e17515 Background: The purpose of the study was to evaluate advantages of combination ozone therapy as an effective factor of radiomodification and radioprotection in the treatment of patients with cervical cancer. Methods: The study included 20 patients with stage IIIb cervical cancer T3бNхM0, the mean age 52 years. Controls (n = 10) received standard chemoradiation therapy; the main group (n = 10) received combination ozone therapy in addition to chemoradiation therapy. Systemic ozone therapy involved intravenous administration of 250 ml ozonized solution of 0.9% sodium chloride, 15 sessions during external beam radiotherapy. Local ozone therapy involved 15-minute instillations into the vagina during brachytherapy. All patients received standard chemoradiation therapy, total irradiation dose to the primary focus 80 Gy, plus weekly cisplatin 40 mg/m2. Results: In the main group, pain and discomfort in the lower abdomen were managed in 20% patients by the 5th session of ozone therapy; 30% - on the 10th day; in 50% - on the 14th day. In the control group, such symptoms in 40% of patients were reduced after 20 days. The main group showed a more rapid regression of tumor infiltration, disappearance of purulent discharge, relief of intoxication syndrome. Manifestations of gastrointestinal toxicity were observed in 10% of the main group and in 100% controls; leukopenia developed in 7 patients in the control group and was not registered in the main group. MRI showed complete clinical regression in 90% in the main group and in 65% in the control group. The treatment was completed within 7 weeks in the main group and 8.5 weeks in the control group. Conclusions: Combination ozone therapy causes a more pronounced antitumor effect, and its radioprotective effects significantly reduce the severity of chemotherapy-induced disorders. It does not aggravate concomitant pathology and shortens the treatment period.

e18030 Background: Chemoradiation therapy (CRT) radiomodified with platinum-based drugs is recommended for patients with inoperable cervical cancer. However, the introduction of chemotherapy medicines increases the number of hematological and non-hematological complications. Prolonged RT significantly decreases survival of patients. This requires more effective methods helping to prevent chemoradiotherapy complications. The purpose of the study was to evaluate the effectiveness of ozone therapy as an accompanying treatment for chemoradiotherapy for cervical cancer (CC). Methods: The study included 20 patients diagnosed with cT3NхM0 CC. The control group (n = 10) received standard CRT with weekly cisplatin radiomodification (40 mg/m2). The main group (n = 10) received CRT with weekly cisplatin radiomodification (40 mg/m2) and additional infusions of 250 ml ozonized solution of 0.9% sodium chloride (a total of 15 infusions). Results: In the main group, CRT did not cause a significant aggravation of gastrointestinal and general symptoms, compared with the initial state. The rate of dyspeptic disorders (nausea, diarrhea, bloating) was 10% in the main group and 100% in controls. In the control group, clinical pain decreased only by the 25th day; 100% of patients developed gastrointestinal toxicity grade 1 (80%) and 2 (20%); 100% of patients complained of weakness during treatment. 7 patients developed leukopenia. Anemia in 5 patients remained at the level of 95g/L, in 3 patients - aggravation up to 80g/L. In the main group, treatment was completed within 7 weeks. In the control group, the duration of CRT was 8 weeks due to toxicity. Conclusions: Ozone therapy in addition to the conventional conservative treatment significantly decreases the severity of CRT-induced complications which allows complete treatment planned, while maintaining optimal treatment duration.

According to the literature, postoperative complications and associated purulent-inflammatory processes occur in 35-60 % of patients, which increases the rates of postoperative mortality to 25 %. The main purpose of our work was to evaluate the effectiveness of the application of the polymeric film «biodep-nano» in the prevention of secondary infection of wounds in patients with hemodialysis. Methods. The results of treatment of 36 patients divided into two groups were analyzed: the main group (18 patients) with applied traditional gauze dressings and the experimental group (18 patients) with additionally applied biodep-nano polymer films (Pat. No. 110594 Ukraine, MPK 2016.01, biodegradable polymer film «biodep-nano» is produced as «Hygienic and prophylactic. Biodegradable polymer film «Biodep-nano» of 50 g according to TU U 20.4-2950221612-001: 2017». Statistical data processing was performed using Student's t-test, 5 % (p≤0.05). Results. The results obtained showed that the temperature of the study area in the main group came to normal on the 3rd day (2.8±0.2 days), and in the group where traditional methods were used – on the 5-7th (5.9±0.4) day, (p<0.05). Reduction of tissue edema by an average of 3.8±1.2 days, and in the control group by an average of 6.1±1.7 days, (p<0.05). With regard to hyperemia, it gradually decreased and in the main group was up to 3.9±2.1 days on an average, and in the control group by 6.0±1.9 days (p<0.05). In the main group, the appearance of purulent discharge was observed in 1 patient, in the control group in 4 cases. The statistical calculations showed a significant statistical difference in the reduction of inflammatory process in both groups. Conclusions. The use of new methods and means of preventing secondary infections after surgery in patients with hemodialysis will enable to carry out dialysis of patients, shorten their stay in the clinic and save lives.

