The clinical confirmation of the relationship between uterine leiomyoma and female infertility is of interest to scholars. The aim of our research was to study obstetric history and the level of sex hormones in women of reproductive age with leiomyoma. Materials and methods. The main group consisted of 90 women of reproductive age with uterine leiomyoma, the control group - 45 healthy women. Gynaecological and obstetric history, hormone content in peripheral blood and pelvic vessels (estradiol, progesterone) were analysed. Statistical processing of clinical material was performed using Microsoft Excel, Statistica 7.0, and Statistica 8.0 for Windows. Results. The frequency of infertility in the surveyed women with LM was 18.9 %. One in three patients in the main group (33.3 %) did not give birth, which was statistically different from the healthy women group. In this case, 25.6 % of women in the main group did not use any methods of contraception. It was found that 51.11 % of patients with LM had threatened miscarriage in their history, 37.5 % – complicated delivery. Pathological changes in the concentrations of estradiol and progesterone in the preferential blood and blood vessels of women with fibroids have been identified. We have found that the content of sex hormones in the local bloodstream has a reliable relationship with the location of the myomatous node. Conclusion. The obtained results may indicate that leiomyoma contributes to the reduction of fertility. This is evidenced by the fact that women with LM are more likely to have the risk of miscarriage, complicated delivery and delivery by caesarean section, including complicated one. The findings have made a significant contribution to the further development of effective strategies for the prevention and treatment of infertility in women with fibrotic uterine lesions.

Introduction. Analysis of uterine leiomyoma incidence indicates not only its growth but also the increase in the number of women of early reproductive age. However, the problem of trigger mechanisms for its development is still uncertain and actual.The objective of the research was to assess the characteristic features of hyperplastic processes in uterus on the background of dishormonal and metabolic disorders.Materials and methods. 60 women of reproductive age with uterine leiomyoma participated in the clinical trial. They gave their informed written consent. The control group consisted of 20 healthy women of reproductive age. Statistical analysis was conducted using material package “STATISTICA for Windows®-6.0” and methods of odds ratio (OR) calculating and its 95% Confidential Interval (95% CI).Results and their discussion. According to the results of the research the main factors contributing to uterine fibroid development in women with obesity were determined. They included: the age of 30-45 years, instrumental intervention in the womb (abortion, diagnostic curettage, etc. (OR = 8.2; 95% CI: 5.1-13.5)); chronic inflammatory diseases of genitalia (OR = 7.9; 2.9-21.9); hormonal imbalances (fibro-cystic breast disease, thyroid disorders (OR = 8.61; 3.1-23.8), liver and gastrointestinal tract disorders (OR = 5.8; 2.1-15.9) sexual disorders and stress inductive factors (OR = 2.6; 1.4-7.1) (usually fibroids occur in 1-2 years after severe stress). There is a significant percentage of adenomyosis and endometrial hyperplastic processes. Growth of tumors in women with obesity increases to 35-40 years of age, and is associated not only with a progressive decrease in the functional activity of the ovaries, their sensitivity to gonadotrophic stimulation, but also with chronic functional exertion of regulation systems on the background of metabolic disorders of hemostasis and homeostasis in this category of women.Conclusions. One of the most significant risk factors for hyperplastic processes of the reproductive organs is obesity and associated metabolic disorders. In case of metabolic disorders activity and hepatocytes on the background of obesity the number of combined forms of hyperplastic processes in the uterus increases by 1.9 times. The main risk factor for fibroids in obese women is hepatocytes dysfunction on the background of a high percentage of hepatobiliary pathology, endocrine pathology – 68.3% (thyroid dysfunction, fibro-cystic breast changes). Morphological and histological structure of endometrium in obese women leads to the growth of endometrial polyps (36.66%).

