Type 2 diabetes mellitus (DM) is the 10th leading cause of death in the world and had a risk 4.06 times greater to have cardiovascular disease (CVD). Fasting plasma glucose and 2-hours postprandial glucose level as diagnostic tests to evaluate DM and also as a predictive factors to CVD. This research uses descriptive analytic research with cross sectional design. The subject of this study is Type 2 DM patients came to Dr Soetomo hospital from January-October 2014. Data was recorded from medical records of patients and was analyzed with Chi Square test with p<0.05. As result, there is 102 Type 2 DM patients with 36 patients have CVD manifestation and 66 patients without complication. There is a significant association between fasting plasma glucose (FPG), 2-hours postprandial glucose (2hPPG) level, and CVD manifestation (p=0.00). So FPG and 2hPPG have a significant association with CVD manifestation. There are other factors that influencing the formation of CVD, hence, further research is needed.

AbstractThe aim of the study was to assess whether a simple substitution of carbohydrate in the conventionally recommended diet with protein and fat would result in a clinically meaningful reduction in postprandial hyperglycaemia in subjects with type 2 diabetes mellitus (T2DM). In all, sixteen subjects with T2DM treated with metformin only, fourteen male, with a median age of 65 (43–70) years, HbA1cof 6·5 % (47 mmol/l) (5·5–8·3 % (37–67 mmol/l)) and a BMI of 30 (sd4·4) kg/m2participated in the randomised, cross-over study. A carbohydrate-reduced high-protein (CRHP) diet was compared with an iso-energetic conventional diabetes (CD) diet. Macronutrient contents of the CRHP/CD diets consisted of 31/54 % energy from carbohydrate, 29/16 % energy from protein and 40/30 % energy from fat, respectively. Each diet was consumed on 2 consecutive days in a randomised order. Postprandial glycaemia, pancreatic and gut hormones, as well as satiety, were evaluated at breakfast and lunch. Compared with the CD diet, the CRHP diet reduced postprandial AUC of glucose by 14 %, insulin by 22 % and glucose-dependent insulinotropic polypeptide by 17 % (allP<0·001), respectively. Correspondingly, glucagon AUC increased by 33 % (P<0·001), cholecystokinin by 24 % (P=0·004) and satiety scores by 7 % (P=0·035), respectively. A moderate reduction in carbohydrate with an increase in fat and protein in the diet, compared with an energy-matched CD diet, greatly reduced postprandial glucose excursions and resulted in increased satiety in patients with well-controlled T2DM.

Objectives Alzheimer’s disease (AD), impaired fasting glucose (IFG), and Type 2 diabetes mellitus (T2DM) were reported associated with smaller brain volumes. Nevertheless, the association of hyperglycemia with brain volume changes in AD patients remains unclear. To investigate this issue, structural magnetic resonance imaging was used to compare brain volumes among AD patients with different fasting glucose levels. Methods Eighty-five AD patients were divided into three groups based on their fasting glucose level as suggested by the American Diabetes Association: normal fasting glucose group (AD_NFG, n = 45), AD_IFG group (n = 15), and AD_T2DM group (n = 25). Sagittal 3D T1-weighted images were obtained to calculate the brain volume. Brain parenchyma and 33 brain structures were automatically segmented. Each regional volume was analyzed among groups. For regions with statistical significance, partial correlation analysis was used to evaluate their relationships with fasting glucose level, corrected for Mini-Mental State Examination score, age, education level, cholesterol, triglyceride, and blood pressure. Results Compared with the AD_IFG and AD_NFG groups, the volume of pons in AD_T2DM group was significantly smaller. Fasting glucose was negatively correlated with pontine volume. Conclusions T2DM may exacerbate pontine atrophy in AD patients, and fasting glucose level is associated with pontine volume.

Background: Diabetes mellitus (DM) is one of the leading causes of death and it is caused by genetics, nutrition, and unhealthy behaviors. Therefore, changes in lifestyle associated with eating behaviors in diabetes mellitus patients greatly impact on their quality of life. There are many factors related with changes in lifestyle of diabetes mellitus patients, especially eating behaviors.Purpose: This study aims to examine the relationships between self-efficacy, psychological stress, family support, and eating behaviors among type 2 diabetes mellitus (T2DM) patients in Sidoarjo, Indonesia.Method: A total of 117 T2DM patients from the Sidoarjo Community Health Center were included in the analysis. Using SPSS IBM 21.0 program, Pearson product moment correlation was performed to analyze data.Results: The findings showed that self-efficacy and family support had positive relationship with eating behaviors (r = .692, p < .001; r = .683, p < .001, respectively). Psychological stress had negative relationship with eating behaviors (r = -.327, p < .001).Conclusion: Self-efficacy, family support, and psychological stress had relationship with eating behaviors. Nurses should pay attention to the factors to make T2DM patients into a long-term commitment toward healthy eating behaviors.

