Apexification procedures are frequently performed on immature permanent teeth with incomplete root formation, open apices and necrotic pulp status with or without  periapical lesions in order to induce a calcific barrier prior to root canal therapy. The elimination and control of infection in the root canal space is critical to the success of these procedures. A healthy 21-year old male presented with pulpal necrosis, a large periapical lesion, incomplete root formation and an open apex on a maxillary right lateral incisor. Triple antibiotic paste was used to achieve antimicrobial control after traditional calcium hydroxide paste medicament failed to resolve the symptoms. Obturation was achieved using MTA and the conventional apexification technique. Excellent healing of the large periapical lesion was achieved without surgical intervention and the 4-year follow-up CBCT demonstrated complete bone fill of the lesion. Clinicians should be aware that alternative antimicrobial medicaments, such as triple antibiotic paste, may be beneficial in situations where conventional medicaments prove unsuccessful. The use of triple antibiotic paste may result in sufficient healing of the periapical lesion to justify placement of an MTA apical barrier without the need for surgical intervention

Sterilization of root canal space is foremost for the success of the endodontic treatment which is usually carried out with intracanal irrigants and medicaments. Triple antibiotic paste (metronidazole, ciprofloxacin, and minocycline) is used to achieve sterilization and healing of periradicular area. In the present case report, the triple antibiotic paste was used for non-surgical management of periapical lesion for 3 weeks. After 3 weeks, the tooth became asymptomatic that was then obturated. Hence, it is confirmed that conventional root canal treatment, along with intracanal medicaments (triple antibiotic paste), can non-surgically manage the periapical lesions and further promotes healing.

Banyak faktor yang menyebabkan kegagalan terapi endodontik antara lain pembersihan dan membentuk saluran akar yang tidak sempurna dan obturasi tidak hermetis sehingga menyebabkan kurangnya kemampuan untuk menghilangkan mikroorganisme yang ada. Saluran akar yang terinfeksi membutuhkan suatu medikamen untuk menunjang keberhasilan dalam perawatan saluran akar.Kalsium hidroksida merupakan salah satu bahan medikamen yang efektif karena memiliki sifat antibakteri dengan spektrum luas, pH tinggi, biokompatibilitas baik, mampu menetralkan endotoksin bakteri, memiliki sifat toksik yang paling rendah, serta menstimulasi pembentukan jaringan keras. Tujuan laporan kasus untuk menunjukan keberhasilan perawatan ulang saluran akar gigi insisivus lateralis kiri maksila dengan lesi periapikal menggunakan medikamen kalsium hidroksida- chlorhexidine. Pasien wanita umur 53 tahun, gigi insisivus lateralis kiri maksila dengan lesi periapikal.Radiografi tampak obturasi kurang hermetis dan radiolusen daerah periapikal. Perawatan ulang saluran akar,diikuti pemasangan pasak fiber frefabricated dan restorasi porselin fuse metal.Keseimpulan setelah evaluasi setelah enam bulan pasca perawatan ulang saluran akar, radiografi menunjukan radiolusen mengecil dan gigi dapat berfungsi dengan normal.Re-Treatment of Root Canal of Maxillary Left Lateral Incisor with Calcium Hydroxide-Chlorhexidine Medicament. There are many factors that cause failure of endodontic therapy. For instances, incomplete cleaning and shaping of root canal and inadequate obturation that results in difficulty to remove the microorganisms. Infected root canal requires a medicament for the success of the root canal treatment. Calcium hydroxide is one of the effective ingredients as medicament because it has broad spectrum antibacterial properties, high pH, good biocompatibility, and it is able to neutralize bacterial endotoxins, decrease tissue toxicity, and stimulate the formation of hard tissue. The purpose of this case report is to show the success of root canal treatment of the left maxillary lateral incisor with periapical lesions using calcium hydroxide-chlorhexidine medicaments. The patient was a woman aged 53, complaining about her left maxillary lateral incisor with periapical lesion. Based on the radiographic evaluation, there was less hermetic obturation and a radiolucent in the periapical. Root canal re-treatment was continued with fiber prefabricated post and porcelain fused to metal crown. After six months of evaluation and endodontic retreatment, it is found that there is a decrease of radiolucency periapical lesion, and her teeth are able to function normally.

Objective To establish the chronology and sequence of eruption of the permanent teeth in subjects with complete unilateral cleft lip and palate. Design Cross-sectional. Data on children presenting complete cleft lip and palate were evaluated. Setting The study was carried out at the Hospital for Rehabilitation of Craniofacial Anomalies, Bauru, Sao Paulo, Brazil. Sample The sample comprised 477 patients with complete unilateral cleft lip and palate, aged 5 to 14 years. Of these patients, 166 were girls and 311 were boys. Results The girls presented, for all maxillary and mandibular teeth, a smaller mean age of eruption than the boys. The maxillary lateral incisor and cuspid adjacent to the cleft presented significantly higher mean ages of eruption than their homologous teeth on the noncleft side.

