Background Infant mortality rate is an indicator of the degree ofhealth in society. In Indonesia, the infant mortality rate remainshigh, with most deaths occurring in the first 24 hours of life.Breastfeeding has been shown to reduce infant mortality, especiallyif undertaken in the first hour of life. This practice is knownas early initiation of breastfeeding (EIB). According to variousstudies, EIB implementation may be influen ced by many factorssuch as knowledge, attitude, behavior, and health care facilities.Objective To assess the level of knowledge, attitude, and behaviorof pregnant women towards EIB.Methods We conducted a descriptive study using questionnaireson 74 pregnant women in the outpatient clinic of Obstetrics andGynecology Department, Dustira Hospital, Cimahi, West Java,Indonesia from November to December 2012.Results Out of 74 respondents, 21 % had a good level of knowledgeon EIB, 23% had an adequate knowledge, and 56% had less thanadequate knowledge on EIB. A positive attitude towards EIBwas found in 65% of the respondents, while 35% had a negativeattitude. With regards to behavior conducive to EIB, 8% ofrespondents had good behavior, 57% had moderate behaviot; and35%had less than adequate behavior.Conclusion Majority of pregnant women have less than adequateknowledge on EIB, a positive attitude towards EIB, and moderateto less than adequate behavior conducive to EIB.

Background The infant mortality rate (IMR) in Indonesia is higher than that in other ASEAN countries. The highest rate of mortality occurs in the first 24 hours of life. Suboptimal breastfeeding initiation is a cause of high IMR. In an effort to decrease infant mortality, implementing early initiation of breastfeeding (EIB) has been encouraged.Objective To assess the success rate and time needed for latching on in EIB implementation.Methods We reviewed medical records of vaginal deliveries at Dustira Hospital, Cimahi, West Java, from June–November 2011.Results From 305 vaginal deliveries, 174 infants received EIB, though only 159 medical records could be assessed. The results showed that 52 % did EIB with a 91.8% success rate (defined as good implementation by WHO) and a 8.2% fail rate. In terms of subjects’ birth weights, the success rate of EIB implementation was 62.5% in the low birth weight (LBW) group, 94.9% in the normal birth weight (NBW) group, and 100% in the large birth weight or macrosomic group. The success rate of EIB implementation was 69.2% in the preterm group and 93.8% in the full term group. The success rate of EIB implementation was 71.4% in the LBW/fullterm group and 55.6% in the LBW/preterm group. The amount of time for infants to latch on was highest within the 30–44 minute group (52.7%).Conclusion The EIB implementation at Dustira Hospital was classified as good and the amount of time to latch on was 30-44 minutes.

One of the most important tasks that healthcare providers face is to reduce infant mortality. The purpose of our study was to assess the effects of routing of pregnant women with preterm birth on the infant mortality rate. Materials and methods: оperating data of the Federal State Statistics Service (Rosstat), operational notifications about death of young children under the age of 1, monthly reports of obstetric hospitals subordinate to Moscow Healthcare Department, annual reports of hospital departments providing neonatal care in medical organizations of Moscow Healthcare Department, and statistical data analysis. Results and discussion. The infant mortality rate in Moscow has been declining in recent decades and has been less than 6‰ annually since 2016. The share of newborns with extremely low birth weight is insignificant in overall rate. At the same time, extremely low birth weight is one of the main causes of infant mortality. As this category of newborns requires specific care, it is better to deliver such babies in perinatal centers that provide high quality medical care. Within 5 years, the share of children with ELBW born in perinatal centers and specialized obstetric hospitals has almost doubled. At the same time, there is an increase in the survival rate of children with extremely low birth weight by the age of one year in obstetric hospitals. Conclusion. Well-planned routing of patient within the large system of obstetric hospitals in Moscow could reduce death rate, increase the survival rate of children with extremely low birth weight and contribute to reducing infant mortality in the region.

The policy of early initiation of breastfeeding has been socialized in Indonesia since August 2007 (Roesli, 2008). The World Health Organization (WHO) has recommended that all babies get colostrum, namely breast milk on the first and second days to fight various infections and get exclusive breastfeeding for 6 months (Ministry of Health 2012). The IMD implementation policy is also expected to reduce infant mortality (IMD). In infant mortality, 40% occurs in the first month of life and early initiation of breastfeeding can reduce these risk factors for death, thereby reducing 22% for 28-day infant mortality. The research design was a cross sectional study using primary data (questionnaire). This sample uses accidental sampling method. The results showed that of the 87 respondents at Puskesmas Palmerah, the age of 20-35 years old mothers who had good knowledge about IMD were 30 respondents (34.5%), respondents with high education (SMA-PT) had good knowledge about IMD as many as 51 respondents ( 58.62%), respondents who work and have good knowledge about IMD are 24 respondents (27.59%), respondents who are experienced and have good knowledge about IMD are 42 respondents (48.28%), respondents who receive information and have Good knowledge about IMD was 69 (79.31%), while the related variables included age, education and experience with a P value of 0.384 greater than α, namely 0.05. Conclusion. Characteristics of age, education level, experience are closely related to the knowledge possessed by pregnant women, especially those related to knowledge of IMD. Meanwhile, the factor that is not related to the success of IMD is the source of information. Suggestion. There needs to be an effort to do outreach activities on Early Initiation of Breastfeeding (IMD) or supporting facilities to increase the knowledge of pregnant women, especially in the third timester.Keywords: Knowledge, IMD, Characteristics

