The article highlights the pathogenesis, clinical manifestations of lesions of the central and peripheral nervous system that have arisen or persist in patients in the postcoid period (Long-COVID-19). Their correct assessment, the use of effective methods of complex treatment, targeted neurorehabilitation contribute to the reversibility of functional disorders, prevention, reduction of disability, improvement of quality of life indicators, prevention of the progression of cognitive, emotional, behavioral disorders initiated by SARS-CoV-2.

Aim. To study was to assess the dynamics of cytokine profile indicators and quality of life in patients with juvenile rheumatoid arthritis under the influence of the standard sanatorium-resort rehabilitation complex. Material and methods. At the stage of sanatorium rehabilitation, 112 patients aged from 8 to 17 years (mean age 13.2±3.7) with juvenile rheumatoid arthritis were examined. The dynamics of the main quality of life indicators were assessed using the CHAQ questionnaire (pediatric version). Changes in proinflammatory (TNF-α, IL-1, IL-6) and anti-inflammatory cytokines (IL-4, IL-10) were evaluated in correlation with quality of life indices. The control group consisted of 25 healthy equals in age. Results. In juvenile rheumatoid arthritis, the cytokine profile changes, expressed in significantly higher indices of both pro- and antiinflammatory cytokines associated with functional disorders and, as a consequence, lower quality of life indicators as compared with healthy equals in age. Under the influence of sanatorium-resort treatment, functional disorders decreased, quality of life increased, levels of TNF-α, IL-1 as well as anti-inflammatory cytokine IL-4 significantly (p<0,05) decreased. As for other both pro- and antiinflammatory cytokines, only non-significant tendency to decrease was registered. Conclusion. It has been established that before the beginning of sanatorium-resort treatment, the quality of life parameters were lower in patients with juvenile rheumatoid arthritis (JRA), while the values of both proinflammatory (TNF-α, IL-1, IL-6) and anti-inflammatory (IL-4, IL-10) cytokines were significantly (p<0,01) higher than in the control group. At the same time, changes in the studied parameters correlated with the activity of the inflammatory process and the severity of the functional insufficiency of the joints. There were noted the positive influence of the standard complex of sanatorium-resort treatment on the examined quality of life parameters and the tendency to normalization of cytokine profile indexes in patients with JRA.

Multiple sclerosis is one of the most important problems of neurology. Onset age of disease is young, and young people, which are able to work often become invalids. That is why multiple sclerosis is medical and social problem. Questions of epidemiology and quality of life of our patients with multiple sclerosis are modern questions of multiple sclerosis problem. Our scientific work is devoted to relation between indicators of quality of life and dietary factors, clinical peculiarities of disease. We received interesting results. We found, that among males most important for quality of life are physical functions and common condition of health. And emotial problems, reduction of social functions, reduction of vitality are most important for quality of life among females.

Clinical-neurophysiological bases of the combined use of Nucleo CMF forte preparation and interference therapy in the complex treatment of patients with vertebrogenic radiculopathies were studied. The beneficial effect of this therapeutic complex on the clinical course of the disease, more pronounced analgesic effect and regression of clinical manifestations, as well as improvement of quality of life indicators were proved. It has been established that the therapeutic effect of the complex application of Nucleo CMF forte and interference therapy lies in the improvement in the afferent and efferent links of the neuromotor apparatus, as well as the functional state of the spinal alpha-motoneurons associated with the acceleration of the regenerative processes. Activation of regenerative processes can also be associated with the restoration of certain morphological elements of the nervous system under the action of Nucleo CMF forte, as well as the normalization of the synthesis of complex lipids involved in cellular metabolism and synthesis of substances for the structural restoration of peripheral nerves. Carrying out neurotropic pharmacotherapy in combination with physiotherapy helps optimize therapeutic tactics in patients with vertebrogenic radiculopathies and is aimed at improving the quality of treatment, reducing the using of number of non-steroidal anti-inflammatory drugs and the frequency of relapse of the disease, and improving the quality of life of patients.

