Systemic sclerosis, although a rare disease, has numerous direct and indirect effects on the oral cavity. This article aims to inform clinicians of the signs and symptoms they may see in patients with the disease and the impact it can have on dental management. The main effect, microstomia, is not just limited to systemic sclerosis, thus the clinical and laboratory techniques described may be helpful when managing other patients with limited mouth opening. CPD/Clinical Relevance: Clinicians should be able to recognize how systemic sclerosis will have an impact on their patient's oral health and implement management plans to prevent deterioration of their dentition. The novel techniques used will provide dentists and dental technicians with alternative techniques that can be employed in a variety of cases when considering prosthetic rehabilitation.

The osteomyelitis is an inflammatory process in bone tissue caused by an infection, commonly related to anaerobic pathogens, frequently Staphylococcus aureus and Streptococcus sp. Several causes have been related such as dentoalveolar infection, trauma, radiation and genetic condition, but it is not common after surgery for third molar extractions, especially in healthy patients. The symptoms of chronic osteomyelitis usually include signs and symptoms such as pain, edema, suppuration, areas of bone sequestration, and limited mouth opening. Among the complications associated with this condition, pathological fracture may occur due to local bone fragility. This paper aimed to report a rare case of pathological mandible fracture due to osteomyelitis after third molar extraction in a healthy male patient treated by a surgical procedure. The procedure consisted of decortication and resection of the sclerotic bone followed by reduce and fixation of mandibular fracture with one 2.4 reconstructive plate and one 2.0 plate with standard screws.

Background: TMJ disorder is define as a condition of acute or chronic inflammation of TMJ in association with mandibular, myofunctional condition, teeth malposition and or malocclusion. TMJ disorder could lead to significant pain and anatomical damage. There are signs and symptoms of TMJ disorder, such as devition of jaw movement, clicking when openingor close the jaw, mastication disorder and decreasing interincisal distance. Purpose: To describe the result of orthodontic treatment with begg technique and trainer for braces on angle’s class I malocclusion with agenese involving 12, 14, 34, and 44 along with TMJ disorder and bad habit which is one side mastication. Case: 19 years old female complaining limited mouth opening, pain on the right side of TMJ when opening the mouth and when chewing hard food and also complaining of multiple diastema on upper and lower teeth. Diagnose : Angle’s class I with  bidental protrusive, 3mm midline midline shifting  to the left side, agenese involving 12,13,34,44, and one side mastication. Treatment : Patient treated witg begg technique and braces which is wear 1 hour at day time distance, and feel comfort when chewing hard  foord  and after 24 months of treatment with fixed begg appliance, malposition and interdigitation are corrected.

Patients who undergo surgical resection of the maxillo-mandibular structures as a result of trauma, infection or malignancy, suffer from psycho-social setbacks which has a profound impact on their over-all quality of life. 1,2 These defects, especially those following maxillectomy, result in oroantral communication, facial deformation, impaired speech and difficulty in deglutition. For the rehabilitation of patients with such defects, surgical and prosthetic treatment options are available. As, not all patients can be successfully rehabilitated with reconstructive surgeries due to postoperative complications like graft rejection, the extent of the surgical defect and high psychological impact factor associated with repeated surgeries, prosthetic rehabilitation proves to be an alternative treatment option. The prosthetic rehabilitation of such patients is challenging as it requires restoration of the lost form, function and aesthetics, under constantly changing state of post-surgical intraoral tissues, with limited mouth opening. The maxillofacial prosthesis designed to close congenital or an acquired tissue opening, primarily of the hard palate, is known as an obturator. 3 The obturator has two functional components, one seals the surgical defect and the other replaces the lost dentoalveolarstructures.4-7 The design of an obturator may vary depending on the extent of the defect, remnant dentoalveolar complex, soft tissue undercuts and existent muscle physiology.8,9 Among the two designs, solid and hollow, hollow obturators are widely used. The bulb portion of the hollow obturator, which accommodates the surgical defect, can be open or closed9,10and its selection depends on the prosthodontist’s clinical decision-making skills and the ease of fabrication. In this article we have discussed the rehabilitated patients with single-piece, openhollow bulb definitive obturator. Patients undergo extensive maxillary surgical resections due to aggressive lesions like malignancies and deep fungal infections. Prosthetic rehabilitation of such patients with an obturator becomes of paramount importance as it separates the oropharynx from the nasopharynx, reduces the risk of recurrent infections, replaces lost dentoalveolar structures, permits intelligible speech, reinstates mastication and deglutition, restores facial contour and patient’s self-esteem. The bulb portion of the obturator extends into the defect and accommodates it, forming a hermetic seal. In this clinical report, we highlight the success of prosthetic rehabilitation of maxillectomy patients using single-piece, open-hollow bulb definitive obturator. The meticulous follow-up carried out reveals the success of the prosthesis and adds practice-based evidence to the maxillectomy rehabilitation outcome.

