Microbiological Assessment of Root Canals Following Use of Rotary and Manual Instruments in Primary Molars

To assess the microflora of root canals in primary molars following use of rotary NiTi files and conventional hand NiTi and stainless steel files. Study design: This randomized clinical trial consisted of a total of 60 first and second primary molars requiring root canal treatment, who were selected from children aged 5-9 years. Based on type of root canal instrumentation, the teeth were randomly assigned to three groups of twenty teeth each; Group A: Rotary NiTi files, Group B: Hand NiTi files and Group C: Hand stainless steel files. Following administration of local anesthesia, isolation with rubber dam was carried out. For the purpose of instrumentation and sampling, the palatal canal of maxillary molars and the distal canal of mandibular molars were selected. Prior to sampling, the orifices of other canals in these teeth were sealed, so as to prevent any contamination. Instrumentation was carried out in each group using respective instruments along with intermittent saline irrigation. Root canal samples were obtained both before and after instrumentation, using sterile absorbent paper points and transferred to a sterile vial with transport fluid. Serial dilutions were prepared and cultured on suitable agar media. Both aerobic and anaerobic microbial counts were made. Data obtained was subjected to statistical analysis using Wilcoxon signed rank test and one-way Analysis of variance. Results: In all three groups, there was a significant reduction in both aerobic and anaerobic mean microbial count following root canal instrumentation. (p<0.001). Conclusion: Rotary NiTi files were as efficient as conventional hand instruments in significantly reducing the root canal microflora.

Introduction: Although rotary instrumentation has been widely studied in permanent dentition, it is a rather new field of study concerning primary teeth. Purpose: We aimed to evaluate apical displacement and time needed for instrumentation of root canals of primary molars by manual and rotary techniques. Materials and Methods: Root canals of 144 extracted first and second primary maxillary molars were randomly divided into 2 groups: I- manual instrumentation (K-files); II- rotary instrumentation (K3 Rotary System®). The canals were radiographed with pathfinding files in place, prepared by both techniques, and instrumentation time was recorded. After preparation, root canals were radiographed again with pathfinding files in place. To analyze the degree of apical displacement, digital images were superimposed using the Adobe Photoshop® software. Results: Mean apical displacement (0.70 mm) in the manual instrumentation group was not statistically different from that in the rotary instrumentation group (0.79 mm). However, mean time for root canal preparation was significantly shorter using the rotary system (128.0 s) than using the manual system (174.0 s) (p<0.05). Conclusions: The use of rotary instrumentation in pediatric dentistry is feasible, offering time-saving advantages in root canal preparation.

Microbes are considered as the primary etiologic agents in endodontic diseases.Disinfection of the root canal is obtained by the combined effect of biomechanicalpreparation, irrigation and intra canal medicament. The aim of the present study wasto assess the antimicrobial activity of intracanal medicaments (formocresol andEndosepton) against two micro organisms (Streptococcus mutans and staphylococcusaureus) isolated from 15 necrotic pulps of primary molars indicated for pulpectomyprocedure. The samples were cultured, and purified using microbiological evaluation.Broth dilution test was performed in our study by preparing test tubes containing10 ml of BHI broth (pH. 7) which then inoculated with strains of the tested bacteriaand incubated at 37 C° for 24 h. After over night incubaction, ten fold dilution weremade in test tubes containing 9 ml of normal saline by adding 1 ml of the inoculum tothe first tube . Then from dilution 10-1 , 0.1 ml of cell suspension was added to 9.9 mlof formocresol and endosepton, then 0.1 ml was taken and spread on duplicates ofBHI agar plates at different intervals and incubated aerobically for 24 h. at 37 C°.Colonies on the plates were counted after incubation and CFU/mL (colony formingunit) was calculated. Our results indicating that there were no significant differencesbetween the intracanal medicaments, but there were high significant differencesbetween the intervals time of the study. We concluded that both materials had greatantibacterial effect against the pathogens commonly isolated from necrotic pulpaltissue of primary teeth.

Objective: To compare the success rate of different obturation procedures in primary mandibular second molars clinically and also by digital radiovisiography. Methods: A total of 40 children aged between 4-8 years with deeply carious mandibular second primary molars indicated for single session pulpectomy were selected. Canals were obturated with Metapex. The 3 study groups (Endodontic plugger, Handheld lentulospiral, Navi Tip syringe) were compared with the control group (reamer) both clinically and radiovisiographically. The data collected were statistically analyzed using Pearson’s Chi-square and Fisher’s exact test. Results: The use of Navi tip syringe led to the least number of voids followed by Endodontic plugger and Reamer and the highest number of voids was reported with Lentulospiral. Navitip presented maximum number of optimally filled cases followed by Endodontic plugger and Lentulospiral and least number of optimally filled cases with reamer. However, there was no statistically significant difference (p>0.05) in any of the groups with clinical (pain and tenderness to percussion) and radiographic parameters (presence or absence of voids and length of obturation). Conclusion: Within the limitations of the present study, though the clinical outcome was statistically insignificant, Navitip syringe exhibited encouraging results and is a promising option for obturation in primary teeth.