The objective: to evaluate the effectiveness of treatment of women with various dysbiotic vaginal conditions with Limenda. Materials and methods. 58 women of reproductive age with BV and mixed nonspecific vaginitis participated in the study. The main group included 30 women, whose treatment was carried out with Limenda drug. The control group consisted of 28 women, whose treatment was performed according to the standard scheme using a combination of drugs. Results. Based on the results of control tests, complete elimination of pathogens occurred in 29 (96.67%) patients of the main group and in 26 (92.86%) patients of the control group. Five days after the treatment, all 30 (100%) women in the main group noted complete disappearance of symptoms on the control visit, and 2 (7.14%) of the control group complained of minor discomfort in the vagina, which required continued treatment. The conclusion. For the treatment of nonspecific vaginitis and bacterial vaginosis, especially chronic and often recurrent forms, it is better to use combined local remedies. The results of this study reliably testify to the high effectiveness of the combined drug Limenda compared with standard treatment regimens. One of the most common benign hyperproliferative diseases of the female reproductive organs are uterine leiomyoma, the frequency of which, according to various literature in women of reproductive age is between 20% and 40%, and is the main indication for surgery, including removal of the organ. Long-term effects of surgical interventions for uterine fibroids have been the subject of numerous debates. In modern literature is not much research on the assessment of the quality of life both in organ treatment of uterine fibroids, and after surgical recovery. Quality of life have undoubtedly significant prognostic value and can be used in selecting the optimal treatment of uterine fibroids, as well as the development of individual approach to postoperative rehabilitation of patients. The objective: was the study of quality of life and the search for possible ways to improve in women after surgical healing uterine fibroids. Materials and methods. A survey of 80 women of reproductive age (study group), which was conducted without hysterectomy applications. Clinical and laboratory examination conducted in the dynamics of the postoperative period and at 6, 12 months and 3 years after surgical recovery. Assessment of comparative analysis require further formation of two groups comparing 20 patients with myoectomy and 20 women with no uterus amputation supravaginal applications. The control group consisted of 20 healthy patients. Assessment of quality of life survey conducted by using common clinical trials and monitoring at the individual questionnaire MOS 36- Item Short-Form Health Survey- MOS SF-36. Statistical analysis of the survey results was carried out using the software package Microsoft Office Excel and Statistica 6.0 for Windows. Results. In women, the main indicators of psycho-emotional and vegetative manifestations were most pronounced immediately after surgery and remained stable in three years with no significant deviations in the comparison group. Among the most frequently observed fatigue (76.25%), sleep disorders (88.75%), irritability and mood changes (76.25%) and emotional lability, anxiety and depressive symptoms. Of The results of evaluation of quality of life should be noted that first disturbed functioning role of women because of poor physical and emotional state, as evidenced by the significant differences obtained on the scale of General Health (GH) (overall health). Three years after the rapid improvement in women’s core group established stable condition posthisterektomy pronounced effect on quality of life. Thus, in total 33.75% of the principal groups characterized as low quality of life, compared with 5.0% of patients in the control group and 10,0% of women with myoectomy. Conclusions. The highest level of dissatisfaction with life studied patients presenting in health – 28.75% (23) cases, slightly less professional – 22.5% (18), emotional – 17.5% (17) and as in the sexual sphere – 13.75% (11) cases. That associated complications related symptoms posthisterektomy syndrome, increases the level of stress and hysterectomy as the body creates prerequisites for psychosomatic disorders. This action provides additional risk factors for the development of psychosocial distress in this group of patients. The authors assessed the quality of life of 263 women of reproductive age. The main group consisted of 139 women aged o18-44 as the patients with active pulmonary tuberculosis disease treated in 2011-2012, and the other group of comparison consisted of 124 healthy women of the same age. To assess the life quality the authors used Russian version of the SF-36 which included questions of medico- social nature. It was proved that all the life quality parameters of the women suffering from tuberculosis disease were