Εισαγωγή: Η τοποθέτηση ενδοπροσθέσεων στα χοληφόρα αποτελεί πλέον μια καθιερωμένη μέθοδο για την αντιμετώπιση κακοήθων και καλοήθων παθήσεων των χοληφόρων. Ωστόσο, το μεγαλύτερο μειονέκτημα των πλαστικών ενδοπροσθέσεων είναι η απόφραξη τους εξαιτίας του μικροβιακού αποικισμό τους.Μέθοδος: Σε διάστημα τριών ετών, 51 πλαστικές ενδοπροσθέσεις αφαιρέθηκαν από 42 ασθενείς. Είκοσι τρεις ενδοπροσθέσεις τοποθετήθηκαν για την αντιμετώπιση κακοήθων και 28 για καλοήθων παθήσεων. Το προς εξέταση υλικό ελήφθη βάσει συγκεκριμένου πρωτόκολλου και εστάλη άμεσα για καλλιέργεια.Αποτελέσματα: Σχεδόν όλες οι ενδοπροσθέσεις παρουσίασαν πολυμικροβιακή ανάπτυξη. Οι συχνότερα απομονωθέντες μικροοργανισμοί ήταν ο Enterococcus spp (74%),η E.coli (62%) και η Klebsiella spp (58%). Η E.coli αναπτύχθηκε συχνότερα σε καλοήθεις απ’ότι σε κακοήθεις παθήσεις (78% vs. 43%, p<0.05). Η Klebsiella spp, η Pseudomonas spp και η Candida spp απομονώθηκαν συχνότερα σε αποφραγμένες σε σχέση με τις μη αποφραγμένες ενδοπροσθέσεις, 68% vs. 37%, 22% vs. 0 και 40% vs. 6% αντίστοιχα (p<0.05). Η E.coli και η Pseudomonas spp παρουσίασαν 34% και 50% αντίσταση στις κινολόνες αντίστοιχα. Το 35% των εντεροβακτηροειδών εμφάνισαν υπερπαραγωγή της Amp-C. Τέλος, ο Enterococcus spp, η Klebsiella spp και η Pseudomonas spp δεν ανέπτυξαν υψηλές αντιστάσεις σε προωθημένα αντιβιοτικά.Συμπέρασμα: Τα συχνότερα απομονωθέντα βακτήρια σε πλαστικές ενδοπροσθέσεις ήταν κατά σειρά ο Enterococcus spp,η E.coli και η Klebsiella spp. Στις αποφραγμένες ενδοπροσθέσεις η Pseudomonas spp και η Candida spp πρέπει να υπολογίζονται στο σχεδιασμό της θεραπείας. Οι κινολόνες φαίνεται ότι δεν είναι πλέον επαρκείς στην αντιμετώπιση χολαγγειΐτιδας σε αποφραγμένες ενδοπροσθέσεις. Τέλος, σε ασθενείς υπό χημειοθεραπεία και σήψη σχετιζόμενη με απόφραξη των ενδοπροσθέσεων θα πρέπει να εξετάζεται η χρήση αντιμυκητιασικών και έναντι του εντερόκοκκου παραγόντων.

Las infecciones del tracto urinario son las infecciones más comunes en todo el mundo y se presentan en todos los grupos de población, se estima que entre el 40% y el 50% de las mujeres  presentarán al menos una durante su vida, y de estas el  11% tendrá una infección al año, siendo la causa más frecuente de origen bacteriano (80%-90%). Constituyen una causa frecuente de consulta en atención primaria, afecta mayoritariamente a las mujeres, generalmente son monobacterianas ,sus  agentes etiológicos incluyen :  Escherichia coli, Klebsiella spp, Proteus spp, Enterococcus spp, Pseudomonas spp y Staphylococcus spp, los cuales presentan prevalencias y perfiles de susceptibilidad antibiótica diferentes entre poblaciones por lo que es relevante contar con esta  información para generar un perfil clínico-epidemiológico propio que permita la implementación de medidas terapéuticas correctas, así como  la atención y control de la resistencia bacteriana, acordes con las particularidades de la población del lugar de estudio.

