The epidemic situation and etiological factors of enterovirus infection in Russia and Vietnam were analysed and compared. The identified strains of enteroviruses of 47 types, which circulated in Russia in 2018–2019, belonged to different species: Enterovirus species A (CV-A2, CV-A4, CV-A6, CV-A8, CV-A10, CV-A16, EV-A71), as well as Enterovirus species B and Enterovirus species C. The strains isolated from 87 children from southern Vietnam hospitalised in 2018–2019 into infectious hospitals while having enterovirus infection with exanthema were also studied. All identified strains were represented by Enterovirus species A: EV-A71 — 59 strains, CV-A10 — 20 strains, CV-A16 — 5 strains, CV-A6 — two strains and CV-A2 — one strain. Out of 59 viruses EV-A71, 53 strains belonged to genotype C4 and 6 strains belonged to genotype B5. The sequences of EV-A71 strains of genotype C4 from South Vietnam formed a monophyletic cluster with the sequences of EV-A71 viruses which circulated during 2016–2018 in different provinces of China, and they were very close to EV-A71 strains of the same genotype from the Yunnan Province. These strains were genetically different from Russian viruses and Vietnamese viruses identified in the years 2003–2005 and 2011– 2012. Most of the cases of enterovirus infection from southern Vietnam (78%) caused by EV-A71 virus of genotype C4 were reported in three provinces located in southern Vietnam in the Mekong Delta. The epidemic process and the etiology of enterovirus infection in Russia and Vietnam have common features. At the same time, the epidemic situation in these countries is not the same. The incidence of enterovirus infection is influenced by geographic, climatic, economic and demographic factors that differ in two countries. In the majority of territories of Russia, the climate is temperate or cold, seasonal rises in the incidence rates of enterovirus infection usually occur in the summer, when people go on vacation, spend a lot of time outdoors and swim in open reservoirs. In Vietnam, a constant high-level temperature, a high population density and a large proportion of children determine the higher incidence of enterovirus infection, especially in the southern provinces of Vietnam, compared to Russia. The fact that more than 20% of the Vietnamese population lives in the Mekong Delta, which is the largest river in Indochina, has a significant impact on the epidemic process of enterovirus infection in South Vietnam. The Mekong River which flows through China, Laos, Thailand, Cambodia and Vietnam, and carries huge streams of water, including rainstorm waters and sewages, from all of these countries into the southern provinces of Vietnam, which have the highest incidence rates of enterovirus infection in the country. The results of the research underline the importance of active epidemiological and virological surveillance of enterovirus infection, which plays the key role in informing the public health authorities about the changes in the epidemic situation in order to take appropriate measures and develop the prevention strategies. The goal of anti-epidemic and preventive measures is to reduce the incidence of enterovirus infection and the economic burden of this infection for Russia and Vietnam.

AbstractBackgroundThe Robert-Koch-Institute reports that during the summer holiday period a foreign country is stated as the most likely place of infection for an average of 27 and a maximum of 49% of new SARS-CoV-2 infections in Germany.MethodsCross-sectional study on observational data. In Germany, summer school holidays are coordinated between states and spread out over 13 weeks. Employing a dynamic model with district fixed effects, we analyze the association between these holidays and weekly incidence rates across 401 German districts.ResultsWe find effects of the holiday period of around 45% of the average district incidence rates in Germany during their respective final week of holidays and the 2 weeks after holidays end. Western states tend to experience stronger effects than Eastern states. We also find statistically significant interaction effects of school holidays with per capita taxable income and the share of foreign residents in a district’s population.ConclusionsOur results suggest that changed behavior during the holiday season accelerated the pandemic and made it considerably more difficult for public health authorities to contain the spread of the virus by means of contact tracing. Germany’s public health authorities did not prepare adequately for this acceleration.

