Efficacy of Habb-e-Asab in diabetic peripheral neuropathy: a randomized placebo control study

Abstract Objectives Diabetic peripheral neuropathy (DPN) is a common diabetes complication. The prevalence of neuropathy is 55% for type 1 and 66% for type 2 diabetes. In Unani medicine neuropathy is known as Khidr (numbness). It is treated with drugs possessing hypoglycemic and analgesic properties, etc. Habb-e-Asab, a polyherbal Unani formulation used for the treatment of Waja-ul-Asab (neuralgia) is routinely used for its indications in neurological pain in Unani medicine. The aim of this study to investigate the efficacy of Habb-e-Asab in diabetic peripheral neuropathy. Methods Thirty patients with DPN were randomly assigned to test (n=20) and control (n=10) groups in a randomized single-blind placebo control study. For 45 days, the test group was given 250 mg Habb-e-Asab twice a day and the control group 250 mg placebo twice a day. The subjective parameters Pain in feet, burning in feet, and tingling in feet was assessed by the arbitrary scale and VAS fortnightly and objective parameters MNSI, and VPT was assessed in pre–post-treatment. Results The research drug revealed highly statistically significant with p<0.001 on VAS score and MNSI whereas VPT is significant with p<0.01 on few points. But control group exhibits no significant effect in any of the parameters. No adverse effects had been reported in either group. Conclusions Our finding indicated that the Habb-e-Asab for 45 days improved and reduced the severity of DPN in a patient with diabetes (CTRI/2018/02/011725).

BACKGROUND: Diabetic peripheral neuropathy (DPN) means the presence of symptoms and/or signs of peripheral nerve damage that occur to people with diabetes, excluding all other causes of neuropathy. Chronic hyperglycaemia leads to increased secretion of tumour necrotic factor-alpha (TNF-α), with the development of micro and macroangiopathy, damage to nerve fibres and local demyelination. AIM: To determine the role of inflammation in the peripheral nerve damage process concerning people suffering from type II diabetes mellitus. MATERIAL AND METHODS: The study included a total of 80 subjects, men and women, divided into two groups: an examined group (n = 50) consisting of subjects with DPN at the age from 30 to 80 years and a control group (n = 30) of healthy subjects aged from 18 to 45. In the investigated group, a neurological examination was performed using the Diabetic Neuropathy Symptoms (DNS) Score and Electroneurography. All the subjects had the blood plasma concentration of TNF-α by ELISA technique. RESULTS: The average value of TNF-α in the test group was 8.24 ± 2.899 pg/ml, while the control group was 4.36 ± 2.622 pg/ml (p < 0.0001). The average value of TNF-α was correlated with the achieved DNS score in the investigated group (p = 0.005). Concerning the linear association of the concentration of TNF-α with the peripheral nerve velocity in the investigated group, no statistical significance was detected. CONCLUSION: Inflammation can play a role in the pathogenesis of diabetic autonomic neuropathy and cranial neuritis.

Objective: To observe the therapeutic effect of salvia miltiorrhiza phenylephrine combined with puerarin sodium on diabetic peripheral neuropathy (DPN) and the effect of serum insulin-like growth factor 1 (IGF-1), free fatty acid (FFA) and human myelin Protein (MBP). Methods: A total of 116 patients with diabetic peripheral neuropathy were randomly divided into observation group (58 cases) and control group (58 cases). The control group was treated with sodium prostaglandin sodium. The observation group was treated with Danshephenolic acid Treatment, treatment for 8 weeks. The changes of total symptom score system (TSS), vibration sensory threshold (VPT), ankle brachial index (ABI) and peripheral nerve conduction velocity (NCV), serum IGF-1, FFA and MBP were observed before and after treatment. Results: The levels of numbness, tingling, burning, paresthesia and total score were significantly decreased in both groups after treatment, VPT, ABI and NCV were improved (P < 0.05), and the above indexes (P < 0.05). The levels of serum IGF-1 were increased and the levels of FFA and MBP were lower in the two groups. The improvement of the above indexes was better than that of the control group (P < 0.05). Conclusion: Salvia miltiorrhiza polyphenate combined with puerarin sodium can significantly improve the vibration sensory threshold and ankle-brachial index, enhance the nerve conduction velocity and relieve clinical symptoms in patients with diabetic peripheral neuropathy. The mechanism may be related to the regulation of serum IGF-1,FFA, The level of MBP is relevant.

