Using a 980 nm, 7w/cm2 Diode Laser in Oral Surgeries

At this paper, we discussed the use of laser in oral surgeries. The aim of this discussion and study were to verify the reliability and efficacy of one of these compact portable diode instruments, emitting a maximum power density of 7W/cm2 not requiring pre-warming or controlling, and delivering a wavelength of 980nm. In addition to that, we compared between the healing time of some cases were treated by diode laser. Recently, laser is one of the most common surgical procedures in the field of oral surgery, implant dentistry, endodontic treatment as well. The role of laser surgery in the oral cavity is well established. The use of diode laser removing a haemangioma as a special case is currently under investigation. The benefits of oral laser surgery are many benefits, i.e., a relatively bloodless surgical and post-surgical course, minimal swelling and scarring and reduction of post-surgical pain are discussed. An interesting cases of removal; a haemangioma, fibroma and performin a frenectomy with a 980nm diode laser are presented.

Reduction of the wound healing time after surgical procedures along with good hemostasis, and the reduction of post-surgical edema, pain and infective complications are generally desirable to both clinicians and patients. Recently, a gel compound containing sodium hyaluronate and four synthetic aminoacids (glycine, leucine, proline, lysine) and marketed as Aminogam® (Errekappa Euroterapici, Italy), has been proposed as a medical device promoting faster wound healing after oral surgery procedures. To assess its achievable clinical benefits, we studied retrospectively 580 cases (290 study cases and 290 control cases) undergoing oral surgery and receiving Aminogam ® gel application. More precisely, cases were divided into 7 groups on the bases of the kind of surgery (teeth extraction, oral surgery in patients taking bisphosphonates, surgical treatment of jaw osteonecrosis related to bisphosphonates therapy, placement of endosseous implants, diode laser surgery of oral mucosa lesions with second intention healing without stitches, diode laser photocoagulation of slow flow vascular malformations and bone surgery). In all instances, Aminogam® gel was applied at least five times a day until the wound healed completely. We compared the elapsed time between surgery and complete healing with Aminogam® application compared to control cases receiving no other drug treatment. Our results confirmed that the overall time of healing is certainly reduced in cases receiving Aminogam® gel regardless of the kind of oral surgery.

Цель. Изучение клеток иммунной системы полости рта у пациентов до проведения операции лифтинга лица, а также сопоставление этих данных с показателями, выявленными при осложнениях в послеоперационном периоде. Методика. Исследованы клетки иммунной системы полости рта 100 женщин (23-68 лет), которым перед операцией лифтинга лица проведено стоматологическое обследование и необходимое лечение при наличии кариеса дентина, клиновидного дефекта эмали зуба, хронического периодонтита или пульпита. Выделенные путем смывов десенной борозды клетки окрашивали моноклональными антителами и анализировали на проточном цитометре. Определяли уровень спонтанного и индуцированного апоптоза. Оценивали уровень фагоцитоза и генерацию супероксидного аниона нейтрофилами. Результаты. Показано, что хронические воспалительные заболевания зубов оказывают негативное влияние на состояние иммунных клеток ротовой полости, сопровождаются повышением генерации супероксидного аниона нейтрофилами, повышением уровня спонтанного и церамид-индуцированного апоптоза клеток десенной борозды. Заключение. Наличие хронической патологии зубов, даже после санации, оказывает негативное влияние на течение послеоперационного периода при проведении операции лифтинга лица, в частности, способствует увеличению времени заживления операционной раны, инфицированию раны с появлением очагов некроза в области операционного шва. The object. The study of the cells of the immune system of the oral cavity in patients before the operation of face lifting, as well as a comparison of these indicators with complications in the postoperative period. Methods. The immune cells from the oral cavity were studied in 100 women (23-68 years), who underwent a dental examination and necessary treatment if they had the dentin caries, wedge-shaped defect of tooth enamel, chronic periodontitis or pulpitis before the facial lifting operation. The immune cells have been isolated by a lavage of gingival sulcus around the damaged tooth. Results. It has been shown that chronic dental diseases made a negative contribution to the oral cavity immune cells. It has been accompanied by the elevated levels of superoxide originating from neutrophils as well as the increased levels of spontaneous and ceramide-induced apoptosis of immune cells isolated from the gingival sulcus. Conclusion. The presence of chronic pathology of teeth even in the case of the preoperative dental sanation has a negative impact on the postoperative period after face lifting, in particular, contributes to the lengthening of the surgical wound healing time, wound infection as well as the partial necrosis of suture.

The analysis of the appealability for the rehabilitation of the oral cavity of staff subject to mandatory periodic examinations at the dentist. The growth of indicators characterizing the dental activity of employees was noted. Subject to the principle of continuity, annual mandatory periodic examinations can be one of the factors in maintaining the dental health of employees.

