Effects of Continuous Nursing Intervention on Exercise Tolerance and Rehospitalization Rate in Patients with Chronic Heart Failure

Objective To observe the effect of continuous nursing intervention on exercise tolerance and rehospitalization rate in patients with chronic heart failure. Methods 134 patients with chronic heart failure admitted to our hospital were divided into two groups, routine nursing intervention group (control group) and continuous nursing intervention group (observation group), with 67 cases in each group. The resting and peak heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the two groups were recorded. The change of 6min walking distance, modified European Heart Failure Self-Care Behavior Scale (EHFSCB-9) and quality of life (SF-36) of the two groups were compared before and after intervention, and moreover, rehospitalization rate of heart failure of two groups 6 months after discharge was compared between the two groups. Results: Before intervention, there was no significant difference between the two groups (P>0.05). After intervention, there was no significant difference in resting and peak HR, SBP and DBP between the two groups and those before intervention (P>0.05). Further comparison between the two groups showed that there was no significant difference in resting and peak HR, SBP and DBP between the observation group and the control group (P>0.05). After intervention, 6min walking distance and SF-36 scale scores (role physical, physiological function, physical pain, energy, health status, social function, mental health and emotional function) were increased in the two groups (P<0.05). Further comparison between the two groups showed that 6min walking distance and SF-36 scale scores (except somatic pain score and role physical score) in the observation group were higher than those in the control group (P<0.05), and the EHFSCB-9 scores in the two groups decreased gradually after intervention (P<0.05). Further comparison between the two groups showed that the EHFSCB-9 scores in the observation group (except low salt diet score and taking medicine based on doctor's advice score) were lower than those in the control group (P<0.05). The rehospitalization rate of heart failure within 6 months after discharge was 11.91% in the observation group, significantly lower than 25.37% in the control group, and the difference was significant (P<0.05). Conclusion: Continuous nursing intervention can strengthen the self-care ability of patients with chronic heart failure, improve exercise tolerance and quality of life, and reduce the rehospitalization rate to heart failure within 6 months.

Background: We aimed to explore the effect of health education and nursing intervention model based on behavioral change theories on self-efficacy and self-management behaviors in patients with chronic heart failure. Methods: Eighty-six patients with chronic heart failure treated in the First Hospital Affiliated to Harbin Medical University (Harbin, China) from November 2016 to January 2018 were enrolled in this study. The patients were evenly divided into control group and observation group (43 patients in each group), according to the random number table method. Patients in the control group received a routine health education intervention. Patients in the observation group received health education based on behavioral change theories in addition to the routine nursing intervention. The health education included mastering of the disease-related theoretical knowledge, daily self-management and exercising guidance. After 8 weeks of intervention, the self-efficacy level, self-management ability and quality of life were assessed and compared between the two groups. Results: The self-management behavior score in the observation group was higher than those in the control group (P=0.002). The chronic disease general self-efficacy (GSE) score in the observation group was higher than those in the control group (P=0.002). The quality of life (MHL) score was in the observation group was higher than those in the control group. The patient satisfaction score in the observation group was higher than those in the control group (P=0.021). Conclusions: Health education based on behavioral change theories can relieve symptoms of chronic heart failure, and improve patients’ self-efficacy level, self-management ability and patient satisfaction.

BACKGROUND: Cardiac failure is one of the most common chronic diseases with high rate of morbidity and mortality. Fatigue and decreased ability to perform daily activities are of the most common complications of this disease. The purpose of this study was to determine the effect of an educational plan based on Roy adaptation model on fatigue and daily activities in patients with heart failure.METHODS: This experimental study was performed on 60 heart failure patients admitted in two educational hospitals supervised by the Kerman University of Medical Sciences in 2019. Sample was randomly assigned into two intervention and control groups. The intervention group received the care plan through a face-toface and group training program in 6 sessions at one month. Interval followed by a follow-up period for 4 weeks later. Fatigue level was calculated based on piper fatigue scale, and daily activities were calculated by Barthes scale.RESULTS: The finding indicated that there was a significant difference between the two groups (control and intervention) after the intervention. The result showed that the intervention group had significantly lower mean scores in fatigue and higher mean scores in daily activities compared to the control group after intervention P ≤ .05. There was a significant relationship between fatigue and daily activities of life with frequency of hospitalization and duration of disease in both groups, P ≤ .05.CONCLUSION: Implementation of Roy model-based education program as a low-cost, effective, and non-aggressive nursing intervention can reduce fatigue, and improve daily activities in patients with heart failure.

