Background. According to the statistics, the incidence of dentoalveolar anomalies, as well as the frequency of orthodontic visits, is constantly growing. Objective. To analyze the efficiency of orthodontic treatment in children during the period of mixed dentition. Methods. We have performed a study on the incidence and severity of dentoalveolar anomalies of the dental status in 7-9-year-old patients vs. 15-17-year-old adolescents after the completion of occlusion. The adolescents were divided into two groups those who underwent orthodontic treatment during the mixed dentition period and those who did not. The evaluation of the childrens dental status was performed using a specialized WHO Map (2013), supplemented by the Dental Maxillofacial Anomalies section. We determined the following indices: DMF (Decayed-Missing-Filled Index), DMF-df, OHI-S, CPI, PAR, DAI, as well as the Littles irregularity index. The need in the different types of dental and orthodontic treatment was estimated, including that per one examined person in a group. Results. In the group of 7-9-year-old we have revealed a high incidence of premature teeth loss (17.0% of patients), dentoalveolar anomalies (73.9%) and their combinations (60.8%). Discussion. Orthodontic treatment during the period of mixed dentition reduces the incidence of the teeth positioning anomalies (crowding and tooth displacement) and dental arch ratio anomalies (completely eliminating the cross-bite). At the same time, early orthodontic treatment does not have a significant influence on the tooth rotation, interdental spaces, deep, open, distal, mesial occlusions and the displacement of dental arches. The severity of dentoalveolar anomalies is reduced in this case. Conclusion. Orthodontic treatment of children during the period of mixed dentition is indicated in the presence of teeth crowding and displacement, as well as in the presence of a cross-bite; concerning other types of dentoalveolar anomalies, early orthodontic treatment is justified only when the patients psychological and functional indices are reduced.

Papillon-Lefevre syndrome (PLS) is a rare autosomal recessive disorder characterized by palmoplantar hyperkeratosis and early onset of severe destructive periodontitis causing premature loss of both deciduous and permanent dentitions at a young age. In this article two cases of patients with Papillon-Lefevre syndrome in late mixed dentition are presented. The objective of these case reports was to illustrate that under a controlled regime of periodontal treatment, orthodontic treatment is possible in patients with Papillon-Lefevre syndrome. In both cases, the deciduous dentition was lost prematurely shortly after eruption. The permanent teeth erupt without any guidance, and this can lead to loss of space, crowding, and collapse of the dental arch. The aim of the treatment was to expand the arch, create space to allow normal eruption of the permanent teeth, and stabilize the occlusion to help the patient achieve a normal facial appearance rather than the collapsed appearance caused by early extractions. OBJECTIVE: The aim of this longitudinal study, comprising young adults without orthodontic treatment, was to assess spontaneous changes in lower dental arch alignment and dimensions. METHODS: Twenty pairs of dental casts of the lower arch, obtained at different time intervals, were compared. Dental casts obtained at T1 (mean age = 20.25) and T2 (mean age = 31.2) were compared by means of paired t-test (p < 0.05). RESULTS: There was significant reduction in arch dimensions: 0.43 mm for intercanine (p = 0.0089) and intermolar (p = 0.022) widths, and 1.28 mm for diagonal arch length (p < 0.001). There was a mild increase of approximately 1 mm in the irregularity index used to assess anterior alignment (p < 0.001). However, regression analysis showed that changes in the irregularity index revealed no statistically significant association with changes in the dental arch dimensions (p > 0.05). Furthermore, incisors irregularity at T2 could not be predicted due to the severity of this variable at T1 (p = 0.5051). CONCLUSION: Findings suggest that post-growth maturation of the lower dental arch leads to a reduction of dental arch dimensions as well as to a mild, yet significant, increase in dental crowding, even in individuals without orthodontic treatment. Furthermore, dental alignment in the third decade of life cannot be predicted based on the severity of dental crowding at the end of the second decade of life.

Orthodontic treatment is thoroughly planned considering the patient’s facial and dental characteristics, the main complaint, treatment time and the orthodontist’s experience. Transposition is a form of ectopia, in which two adjacent teeth exchange positions in the dental arch. Transposition can be partial or complete. This article reports the treatment of a female patient with two kinds of tooth transposition managed in the mixed and permanent dentitions. A girl, aged eight years and three months, came to routine paediatric consultation with an ectopic permanent mandibular left lateral incisor in the mixed dentition. Radiographic analysis indicated partial transposition of the permanent mandibular left lateral incisor and canine (Md.L2.C), and development of a complete tooth transposition between the permanent maxillary right first pre-molar and canine (Mx.C.P1). The patient was treated in two phases. The first, in the mixed dentition, and the second, in the permanent dentition with a three-year follow-up between them. These challenging treatment approaches are described in detail, including the mechanics used. The key points of this treatment were early diagnosis of the ectopic mandibular lateral incisor, use of light forces and interphase patient follow-up. These determined the best time to start the second treatment phase, enabling achievement of aesthetic and functional outcomes, and the results remained stable one year after the end of orthodontic treatment.

