Research objective. To evaluate the features of pregnancy and delivery course, fetal and newborn status in women with confirmed COVID-19.Materials and methods. A retrospective clinical and statistical analysis of 106 pregnancy and delivery case histories and medical records of newborn from women with a gestation period of 22–41 weeks with a confirmed diagnosis of COVID-19 was carried out. Women were treated and delivered on the temporarily converted beds for providing obstetric care to pregnant women, women in labor and postpartum women with suspected and infected COVID-19 and gynecological patients of Municipal Non-Profit Enterprise “Kyiv City Maternity Hospital No. 3” from September 2020 to May 2021.Results. Delivery took place in 48 women. Women with A (II) blood group were significantly predominant – 51 (48.1%) and rhesus-positive factor – 85 (80.2%) (p < 0.05). One third of pregnant women sought medical help on their own – 34 (32.1%). 94 (88.7%) women had a positive PCR test, in 6 (11.3%) cases the diagnosis was confirmed by a rapid test in the admission department; in 6 (11.3%) cases the test results were negative. The leading complaints when pregnant women were hospitalized were fever, cough and fatigue in 85 (80.2%), 90 (84.9%) and 73 (68.9%) pregnant women respectively (p < 0.05). Almost every third woman (39, 36.8%) had a subfebrile temperature – 37.0–37.5 oC. In 15 (14.1%) patients the temperature was 38.0–38.5 oC, in 6 (5.7%) patients over 38.5 oC. At the lung ultrasound the signs of bilateral and polysegmental pneumonia were diagnosed in 44 (53.6%) and 36 (43.9%) cases. Lower and upper lobe pneumonia was diagnosed in 20 (24.4%) pregnant women. Severe lung damage was detected in 29 (35.4%) pregnant women in the form of partial “hepatization” of lung tissue, pleural effusion in the form of black areas of various sizes in the pleural cavity.Conclusions. The main typical SARS CoV-2 complaints in hospitalized pregnant women were fever, cough, fatigue and shortness of breath at rest and during exercise. Such neurological manifestations of SARS CoV-2 as headache, anosmia were noted by almost every third pregnant woman, and parosmia and myalgia were noted by every fourth pregnant woman. The average level of C-reactive protein was above normal in 97.2% of women. Lung ultrasound allowed clearly identifying areas of damage and determining the degree of lung tissue damage.

Abstract: As an infectious transmitted disease, malaria has been a tremendous health problem around the world. Indonesia is determined as one of the 104 malaria-endemic countries in the world. Pregnant women are easier infected by malaria compared with the general population. People suffered from malaria may have low haemoglobin and high TNF-α levels. The increased TNF α level may cause bad complication or mortality in malaria patient. This study aimed to determine the levels of hemoglobin and TNF-α among pregnant woman with malaria in Kabupaten Bolaang Mongondow Utara. This was a cross-sectional study. Samples were obtained by using purposive sampling method. Besides using questionnaire, there were three laboratory tests conducted in this study: peripheral blood smear with Giemsa staining for detection of plasmodia; hemoglobin level with rapid test hemoglobin; and TNF-α level with ELISA. The bivariate analysis showed that of 72 pregnant women there was only one (1.4%) with positive malaria and increased TNF-α level. There was no pregnant woman with malaria had low hemoglobin level. The chi square test showed a P value 1,00 (α = 0,05). Conclusion: There was no relation between plasmodium infection and the levels of TNF α and hemoglobin among pregnant women in Kabupaten Bolaang Mongondow Utara. The pregnancy and delivery can provoke development of retinal detachment in women in the presence of pathological alterations of eye-ground. The rapid development of ophthalmology and amelioration of material and technical resources of medical organizations processing out-patient reception permit hoping for comprehensive examination of eye-ground of all pregnant women. The study was carried out with purpose to evaluate completeness of examination of eye-ground of pregnant women in out-patient conditions. The methods. The questionnaire survey was organized concerning women at the last month of pregnancy being for treatment in department ofpregnancy pathology in a certain maternity hospital under large multifield hospital of Moscow in 2014-2015. The analysis was applied to answers of 481 women concerning completeness of their examination by ophthalmologist at out-patient stage of observation. The analysis of grounds for alertness of ophthalmologists concerning risk of retinal detachment was implemented on the basis of data from state statistical reporting form № 12 about morbidity of Russian population (including girls aged from 15 to 17 years) with eye diseases and its adnexa (2005-2015), myopia (2005-2014), degenerative myopia and retinal detachment with rupture (2014-2015). Conclusions. The study established that every ninth women (11.2%) miss visiting to ophthalmologist during pregnancy; in all the rest, condition of periphery of eye-ground was not examined properly i. e. examination of eye-ground under mydriasis is implemented in 41.2% of cases and in case of one-time visit (54.2%) only in 33.7% of cases. The absence of mandatory examination of eye-ground at wide pupil of pregnant women by ophthalmologists of primary medical care level deprive of any sense recommendation about twice visit to oculist during pregnancy period. The observed deterioration of ophthalmic health ofpopulation provoke no interest in oculists ofprimary medical care level to increasing of alertness related to risk of retinal detachment during natural delivery. There are grounds to expect further increasing of development of retinal detachment in women during natural delivery.

