Background: Health care workers (HCWs) are at risk of various occupational hazards in the hospital, including exposure to bloodborne infections such as HIV and hepatitis B and C virus (HBV and HCV) infection from sharps injuries and contact with body fluids. All health care workers should routinely use appropriate barrier precautions to prevent skin and mucous membrane exposure during contact with any patient's blood or body fluids that require universal precautions. The objective of the study was to evaluate both the knowledge and the practice of standard precautions by nursing   students in the teaching hospital.Methods: It is a cross sectional study conducted in Era’s Lucknow Medical College and Hospital over a period of two months from November 2014 to December 2014 on nursing students. 100 nursing students were included in this study. All the nurses had been professionally active for 1 year or more in direct patient care and hospital hygiene.Results: Most of the nursing students were aware of the concept of universal precaution. 97% of nursing students used to wash their hands before and after patient care. In reasons for use of universal precaution it was found that100% of nursing student said use of universal precaution is to protect health care worker. While 100% said it protects HCW while handling infectious waste. Nursing students knowledge regarding indications of hand hygiene was found 97.Conclusions: The findings of this study highlight the need to implement a programme to improve knowledge on standard precautions.

Background: Improper hand hygiene among health care workers is the main cause for many nosocomial infections. Lack of knowledge and lack of recognition of hand hygiene opportunities during patient care are mainly responsible for poor hand hygiene among health care workers.Methods: Knowledge on hand hygiene was assessed using WHO hand hygiene questionnaire among medical and nursing students of Mysore medical college and research institute.Results: Among 276 medical and 225 nursing students who participated in the study, 26% of medical and 33% of nursing students had received formal training in hand hygiene in the last three years. Less than 50% of students had knowledge on source, routes of transmission of germs and minimal time needed for hand rubbing in hospital setting.Conclusions: The knowledge is limited on hand hygiene actions which shows the importance of inclusion of hand hygiene topic in curriculum.

Abstract Background Multiple factors affect residency education, including duty-hour restrictions and documentation requirements for regulatory compliance. We designed a work sampling study to determine the proportion of time residents spend in structured education, direct patient care, indirect patient care that must be completed by a physician, indirect patient care that may be delegated to other health care workers, and personal activities while on an inpatient general practice unit. Methods The 3-month study in 2009 involved 14 categorical internal medicine residents who volunteered to use personal digital assistants to self-report their location and primary tasks while on an inpatient general practice unit. Results Residents reported spending most of their time at workstations (43%) and less time in patient rooms (20%). By task, residents spent 39% of time on indirect patient care that must be completed by a physician, 31% on structured education, 17% on direct patient care, 9% on indirect patient care that may be delegated to other health care workers, and 4% on personal activities. From these data we estimated that residents spend 34 minutes per patient per day completing indirect patient care tasks compared with 15 minutes per patient per day in direct patient care. Conclusions This single-institution time study objectively quantified a current state of how and where internal medicine residents spend their time while on a general practice unit, showing that residents overall spend less time on direct patient care compared with other activities.

<p>Health care workers (HCWs) are exposed to blood and body fluids (BBF) due to occupational accidents. However, few studies have investigated the prevalence of occupational exposure in Chinese HCWs thus far. There is a clearly a critical need to characterize its epidemiology more fully in China so that effective prevention programs can be implemented. We conducted a retrospective study at a university hospital in China, giving an epidemiological analysis on these exposed HCWs whose pathogens of BBF from patients were positive [human immunodeficiency virus (HIV) / hepatitis B (HBV) / hepatitis C (HCV)]. From July 1st 2011 to June 30th 2014, a total of 155 occupational exposures to positive BBF were reported, with an incidence of 16.64 (/1000 person-years). Percutaneous injuries were the most common type of exposure episodes (89.03%). The most common type of exposed blood-borne pathogens was HBV (83.87%), and the majority of the respondents were nursing students, with an incidence of 34.22 (/1000 person-years). More effective preventive strategies on HCWs’ BBF occupational exposure should be implemented in China, especially for nursing students.</p>

