Background: Stress is very common in medical professionals. Stress begins in the first year of medical school and increases with subsequent years of medical life. Stress decreases overall performance and had a multitude of health-related adverse effect. Yoga has been tried as a stress reduction technique in different populations. In present study yoga was performed in the 1st year MBBS students and impact on stress reduction was studied using PSS-10 stress scale.Methods: Study groups, yoga and control contained 26 and 27 subjects respectively. The yoga group practiced selected yogic asana, pranayama, and yoga nidra 1hour daily 6days a week for 3months. Control group kept in touch and allowed their usual activity as before. The PSS-10 scale used to measure the level of stress in both groups pre and post study.Results: There was a highly significant reduction in the PSS-10 Score (stress level) in the yoga group (P Value <0.0001) but there was no significant change in the PSS-10 Score of control group (P Value = 0.2930).Conclusions: Yoga is an effective modality of stress reduction technique in 1st year medical students. Therefore, yoga should be introduced as a part of the curricula in the first year of medical school. This may be taken as the 1st step in implantation of healthy lifestyle in future health care providers.

Background: Medicine is a profession with high-stress prevalence. There are many stress markers but cortisol is one of the commonly used stress marker. Stress begins in the first year of medical carrier and increases with subsequent years of medical education. There is a decrease in overall academic performance and many health-related adverse effects due to stress. In this study, yoga was performed in the first year MBBS students of BRD Medical College, Gorakhpur and the impact of yoga in stress reduction was studied using serum cortisol as stress a marker.Methods: Study groups, yoga and control contained 26 and 27 subjects including male and female MBBS students. Yoga group practiced selected yogic asana, pranayama, and yoga nidra for 3 months. The control group as a stress marker had been kept in touch and allowed to go on their usual activity as before. Morning (8.00 AM to 9.00 AM) serum cortisol level was used as a stress marker in both group, pre and post-study.Results: There was a significant reduction in morning serum cortisol level (stress level) in yoga group (p-value = 0.0271) but there was no significant change in morning serum cortisol level of the control group (p-value = 0.8573).Conclusions: Yoga is an effective stress reduction technique for medical students. Yoga classes should be introduced in the first year of the medical carrier under the supervision of expert physiologists. This may lead to the implantation of a healthy lifestyle in our future healthcare providers. Yogic practice by health care providers may have long term positive impacts on our healthcare system.

Mercury is considered to be a powerful neurotoxin which leads to mercury poisoning. Mercury can harm us in many ways. Mercury vapour can harm the nervous system in newborn babies and can further lead to cough, tremor and irritability. Being a healthcare provider, it is necessary to know about mercury spill management. To assess the awareness and knowledge of mercury spill management among healthcare providers, an online based survey was created using google forms. The students were asked to answer 10 questions based on mercury spill management and its usefulness. 100 students actively participated in the survey. Correlation analysis was done by chi square test using SPSS software. The results were analysed by Spss software. From the correlation, only 18% of the students in first year, 6% of the students in second year, 2% of the students in third year, 4% of the students in fourth year and 5% of the students in final year were aware of the side effects of mercury with the p value of 0.879 which is statistically insignificant . Similarly, 26% of the first year students, 7% of the second year students, 4% of the third year students, 8% of the fourth year students and 7% of the final year students were aware that hypochlorite solution is used to wipe the mercury spill with p value of 0.323 which is statistically insignificant. On analysing the data it was found that healthcare providers had only little knowledge on mercury spill management. Many healthcare providers were aware of the side effects of mercury and some students were aware of its management. This survey in the current scenario is of utmost importance to reduce exposure of mercury among healthcare providers.