Introducción: Las infecciones del tracto urinario (ITU), constituyen una de las infecciones bacterianas más prevalentes, sus agentes etiológicos incluyen Escherichia coli, Klebsiella spp, Proteus spp, Enterococcus spp, Pseudomonas spp y Staphylococcus spp, los cuales presentan prevalencias y perfiles de susceptibilidad antibiótica diferentes entre poblaciones. Objetivo: Determinar la prevalencia de ITU, uropatógenos y el perfil de susceptibilidad antimicrobiana en una Institución Prestadora de Servicios de Salud (IPS) de Medellín, 2011-2012. Métodos: Estudio de prevalencia en 1.959 individuos atendidos en una IPS de tercer nivel. Se calcularon medidas de resumen, proporciones, razones de prevalencia, Chi cuadrado y Fisher. Se cuantificó la modificación del efecto (confusión o interacción) con análisis estratificado y modelos de regresión logística binaria en SPSS 21.0®. Resultados: La prevalencia de ITU fue 31% los principales agentes etiológicos fueron E. coli (69%), Enterococcus spp (11%) y Klebsiella spp (8%). La ITU y la infección por E. coli fueron estadísticamente mayores en mujeres y adultos mayores. La mayor frecuencia de resistencia de E. coli fue para ampicilina (61%), Ácido nalidixico (48%), TrimetropinSulfa (48%) y Ciprofloxacina (42%); mientras que en Klebsiella spp fue Ampicilina (100%), TrimetropinSulfa (23%), Ampicilina- Sulbactam (22%) y Cefalotina (19%). Conclusión: La elevada prevalencia de ITU, la multiplicidad de uropatógenos aislados, la identificación de grupos de mayor riesgo y la diversidad de perfiles de resistencia antibiótica, evidencian la necesidad de desarrollar investigaciones locales que permitan orientar las acciones en salud y vigilancia epidemiológica, acordes con las particularidades de cada población.

As infecções bacterianas do trato urinário (ITUs) são causa comum de doença em cães, gatos e humanos. Embora bactérias Gram positivas como Staphylococcus spp., Streptococcus spp. e Enterococcus spp., possam ocasionar ITUs, as bactérias Gram negativas (Escherichia coli, Proteus spp., Klebsiella spp., Pseudomonas spp. e Enterobacter spp.) respondem por 75% dos casos. Este estudo teve como objetivo determinar a frequência de diferentes gêneros de bactérias em ITUs em cães e gatos, bem como a sua sensibilidade aos antimicrobianos utilizados na rotina clínica. Portanto, amostras de urina de 100 cães e gatos com sinais de ITU foram coletadas assepticamente, sofrendo avaliação microbiológica por meio de métodos qualitativos e quantitativos, além de urinálise. Todos os isolados foram submetidos a testes de sensibilidade aos antimicrobianos. ITU foi confirmada em 74% dos animais, não havendo predominância quanto ao sexo. No que diz respeito à idade, 85% dos cães e 87% dos gatos tinham idades superiores a seis anos. Noventa e cinco cepas bacterianas foram isoladas, com maior frequência de Escherichia coli (55% do total) dos sorogrupos O6 e O2. Constatou-se níveis elevados de resistência a antimicrobianos nas cepas isoladas. Para as cepas Gram positivas, tetraciclina (46,1%), enrofloxacina, cotrimazol e estreptomicina (42,3% cada) foram as drogas com os maiores índices de resistência. Para as Gram negativas, amoxacilina e tetraciclina apresentaram percentuais acima de 50%. Multiresistência foi verificada em mais de 50% dos principais gêneros isolados. Considerando-se que as cepas de E. coli apresentam potencial zoonótico e forte participação na disseminação de resistência aos antimicrobianos, ressalta-se a importância do papel do médico veterinário na prevenção e controle das ITUs animais e sua contribuição para a saúde pública.

