Epidemiologic Pictures of Kawasaki Disease in Japan: From the Nationwide Incidence Survey in 1991 and 1992

Objective. Since 1970, twelve nationwide epidemiologic surveys of Kawasaki disease (KD) have been conducted throughout Japan every two years to describe KD in Japan. By the end of 1992, a total of 116 848 cases were reported. This paper summarizes the statistical analysis of the latest survey for the 2-year period from January 1991 through December 1992. Method. A questionnaire form and diagnostic guidelines for KD were sent to all pediatric departments of hospitals with 100 or more beds throughout Japan and information was obtained on patients with KD diagnosed during the 2-year period from January 1991 through December 1992. Results. The summary of the results is: 1) the number of patients reported was 11 221 (6604 males and 4617 females; male/female ratio = 1.43) with a yearly incidence rate of 90 per 100 000 children <5 years old; 2) the monthly number of patients was higher in winter and summer, although the monthly difference was not marked; 3) age-specific incidence rates showed a unimodal peak at 1 year of age; 4) the proportion of patients with a family history of KD in a sibling was 1%; 5) the proportion of recurrent patients was 3%; 6) the proportion of patients with cardiac sequelae 1 month after disease onset was 13%; and 7) the number of patients who died was 9, which conforms to 0.08% of total patients. Conclusion. The incidence rates of KD in Japan are ten times higher than those reported in western countries and almost constant over 6 years. The descriptive epidemiology of the disease, which supports the infection theory, does not change for years.

The number of patients with autoimmune pancreatitis who visited hospitals in Japan in 2007 was approximately 2709 (95% confidence interval; range 2540–3040). Because IgG4-related disease is a new clinical entity, there are no data with regard to its prevalence. To estimate the number of patients with IgG4-related disease in Japan, we randomly selected hospitals using stratification and asked them how many patients they had with IgG4-related disease in 2009. The number of patients with Mikulicz’s disease, IgG4-related retroperitoneal fibrosis, IgG4-related renal disease, IgG4-related pulmonary disease, and IgG4-related lymphadenopathy who visited hospitals in Japan in 2009 was approximately 4304 (95% confidence interval; range 3360–5048), 272 (95% confidence interval; range 264–306), 57 (95% confidence interval; range 47–66), 354 (95% confidence interval; range 283–424), and 203 (95% confidence interval; range 187–240), respectively. The total number of patients with IgG4-related disease without autoimmune pancreatitis in Japan was approximately 5190 (95% confidence interval; range 4141–6084). The male : female ratio was 1 : 0.77, and the average of age of disease onset was 58.8 years. The total number of patients with IgG4-related disease in Japan in 2009, including autoimmune pancreatitis, was approximately 8000.

Objectives: The purpose of the present study was to explore the changes in epidemiological, clinical and laboratory characteristics of Kawasaki disease (KD) over a 12-year period in this area. Methods: The medical records of KD patients hospitalized in the Children’s Hospital of Soochow University from January 2006 to December 2017 were retrospectively reviewed and analyzed. To elucidate the changes more clearly in these years, we divided all patients into three groups according to the time of admission, 5-year time intervals apart. Results: We found the number of KD had increased in recent years, although the age compositions had some variances. The male-to-female ratio peaked in patients aged 6–11 months and then decreased with ages. Incidence rates of clinical characteristics were almost the same except for cervical lymphadenopathy. Significant differences were noted in percentage of neutrophils, albumin, C-reactive protein, erythrocyte sedimentation rate (ESR) and platelets. However, only ESR and platelets were significantly different after adjusting the days of disease onset. A trend showing a shorter fever duration before intravenous immunoglobulin treatment was also noted. Conclusions: The number of KD was increasing annually. Our results were indicative of a possibly changed etiology of the disease. However, further large-scale long-term surveillance is warranted.

Results of the statistical analysis of a nationwide survey of patients with Kawasaki disease diagnosed within a 2-year 6-month period, from July 1982 to December 1984, are as follows. (1) The cumulative number of patients reported by the end of 1984 was 63,399 (36,891 boys and 26,508 girls; male to female ratio 1.4). (2) There were two epidemic years, 1979 and 1982, in which the numbers of patients were more than twofold that of the previous years. (3) A curve plotted for age-specific incidence rate showed a unimodal peak at age 1 year. (4) Steroid therapy was used for 6.3% of the patients, aspirin for 89.8%, antibiotics for 57.6%, and γ-globulin for 11.4%. (5) The proportion of sibling cases was 1.4% and that of recurrent cases was 3.9% of all cases reported. (6) The incidence of cardiac sequelae in 1 month after disease onset was 17.2%.