Background: tress at any stage of life virtually produces several respiratory limitations and reduces tolerance to physical efforts, sometimes leading to pulmonary diseases in individuals. Various yoga and Pranayama like Kapalbhati and Nadi-sodhan involves powerful strokes of exhalation, which trains the subject to make full use of diaphragm and abdominal muscles.Methods: This study is performed on 1st yr. medical students whose stress level is in higher side due to academic burden. 55 medical students were selected as participants through counseling and were divided into Yoga group (n= 27) and control group (n=28). Pulmonary functions of subjects were tested using Pony FX advanced desktop spirometer manufactured by Cosmed. Yoga is performed 1 hr/day for 6 days /week for 12 weeks by yoga group.Results: Pulmonary functions tested in Yoga and control group were FVC, FEV1, PEF and FVC/FEV1. There were significant improvements in pulmonary functions in yoga group. In Yoga group (FVC- 5.8% increases, FEV1- 5.2% increase, PEF- 34% increase and FVC/FEV1- 3.15% increase). In control group also, all parameters increased but P value was not significant.Conclusions: This study concludes that practicing Yoga has shown a significant improvement on pulmonary functions in 1st year medical students.

Background: Dehydroepiandrosterone sulfate (DHEAS), secreted by adrenal cortex, acts in the human body as a neurosteroid, cardio-protective, anti-diabetic, anti-obesity and immune-enhancing agent. It is also reported as a youth hormone. But due to various stimulators of stress, and also as an antagonist of cortisol, DHEAS level decreases. Yoga and meditation regulates the level of hormones and neurotransmitters that affect physiological function.Methods: This study was performed on 1st yr. medical students whose DHEAS level was low due to acute stress. 55 medical students were selected as participants through counseling and were divided into Yoga group (n= 27) and control group (n=28). Their morning serum DHEAS level was assessed and yoga group were instructed to practice Yoga (1hr/day for 12 weeks) under supervision of Yoga instructor. No such instruction was given to control group.Results: As a marker of youth and immunity, increase in DHEAS level decreases susceptibility to infections, reduces aging process as well as improves other functions. Yoga and meditation is documented to increase DHEAS level in regular practitioners. Statistical analysis has shown an increase in morning S. DHEAS level in yoga practitioners. Pre-study and post study values were 3.5±2.48 and 3.61±1.73 respectively in yoga group (overall 3.1% increase, P Value = 0.025, significant at 0.05) whereas 3.36±1.98 and 2.58±1.49 respectively in control group (23.2% decrease, P Value =0.84, not significant).Conclusions: This study concludes that practicing Yoga has significantly raised S. DHEAS level in medical students and improved their immunological status as well as enhances mood and behavior.

Patients with cardiac conditions may suffer from anxiety related to prognosis and further rehabilitation. Anxiety could be exacerbated by different factors including miscommunication, which could be attributed to the linguistic barrier, that exists among health care providers. At Saud Al-Babtain Cardiac Center (SBCC), nurses who are non-native Arabic speakers could have difficulty communicating disease-related information at different stages of nursing care. Is it possible to identify the language barrier as a source of anxiety for admitted patients with cardiac diseases? In this cross-sectional, descriptive study, 50 patients were included following the diagnosis of cardiac disease and post-cardiac surgery. A questionnaire that measures anxiety level showed that patients who were handled by Arabic-speaking nurses reported less collective mean for the anxiety domain statements of (20.08) versus those who were handled by Non-Arabic-speaking nurses (28.55, P value = .041). Our finding indicates that anxiety levels increased when there was a language barrier between nurses and patients, which could affect the quality of care delivery at SBCC.

Objectives. To examine prevalence and predictors of digital health engagement among the US population. Methods. We analyzed nationally representative cross-sectional data on 7 digital health engagement behaviors, as well as demographic and socioeconomic predictors, from the Health Information National Trends Survey (HINTS 5, cycle 2, collected in 2018; n = 2698–3504). We fitted multivariable logistic regression models using weighted survey responses to generate population estimates. Results. Digitally seeking health information (70.14%) was relatively common, whereas using health apps (39.53%) and using a digital device to track health metrics (35.37%) or health goal progress (38.99%) were less common. Digitally communicating with one’s health care providers (35.58%) was moderate, whereas sharing health data with providers (17.20%) and sharing health information on social media (14.02%) were uncommon. Being female, younger than 65 years, a college graduate, and a smart device owner positively predicted several digital health engagement behaviors (odds ratio range = 0.09–4.21; P value range < .001–.03). Conclusions. Many public health goals depend on a digitally engaged populace. These data highlight potential barriers to 7 key digital engagement behaviors that could be targeted for intervention. LAY SUMMARY The purpose of this study was to learn more about the smoking habits of U.S. Veterans compared with the rest of the population and to find the best ways to help Veterans quit smoking. This study found that Veterans tend to smoke more than the general population and that some groups of Veterans smoke more than others. When helping Veterans to quit smoking, health care providers should use approaches that are based on evidence, such as cognitive behavioral therapy, cognitive processing therapy, mindfulness-based stress reduction, and biofeedback in addition to the traditional 3A cessation model (ask, advise, refer).

