ABSTRACT Objective: to identify absenteeism of nursing technicians in an Intensive Care Unit and to verify the association between absenteeism and workload. Method: cross-sectional retrospective study of documentary source and quantitative approach. It was performed in an adult intensive care unit with 4,920 measurements from the Nursing Activities Score in 2017, in order to extract the nursing workload, and control absenteeism, considering the absences recorded in the management register of the department. Descriptive and inferential statistical analysis was performed, dividing the intensive care unit into two groups (ICU I and ICU II), according to team shifts. Results: in the period analyzed, 461 absences were observed, with a higher proportion in ICU II (5.35%, p-value=0.00). This group also had the highest Nursing Activities Score (99.76) average. However, workload and absenteeism showed no statistically significant correlation in the analyzed groups (ICU I, r=0.02 and ICU II, r=0.06). Conclusion: there was no statistically significant association between absenteeism and workload of nursing technicians in the Intensive Care Unit. However, the team with the highest workload was the most absent.

ABSTRACT Objective: To analyze near miss cases among newborns hospitalized in the Intensive Care Unit. Methods: An observational, cross-sectional, retrospective study using the STROBE guideline. Data were collected from 1,101 records of live births (newborns). Statistical analysis used the Epi-Info program 3.3.2 and Chi-square and Fisher’s exact tests. Results: A total of 162 newborns were hospitalized, of which 63 had at least one criterion of near miss. The variables that remained associated with neonatal near miss were weight <1.750 g, gestational age <33 weeks and Apgar at 5 minutes <7, pragmatic criteria to identify cases of neonatal near miss morbidity. Conclusion: Prematurity and low birth weight were the factors that contributed most to the near miss outcome among newborns hospitalized in intensive care, a rate two and a half times higher than the number of deaths, according to scientific evidence.

Objective: Evaluate the relation of nursing workload, evaluated by the Nursing Activities Score (NAS), with the occurrence of Ventilator-associated Pneumonia (VAP) in an Intensive Care Unit (ICU) and the impact of VAP on hospitalization costs.Methods: Retrospective cohort study in Adult ICU of a high complexity Brazilian university hospital. The profile, outcomes, costs, and daily NAS from patients were collected. We also proposed some workload indicators based on NAS daily evaluation.Results: The study included 195 patients, 27.17% diagnosed with VAP. VAP was more prevalent in patients diagnosed with trauma on admission. The total costs of care were higher for VAP patients. In all multivariate models tested were predictive for VAP: the patient's intubation that occurs in days prior of the ICU admission day (higher risk if occurs in days prior the ICU admission day) and ventilation time prior ICU (higher risk if higher time). We found others predictors, but these were dependent on the model tested. Additional risk predictors were tracheostomy, propofol use, neuromuscular blocker use and the higher NAS from admission. The protective factors found were the percentage of adequacy of the assignment based in NAS that measure if the workload measured by the NAS was offered and the increment in NAS during the ventilation time.Conclusions: The offering of an adequate nursing work scale (adequate number of professionals for the care), as a function of the nursing workload measured by the NAS, could be effective in the reduction of VAP, hospital stay time and hospital costs.

Introduction: Road accidents are one of the causes of death and it's important to investigate the epidemiological indicators in this context. In this regard, this study aimed to determine the epidemiological causes of road accident mortality in the intensive care unit of Besat Educational-medical Hospital in Hamadan during Nowruz Holidays 2018. Material and Methods: In this descriptive cross-sectional study, data of the patients died for 13 days in Nowruz Holidays in the intensive care unit of Besat Hospital in Hamadan, by census method, was extracted from the medical records of patients by using a checklist from a researcher-made questionnaire. Data were analyzed by Fisher's exact test and SPSS 21 software. Results: Of the 28 Patient admitted to the intensive care unit,16 patients died. Most deaths were in the third decade of life, among single men, with a self-employment job, with diploma education level or cycle education level, with more than 10 days of hospitalization and in the night shift. Based on the results, most of the dead persons were rider (81.25%), suffered head trauma (50%), with head surgery (56.25%), without any underlying illness (93.75%) and without successful experience of cardiopulmonary resuscitation (87.5%). There were no statistically significant differences between the variables and mortality rate (P value > 0/05). Conclusion: Young single men with low level of education are a wide range of road accidents victims. Therefore, it seems it's necessary to reduce mortality from these accidents, health planning such as traffic culture training and driving rules, by using social media, should focus on learning and improving the level of community awareness, especially in this group of people.

