Background: Injuries are an increasingly recognized global, preventable public health problem and are an important cause of mortality and morbidity in adult population. The major causes of injury related deaths may be intentional and unintentional. The major unintentional or “accidental” causes are road traffic accidents (RTAs), falls and drowning whereas the leading intentional causes are suicide and homicide. A robust Surveillance System for Injury Mortality is almost non-existent in our country due to which the data for the same is not available and haphazard. Keeping these factors in mind, the following study was under taken to identify the various epidemiological factors related to fatal injury cases.Methods: A record based retrospective study was conducted in the Department of Community Medicine, VSSIMAR, Burla, Odisha. The data were collected from the autopsy reports preserved at the Dept of FM & T, VSSIMSAR. Variables like age, sex, number of injury cause of death, place of death etc. were collected. Data were entered in Microsoft Excel and analysed using proportions and percentages.Results: The age group 25-44 years recorded the maximum number of deaths (37.49%). Males suffered the highest casualty accounting for 61.85% of deaths. Unintentional fatal injuries constituted 63.58% of deaths. The most number of fatal injuries resulting in deaths were RTAs (36.41%).Conclusions: The age group 25-44 years recorded maximum deaths. Males were the major death victims. RTAs constituted maximum of deaths among unintentional fatal injuries. Homicidal injuries constituted maximum of deaths due to intentional fatal injuries.

Background: Road traffic accidents (RTA) are the only public health problem for which society and decision makers still accept death and disability among young people on a large scale. This study was undertaken to determine the various epidemiological factors influencing RTA with a view to suggest appropriate remedial measures for prevention of accidents.Methods: A hospital based prospective observational study was conducted at a tertiary care hospital in Puducherry. All the patients admitted in the hospital due to RTA from Jan to Dec 2014 were interviewed by using a pre-tested and pre-designed questionnaire. Data was entered in MS Excel and analysed using SPSS version 21.0. Institute ethics committee clearance was obtained.Results: Among 541 participants, 50% of the victims were in the productive age group of 20-40 yrs. The approximate speed of majority of the vehicles (55.6%) at the time of accident was 40-80 km/hr. Among the accident victims, 85.8% of the two wheeler riders did not use helmet and 70.9% of the car drivers did not wear seat belt at the time of accident. Around 39% of the drivers were under the influence of alcohol at the time of accident. Mobile phone is the major cause of distraction (51.9%) at the time of accident.Conclusions: Based on this study it is evident that prevention and control of most of the modifiable factors can bring about a change in the statistics that would ensure our future to be safer.

Background: Ocular injuries in road traffic accidents (RTA) constitute a major but preventable cause of visual morbidity worldwide and so it is of public health importance. Aim of the study was to assess the visual outcome in patients attending tertiary care hospital with ocular injuries following RTA and compare various associated risk factors.Methods: A prospective study was done with 60 cases of ocular injuries following road traffic accidents in Assam Medical College and Hospital to find out the visual outcome and compare various risk factors .Detailed history and clinical evaluation was done with follow up at 1st and 3rd week.Results: Maximum cases were seen in the age group of 30-40 years, more in males (76.66%); with 78.33% in two wheelers. Out of the 60 cases, 38.33% occurred at night, 21.66% was under the influence of alcohol. 15.27% of patients presented with diminished vision, out of which 15% cases reported with severe ocular morbidity like vitreous haemorrhage, lens dislocation, sclera-corneal tear etc.Conclusions: Younger age group was mostly affected with male preponderance, alcohol and two wheelers being quite important risk factors. Early treatment of ocular injuries and use of protective wear result in better visual outcomes.

