The task of a dentist in the treatment of deep caries (pulp hyperemia) is not only to restore the integrity of the tooth, but also to stimulate the production of replacement dentin with the help of a medical pad, to ensure reliable isolation of the pulp from the toxic effects of materials. Having a wide range of different therapeutic pads, but not having methods to objectively assess the condition of the pulp, doctors have to «blindly» give preference to a particular manufacturer. In this work, based on a clinical and laboratory study, an analysis of the effectiveness of calcium-containing pads «Life» and «Dycal» – was performed. The clinical stage was performed on patients in the treatment of deep caries. Changes in the pulp microcirculation were evaluated using laser Doppler flowmetry. During histological examination, samples of tooth cuts were studied for analysis of micro-flows of the dye at the «seal-tooth» border and the depth of penetration of the dye. The clinical results are consistent with laboratory results, which can be used to draw conclusions about the benefits of the «Life» treatment pad in the treatment of deep caries.

Background: We investigated the long-term clinical results of percutaneous transluminal angioplasty (PTA) in patients with peripheral arterial occlusive disease (PAOD) and the influence of different parameters on the primary success rate, the rate of complications and the long-term outcome. Patients and methods: We reviewed clinical and hemodynamic follow-up data of 166 consecutive patients treated with PTA in 1987 in our department. Results: PTA improved the clinical situation in 79.4% of patients with iliac lesions and in 88.3% of patients with femoro-popliteal lesions. The clinical stage and ankle brachial index (ABI) post-interventional could be improved significantly (each P < 0,001), the same results were observed at the end of follow-up (each P < 0,001). Major complications occurred in 11 patients (6.6%). The rate of primary clinical long-term success for suprainguinal lesions was 55% and 38% after 5 and 10 years (femoro-popliteal 44% and 33%), respectively, the corresponding data for secondary clinical long-term success were 63% and 56% (60% and 55%). Older age (P = 0,017) and lower ABI pre-interventional (P = 0,019) significantly deteriorated primary clinical long-term success for suprainguinal lesions, while no factor could be identified influencing the outcome of femoro-popliteal lesions significantly. Conclusion: Besides an acceptable success rate with a low rate of severe complications, our results demonstrate favourable long-term clinical results of PTA in patients with PAOD.

Retention of cationic dispersed rosin size was studied via turbidity measurements on stock filtrate with different alum and dispersed rosin size dosages. Stock charge characteristics were analyzed using both an analysis of charge demand determined via a streaming current detector and an evaluation of zeta potential of the fibers by streaming potential measurement. The results indicated that an optimum amount of alum existed such that good sizing retention was maintained throughout a wide range of dispersed rosin size dosages. However, when an excessive amount of alum was used and fines and colloidal particles were transitioned from anionic to cationic, the cationic size retention was reduced. Laboratory results were confirmed with a paper machine trial. All data suggested that a stock charge study was necessary to identify optimal alum dosage for a cationic dispersed rosin sizing program.

Capillary transit time is determined by the ratio of capillary volume to flow rate. Exercise-induced hypoxemia is thought to occur because of the short transit time of erythrocytes in capillaries. The effect of flow rate on capillary volume (recruitment vs. distension) is controversial. In a perfused left lower lobe preparation in canine lungs, we used laser-Doppler flowmetry (model ALF21R) to monitor changes in blood flow, volume, and transit time in the microvasculature near the subpleural surface. Changes in total flow, blood volume, and total transit time (tt) were also measured. The results showed that microvascular volume approached maximum when flow rate was at resting value (0.4 l/min) and pressure in the pulmonary artery was > 6 mmHg relative to the level of the capillaries. In contrast, the total blood volume increased gradually over a wide range of flow rates. When flow increased 4.2 times (from 155 to 650 ml/min), tt decreased from 7.32 to 3.53 s; meanwhile, microvascular flow increased from 6.0 to 12.7 units and microvascular transit time decreased from 3.14 to 1.81 units. The changes in microvascular volume and transit time were essentially independent of whether the venous pressure was higher or lower than alveolar pressure. At very high flow (6–10 times resting value), tt fell gradually to approximately 1 s. Direct monitoring of transit time with the laser-Doppler also revealed a gradual decline in microvascular transit time as flow rate increased from 2 to 10 times the normal flow. The general theory of axisymmetric hardness tests on nonlinear media is approached from the standpoint of similarity transformations. It is shown how an entire process of indentation can be made to depend on the solution of just one boundary-value problem in scaled variables and with a fixed geometry. Once this single auxiliary solution has been obtained, the values of all physical quantities in the original problem can be generated readily at any stage without further numerical error. Even by themselves the similarity relations provide valuable information about (for example) an invariant connection between the depth of penetration and the radius of contact, or about the variation of penetration with time in a creep test under dead load. Two kinds of material behaviour are considered: (a) nonlinear elastic (modelling strain-hardening plasticity) and (b) nonlinear viscous (modelling secondary creep). In either category the constitutive specification is sufficiently flexible to represent a wide range of actual responses in the context of hardness testing. The analysis for case (a) extends a theory of ball indentation by Hill et al . to a class of indenters with shapes varying from flat to conical. It also prepares the ground for case (6) which is more difficult and calls for a quite different auxiliary problem.