Objective: to evaluate vasopressin (VP) concentration in patients with varying severity of chronic heart failure (CHF), intensity of clinical symptoms, and decreased level of left ventricular ejection fraction (LVEF). Materials and methods. In total, 120 patients (44 males, 76 females) with CHF of varying genesis (mean age 72.12 ± 10.18 years) and 30 clinically healthy individuals (18 males, 12 females) as a control group (mean age 33.4 ± 6.23 years) were examined. All patients underwent comprehensive clinical and instrumental examination in accordance with the standards for patients with CHF. The VP level was determined using ELISA. Statistical analysis was performed using the IBM SPSS Statistics v. 23 software.Results. The patients with CHF had significantly higher blood VP levels compared to the control group (72.91 ± 53.9 pg/ml versus 6.6 ± 3.2 pg/ml respectively; p <0.01). At the same time, patients with stage III CHF had significantly lower VP levels than patients with stages IIВ and IIА (35.61 ± 21.53 pg/ml versus 71.67 ± 48.31 pg/ml and 86.73 ± 59.78 pg/ml respectively; p<0.01). A similar picture was observed for the functional classes (FC). For instance, for CHF FC II and III, the VP level was 91.93 ± 67.13 pg/ml and 77.95 ± 54.01 pg/ml respectively, while for FC IV it decreased to 50.49 ± 28.18 pg/ml (p <0.01). The VP concentration in patients who subsequently perished was significantly lower than in patients who survived (48.79 ± 26.30 pg/ml versus 79.72 ± 57.73 pg/ml; p = 0.012). Moreover, in patients with LVEF <50 %, the VP level was significantly lower than in patients with LVEF >50 % (59.43 ± 42.51 pg/ml versus 86.43 ± 62.46 pg/ml respectively; p <0.05).Conclusion. The observed significant differences in VP in patients with stage III and IV CFH can indicate depletion of neurohumoral mediators in this patient category. However, a correlation between the VP level and the level of LVEF decrease can indicate a significant difference in the role of VP in CHF pathogenesis in patients with preserved and decreased LVEF. This observation requires further research.

The heart failure disease incident rate is high in the world and Indonesia. Heart failure patients suffer from poor sleep quality. This affects the disease recovery process and increases the mortality and morbidity rates. Patients need an intervention to overcome the issue and the role of a nurse is highly needed to overcome it. The purpose of this research was to identify the effects of nursing intervention of therapy of right lateral position on the sleep quality of heart failure patients. The method used was the Quasi Experiment with the pre and post test control group consisting of 15 respondents of intervention group and 14 respondents of control group treated at RSUP M Djamil Padang, determined according to the inclusion criteria. The results of research indicated that there was a significant difference in the sleep quality of right lateral position group and the control group with the value (p = value = 0,001). The statistics test used was the independent T test. Sleep is the necessity of human beings. By giving the intervention of therapy of right lateral position, the issue of poor sleep quality in heart failure patients may be resolved. Health care service, particularly nurses, is advised to apply this therapy as an independent nursing intervention to increase the sleep quality of heart failure patients treated at the hospital.

