Abstract Objectives IL-17A G197A and IL-17F A7488G polymorphisms has been identified to be associated with the susceptibility to many diseases. This study aimed to investigate the frequency distribution of IL-17A G197A and IL-17F A7488G polymorphisms among healthy Sudanese population. A descriptive cross-sectional hospital-based molecular study conducted in different sites throughout Sudan. Two ml blood samples were collected from 717 healthy participants. Demographic data and the medical history of the participants were collected.Results Of the 717 participants, 355 (49.5%) were males and 362 (50.5%) were females, their mean age was 30.2 ± 17.2 and 32.2 ± 16.5, respectively. For IL-17A, the most frequent genotype detected among males and females was IL-17A heterozygote allele (AG); 215 (60.6%) and 194 (53.6%), respectively. Whereas, for IL-17F, the most frequent allele among males and females was the homozygote allele (AA); 298 (83.9%) for males and 322 (89.0%) for females. HWE for genotype distributions of IL-17A was showing statistical insignificance for IL-17A among males and females, P value 0.614. While HWE for IL-17F reached the equilibrium level, P value 0.048. The most frequent age group was those aged between 21 to 40 years; 281 (39.2%). Arab constituted the major ethnicity of the study participants; 418 (58.3%), P value 0.034.

Aim: To find out the most frequent head and neck sites of origin of referred otalgia. Study Design: Descriptive cross-sectional. Place and Duration of Study: Study was conducted at the ENT department of Bahawal Victoria hospital Bahawalpur during January 2019 to December 2019. Methodology: About 500 patients with ear ache were examined and after exclusion 150 were finally analyzed. Detailed history of the patient and clinical examination were done along with radio imaging and endoscopic studies wherever needed. Diagnosis of referred otalgia was made after having normal ear examination along with pathology lying at different head and neck region which share common sensory innervation. For analysis SPSS version-20 was used. Results: The mean age of the study participants was 28.23 ±13 years. The majority of participants were having otalgia of tonsillar origin followed by dental origin and pharyngitis with 31.3%, 23.3% and 16% respectively. Very few of them having otalgia originating from hypopharyngeal carcinoma and laryngeal carcinoma i.e. 3.3% and 2% respectively. While only 4.7% of otalgia were of unknown origin. Female participants were having higher frequency of otalgia which is originated from diseases of temporomandibular joint, hypopharyngeal carcinoma and otalgia of unknown origin in comparison to male participants but differences were insignificant. Otalgia originated from tonsillar and dental causes and due to pharyngitis were most common among the younger age group but having non-significant p-value. Conclusion: It has been concluded that the patients presenting with ear pain, have sometimes no underlying ear pathology. If the ear examination is normal then it is important to examine all other sites of head and neck which share sensory innervation with the ear including teeth, tonsils, pharynx, larynx, nose and paranasal sinuses to find out the exact cause of referred otalgia.

Background and aims: Maternal primary and recurrent infection of cytomegalovirus (CMV) may be transmitted to the fetus during pregnancy and may have complications such as death or growth, along with the development retardation of the fetus and infant. The aim of this study was to determine the prevalence of immunoglobulin G (IgG) and immunoglobulin M (IgM) antibodies against CMV in young women, Sanandaj, Iran. Methods: To this end, 90 women (15-40 years old) referring to a clinical laboratory were randomly selected and announced their informed consent to participate in this cross-sectional study. Demographic information and women’s data were collected, including pregnancy, history of abortion, and history of blood transfusion. Then, women’s sera were measured for CMV IgG and IgM antibodies using the electrochemiluminescence technique. Finally, the data were analyzed by SPSS statistical software. Results: The prevalence of IgG and IgM antibodies against CMV in women was 92.2% (95% CI = 86.5-97.8) and 0%, respectively. In addition, the average CMV IgG antibody level was about 137.52 ± 85.215 SD IU/mL. The results revealed a significant statistical association between IgG antibody and pregnancy (P value = 0.012) while there was no association between CMV IgG antibody and other demographic data. Conclusions: In general, high percentages of women had CMV IgG antibody whereas 7.8% of them were susceptible. They are expected to acquire CMV primary infection, and therefore, the screening of antibodies to CMV is suggested for prenatal care.