Relevance. The relevance of the problem is in the high prevalence of the herpes simplex virus. According to WHO, nearly 90% of the population are carriers. Relapses can occur several times a year, which leads to disability and impairs the quality of life. Currently, there are a large number of drugs for general and local treatment. However, they are toxic and can cause allergic reactions. In recent years, a large number of studies have been devoted to the use of lasers in the treatment of dental diseases. Therefore, the question of the use of a laser in the treatment of herpetic lesions of the oral cavity remains relevant.Purpose. The research purpose was to determine the effectiveness of the use of a diode laser in the treatment of herpetic lesions of the lips.Materials and methods. We conducted a comparative assessment of the effectiveness of the use of a diode laser in monotherapy of herpetic lip lesions in 18 patients. The comparison group consisted of 18 patients, in the local treatment of which antiviral ointments were used.Results. The results of the study showed that the use of a diode laser can reduce treatment time, accelerate the epithelization of foci.Conclusion. The results of our study allow us to recommend the use of a diode laser in the complex treatment of herpetic lesions of the oral cavity.

Background: It is known that hormonal imbalances during pregnancy make women more susceptible to dental problems. High levels of progesterone and estrogen during pregnancy, lead to an increased inflammatory response to dental plaque thus causing predisposing to gum diseases such as gingivitis. If untreated, gingivitis leads to chronic periodontitis which may manifest systemically in form of cardiovascular, endocrine or even respiratory disorders. Also, hyperacidity in the oral cavity due to gastric reflux and vomiting leads to decreased pH thus damaging the tooth enamel making the oral cavity more prone to tooth decay and tooth loss. Studies also show that periodontal disease can also lead to adverse pregnancy outcomes such as pre-term and low birth weight babies. Objectives: We sought to understand the role of oral health in pregnancy. Methods: We identified major articles of interest in the field of oral health in pregnancy and drafted a mini-symposium based on relevant information. Conclusion: Regular dental visits and cognizant efforts to sustain a healthy oral environment can help women in the prevention and treatment of dental issues during pregnancy. The paper highlights the common oral manifestations during pregnancy and their local and systemic impact on the body during pregnancy. Furthermore, it also emphasizes the importance of good oral health practices to counteract the oral complications and the significance of oral health awareness in pregnant women.

Resumo O osso é um tecido conjuntivo especializado, vascularizado e dinâmico que se modifica ao longo do organismo. Quando lesado, possui uma capacidade única de regeneração e reparação sem a presença de cicatrizes, mas em algumas situações devido tamanho do defeito ósseo não se regenera por completo. Assim, se faz necessária a realização de procedimentos de regeneração óssea guiada. Para isso, o implantodontista deve conhecer as bases biológicas da regeneração óssea guiada alveolar e suas indicações. Nesta revisão foram abordadas indicações, vantagens e tipos de biomateriais utilizados para preenchimento do alvéolo dentário imediatamente após a extração dentária sempre que o objetivo for à reabilitação através da instalação de implantes. Mesmo este, biomateriais, apresentando inúmeras qualidades, estudos ainda devem ser feitos a fim de obter a cada dia, um material sintético compatível com o tecido ósseo perdido em quantidades adequadas sem necessitar de cirurgias extra-bucais.ABSTRACT Bone is a specialized vascularized connective tissue that dynamic changes throughout the body. When injured, it has a unique ability to regenerate and repair without the presence of scars, but in some situations due to size of the bone defect does not regenerate completely. Thus, it is necessary to perform guided bone regeneration procedures. For this, the implant dentistry must know the biological bases of alveolar guided bone regeneration and its indications. In this review were addressed indications, advantages and types of biomaterials used for filling the tooth socket immediately after the tooth extraction whenever the goal is rehabilitation through implants installation. Even this, biomaterials, having several qualities, further studies must be done to obtain each day, a synthetic material compatible with the bone tissue lost in proper amounts without the need of extra-oral surgery.

It is well-known that microbiota dysbiosis is closely associated with numerous diseases in the human body. The oral cavity and gut are the two largest microbial habitats, playing a major role in microbiome-associated diseases. Even though the oral cavity and gut are continuous regions connected through the gastrointestinal tract, the oral and gut microbiome profiles are well-segregated due to the oral–gut barrier. However, the oral microbiota can translocate to the intestinal mucosa in conditions of the oral–gut barrier dysfunction. Inversely, the gut-to-oral microbial transmission occurs as well in inter- and intrapersonal manners. Recently, it has been reported that oral and gut microbiomes interdependently regulate physiological functions and pathological processes. Oral-to-gut and gut-to-oral microbial transmissions can shape and/or reshape the microbial ecosystem in both habitats, eventually modulating pathogenesis of disease. However, the oral–gut microbial interaction in pathogenesis has been underappreciated to date. Here, we will highlight the oral–gut microbiome crosstalk and its implications in the pathogenesis of the gastrointestinal disease and cancer. Better understanding the role of the oral–gut microbiome axis in pathogenesis will be advantageous for precise diagnosis/prognosis and effective treatment.