ObjectivesSerum and urinary C-peptide has clinical implications in people with/without diabetes. Recently, C-peptide was detected in hair samples of healthy adults but not studied in people with diabetes. It is not known whether C-peptide can be detectable in nail tissue or not. This study aims to assess the detection of C-peptide in hair and nail samples and to find whether hair and nail C-peptide levels are different in type 1 diabetes mellitus (T1DM) compared with healthy individuals.Research design and methodsIn a prospective case-control study on 41 subjects with T1DM and 42 control subjects, hair and nail samples were collected and prepared. C-peptide was extracted by incubating the samples with methanol and measuring the extract with an immunoassay. The hair and nail C-peptide values were compared between the T1DM and control group and their correlations with each other and with other variables were assessed with a significant level set at 0.05.ResultsHair and nail C-peptide levels were detected in both groups, with significantly lower values in T1DM compared with the control group. T1DM with >7-year diabetes duration had significantly lower C-peptide in serum, nails and hair. Hair and nail C-peptide levels have significant positive correlations with each other and negative correlations with age.ConclusionsWe conclude that C-peptide are detectable in the hair and nails of healthy persons and persons with T1DM. Compared with the healthy persons, persons with T1DM had significantly lower hair and nail C-peptide and significant hair/nail C-peptide reduction after 7 years of diagnosis. Our results suggest that hair and nails are suitable matrices for the measurement of C-peptide in healthy persons and persons with T1DM.

Objectives: This study aims to find the thickness of muscles of lower limbs in patients with diabetic peripheral neuropathy (DPN) along with echogenicity and applying these two findings in the diagnosis of the disease. Methods: This is a case-control study conducted in the clinical neurophysiology unit in Merjan medical city. It includes 73 patients diagnosed to have DPN based on characteristic history and physical examination and documented by nerve conduction study. These patients are matched to 73 control that has matched age and sex to the patient group. Patient and control are examined by high-resolution ultrasound (12 MHz linear probes). We assess muscle thickness and echogenicity of the tibialis anterior, biceps femoris, and abductor halluces brevis. Results: The study showed that there was a statistically significant decrease in muscle thickness and increase in echogenicity in all tested muscles when compared to the control group. Also, we calculated the cut-off value with sensitivity and specificity of muscle thickness in the diagnosis of DPN. Conclusion: Muscle ultrasound is a useful complementary test for the diagnosis of DPN.

Purpose. The probable association between Toxoplasma gondii (T. gondii) infection and diabetes mellitus (DM) is still controversial, and there are several studies with conflicting results. Thus, this study was performed to assess the possible association between chronic T. gondii infection and type 1 diabetes mellitus (T1DM) and T2DM. Methods. In this case-control study, a total of 105 diabetic subjects including 36 patients with T1DM and 69 patients with T2DM were recruited. In addition, 150 nondiabetic subjects were enrolled as controls. Each case group had its own control group. Each participant completed a structured questionnaire obtaining demographic information. Serum samples were examined for T. gondii-specific IgG antibody using enzyme-linked immunosorbent assay (ELISA) method. Results. Analysis revealed that 69.4% and 34.0% of patients with T1DM and control subjects were serologically positive for T. gondii, respectively (odds ratio (OR): 4.41; 95% confidence interval (CI): 1.75–11.06; P = 0.001 ). Moreover, 72.5% of T2DM patients and 29.0% of healthy individuals were seropositive for T. gondii (OR: 6.44; 95% CI: 3.25–12.74; P < 0.001 ). Among risk factors, only contact with cats was significantly associated with IgG seroprevalence in both T2DM patients ( P < 0.001 ) and control subjects ( P = 0.045 ). Conclusion. Although the results showed that chronic T. gondii infection is significantly associated with T1DM and T2DM, there remain many questions regarding the exact mechanisms of T. gondii in the pathogenesis of DM.