Objective: To study the happiness status and related factors among clinic-medical students.Material and Method: A cross-sectional study was conducted in order to survey all medical students, Faculty of Medicine, Prince of Songkla University in 2015. Questionnaires for demographic data, health, activity, human relationships andThai Mental Health Indicators-15 were used. Mean,percentage, chi-square, and fisher’s exact test were used toanalyze the data.Result: There were 403 medical students (80.2%) who completed the questionnaires. Of the medical students, 56.3% were female. Mean age was 23 years old; 26.6% had history of drinking alcohol. The significant factors associatedwith happiness status were medical class, history of drinking, and exercise. Fourth year medical students had asignificantly higher happiness status than 5th and 6th year students (p-value<0.001). Medical students who didn’t havehistory of drinking had a significantly higher happiness status than medical students who had a history of drinking(85.0% and 73.6%, p-value=0.014). Medical students who had a history of exercise had a significantly higher happinessstatus than medical students who didn’t have a history of exercise (84.5% and 73.7%, p-value=0.024).Conclusion: The significant factors associated with happiness status were medical class, history of drinking alcohol,and exercise. Being in the 6th year of medical school and having a history of drinking were significant factors relatedto a lower happiness status. History of exercise was a significant factor related to a higher happiness status.

Background:Foot disease is a common problem in rheumatoid arthritis (RA). Therapeutic decisions are often based on clinical examination (CE) alone, which can be adversely affected by factors such as deformity, obesity, and peripheral edema. Ultrasonography (US) has previously been shown to be more sensitive than CE for detecting synovitis and tenosynovitis in RA forefeet, but few data exist for the hindfoot and ankle.Objectives:The aim of this study was to compare CE and US for the detection of hindfoot and ankle synovitis and tenosynovitis in patients with established RA.Methods:We conducted a cross-sectional study including patients with RA (ACR/EULAR 2010). Demographic data and disease parameters were collected. CE was performed by a rheumatologist for the presence or absence of tenderness, swelling, and mobility restriction of both ankles. The following tendons were examined for tenosynovitis: tibialis anterior (TA) and posterior (TP), fibularis longus (FL), and brevis (FB) (assessed together). In a second time, US examination of the tibiotalar, talonavicular, and subtalar joints and the same tendons as CE was performed by a blinded radiologist experienced in musculoskeletal imaging using a Philips HD11 device with a high-frequency linear transducer. The presence or absence of synovitis and tenosynovitis was recorded, and the composite synovitis score (power doppler / grayscale ultrasound (PDUS)) was measured for each joint. The US score of each patient was defined by the sum of the composite scores of the joints studied (0-30). A p-value <0.05 was considered significant.Results:Sixty-two feet were examined in 31 RA patients (25 women and six men) with a mean age of 54.8±10.8 years old [32-70]. The mean disease duration was 8.5±7.2 years [1-37]. Rheumatoid Factor (RF) and Anti-Citrullinated Peptides Antibodies (ACPA) were positive in 61.3% and 83.8% of cases. The mean DAS28 ESR was 3.8±1.5 [0.6-7].Clinical examination of ankles revealed tenderness in 57.4% of cases, swelling in 38.8% of cases, and restriction in the range of motion in 11.1% of cases. TA tenosynovitis was noted in 14.8% of cases, TP tenosynovitis in 22.2% of cases, and FL and FB tenosynovitis in 31.5% of cases.US showed tibiotalar synovitis in 59.3% of cases, talonavicular synovitis in 64.8% of cases, and subtalar synovitis in 46.3% of cases. TA tenosynovitis was noted in 5.6% of cases, TP tenosynovitis in 22.2% of cases, and FB and FL tenosynovitis in 25% and 11.1% of cases respectively.An association was found between clinical tenderness and US synovitis of the tibiotalar joint (p=0.013) and the talonavicular joint (p=0.027). No association was noted between clinical swelling and US synovitis in these joints.No association was noted between clinical and US tenosynovitis of TA (p=0.279), TP (p=0.436), FB (p=0.495) and FL (p=0.315).Conclusion:Clinical examination of RA ankles may be challenging and needs to be coupled with US, which is more sensitive and accurate in the detection of synovitis and tenosynovitis.Disclosure of Interests:None declared

Asthma in children poses a significant clinical and public health burden. We examined the association between reported neighborhood traffic (a proxy for traffic-related air pollution) and asthma among 855 multi-racial children aged 4–8 years old who participated in the Environmental Influences on Child Health Outcomes (ECHO) cohort. We hypothesized that high neighborhood traffic density would be associated with the prevalence of asthma. Asthma/asthma-like symptoms (defined as current and/or past physician diagnosed asthma, past wheezing, or nighttime cough or wheezing in the past 12 months) was assessed by parental report. The relationship between neighborhood traffic and asthma/asthma-like symptoms was assessed using logistic regression. The prevalence of asthma/asthma-like symptoms among study participants was 23%, and 15% had high neighborhood traffic. Children with significant neighborhood traffic had a higher odds of having asthma/asthma-like symptoms than children without neighborhood traffic [adjusted OR = 2.01 (95% CI: 1.12, 3.62)] after controlling for child’s race-ethnicity, age, sex, maternal education, family history of asthma, play equipment in the home environment, public parks, obesity and prescribed asthma medication. Further characterization of neighborhood traffic is needed since many children live near high traffic zones and significant racial/ethnic disparities exist.