Abstract Introduction Pandemics such as COVID-19 create population-wide stressors that create a natural laboratory for acute insomnia research. This study investigated risk factors and estimated rates of acute insomnia disorder in health care workers at the onset of the COVID-19 pandemic. Methods A Qualtrics survey of more than 2300 health care providers was conducted in a single academic health system on May 15th 2020, comprised of practicing attending physicians, residents and fellows in training, advanced practice providers, and nurses. Six hundred and sixty eight responded (29% response rate). The Research Diagnostic Criteria for Insomnia Disorder was used to diagnose Acute Insomnia Disorder. Results 573 respondent had no missing data pertaining to sleep, with a mean age of 43.4 + 12.5 years and 72% women. The rate of Insomnia Disorder before COVID-19 was 44.5%, while after COVID-19 it was 64.0% - a statistically significant increase. 10.2% of persons with Insomnia Disorder before COVID-19 stated it had resolved during COVID-19, while 43.4% of persons who did not have Insomnia Disorder before COVID-19 developed Acute Insomnia Disorder during COVID-19 (χ2=145.2; df=1; P&lt;0.0001). New cases of Acute Insomnia Disorder increased with female gender, advancing age, and less time spent in direct patient care. Conclusion Insomnia Disorder showed high baseline prevalence before COVID-19, followed by a striking increase in incidence in this sample of tertiary care health care workers. The effects of gender and age were similar to what has been previously published as risk factors for insomnia. The surprising finding that less time spent in direct patient care was associated with more cases of Acute Insomnia Disorder might be related to the poorly understood stresses of working from home during COVID-19. Introduction: Occupational exposure to blood and body fluids (BBFs) is a serious public health concern and it constitute a major risk factor for transmission of infectious diseases such as hepatitis B virus (HBV), hepatitis C virus, and human immune deficiency virus. Therefore, regular adherence to standard precautions remains the best strategy advocated by World Health Organization and Centres of Disease Control for controlling occupational exposure to blood and body fluids. Objectives: To evaluate the knowledge and practice of HCWs, as it relates to exposure to BBFs and adherence to standard precautions. Methods: A cross-sectional study was carried out within 8 months among primary health care workers. Information on knowledge of exposure to BBFs, standard precaution practices and post-exposure management were collected using interviewers’ administered questionnaire and observational checklist. The study was conducted among two hundred and thirteen health care workers in Ekpoma, Edo State. Data were entered, cleaned and analyzed using SPSS for window, version 16 (SPSS Inc., Chicago, IL, USA) statistical package software. Results: Sixty eight (41.7%) of cleaners and 29(17.8%) of nurses/community health extension workers were aware of blood borne infections associated with occupational exposure to BBFs compared to doctors, 3(1.8%) and laboratory workers, 4(2.5%). It was also found out that majority (74.2%) of the participants had poor knowledge of infection control practices. Conclusion: The knowledge and practice of standard precautions was found to be generally poor. Hence, all health care workers should be trained on the principles of infection control and exposure prevention. Also, relevant authorities need to formulate new strategies to create a favorable working environment that will ensure HCWs adherence to standard precautions.

Healthcare workers are at constant risk due to the infections which occur due to exposure of blood and other body fluids from infected patients. Standard precautions must be taken like maintenance of personal hygiene, use of personal protective equipment, etc. to stay safe. Thus, this study assesses the knowledge of health care workers on the importance of PPE in order to improve the awareness towards the usage of PPE during essential periods of risk. The study setting was done through an online survey among health care workers. The collected data were then analyzed in SPSS online software and the results were plotted in the form of bar graphs. From the data, it is inferred that knowledge of PPE among health care workers is well known in the majority of the responders. But more awareness must be created on the importance of PPE for the safety and welfare of all health care workers. Health care workers are at constant risk due to the infections which can occur due to exposure of blood and other body fluids. PPE plays a major role in protecting against these infections. Thus, it is important and mandatory to wear them on a daily basis for a safe and protective work environment.

AbstractBackgroundProtecting health care workers (HCWs) during the coronavirus disease 2019 (COVID-19) pandemic is essential. Serologic testing can identify HCWs who had minimally symptomatic severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infections that were missed by occupational screening based on daily symptom and temperature checks. Recent studies report conflicting results regarding the impact of occupational factors on SARS-CoV-2 seropositivity amongst HCWs.MethodsThe study population included all hospital workers at an academic medical center in Orange County, California. SARS-CoV-2 seropositivity was assessed from a fingerstick blood specimen using a coronavirus antigen microarray, which compares IgM and IgG antibodies against a panel of SARS-CoV-2 antigens with positive and negative controls to identify prior SARS-CoV-2 infection with 98% specificity and 93% sensitivity. Demographic, occupational, and clinical factors were surveyed and their effect on seropositivity estimated using multivariable logistic regression analysis.ResultsAmongst 1,557 HCWs with complete data, SARS-CoV-2 seropositivity was 10.8%. Risk factors for increased seropositivity included male gender, exposure to COVID-19 outside of work, working in food or environmental services, and working in COVID-19 units. Amongst the 1,103 HCW who were seropositive but missed by occupational screening, additional risk factors included younger age and working in administration.ConclusionsSARS-CoV-2 seropositivity is significantly higher than reported case counts even amongst HCWs who are meticulously screened. Seropositive HCWs missed by occupational screening were more likely to be younger, work roles without direct patient care, or have COVID-19 exposure outside of work.Key PointsSARS-CoV-2 seropositivity risk factors amongst health care workers included male gender, nonoccupational exposure, food or environmental services role, and COVID-19 unit location. Those missed by occupational screening were younger, in roles without direct patient care, or exposed outside of work.

