Cognitive impairment is the most disabling symptoms of Multiple Sclerosis (MS). Researchers have been studying factors that could predict cognitive impairment among MS patients hoping to alleviate its consequences and gather indicators of the disease progression. However, there were limited up-to-date articles that review the predictors of cognitive impairment among MS patients. In Malaysia, the topic about cognitive functioning in MS was absent. This review aims to identify and synthesize the research evidence concerning biopsychosocial variables on cognitive impairment among this population for future directions in Malaysia researches. A narrative review was performed by identifying research articles from year 2004 to 2019 through PubMed, EBSCOhost, Scopus and Google Scholar. Full-text articles meeting the selection criteria were included for qualitative synthesis. A total of 12 studies were included. Studies included in this paper found that age, course of disease and anxiety predicts cognitive functioning. Mixed results were found among variables such as gender, disability, fatigue, burden of lesion, depression and educational level. Stress was correlated with cognitive functioning but it was not a significant predictor. Although many factors were found to affect cognitive functioning among MS patients, evidence provided from this review indicates that their relationships might not be linear and direct.

Objective: The purpose of this study was to examine the relationships between cognitive functioning, whole brain magnetic transfer ratio (MTR) imaging, supratentorial 1H-magnetic resonance spectroscopy imaging (1HMRSI), and conventional T1 and T2 imaging in a homogenous sample of SPMS patients. Methods: Nineteen patients underwent a single 90-min imaging session that obtained T1-and T2-weighted images and MTR. 1HMRSI was obtained on 14 of these patients. Patients underwent a neuropsychological battery, which was used to create an integrated measure of cognitive impairment. Cognitive impairment was the dependent variable in two hierarchical multiple regression analyses in which T2 lesion load, T1 lesion load, and MTR or NAA/Cr were entered sequentially. Results: MTR was significantly related to cognitive functioning (ΔR2= 0.22, P= 0.02) after accounting for T2 lesion load (ΔR2=0.33, P= 0.01) and T1 lesion load (ΔR2= 0.00, P=0.98). NAA/Cr was not significantly related to cognitive functioning. Conclusions: Cognitive dysfunction may act as a clinical marker of normal appearing brain tissue pathology in multiple sclerosis.

Abstract Background Approximately half of all patients with multiple sclerosis (MS) experience cognitive impairment, most commonly with regard to new learning and memory. Cognitive dysfunction is a leading cause of disability in MS and it can have profound social and economic consequences for patients and their families. Objective This study was conducted to discover the early cognitive domains affected in multiple sclerosis patients concomitant with the postulated brain atrophy in an Egyptian sample of multiple sclerosis patients. Patients and Methods A cross-sectional observational case-control study conducted on seventy (60) patients who came for follow-up in Ain Shams University hospitals. 40 patients were taken as cases that followed up in MS unit in Ain-Shams university hospitals. 20 participants were taken as controls taken from the general medicine clinics age and sex matched to our patient group. An informed written consent was taken from parents of each person included in the study. Results There was a statistically significant difference between the two groups in brain volumetric changes and in the parameters of cognitive assessment Conclusion Early detection and examination of cognitive functions is important for patient evaluation, follow up and treatment regimen used.

<b><i>Introduction:</i></b> The damage in the endothelium and the neurovascular unit appears to play a key role in the pathogenesis of vascular cognitive impairment (VCI). Although there have been many advances in understanding the physiopathology of this disease, several questions remain unanswered. The association with other degenerative diseases and the heterogeneity of its clinical spectrum establish a diagnostic problem, compromising a better comprehension of the pathology and halting the development of effective treatments. The investigation of biomarkers is an important movement to the development of novel explicative models and treatment targets involved in VCI. <b><i>Methods:</i></b> We searched MEDLINE considering the original research based on VCI biomarkers in the past 20 years, following prespecified selection criteria, data extraction, and qualitative synthesis. <b><i>Results:</i></b> We reviewed 42 articles: 16 investigated plasma markers, 17 analyzed neuropathological markers, 4 studied CSF markers, 4 evaluated neuroimaging markers (ultrasound and MRI), and 1 used peripheral Doppler perfusion imaging. <b><i>Conclusions:</i></b> The biomarkers in these studies suggest an intrinsic relationship between endothelial dysfunction and VCI. Nonetheless, there is still a need for identification of a distinctive set of markers that can integrate the clinical approach of VCI, improve diagnostic accuracy, and support the discovery of alternative therapies.