Background: More than half of all road traffic deaths in the world occur among vulnerable road users including pedestrians, motorcyclists and cyclists whereas; motorcyclists contribute almost one fourth of the death toll of road traffic crashes. Implementation of suitable strategies to address the issues of road traffic crashes is much difficult without information, absolute value of the problem, patterns and risks of injuries & death. Objective: To determine the different patterns, severity and distribution of injuries & their frequency in motorized two wheelers accidents. Study Design: Cross-sectional Study. Settings: Study was conducted in the department of Forensic Medicine at Nishtar Medical University, Multan Pakistan. Duration: One year from January 01, 2018 to December 31, 2018. Methodology: Total 167 cases died of road traffic accidents brought for autopsy and of those, 104 cases were reported to be died of motorcycle accidents involving both riders & pillion riders. The data of victim’s age, gender, site and pattern of injuries were obtained through detailed examination of the hospital record, autopsy reports and police inquest. The injuries over head, neck & thoraco abdominal region were given due consideration being dangerous to life. Statistical analysis was done using SPSS version 20. Results: Of the total 104 cases died in motorcycle accidents; 99 (95.20%) were males & 5 (4.80%) female. Riders died in 81 (77.89%) cases & pillion riders 23 (22.11%). The age group between 21-40 years involved 67 (64.42%) cases; maximum injuries were observed on Head/Face affecting 84 (80.8%) cases while on thorax 26 (25%) cases, Neck/Spine 12 (11.5%), pelvis 14 (13.4%) and abdominal area sustained injuries in 8 (7.6%) cases. As regard pattern of injuries; abrasions were found in 48 (31%) cases, lacerations 38 (24%), contusions 14 (9%) while fractures were observed in 56 (36%) cases. Among fatal injuries, 74 (71%) were found on Head, 7 (6.7%) over cervical spines, 3 (2.8%) on limbs whereas fatal injuries on thorax & Abdomino-pelvic areas were found 6 (9.6%) in each category. Of the total 81 riders, fractures of skull were observed in 59 (73%) cases and of 23 pillion riders, 16 (69.5%) got fracture of skull. Conclusion: Male riders of motorcycles belonging to age range 21-40 years were more prone to fatal injuries i.e., fractures of skull, neck, spine & Abdomino-thoracic injuries. Abrasions, contusions and lacerations were the most common type of injuries. Helmet legislation needs to be supported by strong enforcement and social marketing campaigns. Reducing the incidence by strict implementation of traffic rules and safety education via electronic & print media is need of the day.

Objective: The present epidemiological study was conducted with the objectives of providing an insight into the current use of antidiabetic medications to diabetics and hypertensive diabetics in urban areas and determining how the patient factors influence the prescribing of antidiabetic medications. Methodology: Data of patients of past two years were collected from Bangabandhu Sheikh Mujib Medical University (BSMMU) Hospital, Dhaka, Bangladesh. The details were entered in the structured patient profile form. Data were statistically analyzed using the Microsoft Excel 2007 software. Result: A total of 958 patient’s data were collected and analyzed of which 632 (65.97 %) were males and 326 (34.03 %) were females. These patients were further categorized based on their age. 330 patients (34.45 %) belonged to the age group 20 – 44 years, 504 (52.61 %) to the age group 45 – 65 years and 124 (12.94 %) to the age group 65 – 80 years. 684 (71.4%) patients out of the 958 patients studied were suffering from coexisting hypertension. Co-existing hypertension was found to be more prevalent in the age group 45 – 65 years (67.69%) and was found more in females (84.04%). Conclusion: Metformin was the oral hypoglycemic which was the highest prescribed. In hypertensive diabetics Metformin and Pioglitazone were most frequently prescribed drugs. Biguanides and Insulin were the most commonly prescribed antidiabetics. A combination of two or more drugs of different classes was prescribed to hypertensive diabetics. It is necessary to have an improved understanding of the etiology and pathophysiology of diabetes to focus on research efforts appropriately.

Abstract Background Traumatic brain injury (TBI) is one of the leading causes of mortality and morbidity. Economic impact is much worse in developing countries like Bangladesh, as victims are frequently male, productive, and breadwinners of the families. Objectives The objective of our study was to highlight the etiological pattern and distribution of varieties of head injuries in Bangladesh and give recommendations regarding how this problem can be solved or reduce to some extent at least. Methods From January 2017 to December 2019, a total of 14,552 patients presenting with head injury at emergency got admitted in Neurosurgery department of Dhaka Medical College and Hospital and were included in this study. Results The most common age group was 21 to 30 years (36%: 5,239) with a male-to-female ratio of 2.6:1. Injury was mostly caused by road traffic accident (RTA [58.3%: 8,484]), followed by fall (25%: 3,638) and history of assault (15.3%: 2,226). The common varieties of head injury were: acute extradural hematoma (AEDH [42.30%: 1,987]), skull fracture either linear or depressed (28.86%: 1,347), acute subdural hematoma (ASDH [12.30%: 574]), brain contusion (10.2%: 476), and others (6.04%: 282). Conclusion RTA is the commonest cause of TBI, and among them motor bike accident is the severe most form of TBI. AEDH is the commonest variety of head injuries. Proper steps taken by the Government, vehicle owners, and drivers, and proper referral system and prompt management in the hospital can reduce the mortality and morbidity from TBI in Bangladesh.