Universal precautions are a vital standard set of rules applied to be followed by patients and doctors while carrying out any clinical procedure, but especially in patients with blood-borne diseases or infections. It is carried out to prevent the spread of infection from one person to another. Universal precautions are of great significance to medical personnel, where they expose themselves to numerous infectious diseases. The research aimed to assess and improve knowledge about universal precautions among health care personnel to reduce the rate of harmful exposure and infections among them. A questionnaire comprising 20 questions was created and circulated among 100 health care workers through the online platform &quot;Google forms&quot;. The results were collected and analysed. Statistical analysis was performed using SPSS. It was evident that many people were not aware of the seriousness of universal precautions. Fortunately, many medical personals followed and were aware of universal precaution to an extent. Universal precaution should be followed religiously and judiciously to prevent the spread of deadly diseases.

To evaluate the role of educational intervention on health care workers' (HCWs) compliance to standard precautions and cleaning of frequently touched surfaces at critical care units, forty-nine HCWs at 2 intensive care units (ICUs) and one neonatology unit at Fayoum University hospital were evaluated for knowledge, attitude and practice (KAP) towards standard precautions as well as obstacles affecting their compliance to standard precautions before and after a 32-hour purposed-designed infection control education program. A structured self-administrated questionnaire as well as observational checklists were used. Assessment of Environmental cleaning was investigated by observational checklist, ATP bioluminescence and aerobic bacteriological culture for 118 frequently touched surfaces. Pre-intervention assessment revealed that 78.6% of HCWs were with good knowledge, 82.8% with good attitude and 80.8% had good practice. Obstacles identified by HCWs were as follow: making patient-care very technical (65.3%), deficiency of hand washing facilities (59.2%), skin irritation resulting from hand hygiene products (51%), and unavailability of PPE (38.8%). High significant improvements of knowledge, attitude and practice were detected after one month of educational intervention (P= 0.000). During the pre-interventional period only 30.5% of surfaces were considered clean versus 97.45% post intervention (P< 0.05). The highest Median ATP bioluminescence values were obtained from telephone handset, light switches and Blood pressure cuffs. S. aureus was the most common isolated organism followed by Enterococcus spp and E.coli (52, 38 and 19 surfaces respectively). In conclusion, contentious training of HCWs on standard precautions should be considered a mandatory element in infection control programs

Abstract Background: Health care workers are susceptible to acquiring blood and body fluids borne infections due to their occupations involving contact with patients and their body fluids, although studies conducted in Ethiopia are scarce. Therefore, the aim of this study was to investigate the magnitude of exposure to blood and body fluids among health care workers in governmental health facilities in West Shewa Zone, Ethiopia.Materials and methods: A facility-based cross-sectional study was conducted from May 19 to June 25, 2018. A total of 381 health care workers were selected by simple random sampling from 31 sampled governmental health facilities using proportional to size allocation. Data were collected through self-administered questionnaires, entered into Epi-info version 7, and analyzed by SPSS version 21. Adjusted odds ratio (AOR) with 95% confidence intervals (CI) calculated for variables retained in the multivariable logistic regression and significance declared at p<0.05.Results: Of 377 health care workers who participated, the study found that 233 (61.2%) were exposed to blood and body fluids in their lifetime. Previous needle stick injury (AOR=0.30; 95%CI: 0.12-0.75), type of health facility (AOR=0.42; 95%CI: 0.26-0.68), handwashing practice (AOR=0.15; 95%CI: 0.07, 0.31), and perceiving at risk (AOR=0.16; 95%CI: 0.03, 0.98) were protective factors whereas long work experience (AOR=1.47; 95%CI: 1.13-1.93) was a risk factor for the exposure.Conclusions: Exposures to blood and body fluids during patient care were common among health care workers in the study area. Therefore, health care workers especially those newly hired and working in hospitals should pay due attention to their occupation's safety and regularly practice hand washing during critical times.