Objective To analyze the use of the measurement of uterine cervix length (MUCL) and the fetal fibronectin (fFN) rapid test as predictors of preterm delivery (PTD) in symptomatic pregnant women assisted at the Santa Casa de Misericórdia de Sobral Maternity Hospital. Methods This was a prospective and analytic study involving 53 parturients assisted between September of 2015 and July of 2016; the participants were between 24 and 34 weeks of gestational age (GA) and presented complaints related to preterm labor (PTL) prodromes. Vaginal secretion was collected for fFN testing, and the MUCL was obtained via transvaginal ultrasonography. Results A total of 58.49% of the subjects showed MUCL < 25 mm, and 41.51% were positive in the fFN rapid test. A total of 48 patients were followed-up until their delivery date, and 54.17% resulted in PTL. The relative risk (RR) for PTD in patients with MUCL < 25 mm was 1.83 (p = 0.09, 0.99–3.36, 95% confidence interval [CI]), with a mean time before delivery of 2.98 weeks. Based on fFN positive results, the RR was 3.50 (p = 0.002, 1.39–8.79, 95%CI) and the mean time until delivery was 1.94 weeks. The RR was 2.70 (p = 0.002, 1.08–6.72, 95%CI) when both tests were used. The RR of PTD within 48 hours, and 7 and 14 days were, respectively, 1.30 (p = 0.11, 95% CI 1.02–1.67), 1.43 (p = 0.12, 95% CI % 0.99–2.06), and 2.03 (p = 0.008, 95% CI 1.26–3.27), when based on the MUCL, and 1.75 (p = 0.0006, 95% CI 1.20–2.53), 2.88 (p = 0.0001, 95% CI, 1.57–5.31), and 3.57 (p = 0.0002, 95% CI 1.63–7.81) when based on positive fFN results. The RR at 48 hours and 7 and 14 days considering both tests was 1.74 (p = 0.0001, 95% CI 1.14–2.64), 2.22 (p = 0.0001, 95% CI 1.22–4.04), and 2.76 (p = 0.0002, 95% CI 1.27–5.96), respectively. Conclusion In symptomatic pregnant women, we concluded that the MUCL < 25 mm associated with positive fFN rapid test indicate increased the risk for PTD. Further studies with larger sample sizes could contribute in supporting the results presented in the current study.

One of the most serious problems of modern medicine is sepsis. The number of patients undergoing this complication is 20–30 million (WHO) annually and has no tendency to decrease. Sepsis is characterized by severe multiple organ failure due to a violation of the response of the macroorganism to an infectious agent. Moreover, it is dangerous with high mortality. Sepsis often develops in patients with immunodeficiency conditions, which primarily include pregnant women. The article presents a clinical observation of a case of periostitis in a pregnant woman complicated by a septic state.

