Background: Musculoskeletal injuries both in sport and recreational activities, at each age level, can lead to immobilization. Injury and immobilization can lead to muscle strength decrease and deconditioning. These conditions can be addressed by provision of strength training therapy especially for people with low exercise tolerance such as the elderly and patients with chronic disease. Existing studies stated eccentric exercise can increase muscle strength with less energy expenditure compared to concentric exercise. However eccentric exercise can lead to muscle damage indicated with an acute CK level increase, and eccentric exercise has not been carried out regularly for post muscle injuries and deconditioned patients. The American College of Sport Medicine (ACSM) stated strength increase and muscle hypertrophy require high intensity load strengthening exercise with a minimum load of 70% 1 Maximum Repetition. The recommended load was given to the subjects in this research. Our pleriminary study intervention was well tolerated by healthy subjects.Aim: Comparing acute CK level alteration in high intensity eccentric and concentric muscle strengthening exercise on untrained healthy subjects.Methods: Randomized pre test and post test group design. Subjects of the study (n = 16) were untrained healthy males. CK level was evaluated by laboratory test.Results: Our study showed decline of acute CK level in both groups with p value 0.65 and 0.76, respectively. No significant differences found on both groups.Conclusion: The increase of acute CK level after five weeks of eccentric strengthening exercise is not higher than that in concentric strengthening exercise.

The increasing number of elderly and elderly life expectancy is a challenge for health workers. Decreased cardiac endurance, muscle strength and increased incidence of inflammation will occur in the elderly as we get older. health cardio exercises series-I is an aerobic exercise that has moderate intensity and is carried out for 30 minutes, research is needed to analyze the effect on cardiac endurance, muscle strength and decrease plasma TNF-? levels in the elderly. Research design with experimental method, pre-test and post-test design approach. The study was conducted for 4 weeks on 25 samples which were divided into 3 groups of frequency of health cardio exercises series-I to measure endurance of the heart. A 6-minute road test and muscle strength were used for hand grip strength dynamometer, test back and leg dynamometer. Measurement of plasma TNF-? levels is carried out using the Elisa method. The data was then analyzed using paired t-test to see differences before and after health cardio exercises series-I. The results showed health cardio exercises series-I performed 3x / mg can increase the heart's endurance compared to the frequency of 4x / mg and 5x / mg (-16.11 vs 15.62 vs -47.62 meters). health cardio exercises series-I frequency of 3x / mg and 4x / mg can increase back muscle strength (p-value = 0.019, p <0.05) and the frequency of 5x / mg can increase the strength of attractive shoulder muscles. (p-value = 0.033, p <0.05). Health cardio exercises series-I performed 3x / mg, 4x / mg and 5x / mg did not have a significant effect on plasma TNF-? protein expression in the elderly (0.506 vs 0.856 vs. 0.382, p <0.05). It is suggested that the elderly can do I-1 series heart exercise with a frequency of 3x / mg to increase the heart's lung endurance and back muscle strength, a health cardio exercises series-I 4x / mg series to increase muscle strength of the back back, a series of health cardio exercises series-I frequency of 5x / mg to increase attractive shoulder muscle strength.

The association between fatigue and sarcopenia is not well understood, therefore, this study aimed to compare the sarcopenia measures among elderly with mild and severe fatigue and to determine whether fatigue severity is associated with sarcopenia measures. This was a cross-sectional study conducted on201 elderly (age = 68.45±6.30 years). The elderly was classified into either mild or severe fatigue based on the Fatigue Severity Scale (FSS), meanwhile, sarcopenia measures include SARC-F score, muscle mass (ASM/height2), calf circumference (CC), upper (handgrip) and lower limb muscle strength, as well as physical performance (gait speed). Data were analyzed using independent t-tests and logistic regression. The results showed that elderly with severe fatigue were significantly older, with lower muscle strength, and slower gait speed (all p-value <0.05). After adjusting for age, fatigue severity remained significantly associated with SARC-F score (OR = 1.583, 95% CI = 1.262-1.986, p-value = 0.001) and CC (OR = 1.103, 95% CI = 1.014-1.200, p-value = 0.022). Moreover,when the SARC-F score was removed from the regression model, fatigue severity was significantly associated with CC (OR = 1.088, 95% CI = 1.006-1.178, p-value = 0.036) and gait speed (OR = 0.011, 95% CI = 0.001-0.168, p-value = 0.001). Based on the results, fatigue severity is associated with SARC-F score, CC, and gait speed, therefore, interventions targeted at sarcopenia measures is recommended to optimize physical endurance in the elderly.

Anxiety is one of the most common mental health problems in the elderly. The provision of appropriate and accurate psychopharmacology as well as nursing therapy is not enough, but must be followed up with selected therapy modalities regularly and continuously until the normal functioning of stable normative behavior or adaptive behavior, one of which is group activity therapy. To determine the effect of group activity therapy on the level of anxiety in the elderly. The design of this study is a quasi-experimental design with a pre-post test group design with a total sample of 44 people. The independent variable is group activity therapy (TAK) and the dependent variable is anxiety. The instruments used are questionnaire sheets and observation sheets. The statistical test used the Wilcoxon test with a significance value of = 0.05. The anxiety level of the majority of the elderly before TAK therapy was severe anxiety as much as 40.91% and after TAK therapy the majority of the elderly with mild anxiety level as much as 47.73%. There was a difference in anxiety before and after giving group activity therapy with a P value of 0.000 (P <0.05). Group activity therapy has an effect on reducing anxiety levels in the elderly.