Abstract Background At the end of February 2020 a new case was diagnosed with COVID-19 in north Italy, suddenly followed by new cases. Italian health authorities decided to enforce restrictive measures. Northern areas of Italy were identified as “red areas” to slow down the epidemic and its impact on the healthcare system. From this perspective, the COVID-19 epidemic in Italy constitutes a testing ground for the assessment of the ability of consumers to cope with this health risk. Methods A cross-sectional study on a representative sample of 1000 Italian citizens was conducted over the period 27 February- 5 March exploring the following hypotheses: 1) less engaged individuals are more concerned for the health emergency and feel more vulnerable; 2) less engaged have higher probability to develop negative attitudes and dysfunctional behaviors. Results only the 16% the interviewees presented a high level of engagement. Lower levels of engagement were measured in the southern parts of Italy (not yet touched by the epidemics at the time of data collection). People with lowers levels of engagement reported higher fears for the contagion and sense of vulnerability. Furthermore, they showed the lower levels of trust in the Public Health Authorities, in medical research and in vaccines. Finally, they appeared more dismissive in their preventive behaviors and more disorganized in the fruition of the healthcare services. Conclusions A psychological analysis of processes of attitudinal and role change in the direction of becoming more engaged in health prevention is worthy in order to forecast potential dysfunctional reactions to restrictive health prevention measures and to orient personalized education initiatives to consumers with different level of engagement. Key messages Profiling based on the levels of health engagement is important in order to plan more effective healthcare measures during epidemics. Targeted educational initiatives should take into account citizens' engagement profiles.

AbstractComprehensive population-based data on myeloid neoplasms (MNs) are limited, mainly because some subtypes were not recognized as hematological cancers prior to the WHO publication in 2001, and others are too rare to allow robust estimates within regional studies. Herein, we provide incidence data of the whole spectrum of MNs in Spain during 2002–2013 using harmonized data from 13 population-based cancer registries. Cases (n = 17,522) were grouped following the HAEMACARE groupings and 2013-European standardized incidence rates (ASRE), incidence trends, and estimates for 2021 were calculated. ASRE per 100,000 inhabitants was 5.14 (95% CI: 5.00–5.27) for myeloproliferative neoplasms (MPN), 4.71 (95% CI: 4.59–4.84) for myelodysplastic syndromes (MDS), 3.91 (95% CI: 3.79–4.02) for acute myeloid leukemia, 0.83 (95% CI: 0.78–0.88) for MDS/MPN, 0.35 (95% CI: 0.32–0.39) for acute leukemia of ambiguous lineage, and 0.58 (95% CI: 0.53–0.62) for not-otherwise specified (NOS) cases. This study highlights some useful points for public health authorities, such as the remarkable variability in incidence rates among Spanish provinces, the increasing incidence of MPN, MDS, and MDS/MPN during the period of study, in contrast to a drop in NOS cases, and the number of cases expected in 2021 based on these data (8446 new MNs).

Aedes aegypti represents the principal vector of many arthropod-borne diseases in tropical areas worldwide. Since mosquito control strategies are mainly based on use of insecticides, resistance development can be expected to occur in frequently exposed Ae. aegypti populations. Surveillance on resistance development as well as testing of insecticide susceptibility is therefore mandatory and needs further attention by national/international public health authorities. In accordance, we here conducted a study on Ae. aegypti resistance development towards several often used insecticides, i.e., malathion, deltamethrin, permethrin, λ-cyhalothrin, bendiocarb, and cyfluthrin, in the periurban area of Banjarmasin city, Kalimantan, Indonesia. Our results clearly showed resistance development of Ae. aegypti populations against tested insecticides. Mortalities of Ae. aegypti were less than 90% with the highest resistance observed against 0.75% permethrin. Collected mosquitoes from Banjarmasin also presented high level of resistance development to 0.1% bendiocarb. Molecular analysis of voltage-gated sodium channel (Vgsc) gene showed significant association of V1016G gene point mutation in resistance Ae. aegypti phenotypes against 0.75% permethrin. However, F1534C gene point mutation did not correlate to Ae. aegypti insecticide resistance to 0.75% permethrin. Irrespective of periurban areas in Kalimantan considered as less densed island of Indonesia, Ae. aegypti-derived resistance to different routinely applied insecticides occurred. Our findings evidence that Ae. aegypti insecticide resistance is most likely spreading into less populated areas and thus needs further surveillance in order to delay Ae. aegypti resistance development.