Introduction: The role of virgin coconut oil in the treatment of allergic rhinitis is controversial. Thus, the aim of the present study is to determine the effects of virgin coconut oil ingestion, in addition to standard medications, on allergic rhinitis. We also studied the side effects of consumption of virgin coconut oil. Methods: Fifty two subjects were equally divided into test and control groups. All subjects received a daily dose of 10mg of loratadine for 28 days. The test group was given 10ml of virgin coconut oil three times a day in addition to loratadine. The symptoms of allergic rhinitis were scored at the beginning and end of the study. Results:, the symptom score were divided into nasal and non-nasal symptom scores. Sneezing score showed a significant difference, however the score was more in control group than test group, indicating that improvement in symptom was more in control group. The rest of the nasal symptom and non-nasal symptom score showed no significant difference between test and control groups. Approximately 58% of the test subjects developed side effects from consumption of virgin coconut oil, mainly gastrointestinal side effects. Conclusion: In the present study, ingestion of virgin coconut oil does not improve the overall and individual symptoms of allergic rhinitis, furthermore it has side effects.

Abstract Background Parkinson’s disease is one of the neurodegenerative disorders that is caused by genetic and environmental factors or interaction between them. Solute carrier family 41 member 1 within the PARK16 locus has been reported to be associated with Parkinson’s disease. Cognitive impairment is one of the non-motor symptoms that is considered a challenge in Parkinson’s disease patients. This study aimed to investigate the association of rs11240569 polymorphism; a synonymous coding variant in SLC41A1 in Parkinson’s disease patients in addition to the assessment of cognitive impairment in those patients. Results In a case -control study, rs11240569 single nucleotide polymorphisms in SLC41A1, genes were genotyped in 48 Parkinson’s disease patients and 48 controls. Motor and non-motor performance in Parkinson's disease patients were assessed by using the Movement Disorder Society-Sponsored Revision of the Unified Parkinson’s Disease Rating Scale (MDS-UPDRS). The genotype and allele frequencies were compared between the two groups and revealed no significant differences between case and control groups for rs11240569 in SLC41A1 gene with P value .523 and .54, respectively. Cognition was evaluated and showed the mean ± standard deviation (SD) of WAIS score of PD patients 80.4 ± 9.13 and the range was from 61 to 105, in addition to MMSE that showed mean ± SD 21.96 ± 3.8. Conclusion Genetic testing of the present study showed that rs11240569 polymorphism of SLC41A1 gene has no significant differences in distributions of alleles and genotypes between cases and control group, in addition to cognitive impairment that is present in a large proportion of PD patients and in addition to the strong correlation between cognitive impairment and motor and non-motor symptoms progression.

Abstract Objectives There are several hypotheses on the effects of the rs1738074 T/C single nucleotide polymorphism in the TAGAP gene; however, there has been no study on Turkish pediatric patients. We aimed to investigate the association of celiac disease (CD) and type 1 diabetes mellitus (T1DM) comorbidity with the polymorphism in the TAGAP gene of Turkish pediatric patients. Methods Totally, 127 pediatric CD patients and 100 healthy children were included. We determined the polymorphism by the allele-specific polymerase chain reaction method. We used IBM SPSS Statistics version 25.0 and Arlequin 3.5.2 for the statistical analyses. The authors have no conflict of interest. Results It was determined that 72% (n=154) of only CD patients had C allele, whereas 28% (n=60) had T allele. Of the patients with celiac and T1DM, 42.5% (n=17) and 57.5% (n=23) had T and C alleles, respectively. Of the individuals in control group, 67% (n=134) had C allele, whereas 33% (n=66) had T allele. Conclusions There was no significant difference in the genotype and allele frequencies between the patient and control groups (p>0.05). There was no significant association between the disease risk and the polymorphism in our study group.
