The article is devoted to the peculiarities of clinical manifestations and the severity of a new coronary virus infection in a university student transferred to an observatory organized in a hotel-type hostel to prevent the spread of COVID-19 among students living in hostels. The data on the epidemiological history of the patient, the results of clinical observation and examination are provided. The tactics of managing a patient with a suspected of COVID-19 on an outpatient basis, symptoms that determine the indications for hospitalization, the results of examination and treatment in a hospital, and further observation at the observatory are considered. A new coronavirus infection can lead to a rapid deterioration in the condition of young patients, which does not correlate with indicators indicating lung damage on the 5th day of the disease. Persistent hyperthermia and severe weakness with anorexia can be indicators of the complicated course of the disease, including the development of hyperactive inflammatory syndrome. Hypochromic anemia can be another disease, which is an unfavorable background for the development of COVID-19. The course of the new coronavirus infection in young patients requires careful attention and further study.

The article is devoted to the peculiarities of providing medical care for coronavirus infection in a university student who has been moved to an observatory organized in a hotel-type hostel of the university to prevent the spread of COVID-19 among students living in hostels. The article provides data on the positive experience of using dexamethasone in the treatment regimen for moderately severe disease, follow-up and examination data. The article considers the tactics of managing a patient with suspected COVID-19 in the presence of data for pneumonia according to computed tomography and clinical manifestations characteristic of a new coronavirus infection, which was later confirmed in the study of antibodies to SARS-CoV-2. The importance of round-the-clock medical monitoring of persons placed in the observatory, finding out the details of the epidemiological history from them, as well as determining the tactics of their treatment with limited hospitalization opportunities in a growing number of patients with severe manifestations of COVID-19 requiring inpatient treatment is emphasized. A new coronavirus infection can lead to a rapid deterioration of the condition in young patients, which is not always manifested by severe lung damage according to computed tomography data, but is characterized by persistent fever that cannot be stopped by antipyretics, as well as severe muscle pain, severe asthenia, which can be considered as clinical indicators of the development of hyperactive inflammatory syndrome. The use in the treatment regimen of intravenous dexamethasone at a dose of 0.1 mg / kg of body weight twice led to a rapid normalization of temperature, relief of all clinical symptoms and was not accompanied by any complications both in the early period of convalescence and 2 months after discharge from the observatory.

α1-Antitrypsin deficiency (AATD) is an inherited metabolic disorder in which mutations in the coding sequence of the SERPINA1 gene prevent secretion of α1-antitrypsin (α1-AT) and cause predisposition to pulmonary and liver diseases. The heterogeneity of clinical manifestations in AATD is related to the complexity of biological function of α1-AT. The role of smoking is crucial in the natural history of lung damage progression in severe AATD individuals, even if it also partly explains the heterogeneity in lung disease. Lung damage progression in AATD can also be related to body mass index, exacerbation rate, sex, environmental exposure and specific mutations of SERPINA1. Recent randomised controlled trials, together with previous observational work, have provided compelling evidence for the importance of early detection and intervention in order to enable patients to receive appropriate treatment and preserve functional lung tissue.

Objective: The aim of this study was to assess the knowledge and misconceptions regarding SLE in sixth-year medical students in order to verify their understanding of the general aspects of SLE and provide recommendations to improve their learning experience. Methods: We created a two-part 25-item questionnaire of 25 questions to assess students’ knowledge about epidemiology, clinical manifestations, complications, management, and prognosis of SLE, preferred ways of learning, and opinion on which specialties should include learning about the disease. The questionnaire was distributed to the participants through Google Forms. Results: A total of 200 students from King Abdulaziz University responded to the questionnaire. The prevalence of adequate knowledge among students was 39.5%. There was a significant statistical difference between students with adequate knowledge and those without adequate knowledge according to the number of cases seen during training. There was a higher prevalence of adequate knowledge among female students and those who had a family history of SLE. Most students answered correctly the basic questions about SLE, while there were weak points in the questions about epidemiology and clinical manifestations. The preferred way of learning about SLE indicated by most respondents was to see patients in the wards and analyze the pathophysiology, diagnosis, and treatment of the disease. Most students chose internal medicine and rheumatology as specialties that should include learning about SLE. Conclusion: The findings reflect the need to improve the teaching of chronic medical diseases treated on an outpatient basis and to modify the medical school curriculum so as to prepare future physicians to deal with such cases.