According to the results of the study in the main group of patients who received complex treatment (IPP and neuroleptic), there was a significant positive dynamics of the main clinical manifestations, compared with the control group. All patients reported a significant decrease in the intensity of heartburn and pain.The use of probulin with vegetative-corrective action in the complex treatment of patients with GERD with vegetative dystonia syndrome allowed to more effectively stop the painful manifestations and reduce the emotional experience of bodily discomfort, improve the emotional state and mental health of patients, as well as improve their quality of life.

The article presents the issues of complex treatment of osteoarthritis of the knee with the use of preformed factors in postmenopausal women compared to standard drug therapy. The study included 60 women aged 50 to 75 years with clinical manifestations of osteoarthritis of the knee joints in the postmenopausal period. Complex treatment with preformed factors has been found to be an effective treatment for this category of patients, improving the clinical course of osteoarthritis by 53% and improving quality of life.

Abstract BACKGROUND Cannabinoids, including cannabidiol (CBD) and tetrahydrocannabinol (THC), are a class of compounds found in marijuana. Numerous studies in adults have examined cannabinoid use in management of cancer-related symptoms such as nausea, anorexia, and pain. Less is known about the use in the pediatric oncology population. METHODS A prospective observational study has been ongoing since 2016 at Children’s Hospital Colorado to evaluate cannabinoids’ impact using PedsQL™ modules on quality of life of pediatric patients with central nervous system (CNS) tumors who are 2–18 years old. Laboratory assessments of T-cell activity and pharmacokinetics of CBD, THC and associated metabolites are in process. Diaries with exploratory information on cannabinoid use patterns are being collected. RESULTS Thirty-three patients (14:19; male:female) have been enrolled with a median age of 6.4 years (range, 2.9–17.7 years). The most common tumor type in enrolled patients is embryonal tumors (13/33; 39%). Nine (27%) patients have low-grade glial/glioneuronal tumors, and eight (24%) had high-grade/diffuse midline gliomas. The remaining patients had ependymoma or craniopharyngioma. The median time on cannabinoids is 9 months. Most (n=20) patients have used oral products with CBD and THC. One patient continues on cannabinoid therapy in follow up. Preliminary immune function analyses identified impaired neutrophil superoxide anion production and chemotaxis in patients taking cannabinoids at early time points on therapy. CONCLUSIONS Families of children with various CNS tumors are pursuing cannabinoid therapy for both antitumor and supportive care purposes. Analysis of the impact of cannabinoids on patients’ quality of life is ongoing.

The association of sarcopenia with poor clinical outcomes has been identified in various medical conditions, although there is a lack of quantitative analysis to validate the influence of sarcopenia on patients with lumbar degenerative spine disease (LDSD) from the available literature. Therefore, this systematic review and meta-analysis aimed to summarize the prevalence of sarcopenia in patients with LDSD and examine its impact on clinical outcomes. The electronic databases (PubMed and Embase) were systematically searched from inception through December 2020 for clinical studies investigating the association of sarcopenia with clinical outcomes in patients with LDSD. A random-effects model meta-analysis was carried out for data synthesis. This meta-analysis included 14 studies, comprising 1953 participants. The overall prevalence of sarcopenia among patients with LDSD was 24.8% (95% confidence interval [CI], 17.3%–34.3%). The relative risk of sarcopenia was not significantly increased in patients with LDSD compared with controls (risk ratio, 1.605; 95% CI, 0.321–8.022). The patients with sarcopenia did not experience an increase in low back and leg pain. However, lower quality of life (SMD, −0.627; 95% CI, −0.844–−0.410) were identified postoperatively. Sarcopenia did not lead to an elevated rate of complications after lumbar surgeries. Sarcopenia accounts for approximately one-quarter of the population with LDSD. The clinical manifestations are less influenced by sarcopenia, whereas sarcopenia is associated with poorer quality of life after lumbar surgeries. The current evidence is still insufficient to support sarcopenia as a predictor of postoperative complications.