. Background. Human papillomavirus (HPV) has been established to be the etiological factor of cervical cancer (CC). HPV infection and CC progression involve the direct participation of the E6 oncoprotein. Aim. An analysis of the E6 oncoprotein levels in tissues of the tumor and its perifocal area in HPV-associated cervical squamous cell cancer as an objective indicator of the effect of treatment depending on preoperative chemotherapy. Material and methods. The study included clinical and laboratory data of 237 patients with high-risk HPV infection of the cervix. The patients were divided into 4 groups: two main groups (CC T2а2–2bN0–1M0) and two control groups. Patients in the main group 1 (n=84) received standard neoadjuvant chemotherapy (NACT), in the main group 2 (n=93) — modified NACT with prior plasmapheresis session and a parallel course of nonspecific immunotherapy with Allokin-alpha. Control group 1 (n=40) included patients with CC T1b2–2a1N0–1M0, surgical treatment; control group 2 (n=20) — HPV-positive patients without CC. Levels of E6 were measured in samples of the cervical tumor and perifocal tissues. Results. The lowest levels of the E6 oncoprotein were registered in the group of HPV-positive patients without CC. After modified NACT, E6 levels in tumor tissues remained 4.6 times higher than in intact tissues, and even so, these patients demonstrated minimal E6 levels compared to other CC patients. E6 in tumor tissues was significantly lower than in main group 1 (by 3.3 times) and 8 times lower than in control group 1. E6 levels in the perifocal tissues of patients in main group 2 were 1.9 times lower than in the corresponding tissues of patients in main group 1 and 2.2 times lower than in control group 1. Conclusions. Inclusion of plasmapheresis and inducers of endogenous interferonogenesis into neoadjuvant treatment for cervical cancer can be considered pathogenetically justified, since it affects the key unit of cervical carcinogenesis.

Th e results of using natural radon waters in the form of General or four-chamber baths and ozonopuncture in patients with occupational diseases of the joints and spine at the rehabilitation stage are presented. Studies have shown that the course use of radon therapy in combination with ozone therapy has a pronounced analgesic eff ect, anti-infl ammatory eff ect, stimulates the general sedative eff ect on the central nervous system, helps to restore the volume of movements in the aff ected joints, prevents the weighting of the joint process and the development of dystrophic changes in the musculoskeletal system. The aim of the study is to study the eff ect of natural radon waters combined with ozone therapy on the dynamics of functional indicators based on motor activity data in patients with occupational diseases of the joints.Patients with occupational diseases of the joints in rehabilitation were prescribed general radon or four-chamber baths in combination with ozone therapy (the main group). A control group of patients received radon baths by a similar method without introducing ozone. Before and aft er treatment, studies of functional activity in the aff ected joints were conducted: the volume of movements, the functional Leken index, which takes into account the severity of pain at rest and under motor load, pain assessment on a visual analog scale, echography of the aff ected joints, adaptive response according to L.Kh. Garkavi. Th e use of general or four-chamber radon baths in combination with ozone therapy in the main group allowed to quickly and eff ectively normalize the functional activity of joints, relieve pain, increase the body’s adaptive reserves to external stress factors, normalize sleep and restore working capacity. In the control group, changes in indicators did not have a signifi cant degree of signifi cance. The use of General or four-chamber radon baths in combination with ozone therapy at the rehabilitation stage in patients with occupational diseases of the joints causes an improvement in the functional activity of the musculoskeletal system, accelerates the disappearance of pain, relieves infl ammation, returns performance, normalizes the body’s adaptation to external infl uences.