Background and Objectives: Direct pharmaceutical marketing to physicians by pharmaceutical representatives is effective in changing behavior of health care providers, resulting in less evidence-based prescribing. Although much has been written about pharmaceutical marketing exposures among medical students, less is known about direct marketing exposures before students matriculate. This study examined the types of pharmaceutical representative direct marketing exposures for premedical students and where they occurred. Methods: From June to August of 2017, researchers surveyed students who accepted admission to US public medical schools. These prematriculated students completed our survey just prior to matriculation. The survey inquired about whether the students were exposed to pharmaceutical marketing directly from pharmaceutical salespeople, the types of marketing they observed or received, and where these interactions occurred. Results: Survey participants included 911 prematriculated students from 14 of the 188 medical schools invited to participate. Seventy-one percent (646) of the participants received or observed someone receiving pharmaceutical marketing gifts, small meals or snacks, articles, or samples. The two most common contexts for direct pharmaceutical marketing exposures were during shadowing experiences (54%, 346) and during employment (50%, 323). Conclusions: The findings suggest that it may be common for medical students to have interacted directly with pharmaceutical salespeople or observed other health professionals in these interactions before they matriculate in medical school. Because many of these interactions occur during clinical experiences required by institutions for admission, medical schools and premedical associations should consider delivering conflict-of-interest education early in medical school education or before students matriculate.

Abstract Background: To support the early integration of palliative home care (PHC) in cancer treatment, we developed the EPHECT intervention and pilot tested it with 30 advanced cancer patients in Belgium using a pre post design with no control group. We aim to determine the feasibility, acceptability and perceived effectiveness of the EPHECT intervention.Methods: Interviews with patients (n=16 of which 11 dyadic with family caregivers), oncologists and GPs (n=11) and a focus group with the PHC team. We further analyzed the study materials and logbooks of the PHC team (n=8). Preliminary effectiveness was assessed with questionnaires EORTC QLQ C-30, HADS and FAMCARE and were filled in at baseline and 12, 18 and 24 weeks.Results: In the interviews after the intervention period, patients reported feelings of safety and control and an optimized quality of life. The PHC team could focus on more than symptom management because they were introduced earlier in the trajectory of the patient. Telephone-based contact appeared to be insufficient to support interprofessional collaboration. Furthermore, some family caregivers reported that the nurse of the PHC team was focused little on them.Conclusion: Nurses of PHC teams are able to deliver early palliative care to advanced cancer patients. However, more attention needs to be given to family caregivers as caregiver and client. Furthermore, the home visits by the PHC team have to be further evaluated and adapted. Lastly, professionals have to find a more efficient way to discuss future care.

Despite the existence of medical waste disposal sites, many patients do not know how to discard their unused and expired medications. The goal of this project is to assess health care providers’ knowledge regarding proper medication disposal (PMD) and to measure the effectiveness of a brief provider informational intervention regarding PMD. Changes in knowledge were evaluated using paper pre and post surveys. The surveys were administered anonymously before and after the presentation. Principal findings: A total of 55 healthcare providers (Clinic A n = 25, Clinic B n = 28) received the PMD presentation. 52 (95%) completed the pre and post surveys. Pre-intervention knowledge of PMD was higher in Clinic B where there was an existing medication disposal box (92% accuracy) compared to Clinic B which did not have a medical disposal bin (52% accuracy). Surveys results showed improvement in 36% of Clinic A participants and 14.3% of Clinic B participants (p value = 0.0086). Based on these findings, we can conclude that the presence of a medication disposal bin is positively correlated with provider knowledge regarding PMD. This may be used to encourage the institution of medication disposal bins in more health centers and an increase in medical staff knowledge of the practice.