The article deals with the results of search of the influence of intestinal parasites on the indicators of antilysozyme and anticomplementary activity of microorganism of distal department of intestine. It was established, that the products of intestinal parasites of pigs reduce anti lysozyme activity of such obligate anaerobes as Bifidobacterium spp. to 42.1% (р < 0.01), Prevotella spp. to 14.3%, Clostridium spр. to 66.7% (р < 0,001). With MAFAnM to 38.5% (р < 0.05) in Lactobacillus spp. and to 50.0% (р < 0.01) in Streptococcus spp. Higher compared with the control, antilysozyme activity was in Bacteroides spp. (р < 0.05), Eubacterium spp. (р < 0,001), Propionіbacterium spp. (р < 0.01), Peptostreptococcus spp. (р < 0.05), Fusobacterium spp. (р < 0.01), E. coli lact. «+» hem «-» (р < 0.05), Klebsiella spp. (р < 0.001), Citrobacter spp. (р < 0.01), Enterococcus spp. (р < 0.05), Staphylococcus spp. (р<0.001), Candida spp. (р<0.001) abstracted from the distal intestine of pigs affected with mixed nematodosis - protoozic invasion. With the action of products of vital functions of ascarices, eymeria and balantides, the anticomplementary activity significantly increases in Prevotella spp. (р < 0.01), Clostridium spр. (р < 0.001), Eubacterium spp. (р < 0.001), Propionіbacterium spp. (р < 0.001), Peptostreptococcus spp. (р < 0,001), Fusobacterium spp. (р < 0.001), Lactobacillus spp. (р < 0.05), E. coli lact. «+» hem «-» (р < 0.01), E. coli lact. «-» hem «+» (р < 0.001), Klebsiella spp. (р < 0.05), Enterobacter spp. (р < 0.001), Enterococcus spp. (р < 0.001), Streptococcus spp. (р < 0.001) and only in Bacteroides spp. it was significantly lower (р < 0.001), compared with the control.

The changes in lipolytic, lysozyme and hemolytic activity of the intestinal microflora of piglets invaded with the association of ascaris, imyrium and balantidium have been studied. It has been established that among microorganisms isolated from the distal intestine of piglets, lipase is synthesized by Bacteroides spp., Prevotella spp., Clostridium sp. and Peptostreptococcus spp., E. coli lact. «-» hem +, Enterobacter spp., Streptococcus spp., Staphylococcus spp. and Candida spp. In pigs affected by ascaris, emerya, and balantidium, the number of strains producing lipase was higher compared to healthy ones, namely: Bacteroides spp. – by 20.2%, Prevotella spp. – by 3.7%, Clostridium spp. – 29.2% and Peptostreptococcus spp. – by 14.9%, Staphylococcus spp. – by 67.7%, Streptococcus spp. – by 19.1%, E. coli lact. «-» heme «+» – by 17.3%, Candida spp. – 16.5% and Enterobacter spp. – by 4.8%. Lysozyme activity was detected in Bifidobacterium spp., Lactobacillus spp. and Staphylococcus spp. Significant differences in the penetrance of lysozyme activity of microorganisms in healthy and affected by ascarids, imedium, and balantidias of piglets were observed in Lactobacillus spp. and Staphylococcus spp. (P < 0.001). At the same time, in piglets, compared with healthy animals, the number of Lactobacillus spp. Strains that showed lysozyme activity decreased, while Bifidobacterium spp. and Staphylococcus spp. – grew. Bifidobacterium spp., Eubacterium spp., Lactobacillus spp., E. coli lact. «+» Hem «-», Enterobacter spp. and Citrobacter spp. there is no ability to produce hemolysins. Among lactosonegative gastric sticks, all strains isolated from both healthy and sick pigs showed hemolytic activity. The associative influence of ascaris, imerium and balantidii, showed an increase in the penetrance of hemolytic activity in Bacteroides spp. – by 45.8%, Prevotella spp. – by 26.8%, Clostridium spp. – 42.1%, Propionibacterium spp. – 41.5%, Peptostreptococcus spp. – 29.1% and Fusobacterium spp. – by 16.4%, Klebsiella spp. – by 33.4%, Enterococcus spp. – by 69.6%, Streptococcus spp. – by 66.0%, Staphylococcus spp. – by 64.5% and Candida spp. – by 11.5%.