The objective: to evaluate the results of planned open surgical interventions (POSI) in pregnant women with primary symptomatic chronic vein disease (PSCVD). Materials and methods. The study included 457 pregnant women operated on a routine basis with PSCVD. The patients underwent ultrasound duplex angioscanning (USDA) of the veins of the lower extremities, inguinal canals, and iliac veins. The severity of the clinical manifestations of PSCVD was evaluated on a VCSS scale. With the help of the circadian visual-analog scale, the dynamics after the operative pain were studied. The effect of POSI on uterine tone and cardiac function of the fetus was investigated using cardiotocography. When forming the results for POSI, data from USDA, the severity of clinical and cosmetic manifestations of PCVD, the need for active prevention of thrombophlebetic, thromboembolic and hemorrhagic complications, minimization of surgical childbirth were taken into account. The results of treatment were studied in 2 groups of patients. Patients of the first group were treated with conservative therapy, the second group – POSI. Parametric indicators, which were presented as mean and standard error M(SD), were used for statistical data processing of the study, and their reliability was estimated using Student’s t-test. The species was determined to be significant at p<0.05. Results. 495 POSI were performed at 28-38 weeks gestation in an obstetric hospital (perinatal center), by a vascular surgeon from the staff of the center, who was familiarized with the peculiarities of working with this contingent of patients. Surgery was performed under local anesthesia in 346 (75.49%) pregnant women at one extremity, in 111 (24.51%) – at two in one session. According to the results of the study, it was noted that in the first group 33.7% of patients had a positive clinical result due to the reduction of pain syndrome, while 67.5% of patients had an increase and spread of varicose transformation in the area of saphenous and/or non-saphenous veins with spread of pathological venous reflux in the distal direction. 78% of patients had the need to administer prophylactic doses of low molecular weight heparins, both during pregnancy and in postpartum period. Out of 126 pregnant women with pronounced varicose veins of the external genital organs and perineum in 36 (28.6%) deliveries were performed by caesarean section. In 9 (3%) patients there was an acute thrombophlebitis of the subcutaneous veins of the lower extremities, which required 5 pregnant women to undergo urgent surgery when the inflammatory process had spread to the middle and above along the femoral vein of the large subcutaneous vein. Pregnant women of the second group with PSCVD on the basis of obstetric hospital (perinatal center) POSI were performed in the organization, which laid the ideology of the FTS program, strictly individual indications for intervention in the optimal terms of pregnancy, multidisciplinary management of patients, due to this in 93% of operated patients regression of clinical manifestations of the disease was noted, whch contributed to the correct and safe delivery of pregnancy with 100% absence of preterm birth, abnormalities in fetal development, pregnancy course, negative impact on obstetric and somatic condition of the pregnant woman. No hemorrhagic, thrombophlebic, thromboembolic complications were noted. Patients in the postoperative period did not require medical support, as during the period of pregnancy, during delivery and in the postpartum period. There were no indications for surgical delivery. 2.4% of patients experienced complications of post-operative wounds in the form of cheese, which had no effect on pregnancy and was eliminated before delivery. In the postoperative period, if necessary, it was recommended to use elastic compression class garments 1–2. Conclusion. POSI made at PSCVD in optimal terms of pregnancy in a specialized obstetric hospital by a vascular surgeon in strictly individual indications is safe, both for the fetus and the pregnant woman. It is promising to further study the results of POSI in pregnant women with PSCVD to introduce it into the arsenal of treatment of this pathology. Key words: FTS ideology, planned open surgical interventions in pregnant women.

A study was performed on 100 pregnant women in the outpatient department of gynecology and obstetrics of Maternity and Children Hospital in Al-Diwaniya City during the period between (March to September 2016). One hundred blood samples (50 for patients and 50 for control) were collected under the supervision of the treating gynecologist. The detection of Helicobacter. pylori was done by the use of the serum antibody Rapid test. The results showed that 50 (100%) were positive and 50 (100%) were negative for H. pylori in above method. All blood of patients and control samples were used for the extraction of genomic DNA,where the 107 bp PCR product size. Genotyping of the TNF-α-308 SNP (G/A)was performed by restriction fragment length polymorphism PCR (RFLP-PCR). PCR products were digested with restr NcoI iction enzyme. Individuals with the TNF-α-308(GG) homozygote produced digested DNA bands at 80,and 20 bp bp. A heterozygous genotype ofTNF-α-308 (GA)produced 107 bp,80 bp,and 20 bp bands. Individuals with the TNF-α-308 (AA) homozygote genotype had no amplicon digested and generated only one band of 107 bp. There was a significant difference in the frequency of the TNF-α-308(GG)genotype between H. pylori positive group and H. pylori negative group(72%,78% respectively). Also for GA genotype, there was a significant difference between H. pylori positive group and H. pylori negative group(24%,18% respectively). Concerning the frequency of the TNF-α-308 (AA)genotype between H. pylori positive group and H. pylori negative group, there was no significant difference between the two groups.