The proper knowledge of the root canal system anatomy is crucial to perform successful endodontic treatments. Maxillary lateral incisors usually have a single root. However, aberrant canal configurations of this tooth have been reported in the literature. The clinician should be aware of the difficulties arising from this more complex root canal anatomy and should be able to manage them. A rare case of a maxillary lateral incisor with three root canals is presented in this article; the challenge of diagnosis and treatment in similar situations is also discussed.

Background and Objectives. Multiple antibacterial agents have been mixed and used as an intracanal medicament-like modified triple antibiotic paste (MTAP) to eliminate Enterococcus faecalis (EF), which has been most frequently identified in the cases of failed root canal treatment and periapical lesions. This study is aimed at using a single antibacterial agent, nitrofurantoin (Nit), as an experimental intracanal medicament paste against different clinical isolates of EF bacteria and at comparing its antimicrobial efficacy with MTAP. Materials and Methods. Three strains of EF (S1, S2, and S3) were clinically isolated. A total of 198 straight single-rooted human teeth were collected and divided randomly into three main groups: group N (Nit) (n=90), group M (MTAP) (n=90), and group W (distilled water) (n=18). The main groups were subdivided into three subgroups according to the strain of EF: in groups N and M, subgroups S1, S2, and S3 (n=30), while in group W, subgroups S1, S2, and S3 (n=6). Then, each subgroup of N and M was divided into five groups (n=6) according to the concentrations of Nit or MTAP (6.25, 12.5, 25, 50, and 100 mg/mL). The colony-forming unit (CFU) of EF from the canal lumen and dentinal chips was measured. Results. Nit could eradicate S1, S2, and S3 completely with concentrations of 6.25, 12.5, and 25 mg/mL, respectively, while MTAP showed complete eradication of the three strains only at 25 mg/mL. In all the groups, it was found that the CFU counts of EF in the dentinal chips were higher than those in the root canal lumen. Conclusion. At the concentration of 25 mg/mL, the Nit paste is effective in eradicating EF completely when it is used as an intracanal medicament.

ABSTRACT Radicular cysts are the most common cystic lesions affecting the jaws. They arise from the epithelial remnants in the periodontal ligament as a result of the stimulus from periapical infection following pulpal necrosis. Treatment options to manage large periapical lesions range from nonsurgical root canal treatment or apical surgery to extraction. New approaches involve the use of local drug delivery systems based on microparticles/nanoparticles made from biocompatible polymers along with surgical intervention. Such devices enable the introduction of antimicrobial agents or other drugs directly in the periradicular area or inside the root canal, and the prolonged release of constant concentrations of these agents for fast and better healing. The present case report illustrates the combined endodontic and surgical management of a large periapical cyst with local drug delivery as an adjunct. The case of a five-year-old child is reported, who suffered dentoalveolar injury including subluxation of the right upper lateral incisor and avulsion of the upper central incisors and left upper lateral incisor and laceration in the mucosa. Any injury to the pulp during its development lead to cessation of dentin formation and root growth. Status of the pulp and root development are the decisive factors in the treatment approach. Various treatment options include surgery with rood end sealing,  apexification using calcium hydroxide ,placement of apical plug and regenerative endodontic procedures to induce apexogenesis. This study describes the treatment of an immature permanent tooth which was treated with regenerative approach using 3% NaOCl and 17%  ethylenediaminetetraacetic acid as irrigants,and then medicated with triple antibiotic paste, after that platelet rich fibrin (PRF) and Biodentine were place in the canal. On follow up at 3 ,6,12 months healing of periapical lesion, dentinal thickening, and apical closure were evident. So, PRF can be used for the treatment of immature permanent teeth with periapical lesion, as part of a regenerative endodontic procedures. Introduction: The present study sought to assess the effect of apical foramen cleaning in the repair of periapical lesions detected by radiography. Methods: The sample comprised 980 teeth collected from 25 February 1997 to 15 March 2005 which had been subjected to endodontic treatment and exhibited radiographically visible periapical lesions. The sample was then divided into two groups: Group I, 402 root canal treatments in which cleaning of the apical foramen had not been performed, and Group II, composed of the remaining 578 root canal treatments where the procedure had been performed. After one year, the teeth were clinically and radiographically evaluated. Results: In Group I, 360 canals (89.55%) had no evidence of periapical lesions, versus 521 (90.13%) in Group II. Partial presence of these lesions was observed in 23 canals (5.72%) in Group I versus 27 (4.67%) in Group II. No changes in images were observed in 19 (4.72%) canals in Group I and 30 in Group II. Pearson’s chi-square test showed no statistically significant difference between the Groups I and II (p = 0.732). Conclusions: These findings suggest that foramen cleaning is not a determinant of periapical lesion repair.
