Background: As with substantial increasing in incidence of diabetes mellitus in present century and with huge prevalence in population; the incidence of one of the dreadful complication of diabetes i.e. diabetic foot also increases, leading to increase morbidity and mortality. Fungal infection in foot is one of the underweighted cause of foot ulceration. These microulceration in conjunction with diabetes may leads to fulminant infection. The study comprised of 66 cases out of which 16 served as control. Aim and objectives of our study is to study the incidence of fungal infection in asymptomatic feet of diabetic patients, identification of the type of fungus, comparison with nondiabetics and clinicopathological study of the patients.Methods: The present study was carried at Department of Surgery, MLB. Medical College, Jhansi, the study group was classified into two groups. Control group; this group comprised of non-diabetic patients with foot lesions. Disease group; diabetic patients with foot lesions. Webs scrapings were inoculated in Sabouraud’s media slopes. Cultured media were examined after 3 days for the growth. Identification depends on colonial appearance and the morphology of the spores and by Gram, s and AFB Staining.Results: By this study we conclude that fungal infection in asymptomatic foot of diabetes patient are significantly more than non-diabetic patients which is accentuated by duration, type, glycaemic control and practice of bare foot walking which makes diabetic patient more prone for diabetic foot. So, the foot care of patients of diabetes should include the prophylactic steps to prevent fungal growth.Conclusions: By above study, we reasonably conclude that fungal infection in diabetic patients plays a pivotal role in formation of diabetic foot disease, so by eradication of fungal infection in asymptomatic foot of diabetic patient by general preventive methods and/or pharmacological methods may be beneficial in reducing morbidity and mortality in diabetic foot patients.

Background. One in five adults in Sri Lanka has either diabetes or prediabetes, and one-third of those with diabetes are undiagnosed. Diabetic foot is a debilitating condition affecting up to 50% of patients with both type 1 and type 2 diabetes. The risk of nontraumatic lower limb amputations is 15 times higher in diabetic patients when compared with nondiabetics. Patient education about correct foot care practices is the cornerstone of prevention of diabetic foot disease. Objective. To assess the prevalence of diabetic foot disease, knowledge, and practices about diabetic foot care among diabetic patients. Methods. 334 patients attending the diabetic clinic in Colombo South Teaching Hospital were recruited according to the inclusion and exclusion criteria. Data were collected using 3 questionnaires, and they were filled using the foot examination findings, patients’ medical records, and direct interviewing of the patients. Results. The mean age of the patients included in the study was 58.23 ± 10.65 years while the median duration of diabetes was 10.54 ± 7.32 years. 34.1% patients had peripheral neuropathy, and 29.5% had peripheral vascular disease. Diabetic foot disease according to the WHO definition was present only in 23 (6.9%) patients. There was a significant association between peripheral neuropathy and current or past foot ulcer which took more than 2 weeks to heal ( p < 0.05 ). Knowledge about foot care was less among the studied population, and it was associated with poor foot care practices. Presence of diabetic foot and current or past foot ulcer which took more than 2 weeks to heal were significantly associated with the foot care knowledge and practices ( p < 0.05 ) Conclusion. Improvement of patients’ knowledge about foot care and their practices have a significant impact on the reduction of diabetic foot disease.