Provision of Early Initiation of Breastfeeding (IMD) to newborns in Indonesia is still relatively more low. Early Initiation of Breastfeeding can increase potential success of exclusive breastfeeding for 6 months. Found 40% of infant deaths occur in the first month of life. The infant mortality can be reduced by up to 22% with action of Early Initiation Breastfeeding. This study aimed to analyze the influence of knowledge, support health, and social culture of the provision of Early Initiation of Breastfeeding newborn. This research was conducted at the General Hospital of Makassar to the entire population that has been doing deliveries, with a total sample of 100 respondents. Research shows that there is a relationship between the mother's knowledge by giving Early Initiation of Breastfeeding (p = 0.000). There is a relationship between support services to the administration of Early Initiation of Breast feeding (p = 0.000). There was no relationship between the social and cultural perspectives giving Early Initiation of Breastfeeding (p = 1,000). Enough knowledge about the provision of Early Initiation of Breastfeeding is a need for being able to reduce infant mortality. Health workers support the Early Initiation of Breastfeeding is an appropriate step to encourage mothers to give Early Initiation of Breastfeeding. Social effects of culture against early initiation of breastfeeding sometimes become an obstacle to the provision of Early Initiation of Breastfeeding.

Infant mortality rate by Indonesia Demographic Health Survey in 2012 is still quite high at 32 per 1,000 live births and under five year mortality rates of 40 per 1,000 live births. An effort to prevent infant mortality is by breastfeeding soon after birth or so-called early initiation of breastfeeding and exclusive breastfeeding. Coverage of exclusive breastfeeding nationwide in 2014 amounted to 52.3% is still below the national target of 80%. The aim of this research was to find the efforts to improve the coverage of exclusive breastfeeding and the early initiation of breastfeeding. The Efforts that can do are empowering people through Mother Support Group, socialize the Government Regulation No. 33 of 2012 on exclusive breastfeeding and breastfeeding counselor provides power. The local government is advised to draw up local regulations that support the implementation of the early initiation of breastfeeding program and exclusive breastfeeding so as to have binding legal force for all parties involved as well as to conduct promotion, monitoring, evaluating and supervising the implementation and achievement of the program exclusive breastfeeding.

Chronic Energy Deficiency (CED) is one of the causes of maternal and infant mortality indirectly. Maternal Mortality Rate (MMR) in Grobogan District increased gradually during the period of 2011 – 2012 from 26 cases (114.03/100,000 live births) to 34 cases (151.15/100,000 live births), and decreased sharply to be 22 cases (102.03/100,000 live births) in 2013. Meanwhile, Infant Mortality Rate (IMR) was 194 cases in 2011, rose dramatically in 2012 to be 240 cases, and declined sharply in 2013 to be 166 cases. Low Birthweight Babies (LBWB) was one of the causes of infant mortality because pregnant women suffered from CED during their pregnancies. The aim of this study was to analyse internal and external factors with performance of village midwives in conducting early detection and handling pregnant women with CED. This was an observational-analytic study using cross-sectional approach. Data were collected using structured questionnaires. Population was 243 village midwives at health centres in Grobogan District. As many as 71 village midwives were selected randomly using a technique of stratified random sampling. Data were analysed using analyses of bivariate (chi square test) and multivariate (logistic regression test). The results of this research showed that most of the respondents aged ranging from 20-35 years old (76.1%), had working period less than or equal to 6 years (53.5%), ever handled CED (81.7%). Variables of health facilities (p=0.001), fellow worker support (p=0.016), community support (p=0.001), leader support (p=0.0001) had significant relationship with the performance of village midwives in conducting early detection of pregnant women with CED. Factors jointly influencing the performance were health facilities (p=0.000; Exp(B)=25.395), and leader support (p=0.001; Exp(B)=10.278). As suggestions, health centres and District Health Office need to provide complete health facilities to improve performance of health workers in providing services.