A internação na UTI (Unidade de Terapia Intensiva),  e o processo de adoecimento podem gerar implicações negativas ao sujeito como alterações motoras, cognitivas e na participação nas atividades de vida diária. Muitos pacientes compartilham más experiências vivenciadas durante a internação em UTI (as quais desorganizam o sujeito, identificar as necessidades pode contribuir na elaboração do plano de tratamento adequado às demandas do paciente. Dessa forma,o objetivo do trabalho foi de  caracterizar o perfil desses pacientes atendidos pela Terapia Ocupacional na UTI Adulto. Realizou-se uma pesquisa transversal observacional da qual participaram 37 sujeitos internados na UTI adulto de um hospital público da Zona Leste do município de São Paulo no período de junho a agosto de 2016. Verificou-se que as características mais presentes entre os participantes dos grupos conforme gênero, idade e dias de internação foram:  fraqueza, dor, mobilidade reduzida, limitação no autocuidado, banho e higiene foram os mais presentes entre os participantes. Déficit cognitivo e queixa de memória foram os mais elencados no grupo dos idosos. A internação na UTI e o processo de adoecimento podem gerar implicações negativas ao sujeito como alterações motoras, cognitivas e na participação nas atividades de vida diária. Com base nos resultados identificou-se que os pacientes internados apresentaram fatores que interferem na participação nas atividades de vida diária (autocuidado, banho, higiene, arrumar-se no leito, alimentação e controle de esfíncter), e a atuação da Terapia Ocupacional pode contribuir de forma a minimizar os efeitos deletérios decorrentes da hospitalizaçãoAbstractMany patients live bad experiences during their stay in ICU, that disrupt the subject. Identifying the needs of the person can contribute to the elaboration of the treatment plan. Thus, is importante to identify the profile of the patients treated by Occupational Therapy in an Adult Intensive Care Unit. The study consisted of an observational cross-sectional study in which 37 subjects were hospitalized in the adult ICU of a public hospital in the East of the city of São Paulo from June to August 2016. The presence of weakness, pain, reduced mobility, self-care limitation, bathing and hygiene were the most frequent among the participants according to gender, age and days of hospitalization. Cognitive deficit and memory complaints were the most prominent in the elderly group. ICU hospitalization and the process of illness may cause negative implications for the subject, such as motor and cognitive impairment and difficulties in activities of daily living. The data made it possible to identify the profile of the patients treated by Occupational Therapy in the adult ICU, which makes it possible to guide the practice itself.Keywords: Occupational Therapy, Intensive Care Unit, Adult. ResumenMuchos pacientes comparten de las malas experiencias vivenciadas durante la internação en UCI, las cuales desorganizan el sujeto. De esa forma, se resalta la importancia de identificar el perfil de los pacientes atendidos por la Terapia Ocupacional en la Unidad de Cuidados Intensivos de Adultos. Se realizó una investigación transversal observacional en que fueron evaluados 37 sujetos ingresados en la UCI de adultos de un hospital público de la Zona Este del municipio de São Paulo en el periodo de junio a agosto de 2016. Se verificó que presencia de flaqueza, dolor, movilidad reducida, limitación en el autocuidado, baño e higiene fueron los más presentes entre los participantes de los grupos según género, edad y días de internação. Déficit cognitivo y queja de memoria fueron los más elencados en el grupo de los ancianos. La internação en la UCI y el proceso de adoecimento pueden generar implicaciones negativas al sujeto como alteraciones motoras, cognitivas y en la participación en las actividades de vida diaria. Objetivo: Mensurar a carga de trabalho da equipe de enfermagem de uma Unidade de Terapia Intensiva Neonatal (UTIN) e dimensionar o quadro de pessoal necessário para o suprimento desta demanda. Metodologia: Estudo transversal, documental e quantitativo. Foram coletadas variáveis de caracterização clínica e demográfica da amostra (n=105) de recém-nascidos e da carga de trabalho da enfermagem por meio do Nursing Activities Score (NAS). O dimensionamento foi calculado com base em equação para terapia intensiva e ajustado à Resolução nº 543/2017 do Conselho Federal de Enfermagem. Resultados: A média do NAS da UTIN foi de 749,9. Obteve-se quadro dimensionado de 43 profissionais, com déficit de 17 enfermeiros em comparação ao quadro disponível. ABSTRACT Objectives: to analyze the implementation of the medication time out strategy to reduce medication errors. Methods: this is a quantitative, cross-sectional, inferential study, with direct observation of the implementation of the medication time out strategy, carried out in a cardiac intensive care unit of a university hospital in Rio de Janeiro. Results: 234 prescriptions with 2,799 medications were observed. Of the prescriptions analyzed, 143 (61%) had at least one change with the use of the strategy. In the prescriptions altered, 290 medications had some type of change, and 104 (35.9%) changes were related to potentially harmful medication. During the application of the strategy, prescriptions with polypharmacy had 1.8 times greater chance of presenting an error (p-value = 0.031), which reinforces the importance of the strategy for prescriptions with multiple medications. Conclusions: the implementation of the medication time out strategy contributed to the interception of a high number of medication errors, using few human and material resources.