Objective. To explore the influence of systematic nursing intervention on the life ability and quality of senile dementia patients. Methods. Total of 82 senile dementia patients who were admitted to our hospital from January 2018 to January 2020 were divided into two groups according to the random number table, and the nursing intervention was analyzed. 41 patients in the control group were given routine nursing care, and 41 patients in the observation group were given systematic nursing intervention. Patients were assessed cognitively using the Montreal Cognitive Assessment Scale (MoCA). The Barthel index and SF-36 were used to evaluate the patients’ daily activity function and quality of life. A Symptom Checklist-90-Revised (SCL-90-R) was used to assess mental distress. The Social-Adaptive Function Rating Scale (SAFE) and Social Skills Inventory (SSC) were used to evaluate the patients’ social interaction ability before and after nursing intervention. Nursing satisfaction was distributed to patients in the form of a self-prepared nursing satisfaction questionnaire for scoring. Results. After the nursing intervention, the MoCA scores of patients in the two groups were higher than those before the nursing intervention, and the scores in the observation group were higher than those in the control group P < 0.05 . After the nursing intervention, the Barthel index scores of patients in the two groups were higher than those before the nursing intervention, and the scores in the observation group were higher than those in the control group P < 0.05 . After the nursing intervention, the SF-36 scores of patients in the two groups were higher than those before the nursing intervention, and the scores in the observation group were higher than those in the control group P < 0.05 . After nursing intervention, the SCL-90-R scores of patients in the two groups were lower than those before nursing intervention, and the scores in the observation group were lower than those in the control group P < 0.05 . After the nursing intervention, the SAFE scores of patients in the two groups were lower than those before the nursing intervention, and the safe scores of the observation group were lower than those of the control group P < 0.05 . After the nursing intervention, the SSC scores of patients in the two groups were lower than those before the nursing intervention, and the scores in the observation group were lower than those in the control group P < 0.05 . After nursing intervention, the total satisfaction degree of the control group (80.49%) was lower than that of the observation group (97.56%) P < 0.05 . Conclusion. The implementation of systematic nursing intervention is conducive to improve the cognitive function, activity of life, and quality of life of senile dementia patients who have a positive effect, and nursing satisfaction is higher.

To explore and analyze the impact of Emergency Intensive Care Unit (EICU) critical care patients using eCASH nursing mode intervention on patients’ cognitive function, psychological function, physical function and life self-care ability. Randomly selected 142 critically ill patients in our hospital’s EICU ward from January 2018 to October 2019 as study subjects, and grouped them according to the time of admission. 71 patients in the observation group were intervened by the eCASH nursing model, and 71 patients in the control group received regular nursing intervention. Recording the cognitive, psychological, physical and other functions and self-care ability of the two groups of patients. Before nursing, the Bl score of critical patients in the observation group was compared with the control group (P> 0.05). After the intervention of the observation group patients accepted eCASH nursing model, the Bl score was obviously higher than the control group (P <0.05). According to the SCL-90 table scores, there is no significant difference in the scores of each dimension between the two groups before nursing (P> 0.05). Compare before and after nursing and score except for obsessive compulsive disorder and psychosis (P> 0.05). According to the SCL-90 scale score, there is no significant difference in the scores of each dimension between the two groups before nursing (P <0.05). According to the comparison of the two groups of data after nursing, somatization, anxiety, depression, fear, hostility, paranoia and SCL-90 total score in the observation group were significantly lower than that of control group (P <0.05). Before nursing, the cognitive level of the two groups was compared (P> 0.05). After nursing, the HDS score of the observation group was higher than that of the control group (P <0.05). The CM-PPT score was compared, and the data of the two groups before nursing was compared (P> 0.05). The score of observation group after receiving eCASH nursing was higher than that of control group (P <0.05). The study confirmed that eCASH nursing model for EICU critical patients as early as possible has a significant effect on improving patients ’cognitive function, mental state, physical function, etc. At the same time, it can improve patients’ self-care ability in life and is worthy of promotion.

<p><strong>Objective</strong><strong>:</strong> To analyze the application value of comfort nursing intervention in senile chronic heart failure nursing. <strong>Methods:</strong> The 120 heart failures discharged from our hospital in 2018 and 2019 were selected. The changes in nursing satisfaction, specialty nursing quality indicators and the incidence of pressure ulcers in hospitalized patients were compared and analyzed between the two groups. <strong>Results:</strong> The average satisfaction degree of the two groups was 97.48% and 97.22% respectively, and the difference between the groups was statistically significant (P&lt;0.05). The correct rates of acute left heart failure in the two groups were 99.56% and 98.13%, and the difference between the groups was statistically significant (P&lt;0.05), and two groups of patients with cardiac Ⅲ class above intake control accuracy was 99.95% and 99.99%, and the difference between groups was statistically significant (P&gt;0.05), and the incidence of pressure ulcer was 0.18‰ and 0, and there was no significant difference between the two groups. <strong>Conclusion:</strong> Comfort nursing intervention has achieved good nursing satisfaction and clinical effect in elderly patients with heart failure.</p>