Background: Vernal Keratoconjunctivitis (VKC) is common cause of ocular morbidity in children living in tropical countries. Its diagnosis is based on signs and symptoms of the disease. The study was undertaken to stress upon the demography and clinical presentation of VKC.Methods: Retrospective pre-formed proforma of 155 patients of VKC, who were detected at random in the out - patient department of ophthalmology, ASCOMS, Jammu from May 2016 to April 2017, were analyzed.Results: Mean age at presentation was 10.31 years +4.05. The Male: Female ratio was 4.96:1. Majority of patients reported in the month of May. Mixed type of VKC was predominant. Personal or family history of allergy was seen in 5.8% of patients. Itching (100%) was commonest symptom and palpaberal papillae were commonest sign seen in 78.70% of patients.Conclusions: Clinical pattern of VKC seen in hot and dry climate of Northern India is like that seen in other parts of country.

Background: Cervical spondylotic myelopathy (CSM) is a common spinal cord disorder that develops in elderly people. Anterior cervical decompression and fusion (ACDF) is an effective and reliable procedure for the treatment of CSM. Objective: To find out the results of ACDF by cervical cage with bone graft for the treatment of single level cervical spondylotic myelopathy. Methods: This prospective observational study was conducted in the Department of Orthopedics, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, over a period of two years from March 2015 to August 2017. Forty patients with cervical spondylotic myelopathy were diagnosed on the basis of presenting complains, clinical examination and investigations and were enrolled in this study. The history of the patients was collected, clinical examination was done and relevant investigations were done for each patient. In this study, Nurick grading and VAS scale were used for evaluation of the result. Result: Male gender was predominant in this study; male-female ratio was 2.63:1. Mean age of the patients was 45.9 ± 9.1 years within the range of 30-65 years. Both sides were affected in 14 (35.0%) cases, only right side was affected in 15 (37.5%) cases and only left side was affected in 11 (27.5%) cases. Involved disc spaces were C3/4 (10.0%), C4/5 (22.5%), C5/6 (42.5%) and C6/7 (25.0%). Transientdysphagia was observed in 2 (5.0%), transient para paresis in 1 (2.5.0%), wound infection in 1(2.5.0%) case and damage to the dura was observed in 1 (2.5.0%) case. Signs of fusion were observed in 10 (25.0%) cases after 3 months, 30 (75.0%) cases after 6 months and in all patients after 12months. The result was found to be excellent in 35 (87.5%) and good in 5 (12.5%) cases. Conclusion: Anterior cervical discectomy and fusion by cervical cage with bone graft is an effective procedure for management of CSM.

Background. Cleft lip and cleft palate are among the most common orofacial congenital anomalies. This study is to establish Orofacial Clefts Database for Kumasi, Ghana, with a view to extend it to other cities in future to obtain a national orofacial anomaly database. Methods. A descriptive prospective survey was carried out at eleven selected health facilities in Kumasi. Results. The total number of live births recorded was 27,449. Orofacial anomalies recorded were 36, giving an incidence of 1.31/1000 live births or 1 in 763 live births. The mean maternal age of cleft lip/palate babies was 29.85 years (range 18–40 years). The male : female ratio for the orofacial anomalies babies was 1.3 : 1; the male : female ratio was 0.5 : 1 in the cleft lip group, 1.3 : 1 in the cleft lip and palate group, and 4 : 1 in the cleft palate group. The majority of clefts were unilateral (69.4%, n=25), with females (n=14) outnumbering males (n=11). A family history of cleft was recorded with five babies (13.9%). Associated congenital anomalies were recorded in seven (19.4%) cleft lips and/or palates. Conclusion. The incidence of 1 in 763 live births found in this study indicates that cleft lip/palate is a common congenital anomaly in Kumasi.

Background: Inflammatory myofibroblastic tumor (IMFT) is a rare neoplasm mainly affecting children and young adults. We conducted a retrospective study to evaluate the clinical features and treatment alternatives of childhood inflammatory myofibroblastic tumors.Methods: A total of 19 patients who were pathologically diagnosed with IMT between December 2008 and October 2018 were included. Collected data were demographic information, main complaints, tumor characteristics, treatment, pathological results, immunohistochemical analysis, and prognosis.Results: The male/female ratio was 13:6. The mean age at disease onset was 44.9 ± 33.9 months (range 4 to 111 months). The mean tumor size was 6.5 ± 4.0 cm (range 1.2 to 17.0 cm). The most common site was the abdomen (13/19). The most commonly used detection tool was CT. Eleven patients (57.9%) had aggressive tumor growth, including eight receiving extensive resection and three receiving palliative resection due to high local invasiveness and postoperative chemotherapy. Eight cases whose tumors were completely enveloped received complete resection. Immunohistochemistry was performed for 17 patients and ALK positivity was found in 11 patients. Despite three children lost to follow-up, sixteen patients were followed up for 6 to 132 months (average 63.9 months, median 66 months). Of which, twelve children survived with no evidence of IMT, and four cases (21%) showed local recurrences (two of them died). No distant metastasis was detected.Conclusions: IMT is rare in children with various locations, mostly appearing in the abdomen. Whether the tumor could be completely removed, the location and the invasiveness of surrounding tissues might be highly prognosis-related.

