Quantitative research that aimed to identify the mean total cost (MTC) of connecting, maintaining and disconnecting patient-controlled analgesia pump (PCA) in the management of pain. The non-probabilistic sample corresponded to the observation of 81 procedures in 17 units of the Central Institute of the Clinics Hospital, Faculty of Medicine, University of Sao Paulo. We calculated the MTC multiplying by the time spent by nurses at a unit cost of direct labor, adding the cost of materials and medications/solutions. The MTC of connecting was R$ 107.91; maintenance R$ 110.55 and disconnecting R$ 4.94. The results found will subsidize discussions about the need to transfer money from the Unified Health System to hospitals units that perform this technique of analgesic therapy and it will contribute to the cost management aimed at making efficient and effective decision-making in the allocation of available resources.

OBJECTIVE Identify the direct cost of reprocessing double and single cotton-woven drapes of the surgical LAP package. METHOD A quantitative, exploratory and descriptive case study, performed at a teaching hospital. The direct cost of reprocessing cotton-woven surgical drapes was calculated by multiplying the time spent by professionals involved in reprocessing the unit with the direct cost of labor, adding to the cost of materials. The Brazilian currency (R$) originally used for the calculations was converted to US currency at the rate of US$0.42/R$. RESULTS The average total cost for surgical LAP package was US$9.72, with the predominance being in the cost of materials (US$8.70 or 89.65%). It is noteworthy that the average total cost of materials was mostly impacted by the cost of the cotton-woven drapes (US$7.99 or 91.90%). CONCLUSION The knowledge gained will subsidize discussions about replacing reusable cotton-woven surgical drapes for disposable ones, favoring arguments regarding the advantages and disadvantages of this possibility considering human resources, materials, as well as structural, environmental and financial resources.

To identify the direct cost of procedures related to an outpatient chemotherapy treatment for women with breast cancer. Method: This is a quantitative research, using the case study methodology, performed in an outpatient chemotherapy of a private hospital. The total cost was calculated by multiplying the time spent by professionals involved in therapeutic procedures, the unit cost of direct labor, adding to the cost of materials, drugs and solutions. For performing the calculations, we used the Brazilian currency (R$). Results: The average total cost per chemotherapy session corresponded to R$ 1,783.01 (100%), being R$ 1,671.66 (93,75%) spent with drugs, R$ 74,98 (4.21%) with materials, R$ 28.49 (1.60%) with labor and R$ 7.88 (0.44%) with solutions. Conclusion: The results may support discussions and decision making for the management of costs related to chemotherapy aimed at reducing expenses and eliminating waste without harm to the care provided.

ABSTRACT Objective: To measure the average direct cost of peripherally inserted central catheterization performed by nurses in a pediatric and neonatal intensive care unit. Method: A quantitative, exploratory-descriptive, single-case study, whose sample consisted of the non-participant observation of 101 peripherally inserted central catheter procedures. The cost was calculated by multiplying the execution time (timed using a chronometer) spent by nursing professionals, participants in the procedure, by the unit cost of direct labor, added to the cost of materials, drugs, and solutions. Results: The average direct cost of the procedure was US$ 326.95 (standard deviation = US$ 84.47), ranging from US$ 99.03 to US$ 530.71, with a median of US$ 326.17. It was impacted by material costs and the direct labor of the nurses. Conclusion: The measurement of the average direct cost of the peripherally inserted central catheter procedure shed light on the financials of consumed resources, indicating possibilities of intervention aiming to increase efficiency in allocating these resources.

ABSTRACT Objective: to identify the average direct cost of maintaining the patency of totally implanted central venous catheter with heparin at a Day Hospital of a public hospital of high complexity specialized in the treatment of cancer patients, and estimate the average direct cost of replacing heparin with sodium chloride 0.9%. Method: quantitative, exploratory-descriptive study, with a sample of 200 non-participant observations of the maintenance of totally implanted central venous catheters with heparin. The average direct cost was calculated by multiplying the (clocked) time spent by professionals to complete the procedure by the direct unit cost of workforce, added to the cost of materials and solutions. Results: the estimated total direct cost of catheter maintenance with heparin was US$ 9.71 (SD=1.35) on average, ranging from US$ 7.98 to US$ 23.28. The estimated total direct cost of maintenance with 0.9% sodium chloride in the place of heparin was US$ 8.81 (SD=1.29) on average, resulting in a reduction of US$ 0.90 per procedure. Conclusion: the results contributed to propose strategies to assist in cost containment/minimization in this procedure. The replacement of heparin by 0.9% sodium chloride proved to be an option to reduce the total average direct cost.