Abstract Background Nickel-titanium (NiTi) archwires are routinely used for initial leveling and alignment of teeth in orthodontic treatment. This study aimed to clinically compare the level of pain and tooth alignment in orthodontic treatment with A-NiTi versus Cu-NiTi archwires. Methods In this parallel randomized clinical trial, 88 orthodontic patients (12–25 years) with an irregularity index > 2 mm in the anterior site of the lower dental arch who required non-extraction orthodontic treatment of the lower arch were randomized into two age- and sex-matched groups (n = 44) for treatment with A-NiTi and Cu-NiTi initial archwires. Each archwire was used for 6 weeks. After 6 weeks, the irregularity index was measured, and the level of pain was scored using the Modified McGill pain questionnaire (MPQ) and visual analog scale (VAS) according to the time of onset and duration of pain, and analgesic intake. Data were analyzed by paired t test, independent samples t test, and Chi-square test (P < 0.05). Results The irregularity index significantly decreased in both groups after 6 weeks of treatment (P < 0.001). However, the difference in this respect was not significant between the two groups (P > 0.05). Pain perception (P = 0.487), duration of pain (P = 0.546), and analgesic intake (P = 0.102) were not significantly different between the two groups either. Conclusion Both A-NiTi and Cu-NiTi archwires are equally effective for tooth alignment in the anterior site of the lower dental arch and have no significant difference with regard to the level of pain experienced by patients. <p><strong><em>Background:</em></strong><em> One of the most common inflammation disease in the oral cavity for the past few years is the aggressive form of periodontitis. Common signs of aggressive periodontitis is the mobility of teeth especially in incisive and first molars, and occuring mostly in young patient. Young patients whose losing her anterior teeth, are a real challenge for dentist and prosthodontist. Prosthodontist need to consider both functional and esthetic aspects. <strong>Objective:</strong> Rehabilitation of young adult patient with aggressive periodontitis by an interdisciplinary approach of orthodontist and prosthodontist. <strong>Case Description:</strong> A young woman, suffered from aggressive periodontitis with major complain of her teeth mobility, especially incisive and first molar in mandible. She had undergone periodontal treatment, but the result was failed. The anterior teeth in mandible need to be extracted, therefore patient wished not to be in edentulous state. As preliminary treatment, we choose immediate denture to replace the anterior mandible teeth. We faced difficulties in mandible, because her right canine weren’t in the proper dental arch. So we asked orthodontist to place fixed orthodontics in mandible, to get the canine back in the proper arch. We’ve chosen orthodontic treatment,because we didn’t want to extract the canine teeth. We evaluated in six months and after the canine back in the proper arch, we proceed to long span bridge in mandible as our definitive treatment. <strong>Conclusion:</strong> By not extracting the canine teeth, we got some advantages, especially patient psychically was happier with her own teeth. The collaboration with another dentistry field, provides us better treatment for patient. Introduction: Impacted teeth is complex anomaly of teeth eruption that requires a balanced approach not only in the differential diagnosis of its forms, but choice of rational methods of treatment. The aim: Optimization of the tactics of orthodontic-surgical treatment of patients with impacted teeth based on the development and implementation of computed tomographic indices (KT) and photometric indices (FM) of opening of surgical access (OSA) to crowns of impacted teeth Materials and methods: The results of treatment of 48 patients with delay of permanent teeth eruption have been analyzed. For an objective assessment of treatment results, a group of 24 (aged from 9 to 19 years old ) was formed. All 24 patients had typical clinical situation. Results: Orthodontic correction of patients envisaged, first of all, the elimination of obstacles in the way of teeth eruption, if necessary to provide space in dental arch and simultaneous treatment of associated bite malocclusions. Surgical exposure of impacted tooth crown was carried out and at the same time a triangular shaped guiding channel was formed, base of channel was at the impacted tooth and its angle finished into the dental arch. Precise dimensions and depth of the channel were preliminary planed on computed tomography slices with 3D reconstruction. Mean values of CT width (7,13±0,54mm), and length (6,42±0,78mm) of OSA and CT index (130,79±8,19%) of OSA to impacted teeth crowns were determined. Conclusion: To improve the quality of diagnosis and optimization of methodological approaches to treatment of patients with teeth impaction, we have proposed CT and FM OSA indices to the crowns of impacted teeth. The developed indices serve as specific reference points for optimization of diagnostic process, for reducing of probability of repeated surgical interventions and choosing the optimal path for instrumental orthodontic treatment of patients with impacted teeth

The purpose of this paper is to demonstrate 4 cases with bi-lateral ectopic mandibular canines and to demonstrate in each case which one of the two canines erupted to a normal position in the dental arch. The canines are observed from Orthopantomograms from 4 children (aged 8-13 years of age) in the mixed dentitions stages. Of these children 2 were males and 2 were females. From each child there was between 3-5 Orthopantomograms. These were observed over 3-4 years period. In each of the 4 cases with bi-lateral ectopia of the mandibular canines the one mandibular canine erupted into a normal position, while the other one declined into a horizontal position during the mixed dentition period. These 4 cases demonstrated that the inclination of the canines were important to observe and diagnose from the initial stage (first ortopantomogram). The cases presented, demonstrated that the unsuccessfully erupted canines ending in a horizontal position, were the canines which inclined slightly more that the contra lateral canine already from the initial stage.

Abstract Objective: To comparatively assess clinical failure rate of brackets cured with two different photopolymerization sources after nine months of orthodontic treatment. Materials and Methods: The sample of this study comprised 30 patients who received comprehensive orthodontic treatment by means of fixed appliances. Using the same adhesive, 600 stainless steel brackets were directly bonded and light cured for 10 seconds with the light-emitting diode (LED) lamp or for 20 seconds with the conventional halogen lamp. A split-mouth design randomly alternated from patient to patient was applied. Failure rates were recorded for nine months and analyzed with Pearson χ2 test, and log-rank test at α = .05 level of significance. Results: The overall failure rate recorded with the halogen unit (3.33%) was not significantly different from the failure rate for the LED lamp (5.00%). Significantly more failures were found in boys compared with girls, in the mandibular dental arch compared with the maxillary arch, and in posterior segments compared with anterior segments. However, no significant difference was found between the right and left segments. Conclusion: Both light-curing units showed sufficiently low bond failure rates. LED curing units are an advantageous alternative to conventional halogen sources in orthodontics because they enable a reduced chair-time bonding procedure without significantly affecting bond failure rate.
