Peer counseling programs are increasingly popular, and intervention trials have established efficacy. However, pragmatic, real-world effectiveness of peer counseling programs are limited, but necessary, to justify funding. This study examined the effectiveness of the Loving Support Peer Counseling (LSPC) program to increase breastfeeding duration and reasons for cessation among Women, Infants, and Children (WIC) participants. In a sample of 178 WIC participants, new mothers who received LSPC breastfed for an average of 8.6 weeks longer relative to mothers who did not receive LSPC (p < .001). Although all mothers cited concerns about infants'’ nutritional uptake, WIC mothers who did not receive LSPC reported frustration as the reason for breastfeeding cessation. This study supports the pragmatic, real-world effectiveness of LSPC among WIC mothers.

<p>Dalam rangka mengontrol perilaku dan perkembangan anak pada usia pendidikan dasar, perlu adanya sinergitas serta komunikasi antara pihak sekolah dengan orangtua. Tujuan penelitian ini untuk menganalisis program konseling dalam mengontrol perilaku siswa Sekolah Dasar Unggulan Terpadu Bumi Kartini Jepara. Metode penelitian yang digunakan adalah deskriptif kualitatif. Dalam pengumpulan data menggunakan penelitia lapangan melalui wawancara, observasi langsung dalam implementasi program konseling. Hasil temuan  penelitian menunjukkan ada tiga program konseling yang dilaksanakan di Sekolah Dasar Unggulan Terpadu Bumi Kartini Jepara. <em>Pertama, </em>konseling secara individu yang dilakukan guru kelas dengan siswa. Program ini dilakukan sesuai kebutuhan pada waktu kondisional. <em>Kedua, </em>konseling bulanan antara guru kelas dengan orangtua siswa. Jadwal pelaksanaan setiap hari Jum’at pada akhir bulan. K<em>etiga, </em>program layanan <em>parenting </em>bersama orangtua siswa setiap akhir semester. Program parenting dilakukan dalam bentuk variatif seperti seminar, sarasehan, <em>gathering</em>, dengan tema berbeda setiap semester. <em>Parenting </em>merupakan media <em>update</em> dan sharing informasi tentang perkembangan sikap anak dengan fasilitator ahli. Media komunikasi penunjang konseling adalah grup <em>whatsapp </em>orang tua siswa, guru, pimpinan sekolah, dan buku konseling tentang catatan masalah beserta hasil konseling.<em><br /><br />In order to control the behavior, attitudes, and development of children in the age of basic education, there needs to be synergy and communication between stakeholder of the school and parents. This study aims to analyze counseling programs in controlling the behavior of students of Integrated Primary School Bumi Kartini Jepara. The research method used descriptive qualitative. In collecting data using field research through interviews, direct observation in the implementation of counseling programs. The results of the study are three counseling programs carried out at the Integrated Primary School Bumi Kartini Jepara. The first, individual counseling by class teachers with students. This program is carried out as needed in conditional time. The second, monthly counseling between class teachers and parents. Implementation schedule every Friday at the end of the month. The third, parenting service program with parents of students at the end of each semester. The parenting program is carried out in various methods such as seminars, parent class, gatherings, with different themes every semester. Parenting is a medium for updating and sharing information about the development of children's attitudes with expert facilitators. There are communication media in order to supporting counseling program, such as WhatsApp group of parents, teachers, school leaders, and counseling books about the problem notes and the results of counseling.</em></p>

Abstract Background France has one of the lowest rates in the world regarding breastfeeding initiation and duration. Few studies have explored breastfeeding practices in France since the middle of the twentieth century, or following from initiation to cessation. The purpose of our study was to determine trends in breastfeeding over the past decades regarding public health recommendations, and to examine mothers’ perceptions about factors known to have an impact on breastfeeding support and cessation. Methods From the NutriNet-Santé cohort, 29,953 parous women (launched in 2009 to study relation between nutrition and health), were included in the present study. Using web-questionnaires, they were asked retrospectively if they had breastfed their youngest child or not, and if so, the duration of exclusive and total breastfeeding. For those who had breastfed, we investigated their perceptions about support at initiation and during the entire breastfeeding period and reasons for breastfeeding cessation. We also asked those who did not breastfeed about their perceptions and reasons for infant formula feeding their youngest child. Analyses were weighted according to the French census data. Results In the NutriNet-Santé cohort, 67.3% of mothers breastfed their youngest child. The proportion of breastfed children increased over the past few decades, from 55.0% (95% CI 54.3, 55.6) in the 1970s to 82.9% (82.4, 83.4) in the 2010s. Total and exclusive breastfeeding duration went from 3.3 months and 2.4 months respectively in the 1970s to 5.9 months and 3.2 months respectively in the 2010s. Most mothers felt supported at initiation and during the breastfeeding period. A reported desire to have breastfed longer than two months was 59.5%. Mothers who did not breastfeed did it by choice (64.3%). They did not feel guilty (78.2%) and did not perceive a problem not to breastfeed (58.8%), but almost half of them would have liked to have breastfed (45.9%). Conclusion Breastfeeding duration has increased in the past decades but did not reach the public health recommendations threshold. Targets other than mothers have to be considered for breastfeeding education, like the partner and her environment, to increase breastfeeding practices. Trial registration The study was registered at ClinicalTrials.gov (NCT03335644).