Cervical cancer is one of the widespread diseases that have a negative impact on the reproductive health. However, medical-social risk factors which provoke this pathology aren't studied sufficiently yet. We have identified the most significant medical-social risk factors of cervical cancer's development and progression based on data analysis of a sociological study. Thus, it was found that cervical cancer can't be associated with predominantly late reproductive age of women in conditions of the Republic of Belarus: most of the patients with this pathology (more than 53%) were under the age of 35 years old. The group of patients suffered from cervical cancer was quite homogeneous in their social status, which was average and relatively stable in a large majority of them (more than 90%), despite employment in various sectors of the national economy. Social functioning of these patients wasn't significantly different from the main population of women in reproductive age. Thus, early sexual activity and random early sexual relations weren't characteristic for the majority of them: 86.1±4.46% (the control group – 87.3±5.51%, the comparison group – 78.2±3.96%) and 83.5±4.72% (the control group – 89.1±4.28%, the comparison group – 75.2±2.36%). Moreover, the married patients of the main group had the most ordered sexual behavior (p<0.01; tau=0.572) which determined the low prevalence of artificial abortions among them, including menstrual cycle regulation (less than 8%). However, the revealed different defects of health-saving behavior in the main group of patients (more than 70%), including an insufficient level of valeological knowledge, the presence of common harmful habits and low medical activity, indicate that there are significant reserves for improvement of the preventive activities of medical stuff in the field of primary medical care for the development of a healthy lifestyle among female population of reproductive age and providing clinical examination among this contingent of females in reproductive age with the purpose of cervical cancer prevention.

Improvement of reparative process of the chronic wounds with the insufficiency of peripheral blood suppling in patients suffering from diabetes mellitus is an important issue requiring further investigation. An optimal choice of surgery, effective renewal of blood supply and active stimulation of tissue repair on the cellular level are essential components of success treatment of this problem. Objective of research: From 2017 to 2020, the efficiency of one of the variants of organ-saving surgery performed on 210 patients with an ischemic-gangrenous form of diabetic foot syndrome was studied.Materials and methods. Control group included 104 patients (49.52%), were the treatment carried out in accordance with the standard scheme. The main group (106 patients - 50.47%) in addition to the standard therapy have used regional ozone therapy, vacuum sanitization and local application of autologous platelet-rich plasma (APRP).Results. These proposed measures reduced the period of clinical treatment to 24±1,2 days in the main group compared to 37±2,4 in the control one. 2 years later, patients of the main group confirmed the formation of a foot stump functionally adapted for walking with angular dislocation of fragments of the cuboid and scaphoid bones. Two years after foot amputation due to the ischemic-gangrenous form of diabetic foot syndrome (DFS), the amount of repeated above knee amputations was 1.8 times higher in the control group in comparison with the main group, which is indicative of a reasonable use of the complex of activate a reparative process in the wound. Conclusions: The suggested modification of the organ-saving and partial foot amputation in patients with IV degree of ischemia and DFS is indicative of a possibility to perform such kind of surgery as a variant of choice. The complex of proposed additional measures including regional ozone therapy, vacuum sanitization and local administration of autologous platelet-rich plasma activates a reparative process of a chronic foot wound and promotes an effective treatment of patients with ischemic-gangrenous form of DFS.

Background. Treatment of chronic cerebral ischemia should be aimed both to the main etiological factor and to the pathogenetic mechanisms of cerebral ischemia and hypoxia. The action of ozone therapy is characterized with a multilateral positive effect on the main aspects of chronic cerebral ischemia pathogenesis, such as chronic cerebral hypoxia, impaired microcirculation, rheological disorders, and activation of lipid peroxidation. The use of iodine-bromine baths in chronic cerebral ischemia has been proved to be accompanied by an improvement in cerebral circulation, normalization of the brain bioelectric processes, an initially impaired condition of the autonomic nervous system, and an increase in the level of adaptive capabilities of the body. Aim: to develop effective pathogenetically substantiated methods for the combined use of iodine-bromine baths and ozone therapy in patients with chronic cerebral ischemia to increase the effectiveness of health resort treatment. Methods. In accordance with the tasks set in the work, 90 patients with chronic cerebral ischemia of the degree 1, who were randomized into three treatment groups, were examined and treated. Patients of the control group (n = 30) received iodine-bromine baths, patients of the comparison group (n = 30) received ozone therapy, patients of the main group (n = 30) received iodine-bromine baths and ozone therapy. Examination of patients included Doppler ultrasound, electroencephalography, rheoencephalography, сardiointervalography, lipid metabolism studies, fibrinogen content studies. Results. As a result of treatment, the improvement of clinical symptoms and the normalization of biochemical and neurophysiological parameters were noted in patients of all treatment groups. Overall efficiency (improvement and significant improvement) was 53.3% in the control group, in the comparison group it was 63.3%, and 73.3% in the main group. Conclusion. The authors proved the practicability of rehabilitative treatment of early forms of vascular diseases of the brain at the stage of sanatorium and resort rehabilitation, which is due to significant compensatory reserves and high plasticity of the structural and functional formations of the brain with preserved ability to develop nonspecific elements of the nervous tissue and restore reversibly damaged structures.