Objective: Objective of this study determines the outcome of unipolarhemiarthoplasty in elderly patients with femoral neck fracture at Liaquat University HospitalHyderabad/ Jamshoro. Materials and methods: Total 50 elderly patients were included in thestudy with femoral neck fracture and all the cases were selected from accident, emergency andOPD. All the patients with the history of pregnancy, those patients who were unfit for anesthesia,malignancy, neurological deficits of lower extremity, rheumatoid arthritis and Parkinson’sdisease regarding with systemically disease were excluded from the study. All the patientsunder went surgical hemiarthoplasty treatment with Austin moor-femoral head prosthesia. In thelast all postoperative complications, functional outcome and mortality were noted on proforma.Results: Total 50 patients were included in the study. Mean age of this study was mean±SD64.98±4.13. Females were found in majority with male/ female ratio 1:1.27. Post operative painwas noted in the 50% of the cases and out of them severe pain was noted only in 4% of thecases. Superficial infection was seen in 4% of the cases and deep infection was not found inthe cases and death was occurred in 10% of the patients. On the outcome, excellent resultswere found in the 44.44% of the study participants, good and fair results were seen in 26.66%and 20% respectively, while poor results were seen in 8.88% of the patients. Conclusions: Itis concluded that unipolar hemiarthoplasty is of the reliable procedure by use of Austin-moorfemoral head prosthesia for the treatment of femoral neck fracture in elderly.

Aim: to analyze the structure and changes of colorectal cancer (CRC) epidemiology in the Ulyanovsk region in 2005–2019 according to the regional cancer register.Materials and methods: the study is based on statistical data on the primary detection of CRC in the districts of the Ulyanovsk region (form No. 7) in 2005–2019. Standardized rates of morbidity and mortality were evaluated. Data on the number, gender and age of the regional population were obtained in Ulyanovsk State Statistics Service.Results: the incidence of CRC in the region increased by 1.51 times over the analyzed period (from 31.12 to 49.58 per 100 thousand people). From the total number of newly diagnosed CRC in 2019, the urban population was 25%, and the rural population — 75%. The incidence rates in the districts of the region were assessed. The highest morbidity in males was detected at the age of 60–64 years old, and it was 1.58 times higher than in females. In the adult population under 30 years old, only few cases of colorectal cancer were detected. The main histological type of tumors was adenocarcinoma. Poorly differentiated tumors accounted for about 3%. The highest age-standardized rates for rectal tumors were 10.1 and 12.8 per 100 thousand people in 2005 and 2019, respectively; for the recto sigmoid tumors — 0.9 and 2.3 per 100 thousand people; and for the anal cancer — 0.4 cases per 100 thousand people in 2019. The male/female ratio of deaths from CRC in 2019 was 1.005:1.000.Conclusion: there has been a trend to an increase in the incidence of colorectal cancer among the Ulyanovsk region population in the period from 2005 to 2019. We identified some areas of the region that significantly differ in the incidence of colorectal cancer.

Introduction: Haemorrhoid is the frequent problem in clinical practice of general surgery. Haemorrhoids are dilatation and tortuosity of rectal veins affecting almost 4.5% of the population. Various operative and non operative therapeutic options are in practice to treat haemorrhoids, yet none has been proven cent percent effective. Sclerotherapy is a simple, safe and cost effective procedure that is widely practiced globally to treat haemorrhoids. Many sclerosants are being used with variable efficacies. Objective: To evaluate the outcome of injection Sclerotherapy in first degree and second degree haemorrhoids. Materials and Methods: Fifty patients of both gender having haemorrhoids who attended Outpatient Department (OPD) at BNS Patenga from September 2013 to August 2014 were included in this study. On the basis of history, patients were selected for Digital Rectal Examination (DRE) and proctoscopic examination to confirm the diagnosis. Sclerotherapy was repeated at interval of 6 weeks till symptomatic relief was achieved. Maximum 3 sittings of sclerotherapy were given. Patients were followed up at 3 months interval for 1 year. Results: This prospective study was conducted on 50 cases of first degree and second degree Haemorrhoids. The highest number of patients 21(42%) were in the age group of 21-30 years with majority 38(76%) being male. Male female ratio was 3.16: 1. All the patients presented with painless per-rectal bleeding. Satisfactory results were observed in 29(58%) patients of whom 20(40%) patients were having first degree haemorrhoids and 9(18%) patients were having second degree haemorrhoids. Sclerotherapy was more effective in first degree compared to second degree haemorrhoids patients. Conclusion: Injecting Sclerotherapy is a suitable office procedure for treatment of haemorrhoids. The method is easy, convenient, cheap, well accepted and comfortable for patient. More or less there are no complications and the result is also satisfactory.