Stroke in young adults is a serious medical and socio-economic problem. The relevance and complexity of the problem of ischemic stroke (IS) in young patients is due to insufficient knowledge of this issue, the complexity of medical and diagnostic aspects, as well as the difference in the causes of strokes from those in older age groups. Due to the variety of clinical manifestations, IS is of big interest for cardiologists, neurologists, obstetriciansgynecologists, hematologists, rheumatologists and is also relevant for endocrinologists and nutritionists. This article examines a clinical case of a patient with IS at a young age and a history of obesity and confirmed hemophilia.

This article presents a review of the literature and data from a personal study concerning the clinical manifestations of new coronavirus infection in patients hospitalized during the COVID-19 epidemic depending on their age. It was found that most patients complained about dry or low-productive cough, moderate general weakness. The patients aged 18–30 years old more frequently experienced no cough. The patients aged 31–40 years old significantly more frequently complained about dry cough (p < 0.05). Low-productive cough was less frequent but typical in all age groups and more frequent in patients aged 51–60 years. Moist cough was more frequent in patients over 60 years. Chest pain was registered in patients over 60 years (p < 0.05). The frequency of dyspnea was found to increase with age. More often dyspnea was registered in patients older than 60 years old. No dyspnea was noted in young patients aged 18–30 years. Half of the patients aged 18–30 years had no fever. At hospitalization, the average temperature was within normal or subfebrile, which predominated in patients aged 41–50 years (p < 0.05). The frequency of febrile fever increased in patients with age. There was no significant difference in the frequency of pyretic temperature among all age groups. The patients aged over 30 years complained more about moderate weakness, and patients aged over 40 years complained more about severe weakness. Younger patients more often complained about a sore throat, while older patients had practically no symptoms (p > 0.05). Thoracic pain was significantly more frequently registered in patients older than 60 years old. Young patients aged 18–30 years and 31–40 years had no chest pain.

Annotation. Fainting (syncope) in a given period of life occurs in almost a third of the population, but only in a small number of cases it is a manifestation of a serious disease. The purpose of this observation is to determine the causes of unconscious conditions in young people without a painful history and the absence of any other clinical manifestations and pathological changes in generally accepted laboratory and instrumental examinations. Holter daily ECG monitoring is today considered one of the most important and most effective ways to diagnose heart rhythm disturbances. This article discusses two cases of syncope in young people. The first clinical case occurred in a guy 24 years old. On an outpatient basis, a survey was conducted. All indicators were within the standard values. The patient was diagnosed with Syncopal conditions of unknown origin. When examining the results of ECG monitoring, the cause of syncope was discovered and established. She turned out to be an intermittent form of AV blockade of the II degree of the II type of highly frequent. The patient was recommended to establish an ECS de mand. The second clinical case occurred in a young patient 40 years old. In this patient, with the help of monitoring, a paroxysmal form of ventricular fibrillation was diagnosed. In these two cases, which we examined in this article, only Holter ECG monitoring is of little diagnostic value for verification of syncope in young patients.

The objective: to analyze the course of the new coronavirus infection (COVID-19) in children with active respiratory tuberculosis. Subjects and methods. The article describes results of retrospective analysis of the course of the new coronavirus infection in 25 children (3-12 years old) with active respiratory tuberculosis during the outbreak of COVID-19 in an in-patient TB unit.Results. 24 (96%) persons got infected after the close exposure to the coronavirus infection, and 1 (4%) child didn't get infected. The diagnosis was verified by polymerase chain reaction (detection of RNK of SARS-CoV-2) in 33.3%, by enzyme immunoassay (detection of IgG antibodies to SARS-CoV-2 in 1 month after quarantine removal) in 100%. 58.3% of children with respiratory tuberculosis (14 people) infected with coronavirus infection had minimal respiratory symptoms, that did not differ from signs of other respiratory viral infections; clinical manifestations of the disease were completely absent in 41.7% (10) patients. Coronavirus pneumonia was diagnosed in 4 (16.7%) children who suffered from COVID-19, they had no clinical signs of lower respiratory tract disorders and no body temperature increase; 3 patients had no signs of respiratory viral infection; in all cases, the lung damage detected by computed tomography didn't exceed 10%. In 2 months after the removal of quarantine, no IgG antibodies to SARS-CoV-2 were detected in 2 out of 10 examined children (20.0%), thus there was a chance for re-infection. No mutual aggravating effect of coronavirus infection and tuberculosis was revealed in their combination over the entire observation period (4 months).