ABSTRACT Rapid and specific detection of Shiga toxin-producing Escherichia coli (STEC) strains with a high level of virulence for humans has become a priority for public health authorities. This study reports on the development of a low-density macroarray for simultaneously testing the genes stx 1, stx 2, eae, and ehxA and six different nle genes issued from genomic islands OI-122 (ent, nleB, and nleE) and OI-71 (nleF, nleH1-2, and nleA). Various strains of E. coli isolated from the environment, food, animals, and healthy children have been compared with clinical isolates of various seropathotypes. The eae gene was detected in all enteropathogenic E. coli (EPEC) strains as well as in enterohemorrhagic E. coli (EHEC) strains, except in EHEC O91:H21 and EHEC O113:H21. The gene ehxA was more prevalent in EHEC (90%) than in STEC (42.66%) strains, in which it was unequally distributed. The nle genes were detected only in some EPEC and EHEC strains but with various distributions, showing that nle genes are strain and/or serotype specific, probably reflecting adaptation of the strains to different hosts or environmental niches. One characteristic nle gene distribution in EHEC O157:[H7], O111:[H8], O26:[H11], O103:H25, O118:[H16], O121:[H19], O5:H−, O55:H7, O123:H11, O172:H25, and O165:H25 was ent/espL2, nleB, nleE, nleF, nleH1-2, nleA. (Brackets indicate genotyping of the flic or rfb genes.) A second nle pattern (ent/espL2, nleB, nleE, nleH1-2) was characteristic of EHEC O103:H2, O145:[H28], O45:H2, and O15:H2. The presence of eae, ent/espL2, nleB, nleE, and nleH1-2 genes is a clear signature of STEC strains with high virulence for humans.

The Population-based Cancer Registry of Cali aims to report all new cases in permanent residents within the limits of the city of Cali. Time trends of cancer incidence and mortality are described. The registry has been in continuous operation for 50 years. Methods: Cancer cases reports are obtained actively by visiting all sources of information: hospitals, pathology departments, hematology laboratories, radiotherapy centers, government offices where death certificates are processed and physician’s offices. It is estimated that the reporting is at least 95% complete. Results: Drastic decreases are documented in rates for tumors causally related to infectious agents, especially cancers of the uterine cervix and the stomach. Gradual increases are documented in rates of tumors linked to affluence and the metabolic syndrome, especially cancers of the colon and the female breast. An unexpected increase in the incidence of papillary carcinoma of the thyroid gland in women is reported. Tobacco-related cancers, especially cancer of the lung, showed marked increase in incidence rates around 1970, apparently the beginning of an epidemic similar to the one reported in Western societies. But the increase in incidence stopped around 1980, resulting from a strong anti-smoking campaign launched in Colombia in the 1970s. Conclusions: The findings have influenced prevention strategies implemented by public health authorities, specially the establishment of a city-wide program to prevent cervix cancer via widespread use of vaginal cytology and anti-smoking campaigns. Also, new population-based cancer registries have been established in other Colombian cities as well as in Ecuador.