Abstract: Early Initiation of Breastfeeding (IMD) is the process of feeding the baby immediately after birth, which has important benefits such as support the sustainability of exclusive breastfeeding and potentially reduce IMR and MMR. Based on data by RISKESDAS 2013, the percentage of IMD implementation in Indonesia is only 34.5% and in North Sulawesi is only 29%. Midwives are recognized as professionals who work as female partners including help the mothers in initiating breastfeeding. Therefore midwives are expected to have a good knowledge and a positive attitude towards the IMD so the implementation may increase. This study aims to measure the knowledge and attitude of RSU Prof. Dr. R. D. Kandou Manado midwives toward early initiation of breastfeeding in 2014. This is a cross-sectional study using questionnaire administered to midwives in RSU Prof. Dr. R. D. Kandou Manado. As a results from 62 respondents, 41 respondents (66.1%) had a good knowledge on IMD, 14 respondents (22.6%) with moderate knowledge, 7 respondents (11.3%) with poor knowledge and all respondents (100 %) had a positive attitude towards the IMD. Most of midwife with a good knowledge is those who with age 36-40 years, Bachelor of Midwifery (S-I) and has been working for 6-10 years old.Keywords: knowledge, attitude, midwife, early breastfeeding initiation, IMDAbstrak: Inisiasi Menyusu Dini (IMD) adalah proses bayi menyusu segera setelah dilahirkan, yang memiliki manfaat penting diantaranya yaitu menunjang keberlangsungan ASI ekslusif serta berpotensi menurunkan AKB dan AKI. Berdasarkan data oleh RISKESDAS 2013, persentase pelaksanaan IMD di Indonesia baru sebesar 34,5% dan di Sulawesi Utara baru sebesar 29%. Bidan diakui sebagai tenaga profesional yang bekerja sebagai mitra perempuan termasuk membantu ibu dalam memulai pemberian ASI. Oleh karena itu bidan diharapkan memiliki pengetahuan yang baik serta sikap yang positif terhadap IMD sehingga pelaksanaan IMD dapat meningkat. Tujuan penelitian ini untuk mengetahui pengetahuan dan sikap bidan di RSU Prof. Dr. R. D. Kandou Manado tentang IMD tahun 2014. Penelitian ini dilakukan dengan desain penelitian cross-sectional menggunakan kuesioner kepada bidan di RSU Prof. Dr. R. D. Kandou Manado. Hasil penelitian dari 62 responden, didapatkan bahwa 41 responden (66,1%) telah memiliki pengetahuan yang baik tentang IMD, 14 responden (22,6%) berpengetahuan cukup, 7 responden (11,3%) berpengetahuan kurang serta seluruh responden (100%) memiliki sikap positif terhadap IMD. Mayoritas bidan yang berpengetahuan baik ialah bidan dengan umur 36-40 tahun, berpendidikan S-I Kebidanan dan lama kerja 6-10 tahun.Kata kunci: pengetahuan, sikap, bidan, inisiasi menyusu dini, IMD

AbstractThe infant mortality rate in Indonesia is still high at 24 per 100 live births. One of the causes is infection. Infection can by prevented by controling breast milk because breast milk contains colostrum which is able to maintain the baby’s immune system. The introduction of breastfeeding begins with early initiation of breastfeeding. The coverage of newsborn in Indonesia in 2019 who received early intiation of breastfeeding was 75,58%.The purpose of this study was to determine the between early initiation of breastfeeding and the smooth production of breast milk throught the Literature Review.This reserch is a quantitive research through Literature Review. The articles were obtained from three articles from articles from Garuda Portal which is a critical review of the JBI instrument written by The Joanna Briggs Institute. The results of this study indicate a relationship between the provion of early intitiation og breastfeeding with the smooth discharge of breast milk with p value <0,05 (0,000-0,029).There is a corelation between the previsions of early initiation of breastfeeding the smoothness is producing breast milk.Keywords :giving early initiation of breastfeeding ( IMD), breast milk flow AbstrakAngka kematian bayi di Indonesia masih tinggi yaitu 24 per 1000 kelahiran hidup. Artikel didapatkan dari Searh Ergine Google scholar tiga dan Portal Garuda dua telaah kritis instrument JBI The Joanna Briggs Institute .Hasil penelitian ini menunjukkan adanya hubungan antara pemberian inisiasi menyusu dini (IMD) dengan kelancaran pengeluaran ASI dengan p value<0,05 (0,000-0,029).Terdapat hubungan antara pemberian inisiasi menyusu dini (IMD) dengan kelancaran pengeluaran ASI.Kata kunci: Pemberian IMD ; Kelancaran Pengeluaran ASI.

The maternal mortality rate is 19,500 to 20,000 people every year or occurs every 26-27 minutes. The caus of maternal death is bleeding 30.5%, infection 22,5%, gestosis 17,5 and anesthesia 2%.  The infant mortality rate is around 10,000 to 280,000 per 18-20 minutes. The cause of infant mortality is due to Low Birth Weight (LBW) of 15/1000%.  The infant mortality rate in Indonesia is still the highest problem in other ASEAN countries. The infant mortality rate in Indonesia from 2008 was around 248 per 100,000 live births. Basic Health Research (RISKESDA) 2013 shows there are still 10,2% of babies with LBW, which is less than 2,500 grams. Neonatal death because LBW is basically affected by the nutritional status of pregnant women. This study aims to determine the relationship between the nutritional status of pregnant women and the  incidence  of  LBW. This  quantitative  research  uses  a  case  control  approach  using  a  retrospective approach. The population in this study were mothers who had given birth to babies during the last two years (2016-2017). The sampling technique uses total sampling for control cases by using a ratio of 1: 1 for the case group of 40: 40 samples. Analysis using Chi Square with p value 0,000 (OR=3,500, CI 95%=2,313-5,296). There is a relationship between nutritional status of pregnant women and the incidence of LBW. Health Technology Assessment (HTA) which can seek 1000 first day of life can be a breakthrough in assessing and providing interventions of nutrition in families, especially in pregnant women.