reduced in comparison with the control group of the healthy women. The only exception was the pain syndrome for which the statistical significance wasn’t reached at all and the physical functioning was assessed at the lowest level. The life quality problem was identified almost in half of the women that proved by the obtained data. Analysis of quality of life of the women established that 25-34 years respondents had the highest reducing of the rates. The analyzed group of patients differed from the low level of the socio- economic independence relatively favorable structure of clinical tuberculosis forms but with a high frequency of tubercle bacilli discharging and detection of lung tissue destructing. Generally, women with pulmonary tuberculosis had numerous risk factors for disease and co-morbidities aggravating the main process. The unfavorable health and social hygienic characteristics of the analyzed group of patients contribute to late making a diagnosis and spreading the infection among the population, including the women of reproductive age. Therefore health care professionals need to make efforts to promote healthy lifestyle and prevention of tuberculosis with this contingent that will allow to prevent new cases of disease and to reduce morbidity. The aim of any disease treatment, including tuberculosis should be considered as acts to improve the life quality of patients in case of positive clinical dynamics. Therefore it is important to guide medical practice for the patients’ psychosomatic status correcting in the early stages of disease that will be able to solve health problems more successfully. Thereby, the authors think it is necessary to organize services; to open offices where patients could get psychosocial correction and rehabilitation, to train the specialists.

The objective: on the basis of a comprehensive examination of women of reproductive age to establish the frequency of phenotypes (clinical variants) of polycystic ovary syndrome (PCOS).Materials and methods. 34 patients (main group) who complained of menstrual disorders and/or dermatopathies by recommendation of a dermatologist were examined. The control group is represented by 30 women without gynecological and somatic pathology. The mean age of women in the main group was 26,4±0,9 years and 29,1±0,9 years in the control group (p>0,05). The age of women in the study groups ranged from 18 to 35 years. Patients underwent a comprehensive examination to assess the severity of hirsutism and the severity of acne, as well as the body mass index was determined. All women underwent ultrasound examination in the dynamics and quantitative assessment of the concentration of hormones in the blood plasma, namely cortisol, thyroid-stimulating hormone, prolactin, free testosterone and its index, androstenedione, dehydroepiandrosterone sulfate, 17-α-OH-progesterone, sex hormone binding globulin. Variation-statistical processing of the results was carried out using the program «STATISTICA 13».Results. The results of the conducted research show that 73,5% had menstrual irregularities, and 52,9% – infertility. Acne and hirsutism in every 3rd woman were combined and were diagnosed in 47,1% and 41,2% of women, respectively. Ultrasound signs of polycystic ovaries were found in 94,1% of patients according to the criteria for the diagnosis of PCOS, and in 88,2% – anovulation. According to the laboratory examination, hyperadrogenism was found in 55,9%, which is confirmed by statistically significant (p<0,05) predominance in the main group compared with the control group of androstenedione, free testosterone and its index. In addition, it should be noted statistically significant (p<0,05) higher levels of 17-α-OH-progesterone and prolactin in the main group, but their indicators were within the reference values of the norm. Analyzing the frequency of phenotypes (clinical variants) of PCOS, it was found that phenotype A (classical) occurred in 32,4%. Phenotype B (incomplete classical) was diagnosed in 14,7%, and phenotype C (ovulatory) – only 8,8%. The most often, namely in 15 (44,1%) women with PCOS, the phenotype D (non-androgenic) was established.Conclusions. The results of the conducted research show that in women with PCOS clinical symptoms are characterized by menstrual dysfunction (73,5%), infertility (52,9%) and dermatopathies, namely acne (47,1%) and hirsutism (41,2%). According to the laboratory exanination, hyperadrogenism was found in 55,9%, which is confirmed by statistically significant (p<0,05) predominance in the main group compared with the control group of androstenedione, free testosterone and its index. Among the clinical variants of PCOS, the non-androgenic phenotype (phenotype D) was the most often diagnosed, the frequency of it was 44,1%. Classical (phenotype A) and incomplete classical (phenotype B) were found in 32,4% and 14,7%, respectively. It should be noted that only 8,8% of women with PCOS are diagnosed with phenotype C (ovulatory).