H.influenzae type b, the sero-type which causes meningitis, was isolated from 3 (2·4 %) of 127 naso-pharyngeal swabs from white non-Jamaican children resident in Jamaica, and from 14 (2·9 %) of 473 such swabs from a mixed group of Jamaican children. Much higher frequencies were found in households in which cases of haemophilus meningitis had recently occurred (41 %), in the nursery of an orphan home (up to 70%) and in a day-nursery. In the orphan home nursery the high frequency persisted over a number of months, but no case of meningitis or other relevant disease occurred. A high concentration of this organism is therefore compatible with normal health, and the significance of high concentrations in the homes of meningitis patients is not certain.I am indebted to the Standing Advisory Committee for Medical Research in the British Caribbean for a grant towards the cost of materials; to a number of colleagues at the University College of the West Indies, notably Dr E. Technological advancement, reduction in the cost of materials, and Government support for sustainable development help improvement in solar PV technology in recent years. Photovoltaic technology is majorly employed to generated electricity worldwide in the renewable energy category. To establish a good market base for Solar PV the efficiency of solar cells plays a significant role. Presently, extensive research work is going for efficiency improvement of solar cells for commercial use. The efficiency of monocrystalline silicon solar cells has shown very good improvement year by year. It starts with only 15% in the 1950s and then increases to 17% in the 1970s and continuously increases up to 28% nowadays. The growth in solar photovoltaic technologies including worldwide status, materials for solar cells, efficiency, factor affecting the performance of PV modules, an overview of a cost analysis of PV, and its environmental impact are reviewed in this paper.

In this paper, an efficient, affordable, and portable manual automobile tyre changing tool was designed. The tool operates on the principle of second-class levers, where the load to be overcome is situated between the fulcrum and the effort point. The mechanical advantage (MA) of the bead breaker arm and pry bar assembly of the tool was determined to be 11.5. The standard tube size of the bead breaker arm that can withstand stress due to bending moment was determined to be a 2” (50mm) diameter Schedule 80 steel tube. Also, the dimensions of the standard hexagonal bolts to be used at the fulcrum and load point of the bead breaker arm are M12 × 1.5 × 75 mm, while the standard size of hexagonal nuts to be used with the bolts is M12 × 1.5 mm with a height of 10mm. Moreover, the mechanical advantage (MA) of the mount/demount arm and pry bar assembly of the tool was determined to be 5.22. Also, by comparing the angles of twist of two sizes of the mount/demount arm when the tool is used in mounting or demounting a tyre on a 16” × 7” wheel rim, the standard size of the mount/demount arm that can withstand stresses due to bending moment and torsional moment was determined to be a 60mm × 60mm × 5mm square tube. The cost of materials needed to fabricate the tyre changer summed up to ₦21,000. Ergo, the design provides an alternative portable and relatively affordable tyre changing tool that can be afforded by tyre technicians across Nigeria, and other developing or underdeveloped countries.

The hoist assembly based on the Koeppe friction is a commonly used solution in mining. However, it has some disadvantages. A few centimeters offset of the groove axis can lead to excessive abrasion of linings on the Koeppe friction and pulleys. As a consequence, the mines are forced to bear the direct and indirect costs of replacing the linings such as the cost of materials and service as well as the cost of extended machine and shaft downtime. Last year, the authors undertook a geodetic inventory of the condition of two hoisting machines with a Koeppe winder. Terrestrial laser scanning enhanced with precision total station measurements were performed. Additionally, elements particularly important for the performed analysis (inclination of hoisting machine and rope wheels shafts) were determined by the precision leveling technique. Obtained results were verified using measurements on Szpetkowski’s tribrach. Appropriate selection of the measurement methods in both analyzed examples allowed us to determine the causes of destruction of each hoist assembly component. Based on precise geodetic data, guidelines have been defined for rectification (twisting and shifting the rope pulleys), which seems unavoidable despite the lack of unambiguous legal regulations.

SEMATECH evaluated the impact of various process options on the overall manufacturing cost of a TSV module, from TSV lithography and etch through post-plate CMP. The purpose of this work was to understand the cost differences of these options in order to identify opportunities to significantly reduce cost. Included in this study were multiple process and materials options for TSV etch, liner, and barrier/seed (B/S). For each of these options, recipes were adjusted for post-etch clean, ECD Cu fill and CMP overburden, and the resulting cost impacts were evaluated. The TSV dimensions used in this study are 5x50 μm and 2x40 μm. These cost comparisons included a sensitivity analysis, highlighting the main factors responsible for the differences. Cost of materials, tool cost, and throughput were the primary factors affecting cost differences, especially in barrier/seed deposition. In some cases the contributions from both these sources were comparable. We explain the assumptions used and some of the uncertainties inherent in this work. For example, where materials costs were significant, we extrapolated the cost of new materials from research quantities to those needed to support high volume manufacturing. We had to estimate throughputs and materials costs using our best engineering judgment, because the recipes have not yet been optimized. We also considered that the tools used on some non-critical steps might be fully depreciated, or a lower cost tool such as is used in wafer level packaging. Despite these uncertainties and assumptions, we were able to extract some fairly clear conclusions. The process options include the following B/S variations: For 5x50 μm TSVs, the B/S film structure is TaN/Ta/Ru/Cu, and the options are with and without the Ru and/or Cu layers. For 2x40 μm TSVs, the B/S structure is TaN/Ru/Cu, with different thicknesses of Ru, and the Cu is an optional seed layer for the field. We also discuss the impact of scaling the TSV dimensions on manufacturing costs. This work is continuing to look at different process options and to apply this methodology to MEOL modules such as temporary bond and debond, wafer thinning, and TSV reveal.