Aim: The aim of this case report is to present a series of anatomical variations and endodontic treatments in four two-canal mandibular premolar teeth and three rooted three-canal maxillary second premolar teeth with root canal treatment indications identified via clinical and radiographic examinations. The success of root canal treatment is achieved with a thoroughly examined root canal morphology that has been accurately determined radiographically and clinically before adequate shaping, irrigation, and hermetic filling procedures. Root canals that are not found or not adequately disinfected can cause root canal treatment failure and complications, such as pain, swelling, or persistent fistula, also known as flare-up, after treatment. Canal variations in the teeth were detected via periapical radiographs during the root canal instrumentation stage. Methodology: The endodontic treatments of four two-canal mandibular premolar teeth and one triple-rooted three-canal maxillary second premolar with root canal treatment indications were described. Conclusion: To achieve full success in root canal treatment, anatomical variations should be examined in detail before and during treatment, and treatment should be completed with appropriate techniques.   How to cite this article: Dinger E, Bodrumlu E. The shaping and cleaning of the root canal are very important in root canal treatment. The excessive force and vibration during biomechanical preparation of the root canal may result in failure of the endodontic file. In this study, force and vibration analysis was carried out during root canal preparation. The samples of human extracted (premolar) teeth were provided by the College of Dental Science and Hospital. Endodontic instruments for reciprocating motion, such as the WaveOne Gold file system, had been used for root canal preparation. Force and vibration signals were recorded by dynamometer and accelerometer, respectively. The acquired signals were denoised using the db4 (SWT denoising 1-D) wavelet. Four levels of decomposition were carried out for each signal. The signal denoising technique was used to remove unwanted noise from the acquired signal. FESEM analysis was used to visualize the levels of severity of endodontic files during the cleaning and shaping of the root canal. In most of the cases, the failure occurred due to the improper use of the root canal instrumentation. The optimum amount of force was used to avoid the file failure and provided the proper instrumentation. The curve fitting regression model was used to find the interdependency between force and vibration.

Hypomineralized primary second molars (HPSM) are characterized by enamel opacities accompanied by hypersensitivity and atypical caries lesion, on one to four primary second molars. The correct treatment and follow-ups of those teeth have an important impact on a correct eruption of the first permanent molars and future occlusion. Hence, this report aims to describes a case of a severe HPSM in all second molars of a four-year-old girl and subsequent four-year follow-ups. The rehabilitation involved the placement of four stainless steel crowns on all four second primary molars under general anesthesia. Concerning the available literature and the case severity of HPSM, the treatment approach proposed for the case provided good functional outcome.

Introduction. Root canal instrumentation is performed to achieve cleaning and shaping with maximum preservation of its original anatomy. However, in curved canals this approach may cause excessive damage or canal transportation. The present study aimed to evaluate changes in dentin wall caused by instrumentation of curved canals using two nickel titanium rotary systems, ProTaper and Twisted File. Material and Methods. Twenty five extracted human mandibular first molars with two separated mesial root canals were selected. Distal roots were amputated and 50 canals (25 mesiobuccal and 25 mesiolingual) were randomly assigned into two groups: group 1 - ProTaper, and group 2 - Twisted File. All files were used with torque control engine, following the manufacturer instructions. The final instruments were F3 (group 1) and #30/.06 (group 2). Pre and postoperative cross sections were obtained from coronal, middle, and apical portions. Changes in dentin wall were measured and evaluated using Bramante method and digital image software. Statistical analysis was performed using the Mann Whitney U test (p<0.05). Results. The ammount of dentin removed by ProTaper in cervical and middle canal thirds was significantly greater than with Twisted File (p=0.040 and p=0.043). ProTaper eliminated significantly greater ammount of dentin than Twisted File from mesial wall in coronal sections (p=0.039). Mean differences in distance from the center to other walls were not statistically significant in the middle and apical thirds of both groups (p>0.05). Conclusion. ProTaper system produced greater changes in cross sectional area of the root canal compared to Twisted File system.

ABSTRACTObjectives: To determine the influence of passive ultrasonic irrigation (PUI) on apical extrusion of irrigating solution.Methods: Twenty freshly extracted maxillary and mandibular incisors with single straight root canals were instrumented with ProTaper rotary files. During final irrigation, the root canals were filled with 2.5% sodium hypochlorite (NaOCl) solution and PUI was performed with a piezoelectronic unit. A stainless steel instrument (size 15) was inserted into the root canal and the irrigant was ultrasonically activated for one minute. This sequence was repeated three times, resultingin a total PUI time of three minutes. The volume of extruded irrigant was measured by Pipetman pipettes. The data were statistically analyzed with the Mann-Whitney U test.Results: The mean volume of apical extruded irrigant was 2.15 �L for the PUI group and 14 �L forthe control group. The experimentalgroup extruded significantly smaller amounts of irrigant than the control group (P<.05).Conclusions: The PUI procedure as a final irrigation was associated with less apical extrusion of the irrigating solution. Introduction: Root canal preparation is an important step in root canal treatment. The use of stainless steel K-Files is time-consuming and causes fatigue to patients and operators. The large taper endodontic hand instrument can be used as an option for another preparation instrument. The purpose of this article is to determine the advantages of using large taper endodontic hand instruments as a preparation instrument compared to conventional stainless steel K-Files. Case: The 24-year-old male patient presented with pain in his lower left tooth even though it was not used. Examination of teeth 35, percussion and press (+), palpation (-), CE (-), exploration of root canals with smooth broach (+). There was a radiolucent image in the distal crown that had reached the pulp chamber, radiolucency with a blurred border of 2 mm in diameter at the periapical area, and the dilation of the periodontal ligaments along with the roots. Tooth 35 diagnosis was partial pulp necrosis with periapical lesions. Case Management: Root canal treatment of teeth 35 using hand-used ProTaper with the crown down technique up to F4 WL = 21 mm. Obturation with single cone technique with Endomethasone as a sealer. Discussion: The large taper instrument is very flexible and easily enters narrow and bent root canals, thus it can shorten the working time, reduce the risk of fatigue for patients and operators, improves the cleaning of the root canal system, and consistent root canal formation. The crown down technique used can reduce the risk of preparation errors, prevent debris extrusion and improve obturation quality. Conclusion: Root canal preparation using a large taper endodontic hand instrument can shorten the working time and reduce the risk of fatigue for patients and operators compared to preparations using conventional stainless steel K-File.