Aromatase is the key enzyme, which converts androgens into estrogens. To study the role of aromatase in pathogenesis of endometriosis 57 patients and 15 healthy women of reproductive age were examined. Aromatase activity was detected by reaction of estrogens on aromatase inhibitor letrozol intake. Ovarian source of detected aromatase activity was proved by suppression of reaction on letrozole intake during therapy with gonadotropin- releasing hormone agonist. Aromatase activity in recalculation on antral follicle in endometriosis patients withII–IV stages was higher than in healthy women, though total ovarian aromatase activity was not differed from control group because of low number of antral follicles in endometriosis patients. The disturbance of folliculogenesis in endometriosis is connected probably with hyperestronaemia, which has ovarian and extragonandal origin.

The objective: to study the clinical efficacy of two schemes of using the monocomponent herbal preparation Cyclodynon® in women of reproductive age with complaints of various menstrual cycle disorders diagnosed with latent hyperprolactinemia and second phase failure of the menstrual cycle based on the results of clinical and hormonal studies. Materials and methods. The study included 60 patients aged 25–38 years suffering from PMS. The main group (n = 30) received Cyclodynon® in an ascending pattern: from 1 to the 14th day of the menstrual cycle – 40 drops 1 time a day and from the 15th day to the end of the cycle – 40 drops 2 times a day. The control group (n = 30) – 40 drops 1 time per day until the end of the cycle for three menstrual cycles. Results. After 3 months in the main group there was a more significant decrease in the level of prolactin (from 28.3 to 18.4 ng / ml) as compared with the control (from 27.8 to 23.1 ng / ml). Increased progesterone and estradiol production was also more significantly observed in the main group. The dynamics of normalization of the duration of the menstrual cycle and the volume of blood loss was also more significant in the main group and amounted to 93.3%. Conclusion. The use of an increasing treatment regimen with Cyclodynon® in women with a luteal phase deficiency on the background of hyperprolactinemia has advantages over the use of a standard continuous regimen with respect to a more rapid normalization of hormonal homeostasis, the duration of the menstrual cycle and blood loss. The objective: To determine the features of the microbiocenosis of the genital tract and immunological homeostasis and in women of reproductive age with uterine leiomyoma.Materials and methods. The microbiocenosis of the genital tract and immunological homeostasis of 308 women with uterine leiomyoma aged 23–49 years were studied. The control group included 30 healthy women aged 24–37 years. Results. In women of reproductive age, the development of uterine leiomyoma is associated with a significant violation of the microecology of the genitals, the presence of nonspecific vaginitis and bacterial vaginosis, increased frequency and concentration of pathogenic flora (staphylococci and streptococci with hemolytic properties, enterobacteria), STIs or reducing the concentration of protective microflora.The study of the peculiarities of the functioning of the immune system at the local level in the presence of uterine leiomyoma indicates unidirectional disorders in the state of local immunity and changes in the protective function of cervical mucus in the examined women. Thus, under conditions of uterine leiomyoma, there is a significant activation of local immunological factors, as evidenced by a significant increase in the concentration of IgG, IgA and the appearance of IgM in cervical mucus in 27,5–32,5 % of women with uterine leiomyoma. In addition, there was suppression of phagocytic activity of neutrophils, which was manifested by a significant decrease in activity and intensity of phagocytosis, with a decrease in their functional reserve by NRT test, which creates conditions for intracellular persistence of infection, activation and persistence of inflammatory progression.Conclusions. The obtained data indicate a violation of the microecology of the genitals and the intensity of immunogenesis in women with uterine leiomyoma, which indicates that the inflammatory process of the genitals is one of the significant factors that lead to the emergence and progression of uterine leiomyoma.