Zusammenfassung Gegenstand und Ziel Bakterielle Hautinfektionen kommen bei Reptilien häufig vor. Obwohl viele dieser Infektionen durch multifaktorielle Probleme verursacht werden, ist eine spezifische Behandlung nötig. Ziel der Studie war, das Keimspektrum und die Resistenzlage von aeroben Bakterien in Hautläsionen von Reptilien zu untersuchen. Material und Methoden Tupferproben dermaler Läsionen von 219 Reptilien wurden bakteriologisch untersucht (01/2017–06/2018). Die Identifizierung der Bakterien erfolgte anhand von Selektivnährböden, biochemischen Parametern sowie MALDI-TOF-MS, die Erstellung der Antibiogramme mittels Mikrodilutionsmethode. Ergebnisse Bei den insgesamt identifizierten 306 Keimisolaten handelte es sich überwiegend um gramnegative Spezies. Pseudomonas spp. (n = 48), Citrobacter spp. (n = 31, nur bei Schildkröten), aerobe Sporenbildner (n = 30), Aeromonas spp. (n = 20), Acinetobacter spp. (n = 20), Proteus spp. (n = 15), Staphylococcus spp. (n = 15), Klebsiella spp. (n = 13), Enterococcus spp. (n = 13) sowie Morganella spp. (n = 11) machten den Hauptteil aus, daneben konnten weitere gramnegative (n = 78) und grampositive (n = 12) Bakterienspezies identifiziert werden. Mischkulturen mit 2 (n = 80) oder mehr (n = 16) Keimen traten bei 96 Tieren auf. Von 208 der 306 Isolate wurden Antibiogramme erstellt. Gegenüber Enro- (E) und Marbofloxacin (M) waren viele Isolate sensibel (minimale Hemmkonzentration [MHK] in µg/ml ≤ Grenzwert), beispielsweise Pseudomonas spp. (E: 86,4 % MHK ≤ 0,5; M: 95,5 % MHK ≤ 1), Citrobacter spp. (E: 86,4 % MHK ≤ 0,5; M: 90,9 % MHK ≤ 1) und Aeromonas spp. (E: 75,0 % MHK ≤ 0,5; M: 100 % MHK ≤ 1). Trimethoprim/Sulfamethoxazol erwies sich als wirksam gegen die meisten Citrobacter- (90,9 % MHK ≤ 2/38) und Aeromonas- (75,0 % MHK ≤ 2/38) Isolate. Amikacin war wirksam gegen fast alle Pseudomonas spp. (97,7 % MHK ≤ 16), Citrobacter spp. (95,5 % MHK ≤ 16) sowie Aeromonas spp. (93,8 % MHK ≤ 16). Schlussfolgerung und klinische Relevanz Das Keimspektrum von Reptilienhautläsionen umfasst vor allem gramnegative Bakterien, deren klinische Relevanz für jeden Einzelfall abzuwägen ist. Viele Isolate dieser Studie waren sensibel für Fluorchinolone sowie Aminoglykoside. Da der Einsatz dieser Antibiotika zurückhaltend erfolgen sollte und gegenüber jeder getesteten Antibiotikagruppe auch resistente Isolate identifiziert wurden, wird eine Antibiogrammerstellung empfohlen.

Background: Urinary tract infection (UTI) is a very common infection worldwide and drug resistance makes the treatment more difficult which may lead to complications along with extended hospital stay. Therefore, this study was designed to find out the prevalence of UTI, and antimicrobial susceptibility pattern of isolates collected from patients of UTI.Methods: This study was conducted among 1,760 suspected UTI patients from January 2019 to November 2020. Urine samples from mid-stream were transferred to microbiology laboratory following microbiological standard protocol. Microbial identification was performed by microscopy, colony morphology, and biochemical characteristics. Antibiogram was pursued following the standard Kirby-Bauer disk diffusion method.Results: Our study showed almost 30% of samples were positive and E. coli was found as the highest predominating organism (50.09%) followed by Klebsiella spp., Pseudomonas spp., Enterococcus spp., and Citrobacter spp. The percentage of infection in females was 65.28% which is higher than males. The highest prevalence of UTI has been observed in females aged 21-30 years and males aged 31-40 years. However, E. coli and Pseudomonas spp. showed the highest resistance against Amoxiclav whereas, Klebsiella spp. showed it against co-trimoxazole. Citrobacter spp. showed 100% resistance towards Amikacin, cefixime, nalidixic acid, co-trimoxazole, amoxiclav, ceftazidime, and Enterococcus spp. showed against nalidixic acid and amoxiclav. Moreover, 95.8% isolates collected were resistant towards 3 or more than 3 antibiotics which is very alarming.Conclusions: Changing trend in antibiotic sensitivity profile of the isolates needs to be monitored as there is limited availability of newer drugs.