Background. The aim of this study was to identify the most-feared complications of diabetes mellitus (DM), comparing those with diabetic foot pathology with those without diabetic foot pathology. Methods. We determined the frequency of patients ranking major lower-extremity amputation (LEA) as their greatest fear in comparison to blindness, death, diabetic foot infection (DFI), or end-stage renal disease (ESRD) requiring dialysis. We further categorized the study group patients (N = 207) by their pathology such as diabetic foot ulcer (DFU), Charcot neuroarthropathy, foot infection, or acute neuropathic fractures and dislocations. The control group (N = 254) was comprised of patients with diabetes who presented with common non–diabetes-related foot pathology. Results. A total of 461 patients were enrolled in this study and included 254 patients without diabetic foot complications and 207 patients with diabetic foot problems. When comparing patients with and without diabetic disease, no significant differences were observed with regard to their fear of blindness, DFI, or ESRD requiring dialysis. Patients with diabetic foot disease (61 of 207, 31.9%) were 136% more likely (odds ratio [OR] = 2.36; 95% CI = 1.51-3.70; P = .002] to rank major LEA as their greatest fear when compared with diabetic patients without foot disease (42 of 254, 16.5%) and were 49% less likely (OR = 0.51; 95% CI = 0.34-0.79; P = .002) to rank death as their greatest fear compared with patients without diabetic foot disease. Conclusion. Patients with diabetic foot pathology fear major LEA more than death, foot infection, or ESRD. Variables that were associated with ranking LEA as the greatest fear were the presence of a diabetic-related foot complication, duration of DM ≥10 years, insulin use, and the presence of peripheral neuropathy. Background: The prevalence of diabetes mellitus (DM) in Saudi Arabia is 18.3% in 2020. One of the most common complications that affects diabetic patients is diabetic foot disease (DFD). Patient education is the most effective way to reduce the complications of DFD.Methods: A cross-sectional study was conducted in the period from January to May 2021. A pretested standardized questionnaire was used to collect information upon knowledge, attitude and practice toward diabetic foot care. Data entry was performed using SPSS.Results: The responses of 480 of which 294 male and 186 female, 41.6% have high school degree, 19.3% have bachelor’s degree and 2.2% have master or Ph. D. degree. The mean age of the respondents was 47 years. The 58% of the participant agreed that they might develop reduce flow to their feet. half of the participant were not aware that smoking can reduce blood flow in their feet. There were only 2.3% of the respondents attended a class on how to care of the foot. Participants who received information about foot care from a nurse and physician were 23% and 9.2% respectively. The 97.5% of the participant would like to know how to care for their foot. 75.8% of participants walk barefoot and 42.2% use a comfortable coated shoe.  Conclusions: Participants have inadequate knowledge, attitude and practice about DFD. providing a structured educational program about diabetic foot care has significant impact on diabetic patient to improve their knowledge and practices and to motivate them to have a positive attitude toward diabetic foot care.

Background: Diabetic foot is one of the main reasons that seriously affect the quality of life and disability. Good foot care can prevent 85% amputation of diabetic foot patients. The purpose of this study is to explore the effects of interactive education with conversation map on foot care behaviors among elderly diabetic patients with high-risk diabetic foot.Methods: 96 elderly diabetic patients in Endocrinology Department of the Second Affiliated Hospital of Shandong First Medical University from January to December in 2020 were selected as the research objects. According to the parity of the last two digits of the hospitalization number, the odd number was divided into the control group and the even number was divided into the experimental group. The control group was given traditional health education and interactive education with conversation map for diabetic foot launched by the international diabetes federation was added to the experimental group. After 3 months of intervention, fasting blood glucose, 2 hours postprandial blood glucose, glycosylated hemoglobin and foot care behaviors of the two groups were observed.Results: The fasting blood glucose, 2 hours postprandial blood glucose, glycosylated hemoglobin and foot care behaviors of the experimental group were significantly better than those of the control group and the difference was statistically significant (p<0.05).Conclusions: The interactive education with conversation map can effectively improve the foot care behaviors of elderly diabetic patients with high-risk diabetic foot, and then prevent the occurrence of diabetic foot.

The aim of this study was to determine whether intensive treatment and education strategies for diabetic patients with ulcers help in preventing leg amputation. From August 2005 to March 2007, a diabetic-foot protocol using a multidisciplinary approach was applied at our hospital. All the subjects were educated regarding diabetic-foot disease and its complications and prevention. This report compares the amputation rate in patients receiving the protocol care from August 2005 to March 2007 with those who had standard care from August 2003 to July 2005. Seventy-three and 110 diabetic-foot ulcer patients received protocol and standard foot care, respectively. The incidence of major amputations in the protocol and standard care groups was 4.1% and 13.6%, respectively ( P = .03). Our protocol was associated with improved diabetic-foot care outcomes. It can be used by any hospital to improve outcomes for patients with diabetes.

Aim The aim of this study was to examine the effectiveness of a foot care (FC) camp to enhance diabetic foot care knowledge (DFCK) and diabetic foot care behaviours (DFCB) among diabetic participants in Indonesia. Methods A two-group pre- and post-test quasi-experimental design was used in this study. A total of 72 participants completed a 5-week programme. The participants’ DFCK and DFCB were examined in the fifth week using the Modified Diabetic Foot Care Knowledge (MDFCK) and the Modified Diabetic Foot Care Behaviours (MDFCB) questionnaires. Data were analysed by descriptive and independent t-tests. Results The mean score of DFCK (DFCK and DFCB in this study represent the score of knowledge and behaviors which gathered from MDFCK and MDFCB's questionnaires) in the experimental group after completing the FC camp was significantly better than that in the control group ( p < .001). Similarly, the mean score of DFCB in the experimental group after completing the FC camp was significantly better than that in the control group ( p < .001). Conclusions The FC camp was found to enhance DFCK and DFCB among diabetic patients. Therefore, this programme can be utilised for nursing practice in order to prevent diabetic foot ulcers and foot amputation.