ObjectiveTo study the frequency of venous thromboembolism in hospitalised patients with COVID-19 and correlation with the D-dimers and thromboprophylaxis.DesignCross-sectional descriptive study.Place and duration of studyQueen Elizabeth Hospital, 20 April 2020–13 May 2020.Patients and methodsOne hundred and seven (n=107) patients of PCR-confirmed COVID-19 pneumonia admitted to Queen Elizabeth Hospital, Birmingham, between 20 April 2020 and 13 May 2020 were included in the study using consecutive sampling. Data were collected using the Excel audit tool and included age, gender, weight, estimated eGFR, D-dimer values on admission, intensive care unit admission, presence of respiratory failure, imaging results for evaluation of venous thromboembolism (VTE) and anticoagulation received on admission. The data were entered in the SPSS (V.17) and were analysed. Data were summarised as means±SD, number or percentage as appropriate. A p value of less than 0.05 was considered significant.ResultsThe frequency of VTE was found to be 11.2% in patients hospitalised with COVID-19 pneumonia. The mean D-dimers were 3322.24 ng/mL±9603 ng/mL with the values significantly higher for patients with VTE and those requiring intensive care unit admission. All of the seven patients (100%) with D-dimers value above 2000 ng/mL who underwent imaging were found to have VTE.ConclusionVTE is frequent in patients with COVID-19 pneumonia despite anticoagulation. A higher D-dimers value correlates well with the risk of VTE in these patients and further evaluation of such patients for VTE is necessary especially with D-dimers values above 2000 ng/mL.

Nosocomial infections in an intensive care unit (ICU) are common and associated with a high mortality but there are no published data from the Oceania region. A retrospective study in Fiji’s largest ICU (2011-12) reported that 114 of a total 663 adult ICU admissions had bacteriological culture-confirmed nosocomial infection. The commonest sites of infection were respiratory and bloodstream. Gram negative bacteria were the commonest pathogens isolated, especiallyKlebsiella pneumoniae(extended-spectrumβ-Lactamase-producing),Acinetobacter,andPseudomonasspecies. Mortality for those with a known outcome was 33%. Improved surveillance and implementation of effective preventive interventions are needed.

Latar belakang. Ventilasi non invasif (non invasive ventilation = NIV) dapat menjadi alternatif ventilasi selain intubasi endotrakeal, belum banyak didapatkan penelitian mengenai NIV di negara berkembang. Karakteristik awal pasien mempunyai peranan sebagai faktor risiko kegagalan NIV, tetapi masih didapatkan hasil yang beragam.Tujuan. Menganalisis faktor risiko kegagalan terapi NIV berdasarkan karakteristik awal pasien di Pediatric Intensive Care Unit Rumah Sakit Umum Daerah Dr. Moewardi.Metode. Penelitian analitik dengan desain cross sectional dengan besar sampel 25 anak, dilakukan pengambilan data melalui rekam medik dengan periode november 2016 sampai mei 2018.Hasil. Didapatkan hasil, sebanyak 10 dari 25 pasien (40%) gagal NIV, 22 pasien (88%) sebagai lini pertama sedangkan sebagai penyapihan ventilator sebanyak 3 pasien (12%). Analisis multivariat dengan menggunakan regresi logistik menunjukkan jalur nutrisi enteral (P value 0,028 ;OR 10,64 (1,29-87,56)CI 95%) dapat menjadi prediktor kegagalan penggunaan NIV.Kesimpulan. Pemberian Nutrisi secara enteral pada saat awal penggunaan NIV dapat dijadikan prediktor kegagalan.