The school counseling program is meant to assist students to deal with psycho-social and behavioral challenges, which normally affect their academic performance. In order to achieve this goal, the Kenyan government has previously committed resources towards establishing counseling programs in schools. The aim of this study was to establish how effective these counseling programs are in assisting secondary school learners in Kiambu County to achieve academically. Primary data was derived from 80 high school adolescents, 8 counselors, and 8 academic Dean of Students who were purposively sampled from 8 schools that were randomly selected. The study was qualitatively done, and self-determination theory was used to guide the study. Data were analyzed for the identification of counseling program characteristics and student improvement indices. These were presented in tables, charts, frequencies, and percentages based on the responses from the respondents. Further, a correlation between the two variables of the study was examined. Findings depicted that 65.7% of the students sampled said that counseling services offered at school satisfied their needs, 74.3% reported an improvement in their academic performance as a result of those counseling services and 87% felt more positive about school life after receiving counseling services. The study concluded that comprehensive counseling programs improved academic performance among high school students. The study intended to inform education planners, principals, and administrators on the role counseling would play in enhancing academic achievement among secondary school students.

Abstract Background Despite numerous benefits, many mothers stop breastfeeding soon after birth. A common reason for this is the experience of pain or discomfort. One resource which women use to share their breastfeeding challenges and seek support are online forums. This study aimed to collect data from online forums to explore 1) usage of forums as social support for breastfeeding-related pain; 2) experiences of breastfeeding-related pain; 3) perceptions and strategies to deal with breastfeeding-related pain; and 4) the impact of pain on breastfeeding duration. Methods Data was gathered through searches of online forums based in the UK and USA: Netmums, What to Expect and Mumsnet using key terms: ‘painful breastfeeding’ and ‘sore breastfeeding’. Data extraction took place in July 2018 and included posts made between 2012 and 2018. Data included 123 posts and 193 replies, analysed using thematic analysis. Results The first theme identified was ‘variation in types of pain’, highlighting the variety of painful experiences and their descriptions. In particular, this theme reveals the large variety of different types of pain women experience at different stages throughout their breastfeeding journey, as well as the different pain characteristic they focus on in the description of their experience (i.e., location, sensory or physical aspects). Secondly, the theme ‘perceived causes and explanations for pain’ revealed how women interpret pain experiences either due to a recognised condition or behavioural cause. The third theme ‘cessation of breastfeeding related to pain’ identified. How women experience both physical and psychological struggles (e.g., guilt) related to breastfeeding cessation, with pain being a main factor in considering cessation of breastfeeding. Finally, the theme ‘shared experiences and support’ identified women’s strong need for both practical and emotional support to deal with pain. Many women look for this support through the knowledge exchange offered on the online forums. Conclusions Pain was a key reason for breastfeeding cessation, commonly associated with strong feelings of guilt. The online forums provide a unique form of social support for breastfeeding women to find ways to cope with the pain, while highlighting the urgent need for more appropriate antenatal education on realistic expectation surrounding breastfeeding.

The purpose of this study was to conduct a comprehensive evaluation of a peer helping program in a high school setting. Opinions were obtained from students, staff, peer support members, their parents, and their “clients.” A large sample was obtained for the general student survey (204 students, or 73% of the student population). The results validated the basic assumptions on which peer helping programs are founded, namely, that students most often seek out other students for help with problems and that students are thought to be capable of providing help for their peers. The results indicate that although students have confidence in one another for helping, few students said they would seek help with a problem from a peer counselor. It is thought that existing peer counseling programs are perhaps too small to adequately represent all peer groups within the school and that attempts should be made to expand membership in the program in order to reach more students. It was also indicated that students have greater confidence in a peer's ability for helping with school and relationship problems, whereas less confidence was shown for dealing with problems of a more serious nature such as suicide, death, and pregnancy. This point emphasizes the difference between peer counseling and “professional” counseling programs. It is proposed that for peer helping programs to grow the distinction between support and counseling become clear in the minds of those running the program and that alternate methods for program selection and training need to be explored.

Cardiovascular (CV) disease (CVD) is the main cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Despite optimal glycaemic control, standard antihyperglycaemic therapy failed to impact CV events in intervention trials; therefore, the U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA) issued a guidance to the pharmaceutical industry to specifically assess the CV outcomes and safety of new glucose-lowering drugs. Amongst them, sodium-glucose transporter 2 (SGLT2) inhibitors proved to not only provide good tolerance, few adverse effects, and good glycometabolic control, but also striking reduction in the risk of CV events. In this review, data from the main randomised controlled trials are presented, including post-hoc analyses looking into several aspects of CV protection. Moreover, the main findings from observational real-world studies to date are described, overall reassuring as regards to CV safety and efficacy of SGLT2 inhibitors. Finally, several mechanisms which might contribute to the cardioprotective effect of SGLT2 inhibition are depicted, including findings from recent mechanistic studies.

An association between high pre-pregnancy body mass index (BMI) and early breastfeeding cessation has been previously observed, but studies examining the effect of underweight are still scant and remain inconclusive. This study analyzed data from a nationally representative cohort of 18,312 women (mean age 28.3 years; underweight 20.1%; overweight 8.2%; obesity 1.9%) who delivered singleton live births in 2005 in Taiwan. Comprehensive face-to-face interviews and surveys were completed at 6 and 18 months postpartum. BMI status and breastfeeding duration were calculated from the self-reported data in the questionnaires. In the adjusted ordinal logistic regression model, maternal obesity and underweight had a higher odds of shorter breastfeeding duration compared with normal-weight women. The risk of breastfeeding cessation was significantly higher in underweight women than in normal-weight women after adjustments in the logistic regression model (2 m: aOR = 1.11, 95% CI = 1.03–1.2; 4 m: aOR = 1.32, 95% CI = 1.21–1.43; 6 m: aOR = 1.3, 95% CI = 1.18–1.42). Our findings indicated that maternal underweight and obesity are associated with earlier breastfeeding cessation in Taiwan. Optimizing maternal BMI during the pre-conception period is essential, and future interventions to promote and support breastfeeding in underweight mothers are necessary to improve maternal and child health.