Because of importance of Hospital acquired infections (HAIs), it is critical to conduct surveillance studies to obtain the required data about the regional microorganisms and their susceptibility to antibiotics. This study to investigate antimicrobial resistance pattern among Intensive Care Unit (ICU) patients in a private medical college hospital setup. In a cross sectional study, 100 specimens from patients admitted in the ICU who had signs or symptoms of nosocomial infection were collected from 2012 - 2013. For each patient, samples of blood, urine, tracheal aspirate, sputum, wound swab, pus, and endotracheal tubes were obtained, cultured and analyzed with antibiogram. The most common primary diagnosis were aspiration pneumonia (49%) and UTI (20%) respectively. The most common locations for infection were tracheal aspirate (54%). The most frequent gram negative microorganisms derived from samples were Acinetobacter spp (29%), Klebsiella spp (26%) and Pseudomonas spp (18%). Klebsiella spp, Acinetobacter spp and Pseudomonas spp were most common resistant organisms among all. Klebsiella spp were resistant against Ceftriaxone (84.6%), Ceftazidime (82.6%), Amikacin (46.1%), Gentamicin (66.6%) and Quinolones (65-66.6%) respectively. Acinetobacter spp were resistant against Ceftriaxone (85%), Ceftazidime (88.8%), Cefotaxime (85.7%), Meropenem (79.3%),Amikacin (86.2%), Gentamicin (84.5%) and Quinolons (86.2-89.2%) respectively. Pseudomonas spp were resistant against Ceftriaxone (70.5%), Ceftazidime (66.6%), Amikacin (68.7%), Gentamicin (58.8%), Meropenem (52.9%) and Quinolones (81.2-86.6%) respectively. Meropenem was the most sensitive antibiotic against Klebsiella spp (84.6%) but Cotrimoxazole in case of Acinetobacter spp (60%) respectively. Escherichia coli were mostly isolated from urine, which was sensitive to Amikacin (73.3%) and Meropenem (86.6%) respectively. Gram-negative pathogens obtained from ICU patients in our settings show high resistance to antibiotics. Regular monitoring of the pattern of resistance of common pathogens in the ICUs is essential to up-to-date the use of rational antibiotics regiments.Bangladesh J Medicine Jul 2014; 25 (2) : 47-51

Hospital Acquired Infections (HAIs) are a significant concern for healthcare setups, as it increases the overall cost of treatment, patients stay in hospitals, making them susceptible to secondary and tertiary infections and, sometimes, mortality1. To prevent or control HAIs, evaluating the organisms isolated from the critically maintained areas is considered of epitome importance and everlasting practice in the healthcare industry. Identifying such organisms and screening them for antibiotic resistance is mandatory, but it also helps professionals understand colonization trends. Sensitive areas of healthcare setups were screened monthly from years 2017 to 2020. A total of 4400 samples of hospital hygiene, e.g., intravenous drip stands, ventilator surface, anesthetist’s trolley, patient’s bed, instrument trolley, etcetera, were collected. Isolated organisms were cultured and screened using the CLSI technique. E. coli, Pseudomonas spp., and Klebsiella spp. were found in both previous to COVID current samples. Multidrug-resistant organisms were subjected to molecular characterization to detect the presence of carbapenem genes. Evaluation data of both pre-and during Coronavirus Disease or COVID-19 were compared. The prevalence of pathogenic (Klebsiella spp., E. coli, and Pseudomonas spp.) and non-pathogenic (Staphylococcus aureus and Bacillus spp.) strains in healthcare setups decreased drastically (Klebsiella spp. from 80% to 20%, E.coli from 90% to 10% and Pseudomonas spp. from 80% to 20%). It is possible only because of the awareness in non-specialists and healthcare workers due to the unforeseen critical situation proving to be a blessing for the future generation.

In the structure of diseases of the urinary system in children, microbial-inflammatory lesions of the kidneys and urinary tract are 70–80 %. The prevalence of infections of urinary system in children in the Russian Federation is on average 18–22 per 1000 children’s population. The aim of the study was to analyze the spectrum of infectious agents of urinary infection in children and to determine the dynamics of bacterial resistance. We analyzed 330 cases of urinary system infection in children aged from 2 months up to 18 years in two time peri- ods – 2002–2004 and 2014–2016 (n = 114 and n = 216, respectively). Patients of both periods were divided into three groups: the first one – cystitis, the second one – pyelonephritis and the third group – a combination of pyelonephritis and cystitis. An obligatory criterion for including the patient in the study was the presence of a pathogmonic combina- tion of leukocyturia and bacteriuria, 10 5 KOE/ml, for the infection of the urinary system. The main causative agent of urinary infection in children remains E. coli. An increase is marked in the specific gravity of E. coli, Enterococcus spp. and Klebsiella spp., Pr. vulgaris. The study revealed the growth of E. coli resistance to cephalosporins of II, III, IV generations; decreased resistance of Enterococcus spp. to ampicillin and gentamicin. In the study of the sensitivity of Klebsiella spp. an increase was detected in resistance to cephalosporins of the III generation. We identified the main pathogens of infections of the urinary system and determined bacterial resistance.