Deterioration in muscle mass and muscle strength is common among the frail older persons, cause functional dependence and decrease in the quality of life. Therefore, the identification of cost-effective interventions to prevent or ameliorate frailty is crucially needed. The aim of this study is to determine the effectiveness of a Community-based Muscle Strengthening Exercise (COME) program to increase muscle strength among pre-frail older persons. This study was a quasi-experimental study. A total of 32 older persons aged 60 years and older with pre-frail symptoms were recruited from the community center. The COME was developed based on the Growing Stronger program and the Otago Home Exercise Program. COME was designed to strengthen all of the major muscle groups in the upper and lower extremities. The exercise program was conducted for 12 weeks and divided into 3 parts; (1) to strengthen the body slowly and gently, using only body weight, (2) to introduce dumbbells and ankle weights to increase strength and (3) to add various new ways to boost strength even more. Functional tests were measured before and after the intervention. The results revealed non-significant p-value for pre- and post-intervention comparison for all study outcomes. Observing the values of mean difference, the study intervention was shown to have slightly improved the time up and go (Mean difference = −0.25), and sit-to-stand duration (Mean difference = −0.41) as well as the handgrip strength (Mean difference = 0.68) among the participants. On the assessment of Cohen ES, all three improvements exhibited small effect sizes. Sit-to-stand duration was shown to have most benefited from the intervention with highest ES among the outcome variables (ES = 0.20). COME intervention program among pre-frail older persons showed favorable trend toward improvement of upper and lower extremities muscle strength. This study should be further tested in randomized control trial to confirm its effectiveness.

Lingual strengthening training can improve the swallowing function in older adults, but the optimal method is unclear. We investigated the effects of a new progressive resistance exercise in the elderly by comparing with a conventional isometric tongue strengthening exercise. Twenty-nine participants were divided into two groups randomly. One group performed forceful swallow of 2 mL of water every 10 s for 20 min, and a total of 120 swallowing tasks per session at 80% angle of maximum head extension. The other group performed five repetitions in 24 sets with a 30 s rest, and the target level was settled at 80% of one repetition maximum using the Iowa Oral Performance Instrument (IOPI). A total of 12 sessions were carried out by both groups over a 4-week period. Blinded measurements (for maximum lingual isometric pressure and peak pressure during swallowing) were obtained using IOPI before exercise and at four weeks in both groups. After four weeks, both groups showed a significant improvement in lingual strength involving both isometric and swallowing tasks. However, there was no significant difference between the groups in strength increase involving both tasks. Regardless of the manner, tongue-strengthening exercises substantially improved lingual pressure in the elderly with equal effect.

Aktivitas fisik akan meningkatkan kebugaran jasmani sehingga dapat memperlambat perkembangan kognitif dan penurunan fisik lansia dengan demensia. Demensia merupakan sindrom neurodegeneratif yang timbul karena adanya kelainan bersifat kronis dan progesif disertai dengan gangguan fungsi luhur multipel seperti kalkulasi, kapasitas belajar, bahasa, dan mengambil keputusan. Penelitian ini bertujuan untuk mengetahui kebugaran jasmani lansia dengan demensia pasca aktivitas fisik dengan senam vitalisasi otak. Metode yang digunakan dalam penelitian ini yaitu Metode Experimental. 10 subjek yang berusia 60 – 85 tahun dengan demensia ringan diberi perlakuan senam vitalisasi otak selama 12 kali dengan durasi waktu 25 menit. Setelah melakukan aktivitas fisik dilakukan pemeriksaan tes kebugaran jasmani (testing the elderly) yang terdiri dari: 6 minutes walk test (6 MWT), flexibility: Chair seat and reach test (CSRT), Muscle strength and endurance: Chair stand test (CST), dan Arm curl test (ACT), koordinasi dan kelincahan, 8 Foot up and go test (8 FUGT). Hasil penelitian dengan menggunakan uji T menunjukkan bahwa tes kebugaran memberikan hasil yang signifikan pada tes CST, CSRT (p value 0,0296) dan 8 FUGT 6MWT (pvalue 0,0229) pada tingkat kepercayaan 95%.  Kesimpulannya, aktivitas fisik vitalisasi otak berpengaruh terhadap kebugaran jasmani dengan perbaikan tonus otot dan kognitif pada lansia dengan demensia.

AbstractExercise is the most common conservative intervention for chronic low back pain (CLBP). We have developed an innovative exercise device for the abdominal trunk muscles that also measures muscle strength in a sitting position. The device, which is easy for patients with CLBP to use, allows for lumbar stabilization exercise under pressure. This study aimed to examine the efficacy of abdominal trunk muscle strengthening using the device in improving CLBP. We conducted a two-group non-randomized controlled clinical trial. CLBP patients were allocated into two groups. The strengthening group underwent a 12-week exercise program that included abdominal trunk muscle strengthening using our device and stretching exercises, while the control group received a 12-week stretching exercise program. The outcome measures included the improvement of the abdominal trunk muscle strength measured by the device, pain intensity of CLBP, physical function, and quality of life (QOL). A total of 40 participants (20 in each group) were analyzed. The strengthening group showed better improvement in the abdominal trunk muscle strength, CLBP, physical function, and QOL than in the control group. In conclusion, the strengthening exercise using the device with easy stretching was effective in improving the strength of the abdominal trunk muscles, pain intensity of CLBP, physical function, and QOL.