Background: The widespread prevalence of infertility, the low effectiveness of assisted reproductive technologies (ART), and the high incidence of chronic endometritis (CE) in infertile women determine the relevance of the considered problem. The aim of the study was to determine the clinical and anamnestic, laboratory, and instrumental features of CE associated with infertility and unsuccessful IVF cycles in women of reproductive age. Materials and methods: The study enrollred 150 women of reproductive age with morphologically established CE (main group, n=120) and without CE (control group, n=30). A subgroup I of the main group included 64 patients with infertility and IVF failures, a subgroup II – 56 fertile women. In addition to anamnesis collection and identification of CE clinical features, all patients underwent infectious screening, immunological and immunohistochemical analysis, ultrasound examination of pelvic organs with dopplerometry, and office hysteroscopy. A comparative analysis of the data obtained from subgroups of the main group was conducted. Results: Histological study of endometrial pipelle-biopsy specimens on the 7−10th day of the cycle revealed CE in all patients of the main group. We found prevalence of mean duration of CE in the subgroup I relative to subgroup II ― 5.5±0.06 years and 2.4±0.07 years, respectively (p0.001). Infectious screening showed that 58 (90.6%) patients of the I subgroup had sterile endometrial seeding which was 16.9 times higher than in subgroup II (p0.0001). Immunological analysis determined the presence of AEAT in all patients of the subgroup I, 43 of which (67.2%) were above 265 U/ml, while 51 (91.1%) of subgroup II had no AEAT (p0.001). Immunohistochemical analysis of the endometrium on the 18th−24th day of the cycle established high expression of CD16 , CD20 , CD56 , and HLA-DRII in 58 (90.6%) patients of the subgroup I, whereas in 54 patients (96.4%) of II subgroup high expression of CD16 and CD20 with low amount of CD56- and HLA-DRII-positive cells was registered (p0.001). We determined prognostically significant clinical and anamnestic risk factors predisposing to the development of infertility in patients with CE (p0,05). We revealed certain echographic, dopplerometric, and hysteroscopic criteria of CE demonstrating the critical disruption of endometrial receptivity in infertile women. Conclusion: Most patients (90.6%) with infertility had autoimmune component of CE characterized by prolonged (more than 5 years) course, high serum level of AEAT, sterile endometrial crops, and high expression of inflammation markers CD16 , CD20 , CD56 and HLA-DRII .

The objective: the study of the psychoemotional state in women with infertility, who underwent surgical treatment on the uterine appendages in the planned and urgent order. Materials and methods. 120 women of reproductive age with a tubal peritoneal factor of infertility and benign ovarian formations, an ectopic pregnancy, who underwent organ-preserving surgical intervention on the uterine appendages (the main group) were examined. Planned surgical treatment was performed in 76 (63.7%) of the examined main group (group 1); In urgent order, 44 (36.7%) patients of the main group (the second group) underwent surgical treatment. Depending on the access, laparoscopy (LP) with the use of monopolar or bipolar electrosurgery was performed in 54 women of the 1st group and in 24 patients of the 2nd group, and laparotomy (LT) in 22 patients of the 1st group and in 20 patients - 2 nd group. The control group included 20 patients with reproductive function without infertility (3rd group). Twenty women of reproductive age with infertility without surgery entered the comparison group (group 4). Results The article examines the effect of surgical treatment on the uterine appendages in 120 women with infertility on the psychoemotional state depending on the planned nature of the surgical intervention. Women who underwent surgical treatment in urgent order found that pronounced depression and neurotic manifestations are closely related to sleep disturbances, decreased mood, crying, irritability. Conclusion. A month after surgery, depression, anxiety, nervousness decreased, which indicates positive changes in the mental state of patients and an improvement in the state of internal comfort as a result of the recovery of somatic well-being, increased optimism and self-confidence, which is the result of cosmetic effect and organ-preserving surgical treatment.