Health care costs continue to increase, affecting patients and insurance providers. Complementary health approaches are increasingly used to augment traditional medicine, and integrative medicine (IM) incorporates these complementary approaches into traditional patient care. The IM Department was established in our institution in 2004 and now offers a wide range of services to patients. Our institution offers health care coverage to all benefit-eligible hospital personnel and their eligible dependents. The use of IM has had a surprising and beneficial effect on the health care costs of this small, self-insured health plan. We found that the coverage of certain IM modalities for specific conditions had positive clinical results and resulted in significant cost savings to the insurance plan. At the same time, this partnership supports patients by providing appropriate and effective care, and we have seen success in terms of patient recovery and patient satisfaction. Here, we present the history of the relationship between the insurance plan and the IM Department, how the coverage of IM modalities has expanded, and the current practice at our institution. We demonstrate that this innovative relationship has benefitted patients and resulted in cost-savings for the insurance provider. Therefore, this partnership will continue to expand, thus providing patients with a wide range of treatment options and effective care.

The interferons are the best known of biologic antineoplastic agents. Progress with the clinical application of interferons to cancer has been slow and complicated by the need for attention to a new spectrum of therapeutic and toxic effects manifest by the interferons. This summary of current phase I and II trial results with the interferons establishes their clinical potential. The maximally tolerated dosages of the most common species of interferon alpha produced in eukaryotic cells as well as by recombinant DNA technology in bacteria are now described in a variety of different disease states. "Naturally" produced eukaryotic as well as bacterially synthesized interferons have a similar, wide range of biologic effects in vitro and in vivo. Antiviral, antiproliferative, immunologic, and enzymologic functions of the interferons relevant to antineoplastic functions are under study. Knowledge of these mechanisms should improve the clinical results obtained in human cancer. Species and subspecies differences in the activity of interferons may lead to selective use of the pure interferon subspecies, alone or in combination. The use of the interferons and other antineoplastic biologics, such as antibody or chemotherapy, are subsequent goals that are now on the horizon.

Typhlodromus pyri Scheuten (= T. tiliae Oudms.) is the predominant species of predacious mite on apples in Nova Scotia. It feeds on a wide range of prey species (Herbert, 1959). Chant (1960) investigated the life-history and habits of this species in southeastern England and Herbert (1956) studied its life-history under controlled laboratory conditions. A knowledge of the life-histories of various components of the fauna is basic to an evaluation of predation; therefore, a study of the life-history and habits of T. pyri in the field was undertaken. This was supplemented by a laboratory study in which an attempt was made to duplicate field temperarures. The results of these studies are presented here.

For treatment of complex superficial femoral artery lesions, suitable stent devices are required. The present study details the first long-term results with the long EverFlex nitinol stent. Forty-one EverFlex stents were implanted in 32 patients with either superficial femoral artery occlusions (n = 20) or stenoses (n = 12); mean lesion length was 13.3 cm (range = 8.0–39.0). Patients presented with clinical stage Fontaine IIb (n = 27), III (n = 4), or IV (n = 1). Stent lengths were 10 cm (n = 18), 12 cm (n = 6), or 15 cm (n = 17). Follow-up after 2 months indicated primary and secondary patency rates of 96.9% (n = 31) and 100% (n = 32); ABI improved from 0.57 to 0.91 ( P < .001). After 1 year, revascularization was performed in 6 patients (18.8%). Primary and secondary patency rates were 81.3% (n = 26) and 93.8% (n = 30). In this first long-term evaluation, the long EverFlex nitinol stent achieves excellent clinical results after implantation into complex superficial femoral artery lesions.

Pomegranates have been known for hundreds of years for their multiple health benefits, including antimicrobial activity. The recent surge in multidrug-resistant bacteria and the possibility of widespread global virus pandemics necessitate the need for additional preventative and therapeutic options to conventional drugs. Research indicates that pomegranates and their extracts may serve as natural alternatives due to their potency against a wide range of bacterial and viral pathogens. Nearly every part of the pomegranate plant has been tested for antimicrobial activities, including the fruit juice, peel, arils, flowers, and bark. Many studies have utilized pomegranate peel with success. There are various phytochemical compounds in pomegranate that have demonstrated antimicrobial activity, but most of the studies have found that ellagic acid and larger hydrolyzable tannins, such as punicalagin, have the highest activities. In some cases the combination of the pomegranate constituents offers the most benefit. The positive clinical results on pomegranate and suppression of oral bacteria are intriguing and worthy of further study. Much of the evidence for pomegranates’ antibacterial and antiviral activities against foodborne pathogens and other infectious disease organisms comes fromin vitrocell-based assays, necessitating further confirmation ofin vivoefficacy through human clinical trials.

This paper reports the results of a laboratory study of the effects of (i) manhole proximity to installation site, (ii) pipe slope (i.e., approach velocity), (iii) upstream head measurement location, and (iv) submergence on the "hydraulics" of two types of sewer weirs used by the City of Ottawa. The City's streamlined form of broad-crested weir and its compound-shaped sharp-crested weir were initially calibrated for a "pipe straight"-arrangement (no upstream manhole is present) for a wide range of flow conditions at 0.2%, 0.4%, and 1.0% pipe slopes. The calibration tests were then repeated with the weirs positioned immediately downstream of a model manhole structure ("manhole"-arrangement). Corresponding data sets were compared to determine the effects of (i) approach velocity and (ii) manhole proximity (for two different shapes of manhole "benching") on the respective weir discharge coefficients.Key words: sewer weirs, broad-crested weir, compound sharp-crested weir, calibration, sewer benching, discharge coefficients.