Background: India has an estimated 2.1 million persons living with HIV in 2011. The prevalence of HIV among adult population has consistently declined over the last one decade from 0.4% in 2000 to 0.27% in 2011.HIV transmission from infected mother to baby is known to occur. The effective use of Antiretroviral drugs is known to reduce the risk significantly.Methods: This is a retrospective data analysis to know the prevalence of HIV infection among antenatal women attending a tertiary care hospital. All antenatal females are offered pretest counseling, sample collected and tested as per NACO guidelines by Rapid test and ELISA. Results of the test are disclosed after posttest counseling and kept confidential.Results: The seroprevalence over five years in the present study ranges from 0.466% to 0.278%. Women in the age group 21-25 years shows high prevalence due to peak sexual activity in this age group. 56.66% Husbands of these females are reactive which shows the major source of infection.Conclusions: The present study stresses the need to target the population mainly between 21-30 years in various Government programmes, thereby to reduce the transmission from parent to child.

Background: Traumatic brain injury is a common secondary cause of  Benign Paroxysmal  Positional Vertigo(BPPV). Objective: To determine the incidence and clinical profile of patients with BPPV in mild and moderate head injury. Materials and Methods: A prospective observational study done in patients admitted with  mild and moderate head injury who developed BPPV from January 2020 to March 2020 in a tertiary care hospital of  South India. Demographic and clinical parameters were noted and were treated with canalith repositioning procedures. Results: Among 42 patients admitted with mild and moderate head injury, 9(21.4%) were diagnosed with BPPV in the follow up. The male female ratio was 2.5:1. The mean age was 44 ±21.4years.Two wheeler accidents contributed to nearly half of (55.6%) traumatic BPPV. Posterior canal was commonly involved in 7(77.8%) followed by horizontal in 2 (22.2%).The type of head injury was mild in 7 (77.8%) and moderate in 2 (22.2%) in traumatic BPPV patients. 5 (55.6%) patients with BPPV had abnormal findings in neuroimaging. 4 (44.4%) patients developed recurrent BPPV in our cohort. Conclusion: The incidence of traumatic BPPV was 21.4% in our study. Road traffic accidents due to two wheeler accidents were the common mode of head trauma. Posterior canal was commonly involved in three fourth of cases. Recurrence of vertigo was documented in 4 (44.4%).

Unintentional home injuries are a major cause for childhood morbidity and mortality. Aims and Objective: To find the KAP among mothers about common home injuries and methods to prevent them. To assess how safe the houses of the study population is. Materials and Methods: 200 mothers attending the paediatric OPD of a tertiary care hospital in a sub-urban area of South India were enrolled. They were interviewed with a structured questionnaire. Details regarding socio-demographic aspects, knowledge, attitude and practices about common household accidents and ways to prevent were asked. The safety aspects of their homes were also noted down. The collected data were analysed with sigma plot 13(system software, USA). In the above statistical tools the probability value <0.05 was considered as significant. Result: The mean age of mother was 28.82 ± 4.93 years and age at marriage, 21.7 ± 3.55 years. 54% belonged to lower middle class and nearly 20% each in upper lower and upper middle class. Fall from height, Road traffic accidents followed by electrical injuries were the commonest accidents known by mothers. Socioeconomic status and educational levels had statistical correlation with few of the variables. Conclusion: Knowledge about home accidents is poor in the study population. Most of the houses are not child safe. Parent education about home injuries and training on first aid should be part of newborn follow up.

Background: Medico-legal case (MLC) can be defined as a case of injury or ailment, etc., in which investigations by the law-enforcing agencies are essential to fix the responsibility regarding the causation of the said injury or ailment. Medico-legal cases are an integral part of medical practice that is frequently encountered by medical officers working in casualty. Hence the present study is carried out to find out the frequency and pattern of medico-legal cases reported at a tertiary care hospital and to highlight the vulnerable gender, age, residence and the cause. Methods: It is a record based cross sectional study in which all the MLC cases registered in MLC record book during a period of 6 months (January 13– June 13) were included. Cases found non medico-legal were excluded. Variables considered were gender, age, residence and cause. Findings were expressed in numbers and percentages. Results: Total cases were 2350, of which 1866 (79.4%) were males and 484 (20.6%) were females. Maximum cases were from the age group of 21-30 years i.e., 828 (35.2%). 1440 (61.27%) were residents of urban area. Most of the medico-legal cases registered were due to assault (27.2%) followed by medical examination of prisoners(25.9%), road traffic accidents (19.7%), fall (6.9%), snake bite (6.6%), poisoning (4.3%), domestic injuries (4.3%), unknown bite (2.2%), unknown found at railway station (1.6%), brought dead (0.6%), burn (0.2%) & others (0.5%). Conclusions: Majority of the victims were males, young adults and urban inhabitants. Most common indication for medico-legal cases was assault followed by medical check-up of prisoners and road traffic accidents.