Hypertension is one of the number one causes of death and disability in the world. Hypertension contributes nearly 9.4 million deaths from cardiovascular disease each year. Hypertension can cause undesirable effects, it needs good handling, one of them is by doing a hypertension diet. To carry out a hypertension diet requires knowledge, lack of knowledge can increase risk factors for hypertension. This study aims to determine the relationship of the level of knowledge about hypertension diet to the incidence of hypertension in the elderly at the Baloi Permai Public Health Center Batam City. This research method is an analytic observational with a cross-sectional approach conducted at the Baloi Permai Public Health Center Batam City 2018. Sampling technique is a total sampling with a sample of 64 people in 2018 determined by inclusion and exclusion criteria. The results of the study were analyzed with frequency distribution and then tested with the Chi-square test. Based on the results of this study indicate that of the 64 respondents found elderly who have a good level of knowledge are 41 people (64.1%), 48 people (75.0%) have an age range between 60-70 years. 27 people (42.2%) elderly have the last high school education and 40 people (62.5%) have jobs as entrepreneurs. Elderly people who have normal blood pressure are 40 people (62.5%), and those affected by hypertension are 24 people (37.5%). The elderly who have a family history of hypertension is 21 people (32.8%) and those who do not have a history of hypertension are 43 people (67.2%). Chi-Square Test analysis results show the significance value p = 0.009. This number is significant because the p-value is smaller than the significance level (α) ≤ 5% (0.05), so H0 is rejected and Ha is accepted. Therefore it can be concluded that there is a significant relationship about the level of knowledge about the hypertension diet to the incidence of hypertension in the elderly. From the results of this study it was concluded that there was a relationship between the level of knowledge about the hypertension diet and the incidence of hypertension in the elderly at the Baloi Permai Public Health Center Batam City in 2016.

Backgrounds: Commonly, doctors migrate for postgraduate study in African and Middle-east countries to western countries. In this study, we would like to know the percentage of graduates who intended to study abroad and determine the factors of their traveling.Material and methods: This is cross-sectional questionnaire survey. Medical students were asked about their future specialty preference, reasons, and site of training. Also, some demographic data were collected. The data was analyzed by SPSS. P value of < 0.05 was considered significant. Result and Discussion: 197 students responded to our survey and filled the questionnaire properly. Participants knew about future specialty from the teacher, the internet, senior college and advice from a family member with rate 83 (42.1%), 42 (22.8%), 41 (21.3%), and 27(13.7%) respectively. One hundred five (53.3%) of the responders preferred to get their specialty training outside Sudan while the remainder 92(46.7%) preferred local training.The commonest specialty selected among both group are medicine and surgery respectively. Obstetrics and gynecology are selected mainly among students who prefer local training 14 out of 15 totally, with statistically significant (p-value = 0.0001). Residency has significant related to the site of training with (P value 0.03). International recognition and advanced facilities are the commonest cause of traveling abroad among the students. While free hands on training and social reasons are the most common causes of home postgraduates training.Conclusion: More than fifty percent of the participant prefers abroad post-graduates training. The factors which influents site training can be defined as "Pull and Push" factors.

BACKGROUND: Physical activity is one of the most important factors that affect human health; it reduces the chances of hypertension, coronary heart disease, stroke, diabetes, breast and colon cancer, and depression.OBJECTIVES: The objectives of this study were to measure prevalence of physical activity and to determine the factors affecting the level of physical activity among medical student in Jazan University.METHODS: A cross-sectional study was conducted among the students of medical colleges of Jazan University. A random sample of 419 was determined using most recent physical activity prevalence. A validated self-administered questionnaire was used for data collection.RESULTS: There is a high prevalence of inactivity among study participants (88.1%). Females (91.7%) were more significantly (p value=0.013) inactive than males (83.8%). The most influential barrier perceived by participants is (heavy) academic work as well as lack of places for physical activity; the latter factor being more effective in hindering female students' physical activity.CONCLUSION: Results revealed that the inactivity rate was very high among medical students. The results of this study call for a well-planned intervention at the university level for improving the level of physical activity among university students.