ABSTRACTObjetive: Determine the effect of teaching palliative care on the self-efficacy of elderly with Chronic Heart Failure. Material and Methods: This single-blind randomized clinical trial was done on patients with Chronic Heart Failure attending to the heart clinics of Golestan University of Medical Sciences in 2018. 48 eligible patients with class-III were selected via convenience sampling. Patients allocated into control and intervention group randomly (n=32). Palliative care training was done for intervention group for six weeks. Data was gathered through the Cardiac Self-Efficacy Scale (CSES) on three time points (before, immediately, and one month after intervention). Data were analyzed in SPSS-18 using independent t-test, chi-squared, Fisher's exact, and repeated-measures ANOVA with corrected Bonferroni post-hoc test. Results: Finding showed that before intervention the mean scores of self-efficacy in intervention and control group was 23.95±8.34 and 24.11±9.41, respectively. Immediate and one month after intervention it was 26.30±8.30 vs. 28.95±8.53 in intervention group and 24.68±9.32 vs. 25.53±10.39 in control group. In intervention group there was no significant difference between the mean scores of before with immediate and one month after intervention. However, there was a significant difference between the two time follow up in both intervention (p<0.0001) and control (p<0.003) groups. Conclusion: Although the education of palliative care has increased the ability to care in elderly, it seems that self-managing in complex conditions is required follow-up over time.

Objective. The role of deep learning-based echocardiography in the diagnosis and evaluation of the effects of routine anti-heart-failure Western medicines was investigated in elderly patients with acute left heart failure (ALHF). Methods. A total of 80 elderly patients with ALHF admitted to Affiliated Hangzhou First People’s Hospital from August 2017 to February 2019 were selected as the research objects, and they were divided randomly into a control group and an observation group, with 40 cases in each group. Then, a deep convolutional neural network (DCNN) algorithm model was established, and image preprocessing was carried out. The binarized threshold segmentation was used for denoising, and the image was for illumination processing to balance the overall brightness of the image and increase the usable data of the model, so as to reduce the interference of subsequent feature extraction. Finally, the detailed module of deep convolutional layer network algorithm was realized. Besides, the patients from the control group were given routine echocardiography, and the observation group underwent echocardiography based on deep learning algorithm. Moreover, the hospitalization status of patients from the two groups was observed and recorded, including mortality rate, rehospitalization rate, average length of hospitalization, and hospitalization expenses. The diagnostic accuracy of the two examination methods was compared, and the electrocardiogram (ECG) and echocardiographic parameters as well as patients’ quality of life were recorded in both groups at the basic state and 5 months after drug treatment. Results. After comparison, the rehospitalization rate and mortality rate of the observation group were lower than the rates of the control group, but the diagnostic accuracy was higher than that of the control group. However, the difference between the two groups of patients was not statistically marked ( P > 0.05 ). The length and expenses of hospitalization of the observation group were both less than those of the control group. The specificity, sensitivity, and accuracy of the examination methods in the observation group were higher than those of the control group, and the differences were statistically marked ( P < 0.05 ). There was a statistically great difference between the interventricular delay (IVD) of the echocardiographic parameters of patients from the two groups at the basic state and the left ventricular electromechanical delay (LVEMD) parameter values after 5 months of treatment ( P < 0.05 ), but there was no significant difference in the other parameters. After treatment, the quality of life of patients from the two groups was improved, while the observation group was more marked than the control group ( P < 0.05 ). Conclusion. Echocardiography based on deep learning algorithm had high diagnostic accuracy and could reduce the possibility of cardiovascular events in patients with heart failure, so as to decrease the mortality rate and diagnosis and treatment costs. Moreover, it had an obvious diagnostic effect, which was conducive to the timely detection and treatment of clinical diseases.