Abstract Aim The purpose of this clinical report is to present a description of the prosthetic rehabilitation of a bilateral complete maxillectomy patient using a two piece magnetically connected prosthesis. Background A complete bilateral maxillectomy defect presents a considerable reconstructive challenge for the prosthodontist. It results in devastating effects on cosmetic, functional, and psychological aspects of the patient. Report A 46-year-old woman reported with a chief complaint of missing teeth in the upper jaw. Her primary concerns were a poor facial appearance, inability to chew food, and regurgitation of the food into the nasal cavity. She was diagnosed with carcinoma of the maxillary sinus, for which a bilateral maxillectomy was done followed by post surgical radiation therapy. The prosthetic treatment objectives were to separate the nasal and oral cavities, restore the mid-facial contour, and improve her masticatory functions by providing a full complement of maxillary teeth using a two-piece connected hollow obturator prosthesis connected by a magnet. Summary Insertion and removal of a large prostheses used for rehabilitation of midfacial defects requires good neuromotor coordination and an adequate mouth opening. Because these factors were problematic for this patient, the treatment plan was to fabricate a two piece magnetically connected prosthesis. After fabrication and insertion of the prosthesis, the fit between two sections was evaluated and instructions for insertion, removal, and maintenance of the obturator were given. Systemic sclerosis (SSc) is a relatively rare condition with an immunologically mediated pathogenesis. For reasons that are not clearly understood, dense collagen is deposited in the connective tissues of the body in extraordinary amounts. Although its dramatic effects are seen in association with the skin, the disease is often quite serious with visceral organ involvement. We describe a case of limited mouth opening secondary to diffuse SSc, improvement in mouth opening with passive jaw stretch exercises, and the challenges involved in performing dental procedures for such patients.

The current study experimentally investigates the heat transfer augmentation on the novel axial corrugated heat exchanger tube in which the spring tape is introduced. Air (Pr = 0.707) is used as a working fluid. In order to augment the thermohydraulic performance, a corrugated tube with inserts is offered. The experimental study is further extended by varying the important parameters like spring ratio (y = 1.5, 2.0, 2.5) and Reynolds number (Re = 10 000–52 000). The angular pitch between the two neighboring corrugations and the angle of the corrugation is kept constant through the experiments at β = 1200 and α = 600 respectively, while two different corrugations heights (h) are analyzed. While increasing the corrugation height and decreasing the spring ratio, the impact of the swirling effect improves the thermal performance of the system. The maximum thermal performance is obtained when the corrugation height is h = 0.2 and spring ratio y = 1.5. Eventually, correlations for predicting friction factor (f) and Nusselt number (Nu) are developed.