Introduction: Diabetic foot is defined as the foot of diabetic patients with ulceration, infection, and/or destruction of the deep tissues, associated with neurological abnormalities and various degrees of peripheral vascular disease in the lower limb. Foot ulcers are the principal cause of severe complications and hospitalization among patients with diabetes, substantially increasing the costs of this disease. Objectives: To describe the presentation, distribute the diabetic foot lesions according to Wagner’s classification, and determine the outcome of diabetic foot ulceration in a tertiary care hospital of Peshawar, Pakistan. Materials & Methods:  This study was conducted in the Surgery department of Rehman Medical Institute Peshawar. Thirty nine (39) patients with diabetic foot disease presenting from Jan 2018 to Dec 2019 were included in the study. The patients were evaluated and managed by classifying their disease according to Wagner’s classification for diabetic foot disease. Statistical analysis was done by using SPSS version 26 for descriptive statistics. Results: The most common age of presentation with diabetic foot lesion was between 41-70 years; out of 39 patients, 28 (71.8%) were males and 11 (28.2%) were females. Their mode of presentation was as follows- 22 cases (56.4%) with Ulcer, 9 cases (23.1%) with Cellulitis, 7 cases (17.9%) with Gangrene, and 1 case (2.6%) with Abscess. The most common grade of presentation were grades 3 and 4 Wagner’s lesion followed by Grade 1, and Grade 2. Debridement was the most common treatment modality (56.2%), followed by Amputation (41%). Conclusion: Foot ulceration in diabetic patients is a resource-consuming, disabling morbidity that often is the first step towards lower extremity amputation. Diabetic foot lesions can be easily graded according to the Wagner classification and helps in correlating appropriate treatment to the grade of the lesion for a better outcome.

Diabetic foot ulcers (DFUs) are a serious complication in diabetic patients and lead to high morbidity and mortality. Numerous dressings have been developed to facilitate wound healing of DFUs. This study investigated the wound healing efficacy of silver-releasing foam dressings versus silver-containing cream in managing outpatients with DFUs. Sixty patients with Wagner Grade 1 to 2 DFUs were recruited. The treatment group received silver-releasing foam dressing (Biatain® Ag Non-Adhesive Foam dressing; Coloplast, Humlebaek, Denmark). The control group received 1% silver sulfadiazine (SSD) cream. The ulcer area in the silver foam group was significantly reduced compared with that in the SSD group after four weeks of treatment (silver foam group: 76.43 ± 7.41%, SSD group: 27.00 ± 4.95%, p < 0.001). The weekly wound healing rate in the silver foam group was superior to the SSD group during the first three weeks of treatment (p < 0.05). The silver-releasing foam dressing is more effective than SSD in promoting wound healing of DFUs. The effect is more pronounced in the initial three weeks of the treatment. Thus, silver-releasing foam could be an effective wound dressing for DFUs, mainly in the early period of wound management.

Objective: To assess the knowledge, attitude, and practice of diabetic patients regarding care of their own feet.Methods: This is a cross-sectional study conducted in the inpatient department of surgery at Adichunchanagiri hospital and research centre, B. G. Nagara, Karnataka, India from 1st Nov. 2016 to 31st Dec. 2016. The relation between gender and knowledge, attitude and practices of people with diabetes patients were compared by using the Chi-square test at 95% confidence interval at p<0.05.Results: Out of 51 patients, 72.54% were male and 27.46% female and 45.1% of the patients were in the age range 61-80 y. The mean SD of the age was 60.49±14.02. The mean SD of body weight of the patient was 66.17±8.54. The majority of the patients 45.1% were farmers and 41.7% were illiterate. Most of them did not know the practice of correct foot hygiene (39.22%) and what abnormalities observe in their feet (66.67%). We found that 90.2% patients were engaged in foot self-care practice and more than half of them (54.1%) always inspected their footwear before using it. Interestingly, more women were involved in foot care (100%) as compared to men (86.46%) but statistically not significant (p=0.147).Conclusion: We found that patients were not having sufficient knowledge about the correct hygiene of the foot, what abnormalities to observe in their feet and about ideal footwear. Even though females were showing interest towards self-care examination and foot care practice, it is essential that all the diabetic patients must be educated about the knowledge, attitude and foot self-care practice to prevent diabetic foot related complications.

<p class="abstract">The prevailing pandemic situation by SARS-CoV-2 infection is not only worrisome by the disease per se but also for the accompanying opportunistic infection are in the rise especially in diabetic patients. We presented a case series of post-covid rhino orbital cerebral mucormycosis infection in diabetic patients resulting in high morbidity. The need to present this was to emphasis on the timely surgical and medical intervention needed to reduce morbidity and mortality by the infection. The study highlighted the importance of surgical intervention in the deadly infection.</p>