Abstract Background Lassa virus (LASV) is a highly prevalent arenavirus that affects two to three million people in West Africa. This rodent borne virus which has serious consequences on the population and hospital staff in endemic areas. In this article, we review prevalence of LASV with a focus on the dynamic and epidemiology of the disease of 1969-2019. What informs on the evolution and the extent of the disease in this at-risk zone in order to prepare response measures in the event of an epidemic. Methods We was conducted a retrospective review through literatures search using the AGORA, PubMed, Science Direct, Scopus, researchgate and Google scholar Database on Lassa fever (LF) from West Africa. A total 34 articles were studied from 11 countries. Studies were categorized by host and country, and meta-analysis conducted to determine pooled prevalence estimates for each category Analysis was done using the metaprop command in STATA version 15 and MetaXL software. Results A total of 18.111 individual samples from 11 countries, described in 34 articles were studied. Meta-analysis of twenty six studies have indicated that the pooled prevalence was 19.0% [95% CI (15.0-23.0%), I2 = 97.93%]. There was a high level of heterogeneity between studies; however, the high prevalence of LASV was noted in several countries as Nigeria (12-42%), followed by Sierra Leone (8-43%), and then Guinea (9-40%). Pooled prevalence of LASV for human in studies conducted over the entire review period was 22.0% [95% CI (17.0-28.0%), I2 = 98.0%]; eighteen studies), while she was 9.0% [95% CI (4.0-15.0%), I2 = 97.0%]; eight studies) for Mastomys spp. Conclusion The knowledge of the geographical distribution and epidemiology may have help for disease control efforts and limit the risk of transmission, both locally and internationally. This study is also important in order to guide interventions, public health authorities and inform on the evolution of the disease and its magnitude in the population.

The paper presents analysis of the structure of HIV-infected persons at different stages of the development of the HIV epidemic process in the Tyumen region (no autonomous areas) from the moment of the first case registration until 2018. We used descriptive-evaluative, analytical, statistical and epidemiological research methods taking into account intensive and extensive indicators, the structure of HIV-infected persons, the identification of trend in the epidemic process. Analysis of the long-term and intra-annual dynamics of the morbidity and prevalence of HIV infection in the Tyumen region clearly showed that the epidemiological situation in the region is alarmingly tense, but the concentration of health authorities and the sanitary-epidemiological service have already yielded positive results in this direction.

The purpose of the study was to analyze the comparability of data from the bodies of both the Federal Service on Customers Rights Protection and Human Well-being Surveillance (Rospotrebnadzor) and health authorities in terms of the incidence of the temporary disability in the constituent entities of the Russian Federation. Material and methods. Information on indices of the incidence with the temporary disability, presented in state reports “On the sanitary and epidemiological well-being of the population” of the Territorial Departments of the Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing Surveillance, is examined. The information on incidence indices with the temporary disability, presented in the Unified interdepartmental information-statistical system, is investigated. Result and discussion. The incidence rates with the temporary disability presented by the Russian Federation’s executive bodies in the field of healthcare and used by the Territorial Departments of the Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing Surveillance do not correspond to each other. The composition of data on morbidity with the temporary disability in state reports “On the sanitary and epidemiological well-being of the population” of the Territorial Departments of the Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing has a high degree of heterogeneity. Most government reports do not include data on the incidence of temporary disability. Conclusion. To provide comparability of the working population’s health indices at the national and regional levels, the Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing Surveillance bodies and institutions have to develop the uniform methodological approaches to assessing and controlling the incidence rate with temporary disability. The territorial administrations of the Federal Service for Surveillance on Consumer Rights Protection and Human Wellbeing Surveillance (Rospotrebnadzor) are required to receive data from public health authorities of the constituent entities of the Russian Federation that correspond to data provided for statistical purposes at the federal level.

SUMMARYThroughout December 2010 and January 2011, Queensland experienced widespread flooding due to unusually protracted and heavy rainfalls. In mid-January 2011, four individuals from a small community in Central Queensland were hospitalized with leptospirosis. A further five cases were subsequently identified from around Central Queensland, bringing the total to nine. Microscopic agglutination testing found that serovar Arborea (Leptospira borgpetersenii serovar Arborea) was presumptively responsible for leptospirosis in seven of nine confirmed cases. Serovars Hardjo and Australis were identified in samples from two remaining cases. All cases had exposure to flood water. No single exposure source was identified. This is the first reported outbreak of leptospirosis in Central Queensland and the first report of leptospirosis cases associated with flood water inundation in Queensland. Public health authorities should continue to promote awareness of leptospirosis in flood-affected populations. Healthcare providers must maintain a high level of suspicion for leptospirosis during and after flood events.