A rare episode of early neurosyphilis occurred in a 34-year-old, otherwise healthy, woman. Based on an isolated positive Borrelia burgdorferi serology (later interpreted as a cross-reaction), early ceftriaxone was initiated, in the suspect of Lyme borreliosis. Even after the diagnosis was corrected into that of a neurosyphilis, ceftriaxone administration was continued, until it achieved complete clinical and microbiological success after 24 days of treatment in a day-hospital setting, and three-weekly penicillin administrations. When considering the differential diagnosis, a luetic aetiology should not be underestimated when facing young patients with signs-symptoms of a meningoencephalitis. Our case report was characterized by an extremely low patient's age, compared with the occurrence of tertiary neurosyphilis, more than three years after the last sexual contacts. The diagnosis was confirmed by highly positive treponemal and non-treponemal serum and cerebrospinal fluid serology, and several suggestive clinical manifestations: seizures, altered mentation, cognitive impairment, lip drop, and anisochoria. These concomitant findings, together with a neuroradiological report indicating a diffuse meningoencephalitis, allowed us to confirm the diagnosis of neurosyphilis, together with a demonstrated cross-reaction of B. burgdorferi serology. Although ceftriaxone benefits from its once-daily administration (and can be easily delivered on outpatient basis), it is not the firstline treatment of neurosyphilis. However, our experience demonstrated a favourable and rapid response to ceftriaxone, in the absence of toxicity and disease sequelae.

During the pandemic of a new coronavirus infection one of the most serious complications is pneumonia. When entering a specialized pulmonology Department, the presence of pneumonia is confirmed by computed tomography (CT), the etiology is confirmed by the detection of SARS-CoV-2 RNA by PCR, the article analyzes the indicators of blood analysis when patients are admitted to the hospital. All admitted patients were divided into 2 groups. Group 1 of the study - with a new coronavirus infection Covid-19, confirmed by the detection of PCR-RNA SARS-CoV-2 (n=27). Group 2 consisted of 65 patients (70.7%) with lung tissue damage characteristic of Covid-19 according to CT, who had a negative analysis of PCR-RNA SARS-CoV-2. Statistically significant deviations were shown in a decrease in the percentage and absolute content of lymphocytes, monocytes and platelets, and an increase in neutrophils. The comparative analysis in the groups with positive and negative PCR analysis for the detection of SARS-CoV-2 RNA showed no statistically significant differences. It can be assumed that with the similarity of clinical manifestations and CT-data, a negative result of PCR-RNA SARS-CoV-2 can be diagnosed as «lung damage caused by COVID-19 infection».

The article presents data on the epidemiology, pathogenesis, features of clinical manifestations and diagnosis of multisystem inflammatory syndrome in children (MIS-C) associated with a new coronavirus infection. Objective of the research: to identify clinical manifestations and results of laboratory and instrumental examination of pediatric patients with confirmed MIS-C caused by a new coronavirus infection, the most characteristic features of this pathology and to analyze the amount of therapy based on the assessment of epidemiological data. Results: median age – 8 years (6; 9,5), median duration of the disease at the time of hospitalization – 6 days (3; 6). A positive result of SARS-CoV-2 analysis in rhinopharyngeal swabs by PCR was in 3 patients, antibodies to SARS-CoV-2 by ELISA were detected in 10. Clinical manifestations included prolonged febrile fever in 11 patients, gastrointestinal symptoms of varying severity – in 10, polymorphic rash was noted in 7 cases, conjunctivitis/scleritis – in 7, edema of the eyelids, face, extremities – in 4. CNS lesion manifested itself with general cerebral and meningeal symptoms. All patients had an increase in the level of inflammation laboratory markers. In 2 children chest MSCT revealed characteristic changes for SARS-CoV-2, interpreted as CT 3–4. Therapy included intravenous immunoglobulin infusions, systemic glucocorticoids, antiviral and antibacterial drugs, anticoagulants; two patients received tocilizumab infusions.