The article reflects the results of the use of physical factors to restore the renal parenchyma in the early postoperative period after percutaneous nephroscopy in patients with urolithiasis. Material and methods: the study included 80patients with concretions of the renal pelvis after percutaneous nephroscopy (45men and 35women, average age — 46 years), who were divided into 2 comparable groups: groupI (main group) — 40 patients who underwent intravenous laser blood irradiation (ILBI) and ozone therapy — 6 procedures in the preoperative period, and polarized light and ILBI — 6 procedures in the early postoperative period; groupII (control group), which served as a background in the main group, were prescribed non-steroidal anti-inflammatory drugs after the surgery, taking into account bacterial urine culture. Inclusion criteria: patients with urolithiasis aged 28 to 62 years; indications for percutaneous nephroscopy. Exclusion criteria: concomitant diseases; compliance; pregnancy; acute inflammatory kidney diseases; abnormalities of the upper urinary tract. Results: The combined use of ILBI, ozone therapy and polarized light in the pre- and early postoperative period in patients with urolithiasis after percutaneous nephrolithotripsy allowed improving clinical, laboratory, subjective and objective indicators, as well as shortening the recovery period of the renal parenchyma, which was confirmed by the data of ultrasound studies in the dynamics in the form of a significant reduction in the recovery period of the index of renal vessels resistance and reduction of edema of the renal parenchyma. As a result of the application of the developed complex, the total bed-day of patients’ stay in the hospital decreased, and there was a rapid recovery of capacity to work. Conclusion: The developed method of the combined use of ILBI, ozone therapy and polarized light in the pre- and early postoperative period in patients with urolithiasis after percutaneous nephrolithiasis is pathogenetically justified and highly effective, which allows us to recommend it for wide use in urological practice.

Annotation. Diabetes is one of the most common metabolic diseases in the world with a large number of complications. Threatening conditions that significantly reduce the quality of life and even lead to the death include a diabetic foot syndrome. The aim of the study was to investigate the effectiveness of combined ozone therapy in the treatment of patients with type 2 diabetes with chronic lower extremity trophic ulcers. A prospective study of the effectiveness of using the combined method of ozone therapy in the treatment of trophic ulcers of the lower extremities was conducted in the surgical and rehabilitation departments of the University Clinic of the National Pirogov Memorial Medical University. The study involved 52 patients with diadetes mellitus 2 type, who were randomly assigned to the main (n=26) and control (n=26) groups. Subjective and objective changes in the condition of patients were evaluated: an area of trophic ulcers (by A.V. Kuleshov's method), a rate of ulcer’s defect epithelization (by L. Popova's formula) and the quality of life parameters of patients (by the questionnaire SF-36) before and after the treatment. The endpoint of the study was a complete epithelization of the diabetic trophic ulcer. The treatment was carried out according to the generally accepted methodology in both observation groups, and the combination ozone therapy was additionally used for patients of the main group. The latter provided for the intravenous administration of an oxygen-ozonized 0.9% NaCl solution and local exposure of the trophic ulcer defect to the ozonized mixture by chamber aeration. The course of complex treatment was carried out after removal of necrotic tissue and wound cleansing, consisted of 10 local and general ozone therapy procedures, which were carried out every other day. The data obtained were processed using the SPSS for Windows 10.0 and STATISTICA 6.1 application software package (license number BXXR901E245722FA). One month after, the observation changes of the trophic skin defects and Quality of life indicators showed the significantly (p<0.01) better results of treatment in the main group than in the control group. The endpoint of the study was achieved in 85% of patients after complex treatment versus 29% of patients in the control group after a conventional treatment (p<0.01). Using as modern rehabilitation method as ozone therapy is an appropriate complement to the existing treatment regimens, which can significantly accelerate the clinical recovery of patients and, as a result, improve the quality of life. The particular value of the method confirmed by an absence of contraindications and drug overloads of the patient. The study of the effects of ozone therapy on the metabolism of patients with diabetes mellitus type 2 can be a promising area for further research.