AbstrakHegemoni kolonialisme dalam budaya poskolonial merupakan alasan penelitian inikemudian mengkaji wacana kolonial dalam novel Max Havellar (MH) khususnya dampakditimbulkannya. Dampak dimaksud adalah posisi keberpihakan pemikiran tersirat darikarya tersebut. Hasil pembahasan menunjukkan, secara temporal maupun permanen MHmenyuarakan ketidakadilan dalam kondisi-kondisi kolonial menyangkut penindasan sangpenjajah terhadap terjajah. Hanya saja, upaya mengatasnamakan atau mewakili suarakaum terjajah terbukti mengimplikasikan ciri ideologis statis kerangka kolonialisme(orientalisme); yakni cara pandang Eropasentris, di mana “Barat” sebagai self adalah superior,dan “Timur” sebagai other adalah inferior. Dalam konteks poskolonialisme, MH dengan sifatkritisnya yang berupaya “menyuarakan” nasib pribumi terjajah, justru menampilkan stigmapenguatan kolonialitas itu sendiri secara hegemonik. Artinya, “menyuarakan” nasib pribumidimaknai sebagai keberpihankan kolonial yang kontradiktif, di mana stigma penguatankolonialitas justru lebih terasa, ujung-ujungnya melanggengkan hegemoni kolonial. Tidakmembela yang terjajah, tetapi memperhalus cara kerja mesin kolonial.AbstractThe hegemony of colonialism in the culture of postcolonial society is the reason this studythen examines the colonial discourse in the novel Max Havellar (MH) in particular the impactit brings. The impact in question is the implied position of thought in the work. The resultsof the discussion show that, temporarily or permanently, MH voiced injustice in the colonialconditions regarding the oppression of the colonist against the colonized. However, the effort toname or represent the voice of the colonized has proven to imply a static ideological characterin the framework of colonialism (orientalism); ie Eropacentric point of view, in which “West” asself is superior, and “East” as the other is the inferior. In the context of postcolonialism, MH withits critical nature that seeks to “voice” the fate of the colonized natives, actually presents thestigma of strengthening coloniality itself hegemonicly. That is, “voicing” the fate of the pribumiis interpreted as a contradictory colonial flare, where the stigma of strengthening colonialityis more pronounced, which ultimately perpetuates the hegemony of colonialism. No longerdefending the colonized, but refining the workings of the colonial machinery.

An oleo gum resin guggulu is a product which obtained as a result of gummosis from the bark of Commiphora wightii (Arnott) Bhandari [syn. Commiphoramukul (Hook. Ex Stocks) Family, Burseraceae]. It has been known for its immense applicability in the Ayurveda since time immemorial for the treatment of variety of disorders such as inflammation, gout, rheumatism, impotence, leprosy, obesity, and disorders of lipids metabolism. It is a mixture of phytoconstituents like terpenoids, steroids, flavonoids, guggultetrols, lignans, sugars, and amino acids. This review is an effort to compile all the information available on all of its chemical constituents which are responsible for its therapeutic potential, limitation of guggul extracts and the necessity of novel principles for gum guggul. Nowadays, Guggul is available as the marketed formulation for curing numerous clinical conditions and is accessible in combination with various other ingredients. Though conventional dosage form shows the dominance as patient compliance and easy availability, yet it has found to pose the problems like dose fluctuation, peak-valley effect, non-adjustment of the administered drug, invasiveness etc. Guggul lacks its desired effect due to its low bioavailability and water solubility. This makes it a partial or a deficient therapy for remedy of many signs and symptoms. Novel drug delivery system (NDDS), a new approach and has excluded many of drawbacks exhibited by conventional dosage forms. Some of the novel dosage forms of guggul has been formed like nanoparticles, nanovesicles, gugglusomes and proniosomal gel. But still, the novel formulations for guggul has its less outspread in the market. Guggul can be executed as a profitable drug using NDDS. There is a need to highlight the unidentified and unexplained facts about guggul so as to make it more efficacious and effective in terms of bioavailability and aqueous insolubility.

In December 2019, a unique coronavirus infection, SARS-CoV-2, was first identified in the province of Wuhan in China. Since then, it spread rapidly all over the world and has been responsible for a large number of morbidity and mortality among humans. According to a latest study, Diabetes mellitus, heart diseases, Hypertension etc. are being considered important risk factors for the development of this infection and is also associated with unfavorable outcomes in these patients. There is little evidence concerning the trail back of these patients possibly because of a small number of participants and people who experienced primary composite outcomes (such as admission in the ICU, usage of machine-driven ventilation or even fatality of these patients). Until now, there are no academic findings that have proven independent prognostic value of diabetes on death in the novel Coronavirus patients. However, there are several conjectures linking Diabetes with the impact as well as progression of COVID-19 in these patients. The aim of this review is to acknowledge about the association amongst Diabetes and the novel Coronavirus and the result of the infection in such patients.
