oral health practices among older quilombola women (black population group, descendants of slaves in Brazil). Methods: Qualitative research with elderly women living in a rural community formed by descendants of slaves in Brazil. The study was performed through a semi-structured interview with nine of the rural community residents and following content analysis of the narratives. Results: All elderly women are edentulous (partially or totally) and dental problems that led them to use natural means of pain relief were identified. With the analysis of the transcripts, three main categories emerged: the elderly of Quilombola Rural Community Julia Mulata and edentulism; self-perceived oral health of older women of the Quilombola Rural Community Julia Mulata; Popular practices used in the presence of health problems. Conclusion: Quilombola elderly consider the loss of teeth as natural aging; they present life stories linked to dental problems; seek to solve their dental problems with the use of traditional folk therapies.

The purpose of this paper is to describe the knowledge about mammography and to identify associated factors in female adults and elderly. Data were obtained from two population surveys, one with female adults and another with elderly women from Florianópolis (SC) in 2009 - 2010. A descriptive analysis of the variables was carried out, the appropriate mean of responses about mammography was estimated and crude and adjusted Poisson regression was conducted to identify associated factors. Among adults, 23.1% answered all of the questions appropriately and the appropriate average responses was 7.2 (95%CI 7.1 - 7.3) in a total of 9. In the adjusted model, older age, higher education and income were associated with knowledge about mammography. For the elderly, 15.3% answered all questions appropriately and the average of appropriate responses was 6.4 (95%CI 5.2 - 6.5) and the factors associated with knowledge about mammography in the adjusted model were younger age groups, increased education and income, and identification of mammography as the main diagnostic method for breast cancer. Information about mammography can neither be transmitted in a clear way nor be easily understood; there are also demographic and socioeconomic differences concerning the knowledge about the exam.

An excess of irregularly di storted red cells with spiked forms (acanthocytes. spur cells) has been found in a substantial minority of patient s with seni le dementia of Alzheimer type (7 of 50 patients, 3 of 21 men and 4 of 29 women). Of 100 control patients, 42 men and 58 women), 5 (men and 2 women) showed comparable distortion, but, of these, one man may well have incipient dementia and the others had serious organic di seases which may be associated with comparable erythrocytic changes. The cause of the distortion is not yet clear, but the presence of occasional giant erythrocytes in the absence of general macrocytosis suggests a possible abnormality of cell membrane synthes is. This distortion may be a useful marker in patients with loss of memory. Whether it is a manifestation of a haemopoietic clone or a constitutional anomaly associated with Alzheimer’s disease remains to be seen.