  Introduction: Diabetes self-care is an effort to control type 2 diabetes mellitus. Family support and self-efficacy are needed for elderly people with type 2 diabetes mellitus to increase independence in managing their disease. This study aims to determine the relationship between family support and self-efficacy with self-care behavior in the elderly with type 2 diabetes mellitus at the Elderly Posyandu in Wadungasri Village. Methods: This study is a quantitative study with a correlational analytic research design using a cross-sectional approach. The population in this study was 132 elderly with type 2 diabetes mellitus using a probability sampling method with a technique used simple random sampling and obtained a sample of 100 elderly DM type 2. Results: The results showed that most of the elderly have family support and good self-efficacy. with less self-care behavior. The results of the analysis using the Spearman rank test showed that family support was associated with self-care for people with diabetes mellitus type 2 (p-value = 0.006, ɑ = 0.05). The results of the Spearman rank test analysis showed that self-efficacy was related to self-care for people with diabetes mellitus type 2 (p-value = 0.001, ɑ = 0.05). Conclusion: Family support and self-efficacy gave to elderly people with type 2 diabetes mellitus can influence health behavior patterns that will make self-care behavior well.

AbstractDiabetes mellitus is a chronic disease that cannot be cured, but can be prevented by using the 5 pillars of diabetes mellitus management, one of which is controlling blood sugar levels. Patients with diabetes mellitus need support from the family because the family can influence a person's behavior and lifestyle so that it has an impact on the quality of life of patients with type 2 diabetes mellitus. To determine the relationship between family support and blood sugar levels in patients with type 2 diabetes mellitus through a literature review. The data collection technique uses a literature review method of 5 articles sourced from an online database with electronic searches on Google Scholar, Garba Garuda, and ProQuest published in 2017-2021. The critical analysis instrument used is using a strobe. The results of a literature review on the family support variable were 2 articles with supporting categories as many as 128 respondents, 2 articles with less categories as many as 63 respondents, and 1 article with good categories as many as 22 respondents. Variable blood sugar levels 2 articles in the controlled category as many as 98 respondents, 1 article in the poor category as many as 45 respondents, 1 article in the controlled category as many as 17 respondents, and 1 article in the diabetes category as many as 29 respondents. Teknik pengumpulan data menggunakan metode literature review 5 artikel yang bersumber database online dengan penelusuran elektronik pada Google Scholar, Garba Garuda, dan ProQuest yang dipublish pada tahun 2017-2021. Instrumen telaah kritis yang digunakan yaitu menggunakan Strobe. Hasil literature review pada variable dukungan keluarga 2 artikel dengan kategori mendukung sebanyak 128 responden, 2 artikel dengan kategori kurang sebanyak 63 responden, dan 1 artikel dengan kategori baik sebanyak 22 responden. Variable kadar gula darah 2 artikel dengan kategori terkendali sebanyak 98 responden, 1 artikel dengan kategori buruk sebanyak 45 responden, 1 artikel dengan kategori terkontrol sebanyak 17 responden, dan 1 artikel dengan kategori diabetes sebanyak 29 responden. Introduction: Selective sodium-glucose cotransporter 2 (SGLT2) inhibitors target only the kidney, and they have reduced efficacy when patients with type 2 diabetes mellitus (T2DM) have renal impairment (RI). LX4211 blocks sodium and glucose absorption in the gastrointestinal tract by inhibition of SGLT1, and it enhances urinary sodium and glucose excretion in the urine through inhibition of SGLT2. The dual SGLT1/2 action of LX4211 was anticipated to reduce systolic blood pressure (SBP) in addition to improving glucose control in the setting of RI. Methods: This analysis explored the effect of LX4211 on SBP in a clinical trial of patients with T2DM and moderate to severe RI. Patients (N=31) were randomly assigned to be treated with LX4211 (400 mg, N=16) or placebo (N=15) qd for 7 consecutive days. Postprandial glucose levels after a standard high glucose meal served as the primary measure of pharmacodynamic activity. Baseline and Day 8 trough SBP measures were each an average of 3 seated assessments. Results: Mean baseline characteristics included age 66.4 years, estimated glomerular filtration rate (eGFR) 43.4 mL/min/1.73 m 2 , and SBP 130.9 mmHg. Postprandial glucose area under the curve (sampled from pre-dose to 4 hours post meal) was reduced from Baseline to Day 7 by 169.3 mg*hr/dL on LX4211 compared to placebo (p=0.003). Day 8 SBP reductions were 11.4 mmHg on LX4211 and 0.0 mmHg on placebo (p=0.045 for difference between groups). Patients with greater RI (eGFR <45 mL/min/1.73 m2) treated with LX4211 (N=6) had a 10.5 mmHg SBP reduction compared to 0.3 mmHg on placebo (N=9). The difference between seated and standing SBP did not change with LX4211 (0.0 mmHg change, Day 8 vs. Baseline). There were no reports of hypotension, hypovolemia, no serious adverse events, and no patient discontinued due to an adverse event. Mild hypoglycemia was reported in 1 LX4211 patient compared to 2 placebo patients. Conclusions: LX4211 may reduce SBP and enhance glycemic control in T2DM patients with moderate to severe RI.

We studied 98 native Greek patients with Type 2 diabetes mellitus. Their degree of glycemic control, evaluated with glycated hemoglobin A1c levels, was correlated with perceived family support, assessed with the Family Support Scale. The different social context of Greece—and its corresponding perception of family support—influence glycemic control in a way that is at variance from data of some studies of Anglo-Saxon families. In conclusion, family support should be taken into consideration in the management of native Greek patients with Type 2 diabetes mellitus.
